한국 노인의 비만도에 따른 만성질환 및 영양섭취 실태조사 - 국민건강영양조사 2016~2018 자료를 이용하여 - (Evaluation of Chronic Disease and Nutritional Intake by Obesity of Korean Elderly - Data from Korea National Health and Nutrition Examination Survey 2016~2018 -)

  • The purpose of this study was to investigate the current status of chronic diseases according to obesity in the elderly older than age 65 using data from the 7th Korea National Health and Nutrition Examination Survey (2016~2018). The subjects of the survey were 3,245 elderly older than age 65 who participated in the health survey and nutrition survey, 45.8% of the subjects were males and 54.2% of the subjects were female. 37.5% of all the elderly were obese, and the females (42.4%) were more obese than the males (31.8%) (p<0.001). Diabetes, hypertension, and hypertriglyceridemia had a similar prevalence of 24.6%, 63.0%, and 12.7% in males and females, respectively. Hypercholesterolemia was higher in the elderly females (44.2%) than in the elderly males (24.9%) (p<0.001). Through the logistic regression analysis, it was found that the prevalence of chronic diseases was higher in obesity than normal in elderly males and females (p<0.001). The total food intake increased from 'under weight' to 'obesity' in males (p<0.01) and females (p<0.001). In the case of the elderly males, the higher the degree of obesity, the higher the energy intake (p<0.05), and 'pre-obesity' consumed the most energy in elderly females. As a result of this study, the higher the obesity rate of the elderly, the higher the prevalence of chronic diseases.

체질에 따른 스트레스와 비만도가 대사증후군 유병률에 미치는 영향 (Effects of Stress and Obesity on the Prevalence of Metabolic Syndrome to the Sasang Constitution)

  • Objectives This study is to identify the relationship between stress, obesity and metabolic syndrome (MetS) by Sasang constitution (SC) and to use it as a basic data for the customized healthcare system of chronic disease management. Methods In a cross-sectional study conducted in a rural area of the Gyeongju area in 2014, we extracted data on 1,847 people using data from Korean medicine Data Center (KDC). The SC types diagnosed by Sasang constitutional medicine doctor. The stress was measured by the psychosocial well-being index questionnaire, and the obesity was defined by the body mass index. Results In all SC type, the stress and obesity was associated with MetS. There was a significant difference in the prevalence and trend of MetS according to presence of stress and obesity. The prevalence of MetS was the highest in Taeumin with all the stress and obesity, in order of Soeumin and Soyangin. Regardless SC types, the stress was associated with higher risk of MetS in the obese but not in the nonobese. Conclusion The results of this study indicate that further research is necessary to the SC types for the management of the Metabolic syndrome according to stress and obesity.

체성분 분석 변수와 X-선 소견을 근거로 한 비만과 요통의 관계 연구 (The Study on Relation of Obesity and Low Back Pain Based on Body Composition using Segmental Bioelectrical Impedance Analysis and Radiological Parameter)

  • Objectives : This study was performed in order to investigate the relation of body composition analysis and radiological parameter(lumbosacral angle, lumbar lordortic angle, lumbar gravity line). Methods : This study was carried out with the data from comprehensive medical testing. 75 subject aged 20-59 performed the segmental bioelectrical impedance analysis, questionnaire. And lumbosacral angle, lumbar lordortic angle and lumbar gravity line were measured in the standing position x-ray. Then we analyzed the data. Results : Low back pain(LBP) prevalence in high obesity index(Body Mass Index(BMI), Percentage of Body Fat(PBF), Waist Hip Ratio(WHR)) group was higher than LBP prevalence in normal obesity index group(p<0.01). In LBP group, lumbosacral angle, lumbar lordortic angle were significantly lager than Non-LBP group(p<0.001). And 75% of LBP group indicated abnormal lumbar gravity line ratio(0.67 < Normal lumbar gravity line ratio <1.00). When it comes to analyze relation between obesity index and radiological parameter, no-significant change was seen. Conclusions : This study carried as following research after the study on relation of obesity, LBP and trunk muscle strength. Results from this investigation showed positive correlation between obesity and LBP prevalence. But obesity index didn't indicate significant correlation with structural changes of lumbar vertebrae. When considering prior research, trunk muscle strength changes were more related to LBP prevelence in obese people. This results are expected to explain causes of LBP in obese group.

미국이민 중년 여성의 비만 관련요인과 만성질환에 미치는 영향 (Factors Related to Obesity of Mid-year Korean American Women and Their Prevalence of Chronic Diseases)

  • Purpose: The purpose of this study was to investigate factors related to obesity of mid-age women and to examine the relationship between obesity and chronic diseases including diabetes, hypertension, and heart disease. Methods: Using data from Korean American adults living in California, we analyzed the health behavior (diet, exercise, smoking, and binge drinking) and psychological stress of obese women with body mass index ${\geq}25$ by using chi-square test. Logistic regression was used to investigate independent effects of obesity on chronic diseases, after controlling for risk factors. Results: The prevalence of obesity appeared about 16.9%. The Obese group was less likely than the non-obese group to eat vegetables and more likely feel psychological stress. There were no significant differences in smoking, alcohol intake, and physical exercise between the two groups. Obesity was strongly related to chronic diseases (OR=4.642, 95% CI=1.328-16.222). Conclusion: This study suggests that obesity of mid-age women could be reduced by encouraging health behavior such as eating vegetables daily, performing physical exercise regularly, and taking care of emotional stress. Diet and physical activity interventions and emotional supports should be developed for weight loss and prevention of weight gain in mid-age women.

Early-life exposure to endocrine disrupting chemicals associates with childhood obesity

  • Increasing prevalence of childhood obesity poses threats to the global health burden. Because this rising prevalence cannot be fully explained by traditional risk factors such as unhealthy diet and physical inactivity, early-life exposure to endocrine disrupting chemicals (EDCs) is recognized as emerging novel risk factors for childhood obesity. EDCs can disrupt the hormone-mediated metabolic pathways, affect children's growth and mediate the development of childhood obesity. Many organic pollutants are recently classified to be EDCs. In this review, we summarized the epidemiological and laboratory evidence related to EDCs and childhood obesity, and discussed the possible mechanisms underpinning childhood obesity and early-life exposure to non-persistent organic pollutants (phthalates, bisphenol A, triclosan) and persistent organic pollutants (dichlorodip henyltrichloroethane, polychlorinated biphenyls, polybrominated diphenyl ethers, per- and polyfluoroalkyl substances). Understanding the relationship between EDCs and childhood obesity helps to raise public awareness and formulate public health policy to protect the youth from exposure to the harmful effects of EDCs.

DYNAMIC MODEL PREDICTING OVERWEIGHT AND OBESITY IN KOREAN ADOLESCENTS

  • Adolescent obesity has a high risk of developing into adult obesity and may cause many psychological problems. This paper aims at giving a mathematical model for the obesity of Korean adolescents and predicting how much the prevalence of obesity among adolescents will increase using real data. We estimate that the obesity rate of boys will increase until about 28 ~ 29% in 2025.

서울시내 사춘기 여학생의 비만실태와 식이섭취양상 및 일반환경 요인과 비만과의 관계 (Prevalence of Obesity among Adolescent Girls in Seoul and its Relationship to Dietary Intakes and Environmental Factors)

  • The prevalence of obesity and its related factors in adolescent girls in Seoul area were investigated from April, 18 to June, 29 1985 bymeans of anthropometry, dietary intakes and general environmental factors. Nine hundred and sixty junior high school girls were selected and invterviewed for 24-hour food intakes. Weight, height, a 우 skinfold thickness(triceps, abdominal) were measured and information on the general backgroun of the subjects were obtained by questinnaire. Subjects were divided into six groups according to their relative weights. It was observed that majority of the parents were under fifty years old and had high school education . In general, father was older and higher in educational levels than mother. Two thirds of subjects lived in their own houses and had three to four siblings. AVerage weight and height of subjects were 46.0kg and 154.4cm, which were higher than Korean natianl average. Average relative weight was 29.75 and average Kaup index was 1.925. Skinfold thickness was 13.4mm fo rtricepts and 16.4mm for abdominal . Prevalence of obesity was 12.9% and 4.8% by criteria of relative weight and skinfold thickness. Average intakes of most of nutrients except iron were acceptable but about one third of the subjects had less than 2/3 of RDa for calcium, iron, vitamin A, and riboflavin. As a whole, prevalence of obesity in this study was higher than those observed in the past surveys. Relationship of dietary intakes and general environmental factors that seemed to influence the obesity were integrated as such. As parents educational levels were higher, it tended to be higher in living standard and have fewer family members and siblings. These may influence more nutrient intakes so that fatness of the subjects seemed to be greater. In the meantime, parents fatness was closely related to those of children indicating an involvement of genetic factors. And also, food habits of individuals such as frequency of snack and bedtime snack were related to fatness significantly.

한국 소아 청소년에서 비만 및 대사 이상의 유병률 (The Prevalence of Obesity and Metabolic Abnormalities in Korean Pediatric Population)

  • 목 적: 식생활의 서구화와 운동량의 감소로 우리 나라에서 소아 비만은 증가하고 있는 추세이며 비만아에서는 고혈압, 이상 지질 혈증, 비알코올성 지방간 및 당뇨 등의 합병증이 발생할 수 있다. 이에 저자들은 비만의 유병률 및 비만아에서 동반되는 대사 이상의 유병률을 알아보고 비만의 정도에 따른 대사 이상의 유병률 변화에 대하여 비교해 보고자 하였다. 방 법: 2006년 5월부터 2008년 10월까지 노원 을지병원에서 학생 건강 검진을 받은 8,880명의 초 중 고등학생의 기록을 분석하였다. 비만의 유병률은 체질량 지수와 비만도로 측정하였으며 비만 및 과체중인 소아 청소년 각각 588명과 488명을 대상으로 AST 수치, ALT 수치, 공복 혈당, 총콜레스테롤을 검사하였다. 각 검사 항목별의 유병률과 비만의 정도에 따른 유병률 변화에 대하여 t-test, ANOVA test, chi-square test를 이용하여 분석하였으며 p-value 0.05 미만을 유의한 것으로 판정하였다. 결 과: 체질량 지수에 의해서 비만의 유병률은 7.2% 이었으며 남아 7.8%, 여아 6.5%가 비만이었고 과체중의 유병률은 25.0%로 남녀 각각 28.8%, 21.0%였다. 비만도에 의해서는 12.3%의 소아 청소년이 비만이었으며 경도 비만 6.3%, 중등도 비만 5.0%, 고도 비만 1.0% 이었고 남아와 여아 각각 15.0%, 9.4%가 비만하였다. 고콜레스테롤혈증, AST 수치 상승, ALT 수치 상승, 고혈압의 유병률은 비만의 정도가 증가함에 따라 높아졌으나(p<0.05) 공복 혈당 상승은 통계적으로 유의하지 않았다. ALT 수치 상승의 유병률은 17.7%로 ALT 정상군보다 ALT 상승군에서 고콜레스테롤혈증(24.7%, 19.1%), 고혈압(11.0%, 9.9%), 공복 혈당 상승(2.0%, 5.2%)의 유병률이 높았다(p<0.05). 결 론: 소아 비만의 상당수에서 대사 이상이 동반되며 이로 인하여 성인에서처럼 제2형 당뇨, 심혈관계 질환이 발생할 위험이 증가하게 된다. 본 연구에서는 비만아에서 비만의 정도가 증가할수록 고콜레스테롤혈증, 간 효소치 상승, 고혈압의 유병률이 증가하였으며 소아 비만이 성인 비만으로 이행할 확률이 높음을 생각하면 소아 청소년 시기에 비만을 조기 발견하여 치료, 관리하는데 역점을 두어야 하겠다.

Comparing Non-Communicable Disease Risk Factors in Asian Migrants and Native Koreans among the Asian Population

  • Assessing the health of international migrants is crucial in the Republic of Korea, Asia, and even worldwide. We compared the risk factors for non-communicable diseases among Asian migrants in Korea and the Korean population. This cross-sectional (2015) and longitudinal (2009-2015) observational study comprised a population-wide analysis spanning 2009 to 2015. Asian migrants (n=987,214) in Korea and Korean nationals (n=1,693,281) aged ≥20 were included. The Asian migrants were classified as Chinese, Japanese, Filipino, Vietnamese, and other. The prevalence of risk factors for non-communicable diseases (current smoking, obesity, diabetes mellitus, and hypertension) were analyzed. Regarding the age-adjusted prevalence, direct age standardization was conducted separately by sex using 10-year age bands; the World Standard Population was used as the standard population. Among the participants aged ≥20, the age-adjusted prevalence of current smoking was higher among Chinese and other Asian migrant men than among Korean men and women (p<0.001 and p<0.001, respectively). The age-adjusted prevalence of obesity was higher among Chinese, Filipino, and other Asian migrant women than in Korean women (p<0.001, p=0.002, and p<0.001, respectively). Among the participants aged 20-49, the age-adjusted prevalence of diabetes mellitus and hypertension was higher in Filipino migrant women than in Korean women (p=0.009 and p<0.001, respectively). Current rates of smoking and obesity were worse among Asian migrants of specific nationalities than among native Koreans. The health inequalities among Filipino migrant women in Korea, especially those aged 20-49, should be addressed.

Association of daily carbohydrate intake with intermuscular adipose tissue in Korean individuals with obesity: a cross-sectional study

  • BACKGROUND/OBJECTIVES: The prevalence of obesity, a worldwide pandemic, has been increasing steadily in Korea. Reports have shown that increased intermuscular adipose tissue (IMAT) is associated with an increased risk of cardiovascular disease, independent of body mass index. However, the relationship between dietary intake and IMAT accumulation in the Korean population remains undetermined. The objective of this study was to evaluate regional fat compartments using advanced magnetic resonance imaging (MRI) techniques. We also aimed to investigate the association between IMAT amounts and dietary intake, including carbohydrate intake, among Korean individuals with obesity. SUBJECTS/METHODS: This cross-sectional study, performed at a medical center in South Korea, recruited 35 individuals with obesity (15 men and 20 women) and classified them into 2 groups according to sex. Anthropometry was performed, and body fat distribution was measured using MRI. Blood parameters, including glucose and lipid profiles, were analyzed using commercial kits. Linear regression analysis was used to test whether the IMAT was associated with daily carbohydrate intake. RESULTS: Carbohydrate intake was positively associated with IMAT in all individuals, with adjustments for age, sex, height, and weight. No significant differences in blood indicators were found between the sexes. CONCLUSIONS: Regardless of sex and age, higher carbohydrate intake was strongly correlated with greater IMAT accumulation. This suggests the need to better understand sex differences and high carbohydrate diet patterns in relation to the association between obesity and metabolic risk, which may help reduce obesity prevalence.
