Purpose: The purpose of this study was to develop and examine the feasibility of OSCE for novice nurses. Method: The study was designed using a methodological design. Data were collected from January to February, 2005. Nine stations were developed for the examination. Five minutes was allowed for each station. The examination was administered to 2 groups of novice nurses. Results: OSCE for clinical performance improvement which consisted of nine 5-minute long stations, was developed. Those stations were tested by 79 beginning nurses who were divided into 2 groups. Reliability ranges for Cronbach's alpha were $0.399{\sim}0.811$ and the mean value was 0.797. Total mean score for the OSCE was 66.75. Total score for the 2nd group was significantly higher than the 1st group. The nurses were satisfied with the OSCE method and process. Conclusion: The OSCE can be an effective tool for evaluating clinical nursing skills in novice nurse. More reliable and valid stations need to be developed and then repeated evaluation of the nurses can be done.
Purpose: The purpose of this study was to test the validity of a modified clinical performance examination (CPX) for preclinical students in nursing. Method: 70 nursing students in their second semester of the junior year at C University participated in CPX. Scenarios and checklists were developed by our research team from September to October 2005. Six stations were organized. Evaluation included physical examination of a patient with lung cancer, education on usage of a metered dosage inhaler, and lobectomy postoperative care. Students were randomly assigned to a station. Result: There was a difference in the CPX scores according to stations. The agreement of scoring between trained faculty members and SPs was more than moderate (r=.647). The correlation between the CPX score and the average grade in the previous semester and between the CPX score and the average grade of a paper and pen test of the pulmonary system of adults was low (r=.276; r=.048). Conclusion: Traditional CPX is generally recommended, however, modified CPX is appropriate for preclinical students in the current Korean Nursing school setting if there are additional scoring systems to balance the testing level at each station.

Purpose: It is recently reported that the increasing noise in the hospitals has caused psychological and physiological stress problems with patients, and medical staffs. This study intends to investigate and analyze the noise levels in the sub stations in comparison with those in the wards in general. This study tries also to find some alterative solutions to the immediate problems. Methods: Noise measurements are conducted in advance prior to analyzing their results at two general hospitals with more than 900 beds, in the comprehensive nursing service wards and in the main/ sub stations located in the general wards Results: Although the noise level in the comprehensive nursing service wards has been slightly lower than that in the general wards, this result is over the recommended noise levels. Therefore it is recommended that efforts should be made to low down the noise level as an alternative and to replace the aged carts as well. Implications: The comprehensive nursing service wards are required to take measures against the various noise sources.

Purpose: The main objective of this paper is, to evaluate the unit design for patient-centered care and to draw lessons-learned for further improvement. Methods: This study conducted a case study of the intensive care unit, designed to fulfill patient-centered care in the US. It evaluated the effectiveness of the unit by incorporating several study methods such as plan analyses, direct observations through nursing tracking and behavior mapping, and focus group interviews. Results: The major design decision made in this patient-centered unit was the use of patient rooms with designated family areas and distributed nursing stations. Both design features appeared to be a success on a variety of research metrics and outcomes. The study identified that the patient rooms ultimately help family members to spent more time with their loved ones, which leads to increased satisfaction of family members and nurses also report that they generally enjoy the distributed nurses' stations, which provide a comfortable environment to complete their regular lines of work such as charting, monitoring patients, and collaborating with their colleagues. Implications: Certain design features in intensive care units such as patient rooms with designated family areas and distributed nursing stations could appropriately support hospitals to fulfill patient-centered care.

Purpose: The purpose of this study was to provide fundamental data to develop an educational program for the development of nurses' core competences, and to improve the curriculum in nursing education institutions. Methods: This study, based on a descriptive survey, was conducted with 1,019 nurses working in a tertiary hospital in Seoul. Among 47 core nursing practice items, core nursing practices as perceived by the participants and relevant educational requirements were measured on a four-point scale. The collected data were analyzed using descriptive statistics for the mean and standard deviation, one way ANOVA, and Duncan test as post-hoc test. Results: Regarding the core nursing practice perceived as by the nurses, there were statistically significant differences in 43 out of 47 items according to nursing unit, and in 15 items according to clinical career. Regarding the educational requirements for core nursing practice, there were also statistically significant difference for the same items as perception differences according to nursing unit and clinical career. Conclusion: The results of this study suggest further study on the development of educational programs to advance the clinical expertise of nurses and for the development of curriculum reflecting the actual conditions and requirements in clinical nursing practice fields.
Purpose: The aim of this study was to examine relationships between occupational stress, burnout and job satisfaction of PAs(Physician Assistant). Methods: This study was a descriptive study, and 136 PAs were recruited from hospitals in B metropolitan city. The study was approved by KUIRB, and data were collected from September to October, 2013. Results: A strong positive correlation was observed between occupational stress and burnout (r=.715, p<.001). Strong negative correlations were observed between occupational stress and job satisfaction (r=-.761, p<.001), and between burnout and job satisfaction (r=-.624, p<.001). Conclusion: An intervention program should be developed for PAs to reduce occupational stress and burnout in the health care environment.
Purpose: The purpose of this study was to investigate the mediating effect of perceived appraisal support between neuroticism and occupational stress in firefighters. Methods: The subjects of this study were 156 firefighters who worked at three fire stations in G province in Korea. The collected data were analyzed by t-test, ANOVA, and multiple regression analysis using Baron and Kenny's method for mediation. Results: There were significant relationships between neuroticism and perceived appraisal support (r=.30, p<.001), neuroticism and occupational stress (r=.42, p<.001), and perceived appraisal support and occupational stress (r=.36, p<.001). Perceived appraisal support showed partial mediating effects in the relationship between neuroticism and occupational stress. Conclusion: The impact of neuroticism in firefighters was mediated by perceived appraisal support for occupational stress. This suggests that strategies for enhancing perceived appraisal support in fire fighters should be considered when developing neuroticism management interventions for decreasing their occupational stress.
Purpose: This descriptive correlational study investigated the effects of violent experience and resilience on burnout in emergency medical technicians (EMTs). Methods: The participants were 160 EMTs working in fire stations (safety center and local center) located in U, B, and D city. The collected data were analyzed by descriptive statistics, independent t-test, ANOVA, $Scheff{\acute{e}}$ test post-hoc analysis, Pearson correlation coefficients, and multiple regression analysis using IBM SPSS Statistics 20.0. Results: The number of EMTs getting into ambulances was significantly associated with violent experience, resilience, and burnout. Position of EMTs was significantly associated with both resilience and burnout. In the multiple regression analysis, the subscales of durability and optimism in resilience were significantly associated with burnout after controlling for confounding variables. Conclusion: This study indicated that improving durability and optimism in resilience was important to cope with experience of violence and to prevent burnout in EMTs.
As the elderly population are rapidly growing, the elderly who needs special care and nursing homes for them are also growing. However, many nursing homes were planned without considering the hierarchial space organization such as division of housing unit and nursing unit based on the cognitive and behavioral characteristics of the elderly or accessibility and supportiveness of the staff. This study was to examine the hierarchial characteristics in space organization of nursing homes by using Space Syntax. The subjects were 8 nursing homes in Busan. First, spaces in nursing homes were divided into five areas based on the literature review. Then the hierarchy of space organization for the access layer and layers of living floors was grasped by using G-graph. The access layer has a tree-type hierarchical structure and high visuality. Also, average depth of the space was 5. Then the full integration was analyzed. Mobile area, such as elevator, hallways, or living rooms has a high accessibility and the middle corridor type has commonly been characterized. The nursing stations were analyzed through the control degree and it was low in three nursing homes such as B, C, and G. The low clarity was showed in B, D, and F. The low clarity means that it is relatively difficult to recognize the whole space organization of nursing home. Even though the hierarchy of their living floors was tree-type structure with a high visuality, they has a type of grouping spaces around hallways and spreading, or a ring type.
This study investigated the Infant Institutional Household (IIH) for the care of infants. The purpose was to provide basic information needed to raise Korea's birth rate and to design higher level nursing facilities which can satisfy working mothers who are presently offered few facilities. To achieve this purpose. IIM was divided into 7 sub-functional stations : planning-management, marketing, materials-management, business-management, man power-management, financial-management and control etc. The results are as follows : 1. Kind of facilities : Infants'(full responsibility) Institutional Household. 2. Number of infants : total 19 babies. 3. Ratio of infants to teachers : one to one (Contained assistance teachers) 4. Installing region : convenient traffic place - in Seoul. 5. Building site and kind : the first floor space of 60pyung (198㎡) in the Apartment complex which . contained many apartments of small size. 6. Nursing time : 24-hour day care in weekdays Age of object : age from 1month to 24months old babies(after his/her birth). 7. Kind of services and Cost : mother-like care, cost includes nursing-fee, a deposit and admission-fee. 8. Economic Condition and managing direction : pursuit limited profits with nursing-fee, admission-fee, a loan and national (or self -government of direct) supplementary funds.
