  • Meconium peritonitis is an aseptic peritonitis caused by spill of meconium in the abdominal cavity through one or several intestinal perforations which have taken place during intrauterine life or early neonatal life. We experienced three cases of meconium peritonitis with ileal perforation in two cases 1 day-old male neonate and 2 day-old male neonate, respectively, which had the chief complaint of vomiting and abdominal distension, Literatures are reviewed, briefly.

  • Although cavernous angioma itself is not rare, the epidural spinal localization is uncommon and makes preoperative differential diagnosis difficult. An extraordinary case of a thoracic epidural cavernous angioma in very young age, causing sudden paraplegia is presented. Only 79 cases have been reported in the literatures and among them, this kid was the youngest. A 23-month-old boy was referred to us with a 2-day history of sudden both lower limb weakness. Two days before admission, he got up at morning and was unable to stand and even to move the legs. MRI revealed an epidural mass surrounding spinal cord associated with cord compression at the level of the C5 through T3. Through posterior approach with exposure of C6 to T3 level, the hematomatous mass was removed subtotally due to intraoperative bleeding and its ventral location. After the first operation, the weakness of bilateral lower extremities was improved so as to move gainst the gravity. But the next day, the limb weakness was aggravated as same as preoperative status due to mass effect of new hematoma. The second operation was performed to remove the hematoma and to control the bleeding focus. Several weeks later, the limb weakness was improved and he was able to walk. The literatures about spinal cavernous angioma are reviewed.

Study of Traditional Plants of Jeju Island (Five literatures in Joseon Dynasty period) (조선시대 문헌에 기록된 제주도 전통식물의 통시적 연구-세종실록지리지, 신증동국여지승람, 탐라지, 남환박물, 제주계록을 중심으로-)

  • To understand the traditional knowledge of botanical taxa in Jeju Island, a diachronic study was carried out by comparing the archaic and modern names of the native plants in Jeju Island. To identify the archaic names of the plants, five old documents (Sejongshillokchiriji, Shinjŭngdonggukyŏjisŭngnam, T'amnaji, Namhwanbangmul and Jejugyerok) that were written during the Joseon Dynasty between the 15th and 19th centuries were closely examined. A total of 131 taxa (65 families, 112 genera, 118 species and 13 varieties), 7.3% of the native plants known to be currently present in Jeju Island, was identified. Out of these, 21.4% was the plants found in the southern area. Among the five old documents analyzed for this study, Namhwanbangmul recorded the largest number of plants, 89 taxa. We closely examined the consistency between the modern and archaic names of each plant, and discussed the problems in matching the names of some plants analyzed in this study.

SQUAMOUS CELL CARCINOMA ARISING FROM RESIDUAL ODONTOGENIC CYST;Report of a Case & Review of Literatures (치성낭종으로부터 유래된 편평상피세포암종)

  • Odontogenic cysts are relatively common pathologic lesions found in the oral and perioral structures, but the case of squamous cell carcinoma arising from those cysts are very uncommon. After first reported of that case in 1889 by Herman, Schwimmer collected 56 cases of previously reported squamous cell carcinoma arising in residual odontogenic cyst during about past one century. More than 60% of cases of carcinoma developing in odontogenic cysts arising in inflammatory periapical or residual cyst, and these tumors are usually well-differentiated with relatively good prognosis, and often are diagnosed as benign lesion in radiographic or clinical examination, therefore definitive diagnosis must be made by histologic examintation. We report a case and review the literatures, in our case, 78-year old woman were clinically and radiographically diagnosed as residual odontogenic cyst. But in histologic examination after enucleation of lesion, mass of squamous cell carinoma were observed, but in other area, typical cyst wall and lining epithelium were observed. And in some area, carcinoma in situ and invading squamous cell carcinoma into the lining epithelium were also observed.

Ebstein`S Anomaly: A Case Report of Plication and Tricuspid Valve Replacement (Ebstein 심기형 수술 1례[Plication 및 삼첨판막 이식예])

  • A 8 year old male was admitted to the Department of Thoracic Surgery, Korea University Hospital on June 22, 1978. The chief complaints were cyanosis and exertional dyspnea since at birth. EKG shows BVH and dextrocardia, phonocardiogram revealed the accentuation of second heart sound in aortic area. Echocardiogram from the left ventricle to the base of the heart, there is a discontinuity between the ventricular septum and the anterior aortic margin with a large aortic root & aortic overriding. His cardiac catheterization data and cardiac angiogram shows situs inversus totalis, dextrocardia, right aortic arch, large ventricular septal defect etc., and finally diagnosed Truncus Arteriosus. Edwards type IV with retrograde aortogram and selective bronchial angiogram. This is the first operative case reported as Rastelli operation for Truncus Arteriosus type IV in the literatures in Korea. Authors have experienced I case of Truncus Arteriosus, Edward type IV and Rastelli operation with Dacron Arterial Conduit Graft under cardiopulmonary bypass on July 3, 1978. The procedures were as follows; 2] Cardiopulmonary bypass: Origin of bronchial arteries excised from descending aorta bilaterally; defects in aorta closed. 2] Horizontal incision made high in right ventricle. 2] Ventricular septal defect [Kirklin type I+II] closed with Teflon patch. 4] Bifurcated dacron arterial graft with pericardial monocusp sutured to the bilateral pulmonary arteries. [Diameter 9 mm: Length 7 cm]. 5] Proximal end of the conduit graft anastomosed to right ventricle. [Diameter 19 mm: Length 5 cm]..Total perfusion time was 220 min. The result of operation was poor due to anastomotic leakage and increased pulmonary vascular resistance resulting acute right heart failure. The patient was died on the operation table. Literatures were briefly reviewed.

Development on Native Local Food Contents in Damyang through Ancient Writing Storytelling (담양관련 음식고문헌을 통한 장수음식 콘텐츠 개발)

  • The purpose of this study was the development of traditional food recipes from local food contents related to the historical and cultural heritage in the representative long-lived area Damyang. The information on digging 'story' was collected through the region's representative local native journal, searching for related websites, analyzing old literatures, and interviewing traditional household; 'telling', the part of conveyance, was organized by reviewing the literatures, including historical condition, regional food materials, cuisine, region symbolism, etc. After nutritional analysis, the prepared, balanced, and healthy Korean food dishes using the traditional food contents were displayed. In Damyang, the story was extracted from Heuichoon Ryu (柳希春, 1543-1577) - figura of Damyang - and his collection, 'Miam's diary', and described about native food. The traditional history of many foods that were used mainly as rewarding during one's trouble, showing good faith, and delivering gratitude is recorded in Miam diary. A little effort has been made to express the classical scholar's clean and neat dining table which was the mixed Damyang's clean image and nobleman's straight integrity in the middle of the Choson Dynasty. Nutrition fact of 'Miam's table' was based on the Daily Reference Values of Koreans aged 65 or older.

  • Anaplastic thyroid carcinoma, which is one of the most aggressive tumors of the thyroid, has been seldomly reposted to have altered thyroid function. There have been few reports of patients with anaplastic thyroid cancer presenting in a hyperthyroid state. In the literatures, the mechanism of hyperthyroidism in anaplastic thyroid cancer is supposed that the rapid invasive growth of cancer seems to cause destruction of thyroid tissue and develops a hyperthyroid state, which is thought to be anlalogous to that of subacute thyroiditis and several types of metastatic cancer of the thyroid: rapid tissue necrosis with resultant release of thyroid hormone. Recently, we experienced a case of anaplastic thyroid cancer presenting with rapid growing mass and hyperthyroidism in a 67-year-old woman and report it with the review of the literatures.

  • The Objective of this article is to make a bibliographical introduction and to discuss the historical value of several literatures on the Sasang constitutional medicine established by Jema Lee$(1837{\sim}1900)$. Literatures are as follows: 1. ${\ulcorner}$HamsanSachon Donguisusebowon(咸山沙忖 東醫壽世保元 甲午舊本)${\lrcorner}$; newly found draft of ${\ulcorner}$Donguisusebowon${\lrcorner}$ that contains an old edition written in 1894. 2. ${\ulcorner}$Bojeyeonseol-Supplement of Donguisusebowon(普濟演設-東醫壽世保元補編)${\lrcorner}$; a clinical book which is regarded as to had been published before the first edition of ${\ulcorner}$Donguisusebowon${\lrcorner}$ was printed. 3. ${\ulcorner}$Gyukchigo appendix(格致藁-附錄)${\lrcorner}$, ${\ulcorner}$Sasang Medicine-A Memorandom of the first edition(四象醫學草本卷備忘錄)${\lrcorner}$; which were aquired in Yanbian(China) several years ago. 4. ${\ulcorner}$Donguisusebowon${\lrcorner}$- The first edition published in 1901; owned by Mikki Sakae(三木榮) a Japanese medical historian. 5. ${\ulcorner}$Donguisusebowon${\lrcorner}$ - Manuscript; poetry on natures of herbal drugs are appended.

Epiphrenic Diverticulum of the Esophagus (횡격막직상부 식도계실 3례)

  • The first case was a 20 year old female who has been suffered from epigastric pain, and anorexia for 2 years. A thumb tip sized pulsion diverticulum 4cm above the esophagocardial junction was elicited by esophagogram and on exploration. A diverticulectomy with long esophagocardiomyotomy was performed. The second case was a 30 year old house wife who has had postprandial epigastric pain for 2 months accompained with frequent vomiting. Fiberscopy and esophagogram showed epiphrenic diverticulum of the esophagus. Same operative procedures were carried out and obtained a good result as first case. The third case was a 55 year old house wife who was admitted to this Chest Surgery Department because of regurgitation and intermittent vomiting for approximately 3 months. Esophagogram showed a large epiphrenic diverticulum of the esophagus. On exploration, a tennis ball sized pulsion diverticulum was found on the anterolateral wall of the esophagus. A partial esophagectomy including the diverticulum and esophagoesophagostomy was performed. The specimenshowed some erosive changes of the mucosal surface of the diverticulum and also the esophagus suggestive of diverticulitis and esophagitis. She has been satisfactory result until 4 months postoperatively, when she developed regurgitation and epigastric pain. Esophagogram showed stenosis of the operative site. Readmission and esophageal dilatations were done and improved without any problem. Epiphrenic or supradiaphragmatic diverticulum of the esophagus is a rare condition. Pathophysiologically, the conditions accompanied the spasm of the esophagus, many authors prefered the procedures of a diverticulectomy plus long esophagocardiomyotomy rather than simple diverticulectomy or esophagectomy and esophagoesophagostomy. Here we report the cases and reviewed the literatures.
