노년기의 올바른 영양관리를 위한 영양교육자료 개발 (Development of Nutrition Education Materials for Healthy Aging)

  • The objective of this study was to develop nutrition education materials for older adults, 'nutritional management for healthy aging'. A booklet and four leaflets were developed based on lesson plans. Topics of the lesson plans included eating habit assessment. Korean food guide pyramid, meal planning, eating sensibly and weight management. The titles of the leaflets were 'Eating right for healthy aging', 'Eat calcium-rich foods', 'Enjoy fruits & vegetables' and 'Weight management'. Illustrations and icons appropriate to the texts were designed using illustrator 9.0 and Photoshop 6.0. Booklet (letter size, 5 chapters, 44 pages) and leaflets (B4 size, 6 sections) focused on modifying undesirable eating habits, providing practical tips for desirable behaviors, and behavioral modification such as recording in a food diary, goal setting and increasing self-efficacy. The drafts were pilot-tested by interviews with older adults(n=10), and minor changes were made. The characteristics of revised materials are as follows; i) materials focused on providing desirable eating behaviors for healthy aging, ii) messages were simple and specific, iii) large fonts(13 pt) were used and materials included interesting pictures and illustrations, iv) materials provided tips for balanced diets and recipes fur older adults, v) materials included sections for participation of learners including assessment of nutritional risk factors and obesity, meal planning and games. The revised materials are self-explanatory and can be used by older adults and in nutrition education for older adults.

여대생 대상 체중조절 프로그램의 영양 교육 효과 평가 (Effectiveness of Nutrition Education in a Weight Control Program for Female College Students)

  • The purpose of this study was to evaluate the multilateral weight control program provided by the university health care center for female college students. The program was 8weeks long and composed of nutrition consultation, exercise, and behavior modification. Nutrition education focused on a low calorie and balanced diet, increasing the intake of protein and micronutrients and reducing carbohydrate and fat consumption. Evaluations were made upon completion of an 8-week program and upon follow-up at 6 months after the end of program. A total of 92 women completed the 8-week program, while 20 completed the follow-up survey. Upon completion of the 8-week program, significant decreases in body weight, BMI, body fat (kg), and %body fat were observed, and muscle mass was also reduced. The total energy and carbohydrates intake were also significantly decreased in subjects with weight loss, while the percentage of subjects whose consumption of protein, calcium, iron, vitamin $B_1$, $B_6$, and niacin did not meet the EAR, were decreased. The blood values of glucose, total cholesterol and HDL-cholesterol were decreased. The follow-up survey revealed that the subjects continued to lose weight after completion of the program. In addition, a Mini nutrition assessment revealed that diet quality improved, especially with respect to reductions in the consumption frequency of fried, greasy and salty food. In conclusion, the multilateral weight control program with diet education for female college students was effective for long lasting weight control and improved dietary behavior. Nutrition education for weight reduction should focus on balanced nutrition as well as lowering the total caloric intake.

인천지역 대학생에 의한 대학급식소의 급식서비스에 대한 품질 평가 (Quality Assessment of Performance in the University Foodservice by Students Living in Incheon)

  • This study was conducted to investigate the quality assessment of performance of the university foodservice. Self administered questionnaires were collected from 305 students living in Incheon. Statistical data analysis was completed using a SPSS v. 10.0 program. The results were summarized as follows: 61.4% of male students and 48.1% of female students responded to have lunch regularly. Only 23.4% of male students and 14.1% of female students used the university foodservice daily. Reasons for the irregularity of lunch were responded as “lecture”, “eating when I want” and “appointment with friends”, etc. The quantity, temperature, nutritional value, appearance, hygiene, taste and freshness of foods and price of menus were evaluated as appropriate but using seasonal foods, number of side-dishes, etc. as unsatisfactory. Service speed, cloth hygiene, neatness and kindness of employee were evaluated as appropriate. For the facilities of foodservice, counter location, menu board and lighting facilities were evaluated as appropriate but heating facilities, disposition of tables and chairs of dining hall were as unsatisfactory. Sanitation of floors and walls of dining hall, restroom and utensils, etc. was evaluated as low.

Validation of Self-Administered Dietary Assessment Questionnaires Developed for Japanese Subjects : Systematic Review

  • Several self-administered dietary assessment questionnaires have recently been developed, validated, and used in nutritional epidemiological and clinical studies in Japan. This article describes recent evidence on development and validation of them. After extensive search of published articles both in English and Japanese languages, we identified 25 articles on 13 questionnaires of which validation studies have existed. Number of foods/menus assessed varied from 31 to 169 according to questionnaires. Eleven questionnaires were food frequency type, either with fixed portion size or semiquantitative, and two diet history types. All the 13 questionnaires were validated against intakes assessed with dietary record or 24-hour recall, and only two with biomarkers. Number of subjects used in the studies was between 23 and 350. All the studies used adult subjects. In the studies with dietary record or recall, the correlation coefficient for or orgy intake was between 0.22 and 0.65 (median = 0.44). Median correlation coefficient for nutrients was between 0.21 and 0.61. In the studies with biomarkers, serum marine-origin n-3 polyunsaturated fatty acids and carotenes, and urinary potassium seemed useful biomarkers. In conclusion, recent progress of this field in Japan is remarkable. But more research is needed for validation studies with biomarkers, and the development and validation of questionnaires for children and elderly subjects. (J Community Nutrition 5(2) : 83∼92,2003)

위암환자의 입원시 영양상태 (Initial nutritional status of stomach cancer patients)

  • The purpose of this study is to investigate initial nutritional status of stomach cancer patients. The study subjects were 88 patients with stomach cancer admitted at Kosin University Gospel Hospital in Busan. We assessed the initial nutritional status by anthropometric, biochemical and dietary intake data along with subjective global assessment(SGA). The results are as follows. 1. The mean age, weight, height, triceps skinfold thickness, mid arm circumference, and mid arm muscle circumference of the subjects were 55.9$\pm$11.0years, 60.0$\pm$9.8kg, 162.4$\pm$8.5cm, 10.9$\pm$5.7mm, 26.8$\pm$3.8cm, and 23.4$\pm$3.5cm respectively. The mean body mass index was within the normal range, with 22.7$\pm$2.8kg/m2, while 15.4% of the patients was underweight. The result shows that body fat mass and body protein mass of the patients with stomach cancer were decreased. 2. The mean biochemical data of the subjects were 4.0$\pm$0.5g/dl for albumin, 174.7$\pm$41.9mg/dl for cholesterol, 107.6$\pm$57.2mg/dl for triglyceride, 92.1μg/dl for Zn,  297.0$\pm$103.1mg/dl for transferrin, 1980.0$\pm$0.8$mm^3$ for total lymphocyte count. 3. Daily energy intake was 1997.8$\pm$579.3kcal. And the ratio of carbohydrate, protein, and lipid to energy intake was 72:14:14. 4. The patients were divided into three groups according to SGA performed by an observer. Group A(well nourished) was 55.7% with 49 patients, Group B(moderately malnourished) was 22.7 % with 20 patients, and Group C(severely malnourished) was 21.6 % with 19 patients. The three groups showed a significant difference in body weight(p<0.01), 1 month weight loss %(p<0.001), 6 months weight loss %(p<0.001), body mass index(p<0.01), and mid arm circumference(p<0.05), albumin(p<0.01), energy intake(p<0.05) as well as carbohydrate intake(p<0.05). From these results, it may be concluded that SGA can be used as a nutrition screening tool, and comprehensive nutrition assessment is desirable for those malnourished.

학교급식 영양사의 영양적 품질관리 수행도 평가 (Assessment of Dietitian's Nutritional Quality Management for School Food Service)

  • 학교급식의 영양적 품질관리 기준을 설정하기 위한 자료를 얻고자 총체적 품질 경영(TQM)을 기초로 영양적 품질관리 수행도를 평가하였다. 충북지역 초${\cdot}$${\cdot}$고등학교 급식 영양사 200명을 대상으로 질문지법을 이용하여 영양적 품질관리 수행도와 음식 생산단계별 수행도 및 영양적 품질관리 관련문서 및 서식 사용현황을 조사하였다. 총체적 품질경영을 기초로한 영양적 품질관리 영역에서 평균 3.12/5점의 수행도를 보였으며, 그 중 '리더십' 영역에서 높은 수행도(3.66)를 보였고, '인적자원 중시' 영역에서 가장 낮은 수행도(2.66)를 보였다. 학교유형과 급식 실시시기에 따라 전략계획, 고객 및 시장중시, 사업성과 영역에서 유의적인 차이를 보였다. (p<0.05). 영양사의 인구통계적 변인에 따라서는 연령, 근무경력, 고용상태에 의한 차이를 보였는데(p<0.05), $30\~40$대가 20대보다 리더쉽이 높고, 급식의 전략계획을 더 잘 수행하고 있었다. 리더쉽과 전략계획, 인적자원 중시 영역에서 10년 이상의 경력자가 이하보다 수행도가 더 높았다(p<0.05). 음식 생산단계별 수행도는 평균 3.72/5점의 수행도를 나타내었고, 학교 특성별로는 초등학교 영양사가 중${\cdot}$고등학교에 비해 건열조리법과 분산조리법 및 가열시 온도조절의 효과적 사용을 통해 가열처리 시간을 최소화하여 영양소보존을 더 잘 수행하고 있었다(p<0.05). 영양사의 경우 연령에 따라 차이를 나타내었는데, 30, 40대가 20대보다 구매 영역에서 더 높은 수행도를 보였다(p<0.05). 영양적 품질관리를 위한 관련문서 및 서식 사용현황 평가에서 평균 $60.6\%$의 보유율 을 보였으며, 급식섭취량 조사일지$(42.0\%)$와 직무기술서$(41.0\%)$$50.0\%$ 미만의 보유율 나타냈다. 학교 특성별로는 학교유형별로(p<0.05), 영양사의 경우에는 연령에 따라 유의적 차이를 나타내었다(p<0.05). 영양적 품질관리 수행도와 음식 생산단계별 수행도 및 관련문서${\cdot}$서식 사용현황 간에는 각각 0.55, 0.45의 상관관계를 나타내었다. 학교급식의 영 양적 품질관리(TQM) 수행도 평가 결과, 인적자원 중시에 대한 수행도가 낮으므로, 조리종사원의 영양적 품질관리 향상을 위한 교육 및 지도를 위한 교육프로그램 개발이 필요하다. 음식 생산단계별 영양 손실을 최소화하기 위해 가열조리 단계를 중요관리 점으로 설정하고 이를 관리 할 수 있는 관리기준의 개발이 시급하다. 결론적으로 TQM을 기반으로 한 영양적 품질향상을 위한 기준이 설정되어 실제적으로 적용되어야 한다.

젊은 성인의 영양정보이해력 평가도구 개발 및 타당성 검증 (Development and validation of a nutrition literacy assessment tool for young adults)

  • 본 연구는 젊은 성인을 대상으로 영양정보이해력을 평가할 수 있는 측정도구를 개발하기 위해 수행되었다. 영양정보이해력 평가를 위한 예비문항으로서 우선 7개 영역 65문항이 개발되었다. 작성된 예비문항을 영양전문가 5인에게 자문을 받아 불필요한 문항을 삭제하여 7개 영역 52문항으로 압축시켰다. 영양전문가 패널 20인을 대상으로 내용타당도 검증을 하였으며, Lawshe가 제안한 CVR값을 구하여 내용타당도 검증에 활용하였다. 평가도구 최종 항목으로서 총 6개 영역, 즉 식생활지침, 영양과 건강, 영양소, 다섯가지 식품군 (식품구성자전거), 영양표시, 질병 예방을 위한 영양관리를 포괄하는 30문항이 완성되었다. 이 평가도구를 이용하여 서울과 경인지역에 거주하는 성인 211명을 대상으로 신뢰도를 검증하기 위해 2주 간격으로 검사-재검사 (test-retest)를 실시하였으며, ICC값 (0.64-0.86)과 Cronbach's α 계수(0.83-0.90)를 산출한 결과 모두 높은 수준으로 나타났다. 따라서 본 연구를 통해 개발된 도구는 영양정보이해력을 평가할 수 있는 적합한 내용을 담고 있으며, 신뢰도 또한 매우 높은 것으로 검증되어, 젊은 성인의 다양한 수준의 영양정보이해력을 판별할 수 있을 것으로 본다. 또한 본 연구를 통해 개발된 평가도구는 지역사회 영양중재 프로그램 계획 시 그리고 만성질환의 예방관리를 위한 영양교육을 위해 대상자의 영양정보이해력을 파악하는 진단도구로 활용될 수 있을 것이다.

연하 곤란이 동반된 급성 뇌졸중 환자를 위한 장관 영양 프로토콜 개발 및 효과 (Development and Evaluation of an Enteral Nutrition Protocol for Dysphagia in Patients with Acute Stroke)

  • Purpose: The study was done to develop an evidence-based enteral nutrition (EN) protocol for effective nutritional support for dysphagia in patients with acute stroke, and to evaluate effects of this protocol on clinical outcomes. Methods: A methodological study was used to develop the EN protocol and a quasi-experimental study to verify the effectiveness of the protocol. The preliminary EN protocol was drawn by selecting recommendations from previous well-designed EN guidelines, and then developing additional recommendations based on high-quality evidence. Content validation was assessed by an expert group, and clinical applicability by care providers and patients. The scale-level content validity index of the final EN protocol was 0.99. Assessment was done of differences in percentage of caloric goals achieved and presence of undernutrition, aspiration pneumonia, and gastrointestinal (GI) complications after application of the EN protocol. Results: In the EN protocol group, the percentage of caloric goals achieved ($R^2=.24$, p=.001) and the reduction of GI complications (p=.045) were significantly improved, but the presence of undernutrition (p=.296) and aspiration pneumonia (p=.601) did not differ from the usual care group. Conclusion: Results indicate that the new EN protocol for dysphagia in patients with acute stroke significantly increased their nutritional intake and reduced GI complications.

대구지역 여자노인의 건강상태 및 영양소 섭취 실태조사 (The Health Status and Nutrient Intakes of Elderly Female in Daegu Area)

  • This study was designed to evaluate health and nutritional status of elderly females according to their family type. The participants were 157 elderly women(41 living alone, 45 living with a spouse, and 71 living with family). Among them, the subjects answered that their health rated as good condition, the elderly living with a spouse had significantly higher proportions than those living alone and living with family. Serum cholesterol and fasting blood glucose of the respondents living with a spouse were lower than those of the respondents living alone or with family. A dietary assessment with a 24-hour recall method represented that the elderly living alone had lower nutrient intakes, especially the intake of Ca. Most of women living alone or with family were consuming less than 75% of the Korean Recommended Dietary Allowances(RDA). For the females living with a spouse, the number or nutrients consumed below 7s% or the Korean RDA were significantly lower than that for those living alone and the elderly living with family. As a result of the above analysis, the health and nutritional statement of old women with a spouse was better than that of the old living alone or with family.

문헌과 이탈 귀순자 설문조사를 통해 본 북한인의 보건영양상태 (Assessment of Health and Nutritional Status of North Koreans Utilizing an Exhaustive Literature Search and Survey)

  • North Korea has been experiencing a serious food shortage. However, the extent and magnitude of malnutrition and health problems are virturally unknown because a nationwide food and nutritional survey has never been conducted. To get a better udnerstanding of the situation., an exhaustive literature search on the health and nutritiona conditions of North Koreans was performed. We also obtained anthropometric data on 274 North Korean Defectors who are recently resettlers in the South. Some(137) measurements were taken at the time of entry to the South, and the remaining data was from the health and nutrition mail survey conducted by us on North Korean refugeee re-settlers. Health and inutritional status of North Koreans was found to be less than desirable as shown in high infant mortality rates, high mortality and morbidity rates of malnutriton-related infectious deseases, and smaller body sizes. Striking differences were noted in all of the above health and nutrition indicators between north and South Koreans. Considering the fact that North and South Koreans are homogeneous in their genetic back-ground, ours study results clearly demonstrate that the health and nutrition of North Koreas has deteriorated with the dhronic food shortage, which might have sorsened with a recent food crissi. The smaller and thinner adult stature proevailing in North Korea could play a significant negative role in the country's economy, since lower lean body mass has been associated with reduced work productivity that could hinder economic growth and development.
