Occupational health services in Korea have been operated as dual types: one is operated by occupational health care manager and the other is health care agency without their own personnel. The performance of occupational health service should be different due to the variety of characteristics of health care manager and workplace, qualification of health care manager. This study is to analyze performance of occupational health care services with a particular consideration of job, based on comparing those two types of health care management to show on the basic data for the settlement of more qualitative. health care management system at workplace. For this study, total 391 places in Seoul and Inchon city area: 154 places (39.4%) managed by designated. health care manager and 237 places (60.6%) by the agency with their commission are selected as research samples. Tools for data collection are questionnares have been investigated during the period of 20 September 1993-20 December 1993. Those data are compared with percentiles, mean, standard deviation due to the characterstics of each variable and analyzed for impacting factors with relation to the using multiple regression analysis using SPSS PC program, especially using t -test method in order to compare each type of health care management. Conclusions observed from the tests and each comparison could be summerized as follows : 1. Occupational health care have been accomplished at workplaces with designated people than with agencies people, and coverage rate of the occupational health care services has differences, due to management types. The reason of these results is due to visit only one or two times monthly by the agencies, while their own health care manager obsess, at the workplaces all the times. 2. The common sickness management is the most accomplished item in health care area of occupational health care services, while the preventive care and control for the workers who have serious health problems are insufficient in workers health care area. 3. An insufficient accomplishment of overall health education has been shown because it is difficult to perform health education due to almost no chance of the direct introduction at workplaces. Therefore a strong support system for making and supplying the media is necessary in order to activate indirect health education by means of media. 4. Because health care managers and the agencies managers where take the workplaces for this study are almost nurses who have been comparatively high work site rounding rate about an environmental management at the workplaces, that non-profession can also do it, the activities about the professional area not enough. Therefore, an appropriate referral system should be established in order to complement professional area. 5. Two factors which have an effect on the coverage rate of occupational health care services are : one is those from the workplaces such as type of services, the number of workers, the number of harzadous factors and safety & health committee, the other from health care organization about whether there is its own manager or not.
Purpose: The purpose of this study is to analyze occupational accident compensation insurance coverage and occupational accidents incidence for special-type delivery workers. Methods: The data for occupational accident compensation insurance coverage and occupational accidents from 2012 to 2017 were analyzed through descriptive statistics. Results: Rates of occupational accident compensation insurance coverage of special-type delivery workers decreased gradually from 43.4% in 2012 to 28.5% in 2016, and 29.0% in 2017. Rates of occupational illnesses death per ten thousand workers increased gradually from 2.1‱ in 2013 to 3.1‱ in 2016, and 8.6‱ in 2017. All occupational illness deaths were due to cerebro-cardiovascular diseases. Road traffic accidents and slips accounted for the largest proportion of occupational accidents. Conclusion: Special-type delivery workers have a high risk of industrial accidents, so it is necessary to raise industrial accident insurance coverage and provide professional and systematic occupational safety and health services.
Purpose: The purpose of this study was to determine factors that affect satisfaction of the hospital services among injured workers. Methods: The subjects of this study were 532 injured workers. The questionnaires were selected as literature suggested for explaining satisfaction of the hospital services, general characteristics of injured workers, characteristics of the hospital services. The data were analyzed with descriptive statistics, t-test, ANOVA, multiple regression analysis by SPSS 19.0. Results: There were significant differences in satisfaction scores, depending on the types of disease, health status and possibility to return to work among the injured workers. Also, there were significant differences in satisfaction scores, in terms of medical facilities, food service, specialization of doctors and nurses, cleanliness of hospital, convenience of hospital teaching and counselling of doctors and nurses, supporting workers' compensation process, counselling of workers' compensation and unfair treatment. In the results of the standard multiple regression analysis, food service, medical facilities, specialization of doctors and nurses, health status and hospital cleanliness were significant factors for satisfaction scores of the hospital services. Conclusion: It is necessary to promote food service, medical facilities, specialization of doctors and nurses, hospital cleanliness, health status of injured workers for developing hospital services.
This study was designed to identify the performance of occupational health services of 37 industries located in Kyung in area. The data was collected by a structured questionnaire developed by the Academic society of Community Nursing. This analysis had two factors, one was related to six of the industries, and the other was the actual assignment procedures of the health team members. This study was undertaken from December 4, 1992 to January 21, 1993. The results of the study were as follows: 1. The study group was primarily manufacturing industries which employed 300-1,000 employees. 2. The actual assignment state of occupation health team members with the exception of nurses was not kept to the regulations of the industrial safety health law. 3. The following was the analysis of the performance of occupational health services with two factors: 1). The larger the size of the six industries, the more the performance of health education, health assessment, and health screening. 2) The actual. practice of occupational health team members working environmental measurement, was more frequently performed by a nurse other than health team members together. 4. 1) The subscription rate of the laborers for primary health screening was 94%, and of these 10% had the need of secondary health screening. As a results of the secondary health screening the degrees were 'A' 45%, 'C' 92%, 'R' 21%. Of these degrees 'C', 'R' 4% were follow up cases. 2) 43% of laborers needed special health screening and .of these the subscription rate was 99%. The main item of the special health screening was physical factors. After the special health screening 46% required .follow up, 30% required medical treatment, only reporting 18%, change work 8%, suspension from work place 2%.
Purpose: To provide the data of health manager education program in order to improve the quality of work-sites health management with qualification based job analysis of health managers (Occupational Health Nursing, Industrial Hygienist, Environmental Engineer). Methods: A descriptive research on 132 health managers using IPA and SPSS/WIN. Results: The overall average of importance of health management job was 8.0 (10 being the maximum score). Nurses had significantly higher score in the level of importance per areas and health management jobs. The overall average of performance of health management job was 6.7 (10 being the maximum score). Nurses had higher score in the area of health management. IPA matrix distributions per health management job area showed the correlations in qualification backgrounds and all of 3 main areas. Conclusion: There was difference in the level of importance and performance on health management jobs based on the qualification backgrounds of health managers. To improve the health of workers, an integrated health management must be provided. And to provide this, it is necessary to offer the additional education to health managers with an institutional complementary plan.
Purpose: The purpose of this study was to describe the role experiences of occupational health nurse. The research question was "what are daily experiences in practice?" Methods: The data were collected through the focus-group interviews with nine occupational health nurses and analyzed by the grounded theory of Strauss and Corbin (1998). Results: The core category on occupational health nurse role experience was named as "struggling alone with the dual role". The causal conditions included 'unfamiliar area' and 'new work'. The contextual conditions included 'overload of work, 'tasks beyond OHN job' and 'disharmony with colleagues'. The intervening conditions were analyzed as supportive factors and discouraging factors. There were action/interaction strategies 'getting information about tasks', 'working as a professional', 'establishing a cooperative relationship', 'seeking help' by trying to regulate the phenomenon. Finally they succeed in 'establishing the identity' and 'feeling proud', but sometimes they could be 'suffering from low morale', or 'considering turnover'. Conclusion: Through these results, it can be explained that occupational health nurses have been exerting themselves to establish their role with various situation through the conflicted context. Based on the findings, we suggest developing the diverse continuing education programs to be tailored for OHN's needs and reinforcing the occupational health services under the legal and political support.
This study was carried out to investigate the management and support system affecting to the occupational health nursing services(OHNS) provided in group occupational health agencies(GOHA). Questionnaire was developed and distributed to 82 nurses who were working in GOHA and who agreed to participate in the survey. The results were as follow: 1. OH nurses responded were mostly in the age of twenty to thirties(89%), married(73.7%), technical college graduates(88.9%), worked in hospital(85.4%) and participated more than 1 year in group occupational health services (96.3%). 2. Fifty eight point four percent of the OH nurses worked in number of workplace more than 30 to less than 60 in the OHNS form. The figure of workplaces undertaken by nurses was ranged greatly from 9 to more than 100. Number of employees who cared by nurses were mostly under 5,000 peoples in 93.3%. The types of industry was mostly manufacturing and located in the order of factory complex area, suburban, urban and others. 3. Most OH nurses(87.8%) were fully involved in the OHNS for the SSE. Their working days to visit SSE was 5 days per week(77.8%) and one day in the GOHA at 41.3%. 4. The OH documents using by nurses were found in more than 23 different types. However, they were largely summarized in the types of 'Workplace Health Management Card', 'Personal Health Counselling Card', 'Daily Health Management Report', 'Visiting List of Workplace' and 'Sick Employee List'. 5. The items of laboratory test provided by GOHA were mostly achieved in the purpose of basic health examination. They were used to be the blood pressure check(98.8%), blood sugar test (98.8%), urine sugar and protein(91.4%), SGOT and SGPT(85.3% each), cholesterol (82.9%), hepa vaccine immunization(82.9%), r-GPT(81.7%), hemoglobin(79.3%) and triglyceride(75.5%). 6. The OH nurses(92.7%) followed the work pattern to visit the GOHA before and after small-scale enterprises(SSE) visit by car driven by nurses in 74.3%. They were payed by GOHA for transportation fees in certain amounts. However, nurse is the main person(75.0%) who covers up in case of traffic accident. If the GOHA has no transportation regulation for the formal workplace visit, data showed that nurses had been responsible to take charge(31.7%). 7. The personnel manager who takes in charge for nursing services was 'nurse' in 61.7% and 41.2% worked as the final decision maker related to nursing work. The OH nurses' opinions about factors affecting to the management were classified in the four areas such as 'Nature(Quality) of health professional'. 'Content of OHNS', 'Delivery system of the GOHS', and 'Others'. The factors were indicated highly in 'Authority as health professional', 'Level of perception of director on the OH' and 'Physical work condition for OHNS'. The things that this study suggests in the recommendation would be summarized in such as the management and supporting system working for SSE in the OHNS is necessary to reform thoroughly. The reconsidered aspects might be in the matters of number of workplaces undertaken by nurses, development of effectively practical health documents, preparation for guideline of the laboratory test in the workpleces, establishment of convenient and encouraging support system and cooperation between other health professionals with respect and skill.
In Korea, based on the Revised Law of Occupational Safety and Health a new entity of institution was set up in 1990 to provide occupational health services to SSE in which three sorts of personnel as a team have to be involved. These institutions, in charge of scores to hundreds of workplace area-wide, have been providing occupational health services without payment from employers or employees, and government reimburses through the Occupational Injury Prevention Insurance since 1993. As a service provider, a team is composed of doctors, nurses and industrial hygienists. Undergraduate and postgraduate educations for the SSE occupational health are not specified and the question on the performance of the personnel has been raised. This study was designed to analyze the facilitating factors of and barriers to the performance and its improvement of these personnel. In 1997, the survey was conducted with all 58 institutions. Structured questionnaires were mailed to 200 personnel who were providing the occupational health service for SSE. The response rate was 51.7% for doctors, 58.6% for nurses, and 60.3% for industrial hygienist, respectively. Results are as follows : 1) There is a guideline for occupational service mandated by the government. Under the guideline, the minimum frequency of visiting workplace is assured with six times of doctors, 17 times for nurse and industrial hygienist in a year. There are one doctor for every 200 factories, one nurse and one hygienist for every 100 factories. 2) All respondents have basic qualification for occupational health service. About 16.7%. of doctors are certified in industrial medicine or preventive medicine, and 64.7% of industrial hygienists had first grade certification. Totally 66.7% of personnel have been involved in occupational health for more than one year. 3) As a support system for the performance improvement, 66.3% respond that they have been provided with educational materials, advice related to industrial environment and guidance of MSDS from Korea Industrial Safety Corporation. Most respondents indicate the lack of concern of employers and employees as a main barrier to the improvement of the service. Also they are in the need of the training opportunity more focused on SSE. The Governments policy for SSE is a principal facilitating factor. Training program focused on SSE situation, manpower, technical support, etc. are areas to be improved to have a better occupational health service for SSE in Korea.
Purpose: This study was a descriptive research to identify the effects of communication skills, compassion satisfaction, compassion fatigue on burnout among staff of long term care insurance for the elderly in National Health Insurance Services in Korea. Methods: A descriptive cross-sectional design was used. The participants were 191 staffs of long-term care insurance in National Health Insurance Services. Data were collected via mail from the 24 branch offices which were randomly selected among the total of 226 centers of National Health Insurance Corporation. Stamm's professional quality of life (ProQOL) and Communication Skills Test tool were included in questionnaire to detect. SPSS/WIN 20.0 was used to conduct the descriptive statistics, t-test, ANOVA, correlation analysis, and multiple regression. Results: Compassion fatigue had a significant positive effect on burnout, while Compassion satisfaction had a negative effect on burnout. The explained variance for burnout was 69% and compassion fatigue was the most significant factor in burnout among staff of long term care insurance services. Conclusion: The results indicate that the factors influencing staffs' burnout are compassion fatigue, compassion satisfaction, and education level. Therefore, strategies to decrease compassion fatigue and improve compassion satisfaction are needed to decrease burnout for staffs of long-term care insurance in National Health Insurance Services.
This study was carried out to idedtify Worker's general health diagnosis and specific health diagnosis state and provide the basic data about occupational health nursing services in Kyung Sang Nam Do. Data was obtained from 36 industry in Chang-won, Jin-joo, Geo-jeoi, Chung-moo area during Dec. 20. 1992-Feb. 18. 1993 through questionaire survey. Data was analyzed into frequency, percentage, average, standard deviation and score sum The main findings are as follows : 1) 83.3% of subject was manufactures. The factory which have over 1000 Worker's are 41.7% and 500-999 are 36.1%. 2) Health managers are composed of 5 man power. Doctors occupied in 44.4% of factories, average age of them are 43.9 years, average careers are 4.0 years. Nurses occupied in all factories and their average ages are 27.6, average careers are 3.0 years. Industrial hygienist occupied in 33.3% of factories, environmental hygienist occupied 69.4% of industries, and nurse aids occupied in 19.4%. 3) 99.9% of workers are received general health diagonosis. And 10.8% of workers are received elaborate health diagnosis. Among them 30.9% are C class and 23.4% are D class. Among D class, 50% of workers are treated as work time shortening, work replacement, being under treatment. Total specific health diagnosis' subject are 19.3% of workers but 79.9% of them are received specific health diagnosis. Among them 18.6% are needed follow up treatement. Only 44.9% of them are received follow up treatement. 4) 69.4% of industries have their referral hospital and 97.2% have their clinics. Among Occupational health services, health diagnosis are carried out first. of all and the next, environmental management, industrial diagnosis, health education are carried out.
