A Case of Thyroid Metastasis from Nasopharyngeal Carcinoma (갑상선으로 전이된 비인강암 1예)

  • Metastatic carcinomas to the thyroid gland are rare, and thyroid involvement by secondary carcinomas commonly results from direct the extension of malignant cells from adjacent organs such as the larynx or the trachea. The common primary sites of thyroid metastasis are kidney, breast, lung, and lymphoid tissue. Among head and neck cancers, nasopharyngeal carcinoma has a relatively high incidence of distant metastases to other sites and commonly involving sites are bone, lung, and liver. Recently, we experienced a case of a 43-year-old male who had been presented with neck mass for 3 months. He was diagnosed non-keratinizing nasopharyngeal carcinoma in 1993. And, thyroid metastasis of nasopharyngeal carcinoma was confirmed by total thyroidectomy. So we report this rare case with the review of literatures.

A Case of Hutch's Diverticulum Associated with Severe Bilateral Vesicoureteral Reflux (심한 방광 요관 역류를 동반한 양측성 Hutch's Diverticulum 1례)

  • A bladder diverticulum occurs when the bladder mucosa herniates or protrudes through the muscular wall of the baldder. The majority of congenital bladder diverticula occurs in males. They are the most common in the region of the bladder base, most frequently in the region of the ureteral hiatus, in which case they are known as Hutch's diverticula. They can give rise to obstruction or reflux. We had experienced a case of bilateral Hutch's diverticulum associated with vesicoureterai reflux in a 23 month old male. Chief complaints were urinary frequency and dysuria. Voiding cystourogram and CT scan revealed large bilateral Hutch's diverticulum with bilateral vesiciureteral reflux grade VI. There was evidence of urinary infection. This patient was successfully treated by ureteroneocystostomy. We report this case with a brief review of related literatures.

A Study on Formation of Sleeves - Centering around daydress in England - (소매의 조형성에 관한 연구 -영국의 daydress를 중심으로-)

  • This study was conducted to consider the formation of sleeves from the point of view that dress is one of expressive arts. The formative meaning of a sleeve and its chronological formation according to its formative changes as an evidence showing the Internal changes of social culture were examined, centering around day dress of England in the 11th to 19th centuries. Then, sleeves were formatively classified and their aesthetic values were considered to help do formative, aesthetic expression of dress design in the present times. Literatures and references concerning the history of domestic and foreign dress were used as a method of study. The results of this study are summarized as follows. 1. In the women's dress, the formation of the sleeve focuses on aesthetic decoration. This means that in the androgentric society the status of women are subordinate socioculturally. 2. Sleeves were classified into a fitted shape and a extension shape according to the aesthetic formation. The fluted shape expressed the beauty of body with emphasis old natural body, whereas the extension shape represented aesthetic expression of decoration. 3. The formation of a sleeve has influence on forming an image of dress by making the silhouette of dress natural or distorted, expresses the harmonious beauty of the whole dress, and produces the expressional beauty that gives a distinct characteristic to a dress.

Tessier No. 2 Oblique Facial Cleft Not Associated with Cleft Lip or Palate: a Case Report (구순구개열과 관련되지 않은 Tessier 분류 2 안면열의 교정: 증례보고)

  • Oblique facial cleft is extremely rare. The frequency was reported 1/1300 cases of facial cleft. The cleft appears to be bilateral in approximately 20% and more often on the right when unilateral. Oblique facial cleft is nearly always associated with cleft lip and palate. Thus, the case that is unilateral on the left and not associated with cleft lip or palate is very rare. We experienced a case of 2 years 6 months old Philippine girl who had oblique facial cleft that is not associated with cleft lip or palate. The probable cause and treatment is discussed with a review of literatures.

  • Angiolymphoid hyperplasia with eosinophilia (ALHE) is a rare benign vascular lesion that is characterized by proliferation of small to medium-sized vascular structures lined by histiocytoid or epithelioid endothelial cells and often accompanied by an inflammatory infiltrate comprising lymphocytes, plasma cells and eosinophils. A 34-year-old man without any generalized systemic conditions presented with a slowly enlarging painless swelling in the buccal area. An excisional biopsy was conducted and the diagnosis of Angiolymphoid hyperplasia with eosinophilia was confirmed. We report a case of angiolymphoid hyperplasia with eosinophilia of the buccal area in oral cavity and review the previously reported cases and literatures of angiolymphoid hyperplasia with eosinophilia.

A Phytogeographical Study on the Distribution of Bamboos in the Korean Peninsula (韓半島의 대나무類 分布와 그 環境要因에 관한 植物地理學的 硏究)

  • Correlations between horizontal distributions of bamboos (Bambusaceae) in the Korean peninsula and environmental factors were studied using taxanomic and geographical literatures, both old and current. The vertical distributions of bamboos on Mt. Chiri were also studied, and environmental factors limiting horizontal and vertical distributions were compared. There are 18 species of bamboos (belonging to 5 genera) distributed in the Korean peninsula. The distributional range of each genus were distinct, although overlapped. Northern limit of bamboos of any species was marked by the line connecting Paikryung Island (124。40'E, 38。00'N), Mt. Changsoo, Mt. Myungji, Mt. Myohyung and Myungchum (129。40'E, 41。10'N). The optimum range of bamboos was concluded to be restricted to several southern province, with annual precipitation over 1,200 mm. The limiting factors on the distribution were inferred to be low temperature and duration of it. Mean daily minimum temperature of January and the number of days with daily mean temperatures below zero during January showed close associations with the distributional range, and an environmental factors favouring the distributrion of bamboos appeared to be vicinity of warm sea current, deep and extended snow acculation and southern exposure. The vertical distribution of bamboos on Mt. Chiri was limited by low temperature, unfavorable topographic and edaphic conditions caused by steep slope. Difference in the vertical limits between SE and NW slopes are caused by the differences in temperature and precipitation between the slopes. Bamboos were more abundant in valleys than on the ridge, apparently because the deeper snow in the valleys protected the plants from low temperature, heavy winter winds and desiccation.

A Case of Pedunculated Schwannoma of Aryepiglottic fold (피열후두개주름에 발생한 유경성 신경초종 1예)

  • Schwannoma, also known as a neurilemmomas or neurinomas, is benign peripheral nerve sheath tumors arising from the Schwann cells. About 25~45 % of all schwannomas are found in the head and neck region, and the majority are found in the parapharyngeal space. Schwannoma of the larynx is very rare with an incidence of 0.1~1.5% in all benign laryngeal tumors. Recently, we experienced a case of a pedunculated schwannoma arising from the left aryepiglottic fold in a 80-year-old patient. The tumor was completely excised under direct laryngoscopy with the use of a $CO_2$ laser and subsequently confirmed as schwannoma. So, we report this case with a review of the literatures.

A Case of Severe Serotonin Syndrome Induced by Fluoxetine and Sertraline (Fluoxetine 및 Sertraline으로 유도된 심한 세로토닌 증후군 1예)

  • A 54-year old male patient who was suffering from bipolar I disorder for 19 years and was admitted to the National Bugok Mental Hospital due to a depressive episode, was referred to the Kosin University Gospel Hospital. On arrival at the emergency room, he had confused mentality with disorientation, memory impairment, hypomania, marked anxiety and hyperirritability. The change of neuromuscular activity such as ataxia, gait disturbance, tremor, shivering, myoclonus and epileptic seizures was also shown. In addition, the symptoms and signs of autonomic instability including diaphoresis, tachycardia, hypotension, fever and facial flushing were noticed. The above symptoms developed after the administration of sertraline successive to the discontinuation of fluoxetine without any washout period. The degree of severity seemed to be severe because he had epileptic seizures, fever and hypotension. He was recovered from the severe serotonin syndrome by the supportive symptomatic treatment with sodium valproate, clonazepam, lorazepam and cyproheptadine after cessation of the selective serotonin reuptake inhibitors during hospitalization. Therefore, this rare case of severe serotonin syndrome was reported and related literatures were also reviewed.

  • Granular cell tumor is an uncommon lesion that was first described by Abrikossoff in 1926. It is usually benign tumor that can occur in any parts of the body. The most common region of granular cell tumor is the head and neck, accounting for approximately 30 to 50 percent of all lesions, with the tongue as the single most common site of origin. The larynx is uncommon location, accounting for approximately 3 to 10 percent of the reported case. Herein we report a case of a 41-year-old man with laryngeal granular cell tumor who was successfully treated, especially showing well improvements in his voice after the operation. In addition, a brief discussion of the current literatures regarding the typical features of the tumor are also presented.

  • Toxic shock syndrome(TSS) is a multisystemic disease presenting with high fever, sunburn like rash that subsequently desquamates, and hypotension mainly caused by toxin producing strains of Staphylococcus aureus. It was first reported in 1978 by Todd et al, thereafter many patients have been reported. In children, TSS is rare and must be differentiated from other erythematous febrile diseases such as Kawasaki disease, scarlet fever, drug eruption etc. We experienced a case of TSS associated with staphylococcal cellulitis in 26-month old boy, who was presenting similar symptoms to Kawasaki disease at initial stage of illness. As time passed, the patient represented more typical symptoms of TSS and Staphylococcus aureus was isolated from cellulitis of the right elbow area. Therefore, we report this case with a brief review of related literatures.
