Surgical Repair of Bochdalek Hernia in Adult; A Report of Two Cases (성인에서 발생한 Bochdalek 탈장;수술 치험 2례)

  • Congenital diaphragmatic hernia through Bochdalek foramen, posterolateral diaphragmatic hernia, is the result of a congenital malformation of the posterolateral region of the diaphragm. Bochdalek hernia is the most common type of congenital diaphragmatic hernia, but it is a extremely rare anomaly which is detected in adulthood because it has severe cardiopulmonary and gastrointestinal symptoms after birth immediately.We experienced two cases of Bochdalek hernia which were detected in adulthood. Among them, one was a 43 years old man who was treated with simple closure in left Bochdalek hernia and the other was 41 years old woman who was treated with patch closure using silastic sheet in right Bochdalek hernia. Their postoperative courses were uneventiful. So we report two cases of rare Bochdalek hernia which was detected in adulthood with review of literatures.

  • Dermatomyositis is a rare and idiopathic inflammatory myopathy with a characteristic cutaneous manifestation. A 62-year-old female complained of polyarthralgia that lasted for many years. She was diagnosed with hypomyopathic dermatomyositis by the typical skin rash associated with dermatomyositis but without muscle involvement such as muscle weakness, elevated level of creatinine phosphokinase and aldolase. Her symptoms improved with treatment of hydroxychloroquine and prednisolone. We experienced a case of hypomyopathic dermatomyositis on 62-year-old female patient and report with review of literatures.

  • Whereas oral ranula is relatively common and presents as a cyst in the mouth, the plunging ranula is rare and manifests itself as a mass in the neck with or without an associated oral lesion. When there is a soft anterior neck swelling without oral swelling, diagnosis is still difficult. In such a case, clinical suspicion may be low and the pathologist may be misled by the histologic appearance. Recognition of the diagnosis of plunging ranula is essential for the correct treatment of these lesions. We report a case of a 4-year-old girl and review the literatures to discuss the differential diagnosis and treatment modalities.

A Review of Postmortem Investigation of Joseon Dynasty in the Aspect of Recent Forensic Medicine (조선시대 검시의 과학성)

  • In the era of Joseon Dynasty there was a national system of postmortem investigation with literatures of postmortem inspection. The author reviewed the postmortem findings in the literature(Shinjumoowonrok) to compare with recent knowledges of forensic medicine. Characteristics of old system of postmortem investigation was three instance investigation system and was carried out only with a postmortem inspection without autopsy. The postmortem findings described in the old literature were about various kinds of death, such as cases of suicide and homicide and natural and unnatural death. Most of postmortem findings in the Shinjumoowonrok were accurate and reasonable in the aspect of recent knowledges of forensic medicine.

  • Extrapelvic endometriosis is a rare disease. The majority of extrapelvic endometriosis cases involve scar tissue following obstetric and gynecologic procedures. We have treated two cases of extrapelvic incisional endometriosis. A 39 year old female patient with cyclic vaginal spotting after laparoscopic assisted vaginal hysterectomy due to uterine myoma and a 35 year old female patient with a painful palpable abdominal mass after cesarean section. Both underwent complete excision and were proven to have endometriosis by pathology. Here we report on both cases and review the medical literatures.

  • Ameloblastic fibro-odontoma(AFO) is a rare mixed odontogenic tumor. It is composed of connective tissue characteristic of an ameloblastic fibroma and calcified tissue as a complex or compound odontoma. AFO usually presents itself as an asymptomatic swelling of jaw or failure of tooth eruption. The lesion usually occurs in individual less than 30 years old. The differential diagnosis of this tumor includes odontoma, ameloblastoma, and ameloblastic fibroma. This report describes an ameloblastic fibro-odontoma occurring in maxilla of sixteen-year-old female. The lesion was treated by surgical enucleation and curettage without extraction of the involved canine(#23). This patient has shown no sign of recurrence during postoperative 34 months. So we report our case with review of literatures

  • The literature review for fall in the elderly has been done for the better quality of life of increasing elderly people toward 21 st century. Because 30 to 50% of over sixty five years old persons have experiences of fall, five percent of the fallen have trauma such as bone fracture requiring hospitalization and three quarter of people who die as fall are over 65 year old, fall is important health problem of them. There are very little societal interest in and research related to fall. Therefore, among recent foreign and our literatures studying literature review of frequency of fall, risk factors and assessing method for tall, and the management of fall prevention program, I would like to find research direction. Conclusivelly, we should study extensively the survey of the elderly's fall and on the basis of it. developing fall prevention program, promote the elderly's health through fall prevention.

  • It is known that the Korea has a long history in map production. On the contrary, a few literatures and materials of the ancient maps are available by the time being and there is something yet to study more. Therefore, it is not often discover the publishing date, technical standard of the ancient maps, and etc. The objective of this study is to examine the history of the old maps and is also to show the guideline of the study in future.

  • The calcifying odontogenic cyst is rare lesion having features of both cyst and neoplasm. Clinically it frequently occurs in young adult and more frequent in the anterior area of the jaw. Radiographically, this lesion has several radiopaque foci within unilocular or multilocular radiolucency. The authors experienced cystic and neoplastic types of calcifying odontogenic cysts in the anterior area of the mandible in a 16-year-old male and 21-year-old female patients who suffered from pain, swelling and teeth displacemeent on the affected area. And we discussed the clinical, radiological and histopathological features with a brief review of the literatures.

  • The ameloblastic fibrosarcoma is the malignant counterpart of the ameloblastic fibrosarcoma in which the mesenchymal element has become malignant. Clinically it frequently occurs in the 3rd and 4th decades, and more frequent in the mandible than in the maxilla. Radiographic features are apparent multilocular radiolucency with ill-defined border. The authors experienced two cases of ameloblastic fibrosarcoma of the mandible in a 26-year-old male and a 48-year-old female patients who suffered from pain and swelling on the affected area. And we discussed the clinical, radiological and histopathological features of this disease with a brief review of the literatures.
