고혈압의 위험요인에 대한 환자-대조군 연구 (A Case-Control Study for Risk Factor Related to Hypertension)

  • 고혈압과 관련된 위험요인을 조사하기 위하여 1908년 5월 1일부터 11월 30일까지 대추시내 일반근로자들 가운데 고혈압환자 330명(남자 247명, 여자 83명)과 정상인 1,336명(남자 887명, 여자 449명)을 대상으로 환자-대조군 연구를 실시하였다 성, 연령을 제한요인으로 하여 조사한 위험요인들은 근무부서, 비만도, 음주력, 흡연력, 우유섭취빈도, 식염섭취정도, 고혈압가족력이었다. 다변량분석기법가운데 지수형회귀분석 (logistic regression analysis)에 의해 분석한 결과 남자에서는 음주(odds ratio=3.23), 비만도(odds ratio=2.31), 그리고 식염(odds ratio=1.75)이 고혈압의 위험을 증가시키는데 비해 여자에서는 음주(odds ratio=16.49), 고혈압가족력 (odds ratio=3.70). 비만도(odds ratio=1.74), 그리고 식염(odds ratio=1.73)이 고혈압의 위험을 증가시켰다(p<0.05). 그리고 남녀 모두에서 우유가 고혈압의 위험을 감소시키는 유의한 변수이며(남자 odds ratio=0.69, 여자 odds ratio=0.65) 용량-반응 관계가 있음으로 보아 (p<0.05) 고혈압의 예방요인으로 생각된다. 남녀 모두에서 흡연은 고혈압과 관련이 없었다. 그런데 단순분석결과 여자에서 근무부서가 고협압과 유의한 관련이 있었는데 다변량분석에서는 유의한 관련성이 없었다.

청소년의 가출충동에 영향을 미치는 예측요인 (Predicting Factors on Youth Runaway Impulse)

  • This study is attempted to define risk factor of youth runaway impulse and to structure forecast model through an extensive analysis of the factors influencing the runaway impulse of youth. The subjects were 610 high school students in Seoul and Kyunggido. The collected data was analysed by SAS. The differences between the runaway impulse group and the non-runaway impulse group were subject to chi-square and t-test. Also logistic regression analysis was conducted on the basis of purposeful selection method for constructing the forecast model. The findings are as follows : the major predicting factors of youth runaway impulse are sex(odds ratio=1.886, p=.009), existence of friends of the opposit sex(odds ratio=2.011, p=.007), anti-social personality(odds ratio= 4.953, p=.000), depressive trend(odds ratio= 2.695, p=.000), family structure(odds ratio= 5.381, p=.000), marital relationship(odds ratio =1.893, p=.009) and also between parents and youth(odds ratio=3.877, p=.000), emotional abuse(odds ratio=1.963, p=.003), authoritative controlled rearing(odds ratio=2.135, p=.005) and stress from school(odds ratio=1.924, p=.008). Therefore, the forecast model will be contribute to the nursing intervention for prevention of runaway youth.

뇌졸중에 영향을 미치는 생활습관 요인 -흡연, 음주, 비만, 식습관을 중심으로- (The Lifestyle Factors in Stroke Etiology: Smoking, Alcohol Consumption, Obesity, Perception of Saltness)

  • Stroke is a serious disease despite recent improvement in medical and surgical treatment. Hence, identification of modifiable risk factors for stroke is important. This case-control study was done to demonstrate that relationship between smoking, alcohol consumption, obesity, perception of saltness and the incidence of stroke and to identify that smoking, alcohol consumption, obesity and perception of saltness, after adjusting for age, hypertension. A structured interview was carried out from April 15, 1996 to May 3, 1996 in Yonsei Medical Center. The study group consisted of 59 neurologically confirmed stroke patients as the study group and 59 non-stroke patients as controls. Analysis of the data was done by means of ${\chi}^2$-test and logistic regression analysis. The results were as follows. In the study group: 1) Hypertension in males had a 10.2 odds ratio (p<0.05), cardiovascular disease in females had a 11.3 odds ratio (p<0.05) and a farnily history of stroke in males had a 3.1 odds ratio (p<0.05). 2) Females smoking one or more cigarettes had a 8.3 odds ratio (p<0.1), but males had no direct relationship with odds ratio of 1.5 (non-significant). 3) Alcohol consumption in males had a 0.4 odds ratio, and in females had a 0.8 odds ratio. The odds ratio was decreased in alcohol consumption group (non-significant). 4) Males with more than 20 cigarettes pack-years history had a 2.5 odds ratio (p<0.05), more than 25 Body Mass Index had a 3.1 odds ratio (p<0.05) and more than 220 ml ethanol weekly consumption had a 1.5 odds ratio (non-significant). 5) Female smokers had a 8.3 odds ratio (p<0.1), drinkers a 0.8 odds ratio and more than 25 Body Mass Iidex, a 43.1 odds ratio (p<0.05). 6) Females without saltness perception from a 0.5% salt solution had a 6.8 odds ratio (non-significant). 7) By logistic regression analysis independent risk factors for stroke in males were found to be hypertention, age, and obesity. The study was limited because number of subjects was too small for practical implications. However, like as other results, this study suggest that people should be advised to control hypertension, and obesity since these carry a risk of stroke.

도시.농촌 지역 초등학생의 가족환경, 건강행위 및 건강상태에 관한 비교 (Comparision of Family Environment, Health Behavior and Health State of Elementary Students in Urban and Rural Areas)

  • This research intends to survey family environment, health behavior and health status of the students in urban-rural elementary schools and analyze those factors comparatively, and use the result as basic material for school health teacher to teach health education in connection with family and regional areas. It also intends to improve a pupil's self-abilitiy in health care. The subjects involve 2,774 students of urban elementary schools and 583 student in rural ones, who were selected by means of a multi -stage probability sampling. Using the questionnaire and school documents, we collected data on family environment, health behavior and health status for 19 days. Feb. 2nd 1998 through Feb. 20th 1998. The R -form of Family Environment Scale (Moos, 1974) was used in the analysis of family environment(Cronbach's Alpha =0.80). Questionnaires of Health Behavior in School-aged children used by the WHO in Europe(Aaro et al., 1986) and the ones developed by the Health Promotion Committee of the Western Pacific(WHO, 1995)(adapted by long Young-suk and Moon Young-hee(1996)) were used in the analysis of health behavior, as well documents on absences due to sickness, school health room-visits, levels of physical strength, height, weight and degree of obesity were used to determine health status. In next step, We used them with an $X^2$-test, t-test, Odds Ratio, and a 95% Confidence Interval. 1. In two dimensions of three, family-relationship (t=3.41, p=0.001) and system -maintenances(t= 2.41, p=0.0l6) the mean score of urban children were significantly higher than those of rural ones. In the personal development dimension however, there was little significant difference. Assorting family environment into 10 sub-fields and analyzing them, we recognized that urban children were superior to rural children in the sub-fields of expressiveness (t =3.47, p=0.001), conflict (t=0.48, p=0.001), active-recreational orientation (t = 1.97, p=0.049) and organization (t=4.33, p=0.000). 2. Referring to the Odds Ratios of urban-rural children's health behaviors, urban children set up more desirable behavior than rural children wear ing safety belts (Odds Ratio =0.32, p=0.000), washing hands after meals(Odds Ratio = 0.43, p= 0.000), washing hands after excreting (Odds Ratio = 0.39, p=O.OOO), washing hands after coming - home ( Odds Ratio = 0.75, p = 0.003), brushing teeth before sleeping(Odds Ratio =0.45, p=0.000), brushing teeth more than once a day (Odds Ratio =0.73, p=0.0l2), drinking boiled water (Odds Ratio = 0.49, p=0.000), collecting garbage at home(Odds Ratio=0.31, p=0.000) and in the school(Odds Ratio =0. 67, p=0.000). All these led to significant differences. As to taking milk(Odds Ratio = 1.50, p=0.000), taking care of eyesight(Odds Ratio=1.41, p=0.001) and getting physical exercise in(Odds Ratio = 1.33, p=0.0l9) and outside the school(Odds Ratio = 1.32, p=0.005), rural children had more desirable behavior which also revealed a significant difference. There was little significant difference in smoking, but the smoking rate of rural children(5.5%) was larger than that of urban children(3.9%). 3. Health status was analyzed in terms of absences, school health room-visits, levels of physical strength, and the degree of obesity, height and weight. Considering Odds Ratios of the health status of urban-rural children, the health status of rural children was significantly better than that of the urban ones in the level of physical strength(t=1.51, p=0.000) and the degree of obesity(t=1.84, p=0.000). The mean height of urban children ($150.4{\pm}7.5cm$) is taller than that of their counterparts($149.5{\pm}7.9$), which revealed a significant difference (t =2.47, p=0.0l4). The mean weight of urban children($42.9{\pm}8.6kg$) is larger than that of their counterparts($41.8{\pm}9.0kg$), which was also a significant difference(t=2.81, p=0.005). Considering the results above, we can recognize that there are significant differences in family environment, health behavior, and health status in urban-rural children. These results also suggestion ideas for health education. What we would suggest for the health program of elementary schools is that school health teachers should play an active role in promoting the need and importance of health education, develop the appropriate programs which correspond to the regional characteristics, and incorporate them into schools to improve children's ability to manage their own health management.

국민학교아동의 학습부진에 관련된 요인 (Factors Related to Poor School Performance of Elementary School Children)

  • 국민학생들의 학습부진에 관련된 요인을 조사하여 의학적 예방 및 치료 대책을 고안해 보기 위해, 대구 시내에서 고소득층 밀집지역과 저소득층 밀집지역의 국민학교 1개씩을 선정하고, 각 학교의 2, 4, 6학년 학생 중 학업 성적이 하위 10%에 속하는 학생(학습부진군) 175명과 이에 대한 대조군으로 성적이 상위 5%에 속하는 학생(우수군) 97명을 대상으로 하여 이들에게 시력검사, 청력검사, 신체검사(체중, 신장, 두위)를 한 후, 고대 Stanford-Binet test를 이용한 지능검사와 TAI-K로 시험불안반응검사를 하고 발달소아학 전문의사가 신경학적 검진을 하였다. 아동의 어머니에게는 가정 환경, 어머니의 출산력, 아동의 출생 전후 및 과거력에서 의학적 특이사항, 그리고 행동양상에 관해 설문조사하고, 담임 선생에게 설문지를 통해 학교에서의 행동양상을 조사하였다. 아동의 두발을 적당량 채취하여 두발중 납, 카드뮴, 아연의 함량을 원자흡광분광광도계로 분석하였다. 학습부진과 각 요인과의 단순상관 관계분석에서 통계적으로 유의한 요인들을 이용한 다중지수형 회귀분석 (multiple logistic regression analysis)을 하여 학습부진의 관련요인을 찾았다. 단순분석에서 학습부진과 유의한 관련성이 있는 것은 남아인 경우, 출산순위가 높은 경우, 결손가정인 경우, 부모의 학력이 낮거나 직업이 노동직인 경우, 아동의 체중, 신장 및 두위가 작은 경우, 시력장애가 있는 경우, 지능지수가 낮은 경우, 시험불안반응점수가 높은 경우, 과잉활동성 아동인 경우로 나타났다. 다중지수형 회귀분석결과 학습부진과 관련되는 요인은 아동의 높은 출산순위 (odds ratio=2.06), 남아인 경우(odds ratio=5.91), 결손가정(odds ratio=9.29), 신장이 표준치-1 표준편차보다 작은 경우(odds ratio=11.12), 높은 시험불안점수(odds ratio=1.07), 과잉활동성 장애아동(odds ratio=9.67), 그리고 지능지수(odds ratio=0.85)였다. 두발중 중금속 함량의 분석결과 모두 학습부진과 뚜렷한 관련성이 없었으며, 납과 카드뮴의 함량은 학습부진군과 우수군 모두 남아가 여아에 비해 높았으며, 남은 학년과 역상관 관계(p<0.05)를, 아연은 정상관 관계(p<0.05)를 나타내었다. 학습부진과 유의한 관련성이 있는 요인들 중 조정가능한 것은 출산순위, 신장, 과잉활동성 장애 등으로, 가족계획을 통해 알맞은 수의 자녀를 갖도록 하고, 영유아기에서부터 사춘기에 이르기까지 적절한 영양공급을 하며, 과잉활동성 아동을 조기에 발견하여 의사의 진단을 받아 치료를 받도록 하는 것이 학습부진을 예방 및 해결하는데 도움이 될 것이다. 또한 부모와 담임 선생이 학습부진아에게 본인의 지적능력 이상의 기대나 학습의 강요로 불안을 조장하지 않도록 해야 할 것이다.

An Introduction to Logistic Regression: From Basic Concepts to Interpretation with Particular Attention to Nursing Domain

  • Purpose: The purpose of this article is twofold: 1) introducing logistic regression (LR), a multivariable method for modeling the relationship between multiple independent variables and a categorical dependent variable, and 2) examining use and reporting of LR in the nursing literature. Methods: Text books on LR and research articles employing LR as main statistical analysis were reviewed. Twenty-three articles published between 2010 and 2011 in the Journal of Korean Academy of Nursing were analyzed for proper use and reporting of LR models. Results: Logistic regression from basic concepts such as odds, odds ratio, logit transformation and logistic curve, assumption, fitting, reporting and interpreting to cautions were presented. Substantial shortcomings were found in both use of LR and reporting of results. For many studies, sample size was not sufficiently large to call into question the accuracy of the regression model. Additionally, only one study reported validation analysis. Conclusion: Nursing researchers need to pay greater attention to guidelines concerning the use and reporting of LR models.

Odds Ratio and Probability of Conception of Holstein Friesian Dairy Cows in the Kingdom of Saudi Arabia

  • Logistic Regression Analysis was used to compute the odds ratio (OR) and probability of conception of Holstein dairy cows of AL-MARAIE company. Data consisted of 103,778 reproductive records collected from three farms in the central region in the Kingdom of Saudi Arabia. Records were classified according to lactation number, season of calving and milk level. At first lactation, OR of first service was 0.63 of other services and probability of pregnancy from first services was 0.39. Odds ratio increased to 1.72 at fourth lactation or probability of conception reached 0.63. The probability of conception increased from 0.39 for cows inseminated at first services to 0.75 at fifth service insemination. Odds ratio of cows calving in winter were higher than those calving in summer. Odds ratio of conception of low producing cows is about twice as likely to occur from first service as from other services. However, OR of conception of high producing cows was higher than one (probability=0.56) from first service and increased to 1.63 (probability=0.65) from third service. Odds ratio was in favor of the right uterus horn where probability of conception from first service was slightly greater than 50% in first and second lactations and less than 50% in favor of left horn in later lactations.

40세 이상 중장년의 대사증후군과 백내장과의 관련성 -제5기 국민건강영양조사 자료중심으로- (Relationship between Metabolic Syndrome and Cataract in a Middle-Aged Men over the Age of 40's -Based on the 5th Korean National Health and Nutrition Examination Survey-)

  • Objectives: This study aimed to analyze the relationship between metabolic syndrome and cataract in a middle-aged men over the age of 40's by using the data from the 5th Korean National Health and Nutrition Examination Survey(2010~2012). Methods: The subjects for the analysis were comprised of 5,808 patients who were at least 40 years or more with metabolic syndrome and cataract. The comparison between the metabolic syndrome and ophthalmic diseases according to the health demographic characteristics, health behavior and health conditions was analyzed with ${\chi}^2-test$. The complex samples logistic regression analysis was used to calculate the odds ratio of cataract according to the health demographic characteristics, health behavior and health conditions and the combination of the comprising factors of the metabolic syndrome. Results: After controlling the confounding factors, the odds ratio of cataract in the metabolic syndrome group was OR 1.30, which was significantly high (p<0.01). Furthermore, the odds ratio of cataract was significantly high in the case of males, as the age increased, educational level decreased, with exercising habits of less than 3 days a week, sun exposure of at least 5 hours, with hypertension and diabetes (p<0.01). The results of analyzing the odds ratio for the occurrence of cataract with the components of metabolic syndrome were OR 1.36 (concurrent high fasting blood glucose, high triglycerides, low HDL cholesterol), OR 1.63 (concurrent high fasting blood glucose, high triglycerides, high waist circumference) and OR 1.33 (high triglycerides, low HDL cholesterol, high waist circumference), which showed significantly high odds ratio for cataract occurrence (p<0.05). Conclusions: We come to know that the relation between metabolic syndrome and components are associate with cataract. Therefore the thorough management of metabolic syndrome and components is needed to prevent cataract.
