Evaluation of Secondary Obesity (이차성 비만의 진단)

  • While the rising epidemic of obesity is primarily attributed to a sedentary lifestyle, poor dietary habits, and the aging of the population, secondary causes of obesity generally go undetected and untreated. These include endocrinological disorders, such as Cushing's syndrome, polycystic ovary syndrome, hypothalamic obesity, hypogonadism, and hypothyroidism, as well as genetic, syndromic, and drug-related obesity. We present an overview of the major disorders associated with obesity, highlighting the pathophysiologic mechanisms and discussing the diagnostic and treatment strategies that are most helpful to practicing physicians in recognizing and treating these generally under-detected and undertreated disorders.

Assessment of Anthropometric Obesity Indexes by Correlation with Body Composition (비만치료에 유용한 단순비만지표의 선별)

  • Object : Waist circumference(WC), waist-hip ratio(WHR), waist-stature ratio(WSR), and body mass index(BMI) are commonly used for evaluating obesity. This Research were done to determine what is more sensitive obesity indexes(WC, WHR, WSR, BMI) Correlated with body composition such as body fat mass, body fat(%), visceral fat area, and fat free mass. And what is more sensitively correlated obesity indexes with % changes of body composition during weight reduction treatment. Methods : This clinical retrospective research were carried out 127 cases of female obese outpatients with weight reduction treatment during 1 month. Bioelectrical impedence analysis(for body composition) and body size(for anthropometric obesity indexes) were estimated in pre-treatment and post-treatment to evaluate the obesity indexes. Pearson correlation coefficients were used to select useful obesity index. Result & Conclusion : BMI is useful index for diagnosis and evaluation of obesity. WSR is sensitively correlated with visceral fat area and body fat(%). So, WSR is useful index for evaluating abdominal obesity and risk factors of metabolic syndrome. WC is correlated with both body fat mass and fat free mass. WHR is not optimal for diagnosis and evaluation of obesity.

Development of a Web-Based Obesity Management Program for Elementary School Students (초등학생을 위한 웹기반 비만관리 프로그램 개발)

  • Purpose: The purpose of this study is to develop a 12-week web-based obesity management program for elementary school students and to evaluate the appropriateness of the developed program from experts and users. Methods: The development of a web-based obesity management program proceeded through the stages of analysis, design, development, and evaluation. After the application of the web-based obesity management program, the evaluation of appropriateness were conducted for knowledge of obesity, obesity control behavior and satisfaction of the program. Collected data has been analyzed using SPSS 19.0. Results: The name of the program developed by this study is healthful weight management and its URL is http://ihealth.or.kr. There were 6 major menus and 18 sub menus including diet, exercise and behavior modification in this program. This study showed that this web-based obesity management program had high satisfaction for a system and increase knowledge of obesity and obesity control behavior for obese children. Conclusion: We expect the web-based obesity management program for elementary school students developed by this study will be able to lead the practice and participation of obese children and decrease the degree of obesity.

Nutrition Counseling and Tailored Dietary Intervention for Patients with Obesity (비만 환자에서의 맞춤형 영양 상담과 식사 치료)

  • Nutrition counseling and dietary intervention are essential to obesity management because weight reduction is the consequence of negative energy balance. The first step of the nutrition counseling in patients with obesity is thorough evaluation of the nutritional status. During the nutritional evaluation, amount of energy consumption, dietary habits, and medical and socioeconomic factors influencing diets should be evaluated. Diet interventions including low calorie diet, low fat diet, low carbohydrate diet, and high protein diet are all effective in weight reduction as long as decrease in energy consumption is accompanied. Amount of energy restriction and choice of diet interventions should be individualized based the medical condition and characteristics of each patient.

  • Early evaluation of obesity is important. Obesity is defined as an excessive accumulation of fat that causes harm to health. Among patients who visit a hospital for weight-related issues, diseases other than body fat gain may be present; however, people often cannot distinguish between these issues and concerns. Therefore, among patients who visit the hospital with weight gain concerns, it is necessary to determine whether the weight gain is actually the result of excessive fat accumulation. After being diagnosed with obesity, the cause and degree of obesity, the amount and distribution of body fat, and the degree of risk should be evaluated. Additionally, obesity-related complications should be identified and, even if there are no complications, all related risk factors should be evaluated and managed. In all these processes, diagnostic methods such as history taking, physical examination, body fat measurement, blood tests, and imaging tests are necessary, but history taking and physical examination, which can provide a lot of information from the beginning, should not be overlooked.

  • It has recently become known that the social issue of obesity, caused by increased caloric intake and lack of exercise, is a risk factor in the cause of various adult diseases. Above all, to prevent or cure obesity, we must accurately evaluate the degree of obesity, and we have used BML, WHR, and waist measurements for this purpose. In this paper, we propose an obesity evaluation system based on fuzzy inference using BML and waist measurement. For this purpose, we decided reasoning rule and membership function about BML and waist measurements. The inference result is presented in a descriptive sentence.

Association between beverage intake and obesity in children: The Korea National Health and Nutrition Examination Survey (KNHANES) 2013-2015

  • BACKGROUND/OBJECTIVES: Numerous researches have studied the association between sugar intake and obesity of children in many countries. This study was undertaken to investigate the association between beverage intake and obesity of children by reviewing a database for total sugar contents established in all foods and presented in a nutrition survey by the Korea National Health and Nutrition Examination Survey (KNHANES). SUBJECTS/METHODS: Data of 1,520 children aged 6-11 years in the 6th KNHANES (2013-2015) were analyzed for this study. A database for total sugar intake comprises the total sugar contents of all foods included in the results of a nutrition survey using the 24-hour recall method of 6th KNHANES. Beverages were categorized into carbonated beverages, fruit & vegetable drinks, other drinks, tea, and coffee. RESULTS: The average daily beverage intake of all children was 131.75 g/day, and the average daily total sugar intake in beverages was 13.76 g/day. Carbonated beverages had the highest intake rate (58.85 g/day) and also ranked highest for sugar intake (6.36 g/day). After adjusting for confounding variables, the odds ratio for obesity in children with beverage intake of ${\geq}200mL/day$ significantly increased by 1.83 times (95% CI, 1.11-3.00) as compared to children with beverage intake of < 200 mL/day. Also, a significant increase was observed in the odds ratio for obesity in total children (2.41 times; 95% CI, 1.35-4.33) and boys (3.15 times; 95% CI, 1.53-6.49) with carbonated beverage intake of ${\geq}200mL/day$ when compared with children who consumed < 200 mL/day. CONCLUSION: A positive association is observed between beverage intake and obesity in Korean children. In particular, an intake of carbonated beverages has a positive correlation with childhood obesity in boys. This study can therefore be used as scientific evidence for reducing sugar, and for the continuous management and research on beverages.

The Survey on Construction of Obesity Management Program for Primary, Middle and High Schools (초 . 중 . 고등학교 비만관리프로그램 구축에 관한 연구)

  • This study attempts to develop a program for obesity prevention and management for students in primary, middle, and high school and was carried based on the program development process as suggested by Ewles and Simnett. Data were collected from September 1st to September 30th, 2000. The study method is as follows: 1) Reviewing the present obesity control programs practiced in primary, middle, and high school (196 schools total) in metropolitan, medium-sized, and small cities, 2) Surveying obesity students'(682 students total) perception of the obesity management in their school. 3) Reviewing the annual plan for obesity in primary, middle, and high schools 4) Analyzing the educational data (CD, VIDEO) for obesity The results are as below: I. The media researched related to obesity were mainly CD and video, aimed primarily primary school students and are biased in nature. 2. The results of how to manage obesity in students: the difficulty of obesity education in the primary school, is the arousal of a sense of shame among obese students and the lack of various educational data are the most. In the middle and high schools, a lack of various educational data, and no time or lack of time were prevailing concerns. Paramount in managing obesity is weight control, as answered by majority of primary, middle, and high school students. The major problem of obesity management is the non-cooperation of children. 3. Asked whether the weight controlling aspect of health care class was helpful or not, 51.4% of primary school students, and 71.6% of middle and high school students answered not. The most popular educational method used in health care class is the computer, with a 56.4% approval rating. Most 43.5% of respondents answered that they wanted the content of obesity management program to focus on how to lose weight. Therefore, based on the results, the obesity management program is constructed to the steps of assessment, planning, implementation, and evaluation. Assessment includes administering surveys on health needs among students and their parents and physique tests. Planning includes the set-up of long term and short term educational goal and the establishment of an operation system. Implementation includes education, exercise, behavior modification, and health service programs. Evaluation includes step-by-step evaluations for obesity reduction. After this study is conducted, the following should be further researched: More practical and systematic obesity management programs should be developed for the evaluation results. In addition, it is important to develop an appropriate media for obesity management the primary, middle and high school students.

Effective and appropriate use of weight loss medication in pediatric obesity: a narrative review

  • Over the past few decades, there has been a notable increase in the incidence of pediatric obesity, which is a significant public health concern. Children who are obese have a greater risk of type 2 diabetes, hypertension, dyslipidemia, polycystic ovary syndrome, obstructive sleep apnea, and adult obesity. Lifestyle modification therapy is typically the initial approach to treat pediatric obesity. For patients who do not achieve success with lifestyle modification therapy alone, pharmacotherapy is the next logical treatment option. When selecting an anti-obesity medication (AOM), it is essential to first ascertain the medical background of the patient, including current medications and obesity-associated comorbidities. Evaluation of obesity phenotypes in patients may also be beneficial. AOMs for pediatric obesity include metformin, orlistat, glucagon-like peptide 1 agonists, phentermine, and the phentermine/topiramate combination. Sufficient lifestyle modification therapy should be administered before considering pharmacotherapy and continued after the initiation of AOM. To ensure healthy development, monitoring growth and puberty development during anti-obesity treatments is essential.

The Development and Evaluation of a Motivation-Strengthening Obesity Management Program for Obese Subjects with Visual Impairment (시각장애 비만대상자를 위한 동기강화 비만관리프로그램의 개발 및 효과평가)

  • Purpose: The purpose of this study was to develop and apply a motivation-strengthening obesity management program for obese subjects with visual impairment and evaluate its effects. Methods: The program development and evaluation periods were from March to August 2018. The study design was an experimental study of non-equality, control repeat design. 25 subjects in the experimental group participated in the 12-week obesity management program and the 26 people in the control group did not. Results: The motivation-strengthening obesity management program included stretching exercises for 30 minutes in the morning five times a week. In addition, counseling, education, and group discussions were continued once a week for motivational strengthening. Special custom exercises for blind patients were also completed once a week simultaneously. Both of these activities were continued for 50 minutes a week for a total of 12 weeks. Measurements were recorded periodically to identify the sustained effects of the program. The obese subjects with visual impairment improved their knowledge of obesity, perceived disability, self-efficacy of exercise, and quality of life related to obesity through the motivational obesity management program. Waist circumference, systolic blood pressure, diastolic blood pressure, and fasting blood sugar were also improved in obese subjects. Conclusion: The study findings indicate that this program could be an active intervention for the control of weight gain in participants with visual impairment. Therefore, the motivation-strengthening program based on the ADDIE (Analysis, Design, Development, Implementation& Evaluation) model could be an effective strategy for better health outcomes of obese subjects with visual impairment.
