Bombesin has a high binding affinity to gastrin-releasing peptide receptor (GRPR) and can be used as a targeting ligand in GRPR-related cancers. Because GRPR is overexpressed in prostate cancer, bombesin analogues have been investigated extensively for diagnosis and treatment of prostate cancer. In nuclear medicine, bombesin derivatives labeled with radiometals such as 55/57Co, 64Cu, 68Ga, 99mTc, and 177Lu or radiohalogen such as 131I and 211At were developed as markers for early detection of tumors and theragnostic tool for cancer treatment. This review focuses on the introduction of bombesin-based radiopharmaceuticals that are studied in pre-clinical or clinical research.

We investigated the effect of high dose on the sensitivity of optically stimulated luminance dosimeters (OSLDs) on Co-60 gamma rays and used a commercial OLSD (Landauer, Inc., Glenwood, IL). New OSLDs were chosen arbitrarily and were irradiated with 1 Gy repeatedly. We confirmed the change in the radiation sensitivity after repeated irradiation. The OSLD sensitivity increased up to 3% after irradiating for seven times and decreased continuously after the eighth time. It dropped by approximately 0.35 Gy per irradiation. Finally, after irradiating for 30 times, the OSLD sensitivity decreased by approximately 7%. When the OSLDs were irradiated 10 times with 1 Gy after their irradiation using a high dose of 15 Gy and 30 Gy, their sensitivity decreased by 6% and 12%, respectively, compared to that before high-dose irradiation. The change in the OSLD sensitivity with a high dose could be modeled by an exponential equation. We confirmed the radiation sensitivity variation caused by a high dose, and the irradiation history of dosimeters was considered to reuse OSLDs irradiated with a high dose.

With the development in field of industry and medicine, new machines and techniques are being launched. Moreover, the complexity of the techniques is associated to an increasing risk of incident. Especially, a small error in radiotherapy can lead to a serious patient-related incident, risk management is necessary in radiotherapy in order to reduce the risk of incident. However, in field of radiotherapy, there are no legally binding clauses for risk management and there is an absence of risk management systems at an institutional level. Therefore, we analyzed institutional status of risk management, reporting & classification systems, and risk assessment & analysis in 31 countries. For risk management and reporting systems, 65% of countries investigated had legislation or regulations; however, only 35% of countries used classification systems. It was found that 43% more countries had legislation for risk management in healthcare than those for radiotherapy; 19% more countries had reporting systems for healthcare than those for radiotherapy. For classification systems, 60% more countries had legislation, recommendation, and guidelines in the field of radiotherapy than those for healthcare. Recently, international institutes have published several reports for risk management and patient safety in radiotherapy, owing to which, countries adopting risk management for radiotherapy will gradually increase. Before adopting risk management in Korea, we should precisely understand the procedures and functions of risk management, in order to increase efficiency of risk management because classification & reporting system and risk assessment & analysis are connected organically, and institutional management is needed for high quality of risk management in Korea.

Iatrogenic perforations are severe complications of gastrointestinal endoscopy; therefore, their management should be adequately planned. A 77-year-old man with a history of diverticulosis underwent a colonoscopy for anemia. During the procedure, an iatrogenic perforation occurred suddenly in the sigmoid colon, near a severe angle among the numerous diverticula. Through-the-scope clips were immediately applied to treat it and close mucosal edges. Laboratory tests showed increased levels of inflammation and infection, and although there were no complaints of abdominal pain, the patient had an extremely distended abdomen. A multidisciplinary board began management based on a conservative approach. Pneumoperitoneum was treated with computed tomography-assisted drainage. After 72 hours, his intestinal canalization and laboratory tests were normal. Though this adverse event is rare, a multidisciplinary board should be promptly gathered upon occurrence, even if the patient appears clinically stable, to consider a conservative approach and avoid surgical treatment.
Backgrounds/Aims: Postoperative pancreatic fistula is the key worry in the ongoing debate about the safety and effectiveness of total laparoscopic pancreaticoduodenectomy (TLPD). Laparoscopic-assisted pancreaticoduodenectomy (LAPD), a hybrid approach combining laparoscopic resection and anastomosis with a small incision, is an alternative to TLPD. This study compares the short-term outcomes and oncological efficacy of LAPD vs. open pancreaticoduodenectomy (OPD). Methods: A retrospective analysis of data of all patients who underwent LAPD or OPD for periampullary carcinoma at a tertiary care center in Northeast India from July 2019 to August 2023 was done. A total of 30 LAPDs and 30 OPDs were compared after 1:1 propensity score matching. Demographic data, intraoperative and postoperative data (30 days), and pathological data were compared. Results: The study included a total of 93 patients, 30 underwent LAPD and 62 underwent OPD. After propensity score matching, the matched cohort included 30 patients in both groups. The LAPD presented several advantages over the OPD group, including a shorter incision length, reduced postoperative pain, earlier initiation of oral feeding, and shorter hospital stays. LAPD was not found to be inferior to OPD in terms of pancreatic fistula incidence (Grade B, 30.0% vs. 33.3%), achieving R0 resection (100% vs. 93.3%), and the number of lymph nodes harvested (12 vs. 14, p = 0.620). No significant differences in blood loss, short-term complications, pathological outcomes, readmissions, and early (30-day) mortality were observed between the two groups. Conclusions: LAPD has comparable safety, technical feasibility, and short-term oncological efficacy.

본 논문에서는 18F-FDG PET과 CT에서 추출한 영상인자를 이용하여 비소세포폐암의 전이를 예측하는 머신러닝 모델을 생성하였다. 18F-FDG는 종양의 포도당 대사 시 사용되며 이를 추적하여 환자의 암 세포를 진단하는데 사용되는 의료영상 기법 중 하나이다. PET과 CT 영상에서 추출한 이미지 특징은 종양의 생물학적 특성을 반영하며 해당 ROI로부터 계산되어 정량화된 값이다. 본 연구에서는 환자의 의료영상으로부터 image texture 프절 전이 예측에 있어 유의한 인자인지를 확인하기 위하여 AUC를 계산하고 단변량 분석을 진행하였다. PET과 CT에서 각각 4개(GLRLM_GLNU, SHAPE_Compacity only for 3D ROI, SHAPE_Volume_vx, SHAPE_Volume_mL)와 2개(NGLDM_Busyness, TLG_ml)의 image texture feature를 모델의 생성에 사용하였다. 생성된 각 모델의 성능을 평가하기 위해 accuracy와 AUC를 계산하였으며 그 결과 random forest(RF) 모델의 예측 정확도가 가장 높았다. 추출된 PET과 CT image texture feature를 함께 사용하여 모델을 훈련하였을 때가 각각 따로 사용하였을 때 보다 예측 성능이 개선됨을 확인하였다. 추출된 영상인자가 림프절 전이를 나타내는 바이오마커로서의 가능성을 확인할 수 있었으며 이러한 연구 결과를 바탕으로 개인별 의료 영상을 기반으로 한 비소세포폐암의 치료 전략을 수립할 수 있을 것이라 기대된다.
Radiotherapy patients should maintain their treatment position as patient setup is very important for accurate treatment. In this study, we evaluated patient setup error quantitatively according to Cone-Beam Computed Tomography (CBCT) Gamma Density Analysis using Mobius CBCT. The adjusted setup error to the $QUASAR^{TM}$ phantom was moved artificially in the superior and lateral direction, and then we acquired the CBCT image according to the phantom setup error. To analyze the treatment setup error quantitatively, we compared values suggested in the CBCT system with the Mobius CBCT. This allowed us to evaluate the setup error using CBCT Gamma Density Analysis by comparing the planning CT with the CBCT. In addition, we acquired the 3D-gamma density passing rate according to the gamma density criteria and phantom setup error. When the movement was adjusted to only the phantom body or 3 cm diameter target inserted in the phantom, the CBCT system had a difference of approximately 1 mm, while Mobius CBCT had a difference of under 0.5 mm compared to the real setup error. When the phantom body and target moved 20 mm in the Mobius CBCT, there are 17.9 mm and 13.5 mm differences in the lateral and superior directions, respectively. The CBCT gamma density passing rate was reduced according to the increase in setup error, and the gamma density criteria of 0.1 g/cc/3 mm has 10% lower passing rate than the other density criteria. Mobius CBCT had a 2 mm setup error compared with the actual setup error. However, the difference was greater than 10 mm when the phantom body moved 20 mm with the target. Therefore, we should pay close attention when the patient's anatomy changes.

본 논문에서는 GAN 기반의 영상 생성 방법론을 이용해 delayed PET 영상을 생성하는 연구를 수행하였다. PET은 양전자를 방출하는 방사성 동위원소를 표지한 방사성의약품의 체내 분포를 시각화함으로서 암 세포 진단에 이용되는 의료영상 기법이다. 하지만 PET의 스캔 과정에서 방사성의약품이 체내에 분포하는 데에 걸리는 시간이 오래 걸린다는 문제점이 존재한다. 따라서 본 연구에서는 방사성의약품이 충분히 분포되지 않은 상태에서 얻은 PET 영상을 통해 목표로 하는 충분히 시간이 지난 후에 얻은 PET 영상을 생성하는 모델을 GAN (generative adversarial network)에 기반한 image-to-image translation(I2I)를 통해 수행했다. 특히, 생성 전후의 영상 간의 영상 쌍을 고려한 paired I2I인 Pix2pix와 이를 고려하지 않은 unpaired I2I인 CycleGAN 두 가지의 방법론을 비교하였다. 연구 결과, Pix2pix에 기반해 생성한 delayed PET 영상이 CycleGAN을 통해 생성한 영상에 비해 영상 품질이 좋음을 확인했으며, 또한 실제 획득한 ground-truth delayed PET 영상과의 유사도 또한 더 높음을 확인할 수 있었다. 결과적으로, 딥러닝에 기반해 early PET을 통해 delayed PET을 생성할 수 있었으며, paired I2I를 적용할 경우 보다 높은 성능을 기대할 수 있었다. 이를 통해 PET 영상 획득 과정에서 방사성의약품의 체내 분포에 소요되는 시간을 딥러닝 모델을 통해 줄여 PET 이미징 과정의 시간적 비용을 절감하는 데에 크게 기여할 수 있을 것으로 기대된다.
