The Ornithine Transcarbamylase (OTC) Deficiency Identified by a Novel Mutation (새로운 유전자 돌연변이로 확진된 Ornithine Transcarbamylase (OTC) 결핍증 1례)

  • A urea cycle disorder is a condition caused by a defect of the enzymes in the urea cycle, and deficiency of ornithine transcarbamylase (OTC), which converts carbamoyl phosphate and ornithine into citrulline, is the most common type of the disorder. OTC deficiency induces the accumulation of precursors of urea, ammonia, and glutamine, leading to neurological symptoms including hypotonia, respiratory failure, seizure, lethargy, and coma and sometimes to death. Because OTC deficiency is inherited in an X-linked manner, typical symptoms such as vomiting, poor feeding, and lethargy appear mainly in male neonates. We recently had a case that presented with neonatal onset lethargy, vomiting, and apnea in a 4-day-old boy. He was diagnosed with OTC deficiency by biochemical phenotype, including hyperammonemia and an increased orotic acid level in the urine. Genetic analysis of the OTC gene showed a novel mutation c.780_781insCAGGCAGTGT (p.Ile261Glnfs*35). He was treated for hyperammonemia using continuous venovenous hemofiltration (CVVH) at 118 hours after birth. After 4 days of CVVH, his consciousness and blood ammonia concentration were normalized, and he was discharged at the age of 53 days. At around 12 months of age, bilateral femur fractures and osteomyelitis occurred in this patient. Two months after the fracture, he died of septic shock, insulin-resistant hyperglycemia, and multi-organ failure.

  • Ornithine transcarbamylase (OTC) deficiency is a rare X-linked genetic disorder of urea synthesis in newborns. It is the most common urea cycle disorder and leads to elevated levels of ammonia in the blood. Excessive ammonia can cause various symptoms, including vomiting, lethargy, and coma. Boys have a more serious form of OTC deficiency than girls. If not treated immediately, severe OTC deficiency can lead to neurologic abnormalities, hyperammonemic coma, and death. Because late-onset OTC deficiency, which is more common in girls, presents mild symptoms, it is easy to miss diagnosis and prompt treatment. We describe an 11-month-old girl who presented periodic vomiting, intermittent lethargy, and seizure. She was diagnosed with OTC deficiency by elevated serum ammonia and urine orotic acid levels. Genetic analysis of the OTC gene revealed a missense mutation in exon 5 (c.418G>C). We reported an experience of exact diagnosis and successful treatment of late-onset OTC deficiency in our patient.

Anti-fibrotic effects of L-2-oxothiazolidine-4-carboxylic acid via modulation of nuclear factor erythroid 2-related factor 2 in rats

  • L-2-Oxothiazolidine-4-carboxylic acid (OTC) is a cysteine prodrug that maintains glutathione in tissues. The present study was designed to investigate anti-fibrotic and anti-oxidative effects of OTC via modulation of nuclear factor erythroid 2-related factor 2 (Nrf2) in an in vivo thioacetamide (TAA)-induced hepatic fibrosis model. Treatment with OTC (80 or 160 mg/kg) improved serum liver function parameters and significantly ameliorated liver fibrosis. The OTC treatment groups exhibited significantly lower expression of ${\alpha}$-smooth muscle actin, transforming growth factor-${\beta}1$, and collagen ${\alpha}1$ mRNA than that in the TAA model group. Furthermore, the OTC treatment groups showed a significant decrease in hepatic malondialdehyde level compared to that in the TAA model group. Nrf2 and heme oxygenase-1 expression increased significantly in the OTC treatment groups compared with that in the TAA model group. Taken together, these results suggest that OTC restores the anti-oxidative system by upregulating Nrf2; thus, ameliorating liver injury and a fibrotic reaction.

Synergistic effects of oxytetracycline and tiamulin against swine respiratory bacteria and their pharmacokinetics in the pigs (돼지호흡기세균에 대한 oxytetracycline과 tiamulin의 시험관내 상승작용 및 돼지에서의 약물동태학)

  • The study was carried out to characterize the pharmacokinetics after intravenous (iv, 20 mg/kg) and oral (p.o. 100 mg/kg) administration as oxytetracycline (OTC) and tiamulin (TIA) mixture in swine and to determine interaction between OTC and TIA against various pig pathogenic bacteria. The antibacterial effects of OTC in combination with TIA in vitro showed synergistic effect against Salmonella typhimurium 1925, Pasteurella multocida Type A, P. multocida Type D, Krebsiella Pneumoniae 2001, K. Pneumoniae 1560, K. Pneumoniae 2208, Haemophillus pleuropneumonia S 2, and H. pleuropneumonia S 5, but against additive effect E. coli K88ab and S. choleraesuis on the basis of fractional inhibitory concentration (FIC) index. On the while, after i.v. and p.o. administration of OTC and TIA mixture, each OTC and TIA concentrations in plasma were fitted to an open two-compartment model. After i.v. administration of OTC-TIA mixture, the mean distribution half-life ($T_{1/2{\alpha}}$) of OTC and TIA in plasma showed 0.29 h and 0.17 h, and the mean elimination half-life ($T_{1/2{\beta}}$) of those was 4.36 h and 6.64 h, respectively. The mean volume of distribution at steady state ($Vd_{ss}$) of OTC and TIA was $0.85{\ell}/kg$ and $2.44{\ell}/kg$, respectively. After oral administration of OTC and TIA mixture, the mean maximal absorption concentrations ($C_{max}$) of OTC and TIA were $0.60{\mu}g/m{\ell}$ at 1.07 h ($T_{max}$) and $1.68{\mu}g/m{\ell}$ at 1.85 h ($T_{max}$), respectively. The mean elimination half-life ($T_{1/2{\beta}}$) of those showed 6.84 h and 6.36 h. In conclusion, we could suggest in this study that the combination of OTC and TIA may be recommended for the antibacterial therapy against polymicrobial infections, and both OTC and TIA showed large distribution to tissues and high $C_{max}$ after p.o. administration.

  • Ornithine transcarbamylase (OTC) deficiency is the most common inborn error of urea cycle metabolism; it is inherited in an X-linked manner. The OTC catalyzes the third step of the urea cycle, the conversion of ornithine and carbamyl phosphate to citrulline. Deficiency of OTC leads to the accumulation of ammonia, causing neurological deficits. In most affected hemizygote males, OTC deficiency manifests as hyperammonemic coma that often leads to death in the newborn period, and those who recover from the coma may be neurologically impaired due to the sequelae of the hyperammonemic encephalopathy. In some, late-onset manifestations develop. We report a male neonate with early onset OT deficiency that had apnea and was comatous. On mutation analysis using DNA sequencing after polymerase chain reaction (PCR) amplification of the 10 exons, deletions of 10 bases in codon 285, causing a frame shift was detected in exon 8. The mother and a sister were diagnosed as female carriers. Therefore, genetic counseling and the risk assessment could be provided to the family.

단순의약품(OTC) 판매제도 개선에 관한 연구

  • The main purpose of this paper is to propose an alternative system of the distribution of OTC drugs in Korea. We first investigate the characteristics and the regulations on the distribution of the Korean medicine market and compare it with some foreign countries. We also examine the buying behavior of Korean customers. We propose three different alternatives. A passive policy is to expand the range of OTC drugs. Another one would be to amend the relevant laws. An active alternative is to arrange a separate organization to control the distribution of OTC drugs. The time and the execution of the proposed policies could vary.

Factors Associated with the Use of Over-the-Counter Medications in the Elderly Living Alone (독거노인의 일반의약품 사용에 영향을 미치는 요인)

  • Objectives : This study aimed to describe the use of over-the-counter (OTC) drugs and to identify predictors for their use in the elderly living alone. Methods : This is a cross-sectional study that enrolled 1,099 subjects. Data regarding socio-demographic status, medical condition, cognition, mood disorder and use of OTC drugs were collected using self-administered questionnaire and from a specific semi-structured interview by a trained nurse. Data regarding use of OTC drugs were analyzed using descriptive statistics. Logistic regression analysis was applied to examine factors associated with the use of OTC drugs. Results : The use of OTC drugs were reported by 35.4% of the subjects. Analgesics (13.6%) was the most frequent drugs. Depression (OR=1.10, 95% CI=1.10-1.87) and comorbidities measured by cumulative illness rating scale (CIRS) (OR=1.08, 95% CI=1.03-1.12) were significantly associated with the use of OTC drugs in the elderly living alone. Conclusions : Depression and severity of underlying medical conditions could be a predictor of the use of OTC drugs in the elderly living alone. The clinicians should be vigilant regarding the potential use of nonprescription medications in the elderly.

  • Orinithine transcarbamylase (OTC) deficiency is the most common inborn error of the urea cycle with resulting hyperammonemia, which is medical emergency in newborns.We recently had a case of a boy that presented with lethargy, seizure, hyperammonemia and hypocalcemia in neonatal period. He was diagnosed with OTC deficiency by two consequent ways which are initial biochemical phenotype including hyperammonemia and an increased orotic acid in his urine and genetic analysis of the OTC gene. The OTC gene showed a novel hemizygous mutation c.913C>T (p.Pro305Ser). He was treated by low protein intake, sodium benzoate, phenylbutyrate sodium, L-arginine, and continuous renal replacement therapy (CRRT). After discharge, he has a relatively good prognosis without notable developmental delay. For good prognosis, the duration of hyperammonemia should be shorten. And it can be reached by an early diagnosis. For early detection of OTC deficiency, targeted exome sequencing will be a important role as well as biochemical tests.

Exfoliation of abalone, Haliotis discus hannai by commercial exfoliating reagents (시판 전복박리제의 박리 효과)

  • Three commercial exfoliating reagents, product A (main components: citric acid and vitamin C), B (herb) and C (nicotinamide), were used to study their exfoliation effect on abalone, Haliotis discus hannai from the substrate. The exfoliating reagents A, B, C and oxytetracycline (OTC, control) of 6-31.25 g/L and 12-37.5 g/L exfoliated 81.2-84.8% and 90.3-95% of abalone, respectively. Post-treatment recovery time for the abalone was similar for all the reagents except product C. Recovery period for the abalone immersed in OTC for 5-20 second was slightly shorter than the reagents A, B and C; however, no mortality was observed in any group except with the reagents B (concentration: 20 g/L, immersion time: 5 sec) and C (12 g/L, 10 and 20 sec) that showed negligible mortality of 3.3%. Higher concentration and longer treatment with the reagents resulted in longer recovery time of the detached abalone. Although abalone exposed to the reagents needs slightly longer time to recover than that to OTC, the exfoliation effect is much similar. These results indicate that the commercial exfoliating reagents can replace OTC to detach abalone, though they need to be cautiously handled.

Combinational effects of sesquiterpene lactones isolated from bay laurel (Laurus nobilis) leaves with antibiotics against fish pathogenic bacteria (어류 병원성 세균에 대한 월계수(Laurus nobilis) 잎 유래 sesquiterpene lactone과 수산용 항생제의 병용효과)

  • This study is a report on combinational effects between four sesquiterpene lactones (SLs) from bay laurel (Laurus nobilis) leaves, and oxytetracycline (OTC) or amoxicillin (AMX) against four fish pathogenic bacteria such as Vibrio anguillarum, V. harveyi, Edwardsiella tarda, and Streptococcus iniae. Individually, four SLs exerted little antibacterial activity against fish pathogenic bacteria. However, when combined with OTC or AMX, they showed synergistic interaction against pathogenic bacteria. Especailly, zaluzanin C (1) reduced the MIC of OTC (or AMX) eight-fold. Our results showed that combinations of SLs with antibiotics (ABTs) are more effective than ABTs alone to control pathogenic bacteria. The highest synergistic effect was observed when zaluzanin C (1) was combined with OTC or AMX against V. harvey or S. iniae, displaying significant reductions of MICs up to 8-fold (0.125 to 0.015 ㎍/mL and 0.0078 to 0.0009 ㎍/mL). In addition, zaluzanin C (1) improved the antibiotic potency of OTC against OTC resistant V. harveyi (250 ㎍/mL to 62.5 ㎍/mL). Synergism between ABTs and phytochemical such as SLs could be a therapeutically helpful concept to improve the efficacy of ABTs and prevent antibiotic resistance. These results suggest that SLs can be used as an alternative to reduce antibiotic resistance in aquaculture.
