Effect of Preconditioning Ischemia on Endothelial Dysfunction Produced by Ischemia-Reperfusion in Rabbit Coronary Artery

  • This study was designed to test whether or not 1) ischemia-reperfusion attenuates endothelium-dependent relaxation of coronary arteries and 2) preconditioning protects the arterial endothelium from ischemia-reperfusion injury. In anesthetized open chest rabbits, branches of the left circumflex artery were exposed to different combinations of the experimental conditions; ischemia (15 minutes), ischemia (15 minutes)-reperfusion (10 minutes), preconditioning ischemia, and pre-conditioning fellowed by ischemia-reperfusion. Preconditioning consisted of 3 occlusions of 2-min duration, each followed by n 5-min reperfusion. Rings of the artery exposed to the experimental condition and of normal left anterior descending coronary artery were prepared and suspended for isometric force measurement in organ chambers containing Krebs Ringer bicarbonate solution. The rings were contracted with 29.6 mM KCI. Ischemia alone did not attenuate endothelium-dependent relaxation by acetylcholine. However, ischemia-reperfusion significantly impaired endothelium-dependent relaxation. Endothelium-independent relaxation by sodium nitroprusside was not impaired by ischemia-reperfusion and the constrictive response to acetylcholine was not altered in reperfused rings without endothelium, compared with control rings. Arterial rings exposed to preconditioning followed by ischemia-reperfusion exhibited impaired endothelium-dependent relaxation by acetyl-choline. However, although preconditioning not fellowed by ischemia-reperfusion, attenuated endothelium-dependent relaxation at low concentrations of acetylcholine, the magnitude of the impairment by preconditioning followed by ischemia-reperfusion was significantly less than that of the impairment by ischemia-reperfusion alone. These data demonstrate that ischemia-reperfusion significantly attenuates endothelium-dependent relaxation by producing endothelial dysfunction and preconditioning Protects the endothelium of coronary arteries from ischemia-reperfusion injury.

  • Many attempts for the compfrt, esthetics and improvement of Masticatory function of the patients with removable prosthesis have been made for several decades. The search for the ideal denture occlusion has been going on in an effort to find the tooth form which provides maximum denture stability and masticatory efficiency without damaging the health of the underlying bone. For the purpose, the basic concept of lingualized occlusion were suggested by payne(1941) and pound(1973) discussed a similar occlusal concept and used term " lingualized occlusion." The purpose of this literature study is to clarify and amplify a method to achieve bilateral balanced occlusion with the occlusal arrangement termed " lingualized occlusion." Lingualized occlusion can be achieved by use of anatomic teeth for the mandibular denture. Lingualized occlusion can be used in most denture combination. It is particulary helpful when the patient places high priority on esthetics but a nonanatomic occlusal scheme is indicated by oral conditions such a severe alveolar resorption, a class II jaw relationship or displaceable supporting tissue. Advantages of lingualized occlusion are summarized as follows : 1) Most of the advantages attributed to both the anatomic and nonanatomic forms are retained. 2) Cusp form is more natural in appearance compare to nonanatomic tooth form. 3) Good pnetration of the food bolus is possible. 4) Bilateral mechanical balanced occlusion is readily obtained for a region around arotric relation. 5) Bertical forces are centralized on the mandibular teeth. Lingualized occlusion provides a useful combination of several occlusal concept. Many. advantages of anatomic and nonanatomic occlusions are accomplished but the lingualized occlusal concept is not is not a panacea, and all other procedures still must be carefully excuted.

Analysis of the Creep Effect on the Dural-sac Occlusion in the Lumbar Spinal Motion Segment (크?현상이 요추 운동분절내의 척추경악 교합에 미치는 영향 해석)

  • Occlusion of the dural-sac in the lumbar spine was quantitatively analysed using a one motion segment finite element mode developed in this study. Occlusion was quantified by calculating the cross sectional area chance of the dural-sac. In static analysis. less than 2 kN of compressive load could Produced no dural-sac occlusion. whereas 6kN load reduced cross sectional area by 4%, and produced 7.4%, 10.5% occlusion for additional 8 Nm. 10 Nm extension moments. respectively. In creep analysis, 10 Nm extension reduced cross sectional area and volume of the dural-sac by 6.9% and 2.4%, respectively. However. flexion moment could not produce any occlusion. The results suggested that occlusions may result mainly from slackening of ligamentum flavum and disc budging.

A STUDY ON MASTICATORY PERFORMANCE AND FUNCTION BY POSTERIOR OCCLUSAL SCHEMES IN COMPLETE DENTURE (총의치의 교합면 형태에 따른 저작 효율 및 기능에 관한 연구)

  • This investigation was designed to determine the effectiveness of the posterior occlusal schemes on masticatory activity during mastication in complete denture. Twelve edentulous subjects were selected for this study. All subjects had no past history and no functional abnormality on masticatory system and TMjoint. And, they had residual ridge of favorable morphology, firm mucosa and Class I skeletal jaw relationship, Twelve experimental denture with interchangeable occlusions(0-degree teeth, 30-degree teeth, Levin teeth and S-A teeth) were constructed for this study. The masticatory performance was analyzed by means of standard sieve(10, 16, 20, 30sieve), and the electrical activity from selected muscles(Temporalis and Masseter muscle) was recorded simultaneously with electromyography(Bio-Pak system) as the subject masticated test foods (rice, peanut and gum) with four different occlusal schemes. Mandibular movement was, also, measured with Sirognathography(Bio-Pak system). These recordings were performed in immediately, after 1 week and after 2 weeks of insertion of complete denture. The results were as follows; 1. The average masticatory performance of 0-degree artificial teeth was higher than any other artificial teeth. 2. Masticatory performance in denture wearer was affected preferentially by food and artificial occlusal schemes. 3. During chewing, there was a statistical difference of EMG activity between masseter and temporal muscle(p<0.01). Especially, EMG activity of working masticatory muscle was highly affected by food rather than by artificial occlusal schemes. 4. In denture wearer, the velocity of opening was not affected by food, whereas, the velocity of closing was faster in soft food chewing than in hard food chewing, and the amount of vertical displacement was grater in chewing of soft and large bolus than in chewing of hard and small bolus. However, the amount of lateral displacement showed conversely (p<0.05). 5. It was considered that masticatory performance in denture wearer is not affected by the condition of residual ridge, the history of denture wear, the preference, the adaptation to artificial teeth and the total mesiodistal length of artificial posterior teeth.

Relational matching for solving initial approximation (관계영상정합을 이용한 초기근사값 결정)

  • The objective of this research is to investigate the potential of relational matching in one of the fundamental photogrammetric processes, that is initial approximation problem. The automatic relative orientation procedures of aerial stereopairs have been investigated. The fact that the existing methods suffer from approximations, distortions (geometric and radiometric), occlusions, and breaklines is the motivation to investigate relational matching which appears to be a much more general solution. An elegant way of solving the initial approximation problem by using distinct(special) relationship from relational description is suggested and experimented. As for evaluation function, the cost function was implemented. The detection of erroneous matching is incorporated as a part of proposed relational matching scheme. Experiments with real urban area images where large numbers of repetitive patterns, breaklines, and occluded areas are present prove the feasibility of implementation of the proposed relational matching scheme. The investigation of relational matching in the domain of image matching problem provides advantages and disadvantages over the existing image matching methods and shows the future area of development and implementation of relational matching in the field of digital photogrammetry.

  • In this paper, we present a new method for real-time tracking of human walking around a laser sensor system. The method converts range data with $r-{\theta}$ coordinates to a 2D image with x-y coordinates. Then human tracking is performed using human's features, i.e. appearances of human walking pattern, and the input range data. The laser sensor based human tracking method has the advantage of simplicity over conventional methods which extract human face in the vision data. In our method, the problem of estimating 2D positions and orientations of two walking human's ankle level is formulated based on a moving trajectory algorithm. In addition, the proposed tracking system employs a HMM to robustly track human in case of occlusions. Experimental results using a real system demonstrate usefulness of the proposed method.

Can Transradial Mechanical Thrombectomy Be an Alternative in Case of Impossible Transfemoral Approach for Mechanical Thrombectomy? A Single Center's Experience

  • Objective : Until recently, the transfemoral approach (TFA) was used as the primary method of arterial approach in acute ischemic stroke (AIS). However, TFA resulted in longer reperfusion times and worse outcomes in the mechanical thrombectomy (MT) of patients with complex aortic arches and significant carotid tortuosity. We found that the transradial approach (TRA) is a more favorable alternative approach for MT in such cases. Methods : We performed a retrospective review of our institutional database to identify 202 patients who underwent MT for AIS between February 2015 and December 2019. Patient characteristics, cause of TFA failure, procedure time, intra-procedural complications, and outcomes were recorded. Results : Eleven (5.4%) of 202 patients, who underwent MT for AIS, crossed over to TRA for recanalization, and eight (72%) of 11 achieved successful recanalization (≥modified Treatment in Cerebral Infarction 2b). The mean age (mean±standard deviation [median]) was 82.3±6.6 (76) years, and five of the 11 patients were male. The last seen normal to puncture time was 467.9±264.72 (264) minutes; baseline National Institutes of Health Stroke Scale score was 28.9±14.5 (16). Six (55%) of the 11 patients had right vertebrobasilar occlusions, and the remaining five (45%) had anterior circulation occlusive disease. The time from groin puncture to final recanalization time (overall procedural time) was 78.0±20.1 (62) minutes. The mean crossover time from TFA to TRA was 45.2±10.5 (41) minutes. The mean time from radial puncture to final recanalization was 33.8±10.5 (28) minutes. Distal thrombus migration events in previously unaffected territories occurred in 3/8 patients (37%). At 90 days, three patients (28%) had a favorable clinical outcome. Conclusion : Although rare, failure of TFA has been known to occur during MT for AIS. Our results demonstrate that TRA may be an alternative option for AIS intervention for select patients with subsequent timely revascularization. However, the incidence of distal thrombus migration was high, and the first puncture to reperfusion time was prolonged because of the time taken for the crossover to TRA after failure of TFA. This study provides some evidence that the TRA may be a viable alternative option to the TFA for MT of AIS.

Relationship between vertical components of maxillary molar and craniofacial frame in normal occlusion: Cephalometric calibration on the vertical axis of coordinates

  • Objective: The aim of this study was to evaluate the correlation between the vertical position of maxillary first molar and vertical skeletal measurements in lateral cephalograms by using new linear measurements on the vertical axis of coordinates with calibration. Methods: The vertical position of maxillary first molar (U6-SN), and the conventionally used variables (ConV) and the newly derived linear variables (NwLin) for vertical skeletal patterns were measured in the lateral cephalograms of 103 Korean adults with normal occlusions. Pearson correlation analyses and multiple linear regression analyses were performed with and without calibration using the anterior and posterior cranial base (ACB and PCB, respectively) lengths to identify variables related to U6-SN. Results: The PCB-calibrated statistics showed the best power of explanation. ConV indicating skeletal hyperdivergency was significantly correlated with U6-SN. Six NwLin regarding the position of palatal plane were positively correlated with U6-SN. Each multiple linear regression analysis generated a two-variable model: sella and nasion to palatal plane. Among the three models, the PCB-calibrated model yielded highest adjusted R2 value, 0.880. Conclusions: U6-SN could be determined by the vertical position of the maxilla, which could then be used to plan the amount of molar intrusion and estimate its clinical stability. Cephalometric calibration on the vertical axis of coordinates by using PCB for vertical linear measurements could strengthen the analysis itself.

Role of Ischemic Preconditioning in the Cardioprotective Mechanisms of Monomeric C-Reactive Protein-Deposited Myocardium in a Rat Model

  • Background: The deposition of monomeric C-reactive protein (mCRP) in the myocardium aggravates ischemia-reperfusion injury (IRI) and myocardial infarction. Ischemic preconditioning (IPC) is known to protect the myocardium against IRI. Methods: We evaluated the effects of IPC on myocardium upon which mCRP had been deposited due to IRI in a rat model. Myocardial IRI was induced via ligation of the coronary artery. Direct IPC was applied prior to IRI using multiple short direct occlusions of the coronary artery. CRP was infused intravenously after IRI. The study included sham (n=3), IRI-only (n=5), IRI+CRP (n=9), and IPC+IRI+CRP (n=6) groups. The infarcted area and the area at risk were assessed using Evans blue and 2,3,5-triphenyltetrazolium staining. Additionally, mCRP immunostaining and interleukin-6 (IL-6) mRNA reverse transcription-polymerase chain reaction were performed. Results: In the IRI+CRP group, the infarcted area and the area of mCRP deposition were greater, and the level of IL-6 mRNA expression was higher, than in the IRI-only group. However, in the IPC+IRI+CRP group relative to the IRI+CRP group, the relative areas of infarction (20% vs. 34%, respectively; p=0.079) and mCRP myocardial deposition (21% vs. 44%, respectively; p=0.026) were lower and IL-6 mRNA expression was higher (fold change: 407 vs. 326, respectively; p=0.376), although the difference in IL-6 mRNA expression was not statistically significant. Conclusion: IPC was associated with significantly decreased deposition of mCRP and with increased expression of IL-6 in myocardium damaged by IRI. The net cardioprotective effect of decreased mCRP deposition and increased IL-6 levels should be clarified in a further study.

Multi-camera-based 3D Human Pose Estimation for Close-Proximity Human-robot Collaboration in Construction

  • With the advance of robot capabilities and functionalities, construction robots assisting construction workers have been increasingly deployed on construction sites to improve safety, efficiency and productivity. For close-proximity human-robot collaboration in construction sites, robots need to be aware of the context, especially construction worker's behavior, in real-time to avoid collision with workers. To recognize human behavior, most previous studies obtained 3D human poses using a single camera or an RGB-depth (RGB-D) camera. However, single-camera detection has limitations such as occlusions, detection failure, and sensor malfunction, and an RGB-D camera may suffer from interference from lighting conditions and surface material. To address these issues, this study proposes a novel method of 3D human pose estimation by extracting 2D location of each joint from multiple images captured at the same time from different viewpoints, fusing each joint's 2D locations, and estimating the 3D joint location. For higher accuracy, the probabilistic representation is used to extract the 2D location of the joints, considering each joint location extracted from images as a noisy partial observation. Then, this study estimates the 3D human pose by fusing the probabilistic 2D joint locations to maximize the likelihood. The proposed method was evaluated in both simulation and laboratory settings, and the results demonstrated the accuracy of estimation and the feasibility in practice. This study contributes to ensuring human safety in close-proximity human-robot collaboration by providing a novel method of 3D human pose estimation.
