상공정맥 증후군 -Dacron & Nylon 환치수술 2예- (Superior Vena Cava Syndrome -2 Bypass Graft Cases-)

  • Superior Vena Cava Syndrome: Dacron and Nylon graft between the left innominate vein and the right atrial appendage. Two cases with typical superior vena cave syndrome treated by by-pass graft between the left innominate vein and the right atrial apepndage were presented. One of them was a 58 year old farmer who suffered from marked swelling of the neck and upper half of body, the other was a 50 years old government employee who had acutely progressive symptoms of superior vena cave obstruction. Both of cases revealed that [1] cubitel venous pressure was markedly increased. [2] tumors were noted in the posterior mediastinum by laminography. [3] preoperative cavogram showed the occlusion of superior vena cava and marked collaterals. Dacron and Nylon graft were inserted between the left innominate vein and the right atrial appendage. Postoperatively, the symptoms were relieved markedly, showing edema free face and decreased cubital venous pressure. Postoperative cavogram showed patent graft. Histologically the first case was diagnosed as squamous cell carcinoma and the second as undifferentiated carcinoma, originated probably from bronchus. Total doses of 3150 r X-ray irradiation and 5000 mg of 5-FU were administered in each cases. The first case expired 11 months postoperatively without recurrence of superior vena cave obstruction symptom and the second case is living now without obstruction signs, 4 months after by-pass operation.

  • 본 연구에서는 초경량성 마그네슘 재료를 엔진 블록, 실린더 헤드커버 등과 같은 박용기관에 적용하기 위한 환경 친화적 표면처리의 개발에 대하여 고찰하였다. 또한 이온플레이팅법에 의해 마그네슘 박막을 제작하고, 그 제작조건에 따라 변화하는 막의 결정배향성과 몰포로지가 경도특성에 미치는 영향을 해명하고자 하였다. 마그네슘 박막의 경도측정 결과, 아르곤 가스압의 증가에 따라 그 경도값이 상승하였 는데, 그 원인은 결정립계에 의한 강화와 성분 외 가스입자의 흡장효과에 의한 것으로 사료된다.

온성 방법에 따른 수종의 임시 수복용 레진의 착색시의 색 안정성에 관한 연구 (A Study on the Color Stability of Temporary Resins according to Polymerizing Methods and Staining)

  • Dental temporary resin should have adequate strength and similar esthetics as natural teeth. Recently esthetics has become a high priority in clinical dentistry. Thus the evaluation of color stability of dental temporary resin has become an important part in dental research. The purpose of this study was to evaluate the difference in color stability of 3 resins($Snap^{(R)}$, Temporary bridge $resin^{(R)}$, Trim $II^{(R)}$) according to polymerizing methods and stainning. The resins were divided into groups by 2 types of polymerizing methods ( air or pressure and heat curing) The resins were dyed with coffee, chlorhexidine and artificial saliva and then the color stability was evaluated with a spectrophotometer(CM-503i, Minolta, Japan). The results were as follows ; 1. In the color stability according to polymerizing method, pressure and heat polymerizing method was higher than air polymerizing method. 2. The color stability according to staining fluid decreased in the order of I, II, III group, there was significant difference among the groups of staining fluid. 3. The color stability according to resins decreased in the order of $Snap^{(R)}$, Trim $II^{(R)}$, Temporary Bridge $resin^{(R)}$, The $Snap^{(R)}$ resin was highest color stability, there was significant difference among the 3 resins.

이중초음파검사상 빗장밑혈류빼앗김증후군이 관찰된 환자에서의 스텐트 삽입 후 추적 (Duplex Sonography in Subclavian Steal Syndrome Treated by Stent Insertion)

  • 빗장밑혈류빼앗김증후군은 왼쪽 근위부 빗장밑동맥이나 오른쪽 팔머리동맥에 심한 협착 혹은 폐색으로 인해 반대쪽 척추동맥으로부터 관류압이 감소된 병변쪽 빗장밑동맥으로 혈류가 역으로 유입되는 현상이 나타나게 되고, 그 결과로 척추-뇌바닥혈류부전이나 허혈성 말초동맥질환 증상이 동반되는 경우를 말한다. 빗장밑동맥을 포함한 뇌혈관 협착증에서 신경중재술을 이용한 혈류의 회복 및 방향 교정은 환자의 증상과 예후를 좋게 한다고 알려져 있다. 저자는 오른팔에 비해 왼팔 혈압이 12 mmHg 낮은 환자에서 이중초음파검사를 시행하였고, 왼쪽 척추동맥에서 빗장밑혈류빼앗김증후군의 특징적인 혈류파형이 관찰되어 혈관성형술 및 스텐트 설치술을 통해 성공적으로 치료된 증례를 보고하고자 한다. 6개월 후 추적검사에서 왼쪽 척추동맥의 혈류파형은 정상으로 회복되었고, 양팔의 혈압 차는 5 mmHg로 감소하였다.

Influence of marginal bone resorption on two mini implant-retained mandibular overdenture: An in vitro study

  • PURPOSE. To investigate the biomechanical effect of marginal bone resorption (MBR) on the mandibular mini implant (MI)-retained overdenture (MI-OD) on the edentulous model. MATERIALS AND METHODS. The experimental mandibular edentulous model was modified from a commercial model with 2 mm thick artificial soft tissue under denture base. Two MIs (Φ2.6 mm × 10 mm) were bilaterally placed between the lateral incisor and the canine area and attached with magnetic attachments. Three groups were set up as follows: 1) alveolar bone around the MI without MBR (normal group), 2) with MBR to 1/2 the length of the implant (resorption group), and 3) complete denture (CD) without MI (CD group). Strain around the MI, pressure near the first molar area, and displacement of denture were simultaneously measured, loading up to 50 N under bilateral/unilateral loading. Statistical analysis was performed using independent-samples t test and one-way ANOVA (α=.05). RESULTS. The strain around the MI with MBR was approximately 1.5 times higher than that without MBR. The pressure in CD was higher than in MI-ODs (P<.05), while there was no statistical difference between the normal and resorption group (P>.05). Similarly, the CD demonstrated a greater displacement of the denture base than did the MI-ODs during bilateral and unilateral loadings (P<.05). CONCLUSION. The strain around the MI with MBR was approximately 1.5 times higher than that without MBR. The pressure on posterior alveolar ridge and denture displacement of MI-ODs significantly decreased compared to CDs, even when MBR occurs. Bilateral balanced occlusion was recommended for MI-ODs, especially when MBR occurred.

  • Ventriculoperitoneal [VP] shunt is a common treatment for hydrocephalic patients. However, complications, such as shunt tube occlusion, infection, intracranial hemorrhage, seizure can occur. Of these, intracranial hemorrhage may occur due to intracranial vascular injury or a rapid decrease of intracranial pressure [ICP]. Most of these hemorrhages are subdural hematomas [SDH] while a few are epidural hematomas [EDH]. It is extremely rare for an intracranial hemorrhage to occur due to an extension of the bleeding from an injured extracranial vessel. We report two cases of EDH due to occipital artery injury following VP shunt and extraventricular drainage [EVD].

상백피 수성 엑기스의 혈압강하작용에 관한 연구 (A Study on the Hypotensive Action of Mori Radicis Cortex Water Extract in the Rabbit.)

  • Intravenous injection of Mori Radicis Cortex Water Extract (MWE) regularly caused the does-related, lowering of blood pressure in the rabbits anesthetized with urethane, and then did not show the cumulative effect and the tachyphylaxis. The hypotensive effects of MWE were inhibited by atropine, chlorisodamine, phentolamine and bethanidine, while not altered by diphenhydramine, propranolol and cyproheptadine. Atropine after chlorisondamine did not alter the effect of MWE. MWE potentiated the pressor effect of nore-pinephrine, but did not carotid occlusion was inhibited by previous administration of MWE. It is conclude that MWE elicits hypotensive action in the rabbit by the centrally induced cholinergic effect and the inhibitation of responses to sypathetic adrenergic nerve activation.

Magnesium Thin Films Possessing New Corrosion Resistance by RF Magnetron Sputtering Method

  • Magnesium thin flims were prepared on cold-rolled steel substrates by RF magnetron sputtering technique. The influence of argon gas pressure and substrate bias voltage on their crystal orientation and morphology of the coated films were investigated by scanning electron microscopy (SEM) and X-ray diffraction, respectively. And the effect of crystal orientation and morphology of magnesium films on corrosion behaviors was estimated by measuring anodic polarization curves in deaerated 3%NaCl solution. From the experimental results, all the sputtered magnesium films showed obviously good corrosion resistance to compare with 99.99% magnesium target of the sputter-evaporation metal. Finally it was shown that the Corrosion-resistance of magnesium films can be improved greatly by controlling the crystal orientation and morphology with effective use of the plasma sputtering technique.

코골이 및 폐쇄성 수면무호흡증의 구강내 장치 치료시 부작용 및 관리 (Management of complications of oral appliance therapy for snoring and obstructive sleep apnea)

  • Oral appliances therapy is becoming increasingly recognized as a successful treatment for snoring and obstructive sleep apnea(OSA). Compared with continuous positive airway pressure(CPAP), the gold standard therapy for OSA, oral appliance therapy are less efficacious for severe OSA but are more acceptable and tolerable for patients, which in turn, may lead to a comparable level of therapeutic effectiveness. Nevertheless, the various side effects of oral appliance therapy, such as, increased salivation or dryness, pain or discomfort in the teeth or gums, occlusal discomfort in the morning, temporomandibular disorders, dental and occlusal changes may cause discontinuation of treatment or changes in treatment plan. Therefore, oral appliance therapy should be provided by a qualified dentist who can evaluate oral tissues, occlusion, and temporomandibular joints, and prevent and manage the possible side effects.

Cerebroprotective Effect of Nociceptin on Transient Focal Cerebral Ischemia in Rats

  • This study aimed to investigate the cerebroprotective effect of nociceptin on transient focal cerebral ischemia in Sprague-Dawley rats by determining the changes in regional cerebral blood flow (rCBF) and the infarct size. Right middle cerebral artery (MCA) was occluded for 2 hours, and thereafter was followed by reperfusion by an intraluminal monofilament technique. An open cranial window was made on the right parietal bone for determination of continuous changes in rCBF by laser-Doppler flowmetry. The infarct size was morphometrically determined using the 2,3,5-triphenyltetrazolium chloride technique. In normal rats, nociceptin ($0.01\~100\;nmol/kg$, Lv.) increased rCBF and decreased cerebral arterial resistance in a dose-dependent manner. Systemic arterial blood pressure was little affected by nociceptin at the doses of 0.01 and 0.1nmol/kg, but dose-dependently reduced at the doses of 1 nmol/kg or more. In transient cerebral ischemic rats, nociceptin ($0.01\~0.1$ nmol/kg, i.p.) significantly attenuated the postischemic cerebral hyperemia, and progressively increased rCBF. The improving effect of nociceptin on the postischemic rCBF response was markedly blocked by pretreatment with $[Nphe^1]nociceptin(1-13)NH_2$ (1 nmol/kg, i.p.), a selective nociceptin receptor antagonist, but not by naloxone ($3{\mu}mol/kg$, i.p.), a selective opioid receptor antagonist. The cerebral infarct size was significantly reduced by nociceptin ($0.01\~0.1$ nmol/kg) administered i.p. 5 min after MCA occlusion in transient cerebral ischemia of 2-hour MCA occlusion and 22-hour reperfusiion. It is suggested that nociceptin improves the postischemic cerebral hemodynamics and thereby has a cerebroprotective effect in transient focal cerebral ischemia.
