A STUDY OF HOLOGRAPHIC INTERFEROMETRY ON THE INITIAL REACTION OF MAXILLOFACIAL COMPLEX TO THE MAXILLARY PROTRACTION USING THE ANTENNA TYPE MODIFIED PROTRACTION HEAD GEAR (Modified Protraction Headgear를 이용한 상악골 전방 견인시 악안면골의 초기반응에 관한 Holographic Interferometry 연구)

  • The majority of the commonly used protraction headgears for the protraction of small and/or retropositioned maxilla not allow a change in the point of force application or direction of the force delivery to attain predictable results because of the position of the upper and lower lips to avoid discomfort to the patient. The purpose of this study was to investigate the initial reaction of maxillofacial complex according to the change of force variables such as direction and point of force application with designing an antenna type-modified protraction head gear. A macerated human skull with well aligned upper teeth was used to experimental model and the investigation was done by double exposure holographic interferometry. Fringe patterns of each protraction conditions were compared and analized. The results were as follows. (Frontal view) 1. The Counterclockwise rotation of the maxilla was showed by parallel protraction to occlusal plane and the fringe was decreased in number as higher point of force application. 2. Generally, the number of fringe was increased in 500gm of protraction force than in 300gm. 3. When apply the protraction force to the maxilla with rapid palatal expansion, the direction of fringe patterns was differed from the protraction without expansion. 4. In most of cases, the counterclockwise rotation was decreased in case of the direction of the force is $20^{\circ}$ downward to occlusal plane compared to the parallel direction. 5. At the point of force application is 15mm above and the direction of force is 20 downward to occlusal plane , the translation of the maxillary complex was showed. (Lateral view) 6. The direction of fringe patterns of the facial bones were differed each other by the sutures, and showed almost parallel when apply the 300gm and 500gm of protraction force. 7. In case of rapid palatal expansion with protraction of the maxilla, the fringe patterns between the maxillary area and the area from the posterior of the maxillary first molar to the pterygomaxillary fissure were differed. In case without rapid palatal expansion, the changes of direction and point of the force application did not affect to the direction and the number of the fringe patterns.

Full mouth Rehabilitation in a Patient with Occlusal Collapse with Vertical Dimension Increase (교합 붕괴 환자에서 수직 고경을 증가한 보철 수복 : 증례 보고)

  • In a case of multiple posterior teeth loss, antagonistic teeth extrude to the edentulous space and compensatory occlusion on the remained anterior teeth leads to occlusal trauma. Extrusion of antagonistic teeth breaks down occlusion plane and loss of posterior support bring about severe wear of remained teeth. In this situation, it is needed to restore remained teeth and edentulous space by increasing vertical dimension to obtain prosthodontic rehabilitation space and to correct occlusion plane. In this case report, the patient had a masticatory problem with loss of posterior teeth support and an esthetic problem of shortened anterior teeth. Before the tooth preparation for the prosthodontic restoration, the patient used removable device for 2 months to increase vertical dimension reversibly. After that, he got provisional fixed restoration with irreversible tooth reduction and used it for 3 months. It had spent 5 month to evaluate the adaptation state on final restoration with incresed vertical dimension. The increasing amount was 3 mm, which was relatively in less degree and masticatory system adapted to the increased vertical dimension without any pathologic changes. Final restoration was made to have equal-intensity contacts on all teeth in a verifiable centric relations and immediate disclusion of all posterior contacts the moment the mandible moves in any direction from centric relation. In addition, metal occlusion surface on posterior teeth was applied to prevent excessive muscle activation, occlusal trauma and the porcelain fracture.

Dentoalveolar compensation according to skeletal discrepancy in Normal occlusion (전후방적 악골 관계에 따른 치아치조성 보상작용에 관한 연구)

  • The Purpose of this study was to investigate the dentoalveolar compensation according to anteroposterior skeletal discrepancy in normal occlusion and to evaluate cephalometric parameters that quantitatively describe dental compensations. The study consisted of 90 subjects (50 males. 40 females) who were selected among specimens of normal occlusion at Seoul National, University Dental Hospital, Dept. of Orthodontics. Lateral cephalograms in centric occlusion were traced and digitized for each subject. According to the anteroposterior skeletal pattern the sample was divided into three groups. Cephalometric data were analyzed for the three groups using the SPSS program. Independent t-test, correlation analysis and regression analysis were carried out. The results were as fellows: Dentoalveolar compensation was found in upper and lower incisor inclination and occlusal plane inclination. As the mandible located anterior to the maxilla, the maxillary incisors incisors more labially. the mandibular incisors more lingually, and the occlusal plane continued to flatten. The dental parameters most correlated with anteroposterior skeletal discrepancy were L1 to SN and L1 to FH. Among the compensatory dentoalveolar changes, lower incisor inclination was strongly related to the anteroposterior jaw relationship and played au imposrtant role in obtaining a normal incisor relationship U1 to PtGn and L1 to APog were constant irrelevant to anteroposterior skeletal discrepancy.

Full mouth rehabilitation with implant fixed prostheses using POP bow system and 3D printing gothic arch tracer in a patient with unilateral facial nerve palsy (편측성 안면 신경마비 환자에서 3D printing gothic arch tracer와 POP bow system을 이용한 전악 임플란트 고정성 보철 수복)

  • In patients with unstable mandibular movements who are accompanied by involuntary facial muscle movements and asymmetrical changes in the facial features, many difficulties exist in determining the vertical dimension of occlusion, recording centric relation, and transferring information about the occlusal plane. In this case, for a patient with unilateral facial nerve palsy who was edentulous for a long period of time, a CAD-CAM provisional prosthesis was fabricated using a 3D printing customized gothic arch tracer and a prefabricated occlusal plane recorder, the POP bow system, to achieve a stable interarch relationship. Afterwards, a full mouth implant fixed prosthesis was fabricated by delivering an appropriate vertical dimension of occlusion, a reproducible centric relation, and a correct three-dimensional occlusal plane, and continuous maintenance was performed to restore functional and aesthetic oral health to the patient.

  • The purpose of this study is to obtain the enshortening and elongation rate of image in orthopantomograph. The subjects were consisted of 15 dry skulls attached with radiopaque materials on each anatomical points. The length measurements were performed between two points on dry skull, and between two points on film. The results are as follows: 1. The enshortening and elongation rate between two anatomical points (1) ANS↔infraorbital foramen (left:+1.3%, right:+0.7%) (2) ANS↔maxillary tuberosity(left:-11.7%, right:-14.3%) (3) Zygomatic arch length(left:-4.8%, right:-8.9%) (4) first molar↔infraorbital foramen (left:+19.8%, right:+24%) (5) inter-infraorbital foramen length(-21.4%) (6) inter-first molar length (-31.8%) (7) inter-mental foramen length(+1.4%) (8) mental foramen↔mandible angle (left:+3.3%, right:+3.3%) (9) mental foramen↔lingula(left:+8.2%, right:+3.3%) (10) mental foramen↔condyle head(left:+5.2%, right:+4.5%) (11) mandible↔condyle head (left:+15.4%, right:+16.4%) 2. The closer the object is to the occlusal plane and the median line, the smaller it appears.

A STUDY ON THE EFFECT OF TREATMENT IN THE SKELETAL CLASS III MALOCCLUSION PATIENT WITH ANTERIOR CROSSBITE (전치부 반대 교합을 동반한 골격성 3급 부정교합 환자의 치료에 관한 연구)

  • The purpose of this study was to investigate the treatment effects on anterior crossbite patients using chin cap and labiolingual arch appliance. In the present study, I statistically evaluated measurement values on a cephalogram before and after treatment form twenty anterior crossbite cases. The results were as follows: 1. In the craniofacial patterns, cranial base increased after treatment. 2. In the maxillo-mandibular relationship, ANB and Facial convexity significantly increased. 3. In the denture patterns, occlusal plane, Ul to FH and Overjet significantly increased. On the contrary overbite decreased significantly. Lower incisors inclined lingually. 4. In the soft tissue profile changes, lower lip protrusion was signifacantly improved.

  • $CO_2$ laser beam was focused a ZnSe lens onto the center of the occlusal surface of extracted lower molars. K-type thermocouple was contacted with the pulp chamber and the changes of temperature in the pulp chamber during and after the laser irradiation were measured as function of the power of laser beam, the time of laser irradiation and thickness of the sample. An empirical formula for temperature effect was derived from the measured data.

  • Statment of problem : Little in known about the importance of selection of various double crowns as retainer in determining the outcome of treatment with double crown retained removable partial denture. Purpose : To obtain information about the effects and the results of this treatment modality. Material and methods : This study describes 61 double crown retained removable partial dentures worn by 51 patients from Samsung Medical Center, Seoul a time ranging between 6 and 48 months and evaluate occlusal contacts on the denture teeth, denture movement, incidence of denture relining, denture retention, fracture of dentures and abutments, hygiene, residual ridge inflammatory changes, number of lost abutment,. interruption of denture use. Results : One tenth of all the restorations were relined. Restorations in 21 arches fractured repeatedly for various reasons . There was no apparent interrelationship between fractures and the five groups. Conclusion : Good prognoses of removable partial dentures were shown in all groups(Kennedy Classes I, II, III, combination and few remaining abutment).

A CASE REPORT OF ANGLE'S CL.III MALOCCLUSION (Angle씨 제III급 부정교합의 치험일례)

  • A 10 year & 11 month old girl patient who had Angle's Class III malocclusion were treated by chin cap and fixed appliances. The treatment results obtained were as to]lows: 1. The anterior crossbite was corrected. 2. The functional overbite & overjet were established. 3. The favorable molar relationships were achieved. 4. The forward growth of the mandible was restrained. 5. The axial inclination of the upper & lower incisors were changed and the upper dental arch length was increased. 6. The facial profile was improved resulting from the good upper & lower jaw relations. 7. There was no harmful changes on the teeth S the periodontal tissues after treatment. 8. The good occlusal stability was showed after 1 year of retention.

  • The purpose of this study was to investigate the dimensional changes and surface morphology of dentures processed by various polymerization conditions. The measurements were done by taking radiograph and using vernier calipers and each specimen was observed on scanning electron microscope. Results obtained were as follows. 1. The difference of dimensional stability was not recognized between various polymerization conditions(heat-cured resin, pour-type resin, microwave-cured resin, and injection molding resin). 2. There were expansion and shrinkage in the occlusal dimension, shrinkage in the frontal dimension, and expansion in the lateral dimension. 3. Scanning electron microscope pictures of heat-cured resin showed dense and regular surface morphology. 4. Microwave-cured resin surface appeared more regular and smooth than pour-type resin but less dense and more irregular than heat-cured resin. 5. Scanning electron microscope pictures of pour-type resin with the lowest dimensional change showed the most irregular surface morphology.
