Rhinoplasty Education Using a Standardized Patient Encounter

  • Background Comprehensive aesthetic surgery training continues to be a challenge for residency programs. Our residency program developed a rhinoplasty-based objective structured clinical examination (OSCE) based upon validated methods as part of the residency education curriculum. We report our experience with the rhinoplasty-based OSCE and offer guidance to its incorporation within residency programs. Methods The encounter involved resident evaluation and operative planning for a standardized patient desiring a rhinoplasty procedure. Validated OSCE methods currently used at our medical school were implemented. Residents were evaluated on appropriate history taking, physical examination, and explanation to the patient of treatment options. Examination results were evaluated using analysis of variance (statistical significance P<0.05). Results Twelve residents completed the rhinoplasty OSCE. Medical knowledge assessment showed increasing performance with clinical year, 50% versus 84% for postgraduate year 3 and 6, respectively (P<0.005). Systems-based practice scores showed that all residents incorrectly submitted forms for billing and operative scheduling. All residents confirmed that the OSCE realistically represents an actual patient encounter. All faculty confirmed the utility of evaluating resident performance during the OSCE as a useful assessment tool for determining the Next Accreditation System Milestone level. Conclusions Aesthetic surgery training for residents will require innovative methods for education. Our examination showed a program-educational weakness in billing/coding, an area that will be improved upon by topic-specific lectures. A thoroughly developed OSCE can provide a realistic educational opportunity to improve residents' performance on the nonoperative aspects of rhinoplasty and should be considered as an adjunct to resident education.

Development of an Objective Structured Clinical Examination Checklist and a Post-Education Questionnaire for Musculoskeletal Ultrasound Training Focusing on Volar Wrist and Carpal Tunnel Syndrome

  • Background: The objective structured clinical examination (OSCE) is used in the colleges of Korean Medicine, but few studies have validated the OSCE evaluation criteria or post-education questionnaires. Diagnostic ultrasound is used in Korean medicine treatment including acupuncture, acupotomy, and pharmacopuncture to increase the safety and efficacy of treatment. We aimed to develop and validate a OSCE checklist and questionnaire for diagnostic musculoskeletal ultrasound training. Methods: A OSCE checklist and rubric for diagnostic ultrasound training, and questionnaire was developed using literature research. Eight expert panelists verified each draft item in a single-round survey. Items with a content validity ratio (CVR) < 0.75 were excluded or modified to reflect the experts' opinions. Results: The OSCE checklist and rubric for diagnostic ultrasound training focusing on volar wrist and carpal tunnel syndrome included: 15 items revised according to CVR and expert opinions, the pre-examination procedure, structures to be identified by ultrasound, scans with 2 diagnostic criteria for carpal tunnel syndrome, an explanation of the exam results, and the post exam procedure. The questionnaire consisted of 15 items, including the overall evaluation of training, the effect of the OSCE, and the perception of the ultrasound. All 6 self-evaluation items were not revised, as they had a CVR of ≥ 0.75. Conclusion: An ultrasound OSCE for scanning the volar wrist and diagnosing carpal tunnel syndrome was developed using 15 validated tasks, 15 survey questions about ultrasound training, and 6 questions for self-evaluation. These results may be used in the future for education in diagnostic ultrasound.

구조화된 객관적 임상시험(OSCE) 실시 후 간호학생들의 임상수행능력에 대한 자기평가경향 분석 (Analysis of Trends in Self-assessment of Performance of Clinical Skills in Nursing Students after OSCE)

  • Purpose: The self-assessment ability of students is important in acquiring clinical skills. The study explored the self-assessment behavior of nursing students after OSCE (Objective Structured Clinical Examination). Methods: The participants were 90 nursing students. They assessed their performance just after an OSCE (assessment 1). They were given OSCE checklists and re-assessed their performance level (assessment 2). Assessments 1, 2, and an assessment by professor were compared and analyzed. Results: Students assessed themselves higher than professor. But, when students were divided into three groups according to score level, different behaviors were evident between the groups. The high-score group assessed themselves lower than the professor, while the mid-and low-score groups assessed themselves higher than the professor. Students' self-assessment more closely approximated the professor's assessment when they were given checklists. The correlation between assessments 1 and 2 was stronger in high-score group and weak in low-score group. Conclusion: The study results indicate that students tend to assess their skills higher than the professor, but their scores were more in line with the professor when they were provided with checklists, and students' self-assessment behavior differed in the different score groups, suggesting a need for customized feedback and a concern for students with low scores.

임상술기능력평가와 교과목간의 학업성취도 분석을 통한 소아치과학의 교육과정 평가 (Evaluating Course of Pediatric Dentistry on Correlation Analysis between Dental Students' Achievements of Subjects and OSCE)

  • 본 연구는 졸업시점의 치의학전문대학원생 79명을 대상으로 소아치과학 OSCE(Objective Structured Clinical Examination)성적과 이전까지 이수한 소아치과학 교과목들 성적간의 관계를 분석함으로써 소아치과학 교육과정 운영을 평가하고 발전적 시사를 도출하는 데에 목적이 있다. 소아치과학이론 교과목은 이전 이수과목인 임상전단계실습 성적(r = 0.449, p = 0.000), 이후 이수과목인 임상실습성적(r = 0.437, p = 0.000)과 상관관계가 있었다. 하지만 졸업시점의 OSCE성적은 단지 임상전단계실습 과목 성적(r = 0.302, p = 0.003)과 상관관계를 보였다. 이는 OSCE 신뢰도가 Cronbach's ${\alpha}0.733$을 고려하면 OSCE 자체 문제보다는 교육과정 상에 있어서 임상실습 교과목 운영에 문제가 있음을 추론할 수 있다. 치과대학병원에서 교수의 특진을 선호하고 학생들에 의한 진료를 기피하는 진료 현장에서 학생들에 대한 직접적인 지도와 피드백을 통해 임상역량을 강화하기 위해서는 학생 지도를 위한 교수 수 확보와 같은 교육여건이 마련되어야 한다. 또한 이론적 지식 및 술기교육이 임상진료 현장에서 잘 적용되고 심화되기 위해서는 교육과정에 대한 보다 구체적인 평가와 함께 임상실습 단계에 있어서도 기본적 술기에 대한 지속적인 교육과 평가가 점검되도록 하는 실질적 개선 방안이 모색되어야 한다.

  • 목적 본 연구의 목적은 Six Dimension Scale of Nursing Performance (Six-D Scale)와 간호학생 스트레스 평가지수 (SNSI)의 한국어 버전 개발 과정을 기술하고, 천식 및 1형 당뇨 모듈을 내용으로 한 시뮬레이션 기반 객관구조화시험(OSCE)을 이용하여 신뢰도 및 타당도를 평가하기 위함이다. 방법 본 연구는 단편적 서술연구이며, 4년제 간호학과 4학년 학생 51명을 대상으로 수행되었다. 도구의 신뢰도는 Chronbach's 를 이용한 내적 일관성을 평가하였고, OSCE는 검사자간 신뢰도를 계산하였다. 타당도 평가는 Six-D Scale과 SNSI의 하부영역을 OSCE, 평점 및 자아효능감과의 상관관계를 분석함으로써 구성타당도로 확인하였다. 결과 두 도구의 Chronbach's ${\alpha}$는 .82-.95였고, OSCE의 검사자 간 신뢰도는 .75 (천식)-.87 (당뇨)이었다. Six-D Scale은 OSCE (r=.109-.272) 및 자아효능감과(r=.005-.161)는 양의 상관관계를, 평점과는 음의 상관관계를(r= -.246 ~ -.394) 보였으나, 통계적으로 유의하지는 않았다. SNSI는 OSCE (r= -.007 ~ -.238), 평점(r=-.092 ~ -.426) 및 자아효능감(r=-.246 ~ -.394) 모두와 음의 상관관계를 나타냈으며, OSCE를 제외하고는 모두 통계적으로 유의하였다. 결론 본 연구의 결과 간호수행능력의 Six-D Scale, SNSI 모두 적절한 신뢰도를 보였다. 타당도 검증 결과, 간호학생스트레스 도구는 적절한 타당도를 나타내었으나, Six-D scale은 적용에 신중을 기해야 하는 것으로 판단되었다. 따라서 자가 보고형 임상수행능력 평가는 좀 더 다양한 학생들을 대상으로 타당도 검증이 더욱 깊이 있게 평가되어야 할 것이며, 간호학생의 스트레스는 학생들의 임상수행능력에 영향을 미치는 주요한 변수로 간주되어 연구 및 교육에 적극 고려되어야 할 것이다.

Comparative study between an intensive small group teaching and a 1-year clinical practice on OSCE

  • Objectives: Our school introduced a new curriculum based on faculty-directed, intensive, small-group teaching of clinical skills in the third-year medical students. To examine its effects, we compared the mean scores on an OSCE between the third- and fourth-year medical students. Methods: Third- and fourth-year students did rotations at the same five OSCE stations. They then completed a brief self-reporting questionnaire survey to examine the degree of satisfaction with new curriculum in the third-year students and clinical practice in the fourth-year students, as well as their perception of confidence and preparedness. We analyzed the OSCE data obtained from 158 students, 133 of whom also completed the questionnaire. Results: Mean OSCE scores on the breast examination and wet smear stations were significantly higher in the third-year group (P < 0.001). But mean OSCE scores of motor-sensory examination and lumbar puncture were significantly higher in the fourth-year group (P < 0.05). The mean OSCE scores had no significant correlation with satisfaction. In addition, the self-ratings of confidence had a high degree of correlation with satisfaction with new curriculum (r = 0.673) and clinical practice (r = 0.692). Furthermore, there was a moderate degree of correlation between satisfaction and preparedness in both groups (r = 0.403 and 0.449). Conclusions: There is no significant difference in the effect on the degree of clinical performance and confidence between an intensive-small group teaching and a 1-year clinical practice. If combined, intensive small group teaching and clinical practice would be useful to improve the degree of ability and confidence in medical students.

한방부인과학에서 임상실습교육의 만족도와 관련요인 분석 (The Analysis of Satisfaction with Clinical Training and the Related Factors in the Korean Medicine Obstetrics and Gynecology)

  • Objectives: The purpose of this study is to analyze csatisfaction with clinica1 training in the department of Korean Medicine Obstetrics and Gynecology and the related factors. Methods: All the students on the graduating class of school of Korean Medicine, Dong-Shin University must be trained for 1 year in the department of Korean Medicine Obstetrics and Gynecology according to clinical training guideline including Objective Structured Clinical Examination (OSCE). After completing clinical training, we distributed questionaries to them about a reflex of the object of study, usefulness, difficulty, satisfaction and preference. And then, we analyzed the related factors including descriptive statistics, frequency analysis and correlation analysis by SPSS 12.0. Results: In general, a reflex of the aim of study (7.31±1.59), satisfaction with contents (7.44±1.58), and difficulty (6.13±1.34) were evaluated restectively. There is no statistically significant difference with sex. There is a statistically significant positive correlation between a reflex of the aim of study and satisfaction with contents. The primary preferences of clinical practice were Clinic Training, Ward Training, PBL (Problem Based Learning), OSCE. In OSCE, a reflex of the aim of study (7.04±1.83) and usefulness (7.02±1.97) were highly evaluated. There is no statistically significant difference with sex. The primary preferences for clinical OSCE practice were breast examination, genital examination, ultrasound scan, and pap smear. Facilities and equipment (6.84±2.01), contents of clinical training (7.29±1.83), the handout (7.20±1.98) and time scheduling (7.29±1.90) were evaluated respectively in general composition of clinical training. Conclusions: Clinical training program in the department of Korean Medicine Obstetrics and Gynecology can be highly estimated from the viewpoint of satisfaction with clinical training and the related factors.

의과대학생은 시험을 준비하기 위해 어떻게 공부하는가: 평가 전 인지 및 메타인지 활동 (How Do Medical Students Prepare for Examinations: Pre-assessment Cognitive and Meta-cognitive Activities)

  • Although 'assessment for learning' rather than 'assessment of learning' has been emphasized recently, student learning before examinations is still unclear. The purpose of this study was to investigate pre-assessment learning activities (PALA) and to find mechanism factors (MF) that influence those activities. Moreover, we compared the PALA and MF of written exams with those of the clinical performance examination/objective structured clinical examination (CPX/OSCE) in third-year (N=121) and fourth-year (N=108) medical students. Through literature review and discussion, questionnaires with a 5-point Likert scale were developed to measure PALA and MF. PALA had the constructs of cognitive and meta-cognitive activities, and MF had sub-components of personal, interpersonal, and environmental factors. Cronbach's ${\alpha}$ coefficient was used to calculate survey reliability, while the Pearson correlation coefficient and multiple regression analysis were used to investigate the influence of MF on PALA. A paired t-test was applied to compare the PALA and MF of written exams with those of CPX/OSCE in third and fourth year students. The Pearson correlation coefficients between PALA and MF were 0.479 for written exams and 0.508 for CPX/OSCE. MF explained 24.1% of the PALA in written exams and 25.9% of PALA in CPX/OSCE. Both PALA and MF showed significant differences between written exams and CPX/OSCE in third-year students, whereas those in fourth-year students showed no differences. Educators need to consider MFs that influence the PALA to encourage 'assessment for learning'.

한의학교육에서 객관구조화진료시험과 진료수행평가에 대한 연구 동향과 후속 연구 및 교육에 대한 제언: 주제범위 문헌고찰 (Presentation on Research Trends and Suggestion for further research and education on Objective Structured Clinical Examination and Clinical Performance Examination in Korean Medicine Education: Scoping Review)

  • Objectives : This study reviewed existing studies on clinical examination of Korean medicine to investigate the trends and gaps of existing research on clinical performance examination (CPX) or objective structured clinical examination (OSCE) in Korean medicine education. Methods : We conducted a scoping review according to the five steps suggested by Arksey and O'Malley. Six databases(RISS, OASIS, KMBASE, KISS, ScienceON, and DBpia) were searched and studies published from 2012 to July 15, 2022 were considered. The subjects of the study were limited to domestic studies about OSCE or CPX conducted in Korean medicine education. Results : Among the 25 selected papers, 17 studies implemented OSCE or CPX in practice. Those studies were conducted in 8 clinical departments and 9 of them used standardized patients. All 14 survey studies reported positive answers in satisfaction, efficacy, and self-assessment. OSCE, CPX scoring items were developed by modifying existing tools or developing through expert surveys or through literature review and significant cases. Conclusions : This study is expected to be used as basic data for following studies and promote clinical examination. From now on, the colleges of Korean medicine should proceed research on large-scale students about extended subjects through various research methods to obtain objective results that can be generalized and acquire validity and reliability. In order to promote such research, it is necessary to induce cooperation from institutions related to Korean Medicine and local universities to develop modules, implement, and conduct post-evaluation.
