한국인의 비만도에 따른 비만관련질환의 유병률 증가 (Increasing Prevalence of Obesity Related Disease for Koreans Associated with Overweight and Obesity)

  • Objective : To develop a boner understanding of the relationship between weight status and the prevalence of obesity related diseases in the Korean population. Methods : The 1998 Korean National Health and Nutrition Survey was used and 10,880 persons who had previously taken health examinations were selected for study. The Korean Society for the Study of Obesity's classification of weight status was used. Hypertension, diabetes mellitus, dyslipidemia, osteoarthritis, chronic heart disease, stroke were included as obesity related disease. A logistic regression model was developed to estimate the prevalence odds ratio by obesity class adjusted for demographic and socioeconomic factors and we converted the odds ratio to a prevalence ratio using the base line prevalence of disease to aid in the interpretation of the ratios. Results : The prevalence of obesity was 26.3% based on the KSSO classification $(BMI\geq25)$. A graded increase in the prevalence ratio was observed with increasing severity of overweight and obesity for all health outcomes with the exception of chronic heart disease in men and stroke in both men and women. With normal weight individuals as the reference, for men who were younger than 50 years, the prevalence ratios were highest for hypertension BMI<23-25: 1.70(95% CI=1.41-2.05), 25$BMI\geq30$: 4.83(95% CI=3.70-5.84). The prevalence ratios for dyslipidemia were as high as hypertension, but were lower than hypertension for diabetes mellitus and osteoarthritis. Prevalence ratios generally were greater in younger adults. The prevalence of having 2 or more obesity related diseases increased with weight status category, except in people who were older than 50 years. Conclusions : Based on results, obesity is an increasingly important health problem in Korea and the disease burden increases according to weight status. For Korean adults, the strongest relationship was seen between weight status and hypertension and dyslipidemia. In older people the impact of excess weight and obesity is stronger than that seen in younger people. Increased efforts in the study of obesity and prevention and treatment of obesity and obesity related disease are required.

연령에 따른 비만과 치주질환과의 관련성 (The relationship of obesity and periodontal disease by age)

  • Objectives : The purpose of the study is to find the association between obesity and periodontal disease by age in adults. Methods : The subjects were 5,728 adults in the first year of the Fifth National Health and Nutrition Examination Survey in 2010. The questionnaire included oral examinations, periodontal disease, and body mass index screenings. Chi-square test was performed to confirm the periodontal disease. Results : The impact of obesity and waist circumference on body mass index (BMI) showed high risk of periodontal diseases. The risk of periodontal disease according to BMI by age was closely related to obesity ranged form 35 to 59 yeard old. The increased waist circumference and those who were in 35 to 59 and over 60 years old had a significant association with the periodontal disease risk. Conclusions : Obesity and periodontal disease are closely related factors. Those aged from 35 to 59 are the high risk groups of potential obesity and periodontal disease. It is very important to control well-balanced nutrition and physical activity that can prevent the progression of periodontal disease.

Risk Factors for Sarcopenia, Sarcopenic Obesity, and Sarcopenia Without Obesity in Older Adults

  • Background: Muscle undergoes change continuously with aging. Sarcopenia, in which muscle mass decrease with aging, is associated with various diseases, the risk of falling, and the deterioration of quality of life. Obesity and sarcopenia also have a synergy effect on the disease of the older adults. Objects: This study examined the risk factors for sarcopenia, sarcopenic obesity, and sarcopenia without obesity and developed prediction models. Methods: This machine-learning study used the 2008-2011 Korea National Health and Nutrition Examination Surveys in the analysis. After data curation, 5,563 older participants were selected, of whom 1,169 had sarcopenia, 538 had sarcopenic obesity, and 631 had sarcopenia without obesity; the remaining 4,394 were normal. Decision tree and random forest models were used to identify risk factors. Results: The risk factors for sarcopenia chosen by both methods were body mass index (BMI) and duration of moderate physical activity; those for sarcopenic obesity were sex, BMI, and duration of moderate physical activity; and those for sarcopenia without obesity were BMI and sex. The areas under the receiver operating characteristic curves of all prediction models exceeded 0.75. BMI could predict sarcopenia-related disease. Conclusion: Risk factors for sarcopenia-related diseases should be identified and programs for sarcopenia-related disease prevention should be developed. Data-mining research using population data should be conducted to enhance the effectiveness of early treatment for people with sarcopenia-related diseases through predictive models.

Obesity and chronic kidney disease: prevalence, mechanism, and management

  • The prevalence of childhood obesity is increasing worldwide at an alarming rate. While obesity is known to increase a variety of cardiovascular and metabolic diseases, it also acts as a risk factor for the development and progression of chronic kidney disease (CKD). During childhood and adolescence, severe obesity is associated with an increased prevalence and incidence of the early stages of kidney disease. Importantly, children born to obese mothers are also at increased risk of developing obesity and CKD later in life. The potential mechanisms underlying the association between obesity and CKD include hemodynamic factors, metabolic effects, and lipid nephrotoxicity. Weight reduction via increased physical activity, caloric restriction, treatment with angiotensin-converting enzyme inhibitors, and judicious bariatric surgery can be used to control obesity and obesity-related kidney disease. Preventive strategies to halt the obesity epidemic in the healthcare community are needed to reduce the widespread deleterious consequences of obesity including CKD development and progression.

Serum branch chain amino acids and aromatic amino acids ratio and metabolic risks in Koreans with normal-weight or obesity: a cross-sectional study

  • Objectives: Metabolic disease is strongly associated with future insulin resistance, and its prevalence is increasing worldwide. Thus, identifying early biomarkers of metabolic-related disease based on serum profiling is useful to control future metabolic disease. Our study aimed to assess the association of serum branched chain amino acids (BCAAs) and aromatic amino acids (AAAs) ratio and metabolic disease according to body mass index (BMI) status among Korean adults. Methods: This cross-sectional study included 78 adults aged 20-59 years in Korea. We compared serum amino acid (AA) levels between adults with normal-weight and adults with obesity and investigated biomarkers of metabolic disease. We examined serum AA levels, blood profile, and body composition. We also evaluated the association between serum AAs and metabolic-related disease. Results: The height, weight, BMI, waist circumference, hip circumference, waist-hip-ratio, body fat mass, body fat percent, skeletal muscle mass, systolic blood pressure, and diastolic blood pressure were higher in the group with obesity compared to normal weight group. The group with obesity showed significantly higher levels of BCAA, AAA, and BCAA and AAA ratio. Further, BCAA and AAA ratio were significantly positively correlated with triglyceride, body weight, and skeletal muscle mass. The evaluation of metabolic disease risks revealed an association between the ratios of BCAAs and AAAs, hypertension, and metabolic syndrome. Conclusions: Our study is showed the associations between BCAA and AAA ratio, obesity, and obesity-related diseases using various analytical approaches. The elevated BCAA and AAA ratio could be early biomarkers for predicting future metabolic diseases in Korean population.

한국인 성인 비만의 사회경제적 비용 (Socioeconomic Costs of obesity for Korean Adults)

  • Objective : To estimate the socioeconomic costs of obesity in Korea,1998. Methods : The 1998 National Health and Nutrition Examination Survey(1998 NHNES) data was used and 10,880 persons who had taken health examinations were selected for study. Essential hypertension, NIDDM(non insulin-dependent diabetes mellitus), dyslipidemia, osteoarthritis, coronary heart disease, stroke were included as obesity related disease. The data of direct costs of obesity was obtained from the National Federation of Medical Insurance. The category of indirect costs was the loss of productivity caused by premature death and admission, time costs, traffic costs, nursing fees due to obesity. Multiple logistic regression model was developed to estimate prevalence odds ratio by obesity class adjusted demographic and socio-ecnomic factors and calculate PAF(Population Attributable Fraction) of obesity on obesity related disease. And we finally calculated the socioeconomic costs of obesity in relation to BMI with PAF. Results : The direct costs of obesity were 2,126 billion${\sim}965$ billion Won in considering out of pocket payment to uninsured services, and the indirect costs of obesity were 2,099 billion${\sim}1,086$ billion Won. Consequently, in considering out of pocket payment to uninsured services, the socioeconomic costs of obesity were 4.225 billion${\sim}2,050$ billion Won, which corresponded to about $0.094%{\sim}0.046%$ of GDP and $1.88%{\sim}0.91$ of total health care costs in Korea. Conclusions : Obesity represents a major health problem with significant economic implications for the society. This results are conservative estimates as far as all obesity related disease and all health care and indirect costs were not included due to missing information. further studies are needed to caculate socioeconomic costs of obesity more exactly.

롱코비드가 비만에 미치는 영향과 그에 대한 한의학의 역할 (The Effect of Long Coronavirus Disease on Obesity and the Role of Korean Medicine)

  • While the Coronavirus disease 2019 (COVID-19) pandemic is gradually turning into an endemic disease, concerns about post COVID-19 conditions (Long COVID) are emerging. Obesity is a major risk factor for severe complications of COVID-19, and COVID-19 has a wide range of effects on obesity and metabolic function. This paper aims to examine the interaction between COVID-19 and obesity, the effects and mechanisms of long COVID on obesity, and the role of Korean medicine on long COVID-related obesity. Obesity may worsen with cardiometabolic damage and psychosocial insecurity during COVID-19 and long COVID-induced neuroinflammation, systemic inflammation, mitochondrial dysfunction, and hypoxia also may aggravate obesity. Korean Medicine treatments, which have been widely used to treat obesity, have the potential to improve obesity in the era of long COVID by intervening in these mechanisms.

비만치료와 관련된 산삼복합약침요법에 대한 연구 동향 고찰 (Review on Studies of Wild Ginseng Complex Pharmacopuncture Related to Obesity Treatment)

  • Objectives: The purpose of this study was to review the studies of the wild ginseng complex pharmacopuncture related to obesity treatment. Methods: We searched the papers with key words of 'wild ginseng complex pharmacopuncture', 'wild ginseng pharmacopuncture', 'obesity', 'fat', 'weight' in Oriental medicine Advanced Searching Integrated System, KRpia, Koreanstudies Information Service System, Research Information Sharing Service, KoreaMed and PubMed, Scopus. We classified the papers by year, content and study type. Results: There were 7 studies about the wild ginseng pharmacopuncture related to obesity treatment. Cell studies were excluded because they were not related to obesity. Analysis of 3 animal experiments and 4 clinical studies were conducted to describe each research subject, method, and research results. Conclusion: More interest and further research will be needed on wild ginseng pharmacopuncture related to obesity treatment in the Korean medicine to achieve clinical application and to develop treatment protocols for the obesity disease.

과체중 및 비만 여성에서 한방비만변증에 따른 체성분 및 대사관련 지표의 상관성 (Relation between Obesity Pattern Identification and Metabolic Parameters in Overweight and Obese Women)

  • Objectives: We conducted this study to analysis obesity pattern and obesity related blood parameters. Methods: A total of 64 overweight and obese (body mass index [BMI] ${\geq}23cm/kg^2$) women who had no other disease was recruited. Body composition and obesity related blood parameters were measured. Also subjects were given and filled out the Obesity pattern identification questionnaire. We analyzed the differences of body composition and blood parameters and measured correlations of BMI and blood parameters in each obesity pattern. Results: The distribution of obesity pattern was liver depression (35.6%), food accumulation (47.5%) and deficiency (pi and yang deficiency, 22.0%), in order. There were no significant differences age, body composition and obesity related blood parameters between obesity patterns. BMI and obesity related blood parameters, however, showed significant correlations depending on obesity patterns. Conclusions: We concluded that correlations between BMI and obesity related blood parameters were differed depending on obesity patterns.

  • Obesity is a metabolic disease associated with multiple hormonal abnormalities. Therefore, obesity management aims at balancing these endocrine malfunctions nowadays. Although many studies proved interactions of hormones related with obesity, there are still lots of controversies. Most of these malfunctions are more pronounced in central, visceral obesity than in peripheral obesity. Recently, it is revealed that a central lesion of endocrine malfunction in human visceral obesity is probably related with a hypersensitivity of hypothalamopituitary-adrenal(HPA) axis. Probably associated with this axis, Insulin and cortisol promote lipid accumulation by expressing lipoprotein lipase activity, while sex hormones and growth hormone exert the opposite effects. Also reviewed was thyroid hormone which is closely related with thermogenesis. Serotonin is prescribed as antidepressant and it is applied to some eating disorders. Recently, leptin made in fat deposit also took attentions in terms of regulator of appetite and messenger of sex signal.
