Development of a Predictive Model forOccupational Disability Grades Using Workers'Compensation Insurance Data (산재보험 빅데이터를 활용한 장해등급 예측 모델 개발)

  • Purpose A prediction model for occupational injuries can support more proactive, efficient, and effective policy-making. This study aims to develop a model that predicts the severity of occupational injuries, classified into 15 disability grades in South Korea, using machine learning techniques applied to COMWEL data. The primary goal is to improve prediction accuracy, offering an advanced tool for early intervention and evidence-based policy implementation. Design/methodology/approach The data analyzed in this study consists of 290,157 administrative records of occupational injury cases collected between 2018 and 2020 by the Korea Workers' Compensation & Welfare Service, based on the 'Workers' Compensation Insurance Application Form' submitted for occupational injury treatment. Four machine learning models - Decision Tree, DNN, XGBoost, and LightGBM - were developed and their performances compared to identify the optimal model. Additionally, the Permutation Feature Importance (PFI) method was used to assess the relative contribution of each variable to the model's performance, helping to identify key variables. Findings The DNN algorithm achieved the lowest Mean Absolute Error (MAE) of 0.7276. Key variables for predicting disability grades included the severity index, primary disease code, primary disease site, age at the time of the injury, and industry type. These findings highlight the importance of early policy intervention and emphasize the role of both medical and socioeconomic factors in model predictions. The academic and policy implications of these results were also discussed.

Changes of Pulmonary Disability Grades according to the Spirometry Reference Equations (폐기능 예측식에 따른 폐환기능 장해도 변화)

  • Background: The aim was to estimate the differences between pulmonary disability grades according to the spirometry reference equations (the Korean equation and the Morris equation). Methods: Spirometry was performed on 16,916 male and 1,353 female special examination for pneumoconiosis, in the period of 2007~2009. Changes in predictive values for forced expiratory volume in one second ($FEV_1$), forced vital capacity (FVC) and $FEV_1$/FVC and in disability grade were evaluated using both equations. Results: Mean FVCs for men and women were 4,218.7 mL and 2,801.5 mL in predictive values after the application of the Korean equation, and 3,763.9 mL and 2,395.6 mL after the Morris equation, respectively. Compared with the Morris equation, the Korean equation showed 10.8% and 14.5% of excesses for men and women (p<0.001). Mean $FEV_1s$ for men and women were 3,102.5 mL and 2,107.1 mL in the Korean equation, and 2,667.8 mL and 1,699.6 mL in the Morris equation, respectively. Compared with the Morris equation, the Korean equation showed 14.0% and 19.3% of excesses for men and women (p<0.001). Men and women who showed the changes of disability grades using the Korean equation in place of the Morris equation were 23.9% (4,052/16,916) and 22.9% (311/1,353) on FVC, and 23.1% (3,913/16,916) and 10.7% (145/1,353) on $FEV_1$. Conclusion: Applying different reference equations for spirometry has resulted in changes for disability grades in special examination for pneumoconiosis.

Relationship of Stress and Aggression on Schoolchildren with Physical disability (학령기 지체장애 아동의 스트레스와 공격성 비교연구)

  • Purpose : The purpose of this study was to know the relationship of stress and aggression of children with physical disability. The study also tried to search the factors affecting aggression and offer the information to the department of occupational therapy for intervention. Method : We distributed questionnaire to 105 children who have disability grades from hospitals in Gyeongsangbukdo, Gyeongsangnamdo, Daegu, and Busan and collected the data. We used descriptives for analysis of general characteristics, t-test and one-way ANOVA for stress and aggression according to characteristics, and Pearson correlation coefficient and stepwise regression for stress and aggression. Result : The first, stress score was 2.65, aggression was 2.53, and physical aggression was 3.01 that is the highest score in the items. The second, there was a significant difference of physical aggression(p=.021) in comparison of the grades. There was a significant difference of stress(p=.048), total aggression(p=.040), and physical aggression(p=.047) in comparison of gender. There was significant difference of stress(p=.035) and total aggression(p=.042) in satisfaction of school. The third, there was a significant correlation of total aggression(r=.475), physical aggression(r=.568), language aggression(r=.311), anger(r=.397), and hostility (r=.491) quantitatively in correlation of stress and aggression. The fourth, the factors affecting aggression of children with physical disability were stress, male, and satisfaction of school(F=61.187, p<.01). Conclusion : We knew that factors affecting aggression of children with physical disability were their stress and some of general characteristics.

A Comparative Study on Evaluation Methods of Permanent Impairment in Korea (우리나라의 신체장애평가법에 관한 비교연구 - 신체장해등급법, McBride법, 미국의학협회 (AMA)기준법을 중심으로 -)

  • In this study, literature review was done to examine and compare the current status and problems of different evaluation approaches toward permanent impairment in Korea. Alternatives and improvements in the current approaches in Korea were suggested. Series of cases were also examined to compare different approaches applied to the real cases, using 105 cases from a hospital data and another 207 cases from a insurance company data. The main findings of the literature review are as follows; 1. The current evaluation methods of permanent impairment in Korea are grouped into two categories, grading and rating. Gradings of impairments are expressly specified in 17 various statutes. 2. In Grading methods, the rigid system of 74 different grades has been adopted uniformally for the convenience of administration, which may not be, appropriate or valid from medical and scientifical aspect. 3. The adventage of McBride method is assessment of occupational disability rate. However the classified compensable occupations are only 280 and limited to manufacturing industries in 1960s'of U.S.A., which is not appropriate to current Korean circumstances. Especially, the job list does not include managerial officers or mental workers. 4. AMA Guides is the scientific and reasonable method for the assessment of physical impairment rate. However compensation and reparation of impairment case is difficult because this method cannot assess the disability rate according to occupation, age, etc. The results of cases comparative study are as follows: 5. The physical Impairment could be compared in 167 out of total 312 cases, and for the cases of complex impairment, McBride method underestimate physical impairment rate compared with AM A method. 6. When disability late was assessed, occupation was considered the compensation of only 85 cases, and age was used in only 21 cases. This was because occupation and age compensation in McBride method are unreasonable. 7. The most Ideal alternative is to assess physical impairment according to AMA method and then to develop a compensation method appropriate for the circumstances of Korea society.
