팔기능 장애가 있는 유방암절제술 환자의 가정치료융합프로그램이 작업수행력, 삶의 질, 우울감에 미치는 효과 연구 (The Effect of home education convergence program on arm functions, occupational performance, quality of life, and depression in mastectomy patients with arm function impairment)

  • 본 연구는 유방암절제술환자 대상으로 가정치료융합프로그램을 실시하여, 유방암절제술환자의 팔 기능, 작업수행력, 삶의 질, 우울감에 미치는 영향에 대해 알아보았다. 대상자는 유방암절제술환자 20명을 대상으로 12주간 주 2회 40분간 외래로 작업치료실에 방문한 환자를 실시하였다. 중재 전과 후의 팔 기능을 알아보기 위해, 한국형 상지 장애 척도(Korean version of the Disability of the Arm, Shoulder, and Hand; K-DASH)와 부종을 줄자로 측정하였고, 작업수행력을 알아보기 위해 캐나다작업수행측정(Canadian Occupational Performance Measure, COPM)실시하였으며, 삶의 질을 알아보기 위해 Functional Assessment Cancer Therapy-Brest(FACT-B)을 사용하였으며, 우울감을 알아보기 위해 자기평가 우울척도(Beck Depression Inventory, BDI)를 이용하였다. 중재 결과 팔 기능은 유의하게 증가하였으나, 부종은 유의하게 감소하지 않았으며, 작업수행력, 삶의 질에서 유의하게 향상하였으며, 우울감은 유의하게 감소하였다. 따라서 가정치료융합프로그램은 유방암절제술환자의 팔 기능, 작업수행력, 삶의 질 우울감에 긍정적인 영향을 미친다는 사실을 알 수 있었다.

일개 병원 임상간호사의 감정노동과 직무스트레스에 관한 연구 (The Relationship Between Emotional Labor and Job Stress of Clinical Nurses in a Hospital)

  • Purpose: The purpose of this research is to understand the relationship between emotional labor and job stress for clinical nurses. Method: The information for this research has been gathered in method of questionnaire for 202 nurses working in a hospital from January 30 to February 10, 2006. Results; The emotional labor for clinical nurses showed 2.43 on a scale of 4 and the job stress showed 49.84 out of 100. The correlation between the emotional labor and job stress showed that the emotional labor was higher, job stress was also higher. The result was that the factors that influenced the emotional labor were health status in a subjective point of view, working department, and the personality. The factors that influence job stress were self-efficacy, working department, and health status in a subjective point of view. Conclusion: The result of the research showed that emotional labor and job stress for clinical nurses were high and the strategy for reduction in those two areas is needed. Especially, diversified plans for job demand and occupational climate in an area of job stress that is correlated to emotional labor will be needed to minimize the emotional labor and job stress.

반도체 산업의 작업환경과 질병의 경험들 (Working Environment and Experiences of Diseases in Semiconductor Industry)

  • Objectives: Information of chemical in electronics industry has not been known enough, although workers' health hazards by chemicals in this industry have been regarded serious. The who always contribute to reveal and control the health hazards and risks in electronics industry have not been the industry but the occupational or environmental victims and grassroot organizations. The similar phenomena can be observed in Korea. Methods: The experiences of individual workers on the occuapational safety and health in semiconductor factory were heard separately and integrated with related literatures. Results:: The first part of this article is largely based on the memories and experiences of individual workers of semiconductor factories, mainly from S semiconductor. Common features of those experiences can be a good resource for detail understanding of the workplace, especially under the situation without any transparent disclosure of information by the governments or the companies. The second part of this article deals with several factors to be considered for control of exposure in workplace; characteristics of workplace and workforce, types of chemicals used, and the way of using the chemicals. Then the current social issues on work-relatedness of cancers of S semiconductor workers are presented briefly. Conclusions: As a conclusion, current OSH situations and issues in Korean electronics industry raise the need of changes in OSH culture. General adaptation of precautionary principle, internalization of costs, and extended responsibility of producers are needed urgently. The OSH professionals both in public and private sectors should support these agendas under their social obligation to protect workers' health.

Cadmium induces apoptosis in human lung fibroblast by inducing oxidative stress: A role of Bax and Bcl-2

  • Cadmium (Cd) is an inorganic toxicant of great environmental and occupational concern which was classified as a human carcinogen in 1993. Occupational cadmium exposure is associated with lung cancer in human. In the present study, we established the mechanistic basis of apoptotic cell death induced by Cd in WI38 human lung fibroblast. Cd at 20-80 ${\mu}$M decreased viablility of cells in a concentration-dependent manner. PI staining, TUNEL staining and DNA fragmentation analysis demonstrated the apoptotic cell death by Cd. 항암화학요법을 받고 있는 한국 대장암 환자의 삶의 질 영향 요인 (Factors Influencing Quality of Life during Chemotherapy for Colorectal Cancer Patients in South Korea)

  • Purpose: The purpose of this study was to investigate the levels of physical symptoms, anxiety, depression, and quality of life (QOL) during chemotherapy for colorectal cancer patients in South Korea and to identify factors influencing their QOL. Methods: Data were collected from 144 colorectal cancer patients receiving chemotherapy during 2012 at one general hospital located in Seoul. Physical symptoms were measured by the M. D. Anderson Symptom Inventory-Gastrointestinal Cancer Module, and anxiety and depression were measured by the Hospital Anxiety Depression Scale. QOL was measured by the Functional Assessment of Cancer Therapy-Colorectal. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, $Scheff{\acute{e}}$ post hoc test, Pearson correlation and stepwise multiple regression. Results: Mean age of the participants was 56.6 and most of them were not employed. In terms of cancer stage, 38.2% were in stage 3, followed by stage 4 (34.7%). The most frequent symptom was lack of appetite, followed by sleep disturbance and fatigue. The mean score for anxiety was 5.40 with a prevalence of 23% and that of depression 8.85 with a prevalence of 64.6%. The mean score for quality of life was 81.93 out of 136 and 75.3% of the variance in QOL was explained by depression, symptoms, anxiety, treatment place, and occupational status. Depression was the strongest predictive factor. Conclusion: Oncology professionals need to pay special attention to relieving depression as well as physical symptoms to improve QOL during chemotherapy for colorectal cancer patients.

Breast Cancer Association with CYP1A2 Activity and Gene Polymorphisms - a Preliminary Case-control Study in Tunisia

  • The aim of the present study was to evaluate the relative contribution of CYP1A2 isoforms (-3860 G/A, -2467T/delT and -163C/A) in control subjects and breast cancer patients to the metabolism of caffeine in human liver. Restriction fragment length polymorphism analysis of PCR-amplified Fragments (PCR-RFLP) was used for the genotyping of CYP1A2 SNPs and HPLC allowed the phenotyping through the measurement of CYP1A2 activity using the 17X + 13X + 37X/137X urinary metabolite ratio (CMR) and plasma caffeine half life (T1/2). The CYP1A2 -3860A genotype was associated with a decreased risk of breast cancer. In contrast, distributions of the CYP1A2 -2467T/delT or -2467delT/delT and -163A/C or A/A genotypes among breast cancer patients and controls were similar. When the genotype and phenotype relationship was measured by comparing the mean CMR ratios and caffeine half life within the genotype groups between subjects and breast cancer patients, there were no significant differences except for -3860 A, most of them being homozygous for the -3860 G/G SNP and had a significant higher mean CMR ratio and half life than those with -3860 G/A (P=0.02). The results of this preliminary study show a significant association between CP1A2 -3860 G variant and CYP1A2 phenotype which must be confirmed by further large-size case-control studies.

Predictors of Quitting Tobacco - Results of a Worksite Tobacco Cessation Service Program Among Factory Workers in Mumbai, india

  • Background: Tobacco cessation would provide the most immediate benefits of tobacco control to prevent tobacco related disease morbidity and mortality. Methods: A tobacco cessation program involving individual and group behavior therapy was implemented in three stages at a worksite. Tobacco quit rates were assessed at the end of each contact session. Results: Out of the 291 tobacco users identified, 224 participated in the tobacco cessation interventions. At the end of three interventions, 38 (17%) users had successfully quit tobacco use. Presence of clinical oral pre-cancer lesion was found to be associated with quitting (p=0.02). Also tobacco users with oral pre-cancer lesions were around three times more likely to quit than those with no lesions (OR= 2.70 95% C.I= 1.20 - 6.05). Conclusion: Cost effective multi-pronged tobacco cessation approaches, inbuilt into other occupational health and welfare activities, are acceptable and feasible to achieve long term sustainable tobacco cessation programs at worksites.

전처리방법에 따른 불용성 6가 크롬 분석에 관한 연구 (Pretreatment Effect on the Analysis of Insoluble Hexavalent Chromium)

  • Hexavalent chromium($Cr^{+6}$) compounds are considered to be particularly hazardous, primarily because of the associated risk of allergic reaction and cancer. The analytic method of hexavalent chromium such as the s-diphenylcarba-zide(DPC) method and all ether previously used methods are often made uncertain due to significant interferences from organic components. This report can provide a technique for the more rapid and simple determination of total hexavalent chromium. than other currently using methods. The s-diphenylcarbazide method proposed by the U.S. National Institute for Occupational Safety and Health has low recovery rate(15.67 - 48.20%) due to interference, iron chloride and nickel chloride. A microwave oven technique has high recovery rate(about 70%) of insoluble hexavalent chromium. For the difference of ionic charges of $Cr^{+3}$-ethylenediamine tetraacetic acid(EDTA) chelate and $CrO_4{^{-2}}$, we could detect them simultaneously by ion exchanged high performance liquid chromatography. The confirmation of $Cr^{+3}$ and $Cr^{+6}$ were checked by fraction collector and flameless atomic absorption spectrometer. We observed that the small amount of hexavalent chromium is converted to trivalent chromium due to enhancement of chromium reduction by $Fe^{+3}$ or $Ni^{+2}$. As a result of this study, on the analysis of insoluble hexavalent chromium with microwave oven was used for, it may be better and more precise analysis after pretreatment by 2% NaOH-3% $Na_2CO_3$ and then analysis UV-spectrophotometer. It should be done for various studies on insoluble hexavalent chromium on the basis work environmental monitoring so called welding, painting etc.

Associations of Most Prevalent Risk Factors with Lung Cancer and Their Impact on Survival Length

  • Lung cancer is one of the most common malignancies in the world. Its incidence and mortality rates are on the rise in Pakistan. However, epidemiological studies to identify common lung cancer determinants in the Pakistani population have been limited. In this study, data of 440 cases and 323 controls were collected from different hospitals in Peshawar and Islamabad, along with information about socio-demographic factors including age, sex and smoking. Univariate and multi-factorial analyses of socio-demographic factors in association with each other were also performed. Overall survival analysis highlighted that, out of 440 patients in the lung cancer dataset, 204 people were uncensored with a median survival time of 13 months (95% CI=12-18). There were 41 femaleand 399 male patients. Differences were observed between length of survival in the males and females (${\chi}12$ = 6.1; p-value = 0.01). Gender was observed to be significantly related to survival (p-value< 0.01), with better survival in females (hazard ratio=2). Cox regression was extended to adjust for the covariate age (z = 2.5; p-value = 0.02). Survival analysis was also performed on the basis of smoking groups (current smokers, former smokers and never smoked individuals) and smoking duration (smoking duration >10 years, <10 years and never smoked). Smoking duration was significantly associated with survival (p-value < 0.01), with better survival in never smokers in comparison to both smoking for greater or less than 10 years. Strong associations were observed for smoking group with duration greater than 10 years, OR=6.1(3.9-9.5) on univariate and multifactorial analysis OR=11.3(CI=6.8-19.3).

Projection of Cancer Incidence and Mortality From 2020 to 2035 in the Korean Population Aged 20 Years and Older

  • Objectives: This study aimed to identify the current patterns of cancer incidence and estimate the projected cancer incidence and mortality between 2020 and 2035 in Korea. Methods: Data on cancer incidence cases were extracted from the Korean Statistical Information Service from 2000 to 2017, and data on cancer-related deaths were extracted from the National Cancer Center from 2000 to 2018. Cancer cases and deaths were classified according to the International Classification of Diseases, 10th edition. For the current patterns of cancer incidence, age-standardized incidence rates (ASIRs) and age-standardized mortality rates were investigated using the 2000 mid-year estimated population aged over 20 years and older. A joinpoint regression model was used to determine the 2020 to 2035 trends in cancer. Results: Overall, cancer cases were predicted to increase from 265 299 in 2020 to 474 085 in 2035 (growth rate: 1.8%). The greatest increase in the ASIR was projected for prostate cancer among male (7.84 vs. 189.53 per 100 000 people) and breast cancer among female (34.17 vs. 238.45 per 100 000 people) from 2000 to 2035. Overall cancer deaths were projected to increase from 81 717 in 2020 to 95 845 in 2035 (average annual growth rate: 1.2%). Although most cancer mortality rates were projected to decrease, those of breast, pancreatic, and ovarian cancer among female were projected to increase until 2035. Conclusions: These up-to-date projections of cancer incidence and mortality in the Korean population may be a significant resource for implementing cancer-related regulations or developing cancer treatments.
