The Influence of Job Stress on Job Attitude and Moderating Effects of Organizational Support among Hospital Nurses (임상간호사의 직무스트레스와 직무태도의 관계에 있어 조직지원인식의 조절효과)

  • The purpose of this study is to analyze the effects of job stress on job attitude among nurse practitioners, determine moderating effects of perceived organizational support in this process, and provide preliminary data to devise a scheme for managing job attitude effectively in nursing manpower. The research model was composed of 7 sub-variables of job stress, which was a independent variable; job attitude, which was a dependent variable, was divided into job satisfaction and organizational commitment; and variables of moderating effects included perceived organizational support. The data for empirical analysis of the model were collected from 300 nurses working in one university hospital and two general hospitals in Busan by using structured self-administered questionnaires. The main results of empirical analysis were as follows: Perceived organizational support was effective in preventing job stress from worsening job attitude. Moderating effects of perceived organizational support were found to exist in relations between job stress and organizational commitment. These results imply that a sense of organizational support has both direct and moderating effects on job attitude and can be a good means of managing job attitude.

  • Quality management is a recent phenomenon. Advanced civilizations that supported the arts and crafts allowed clients to choose goods meeting higher quality standards than normal goods. There are many methods for quality improvement. Health care, refers to the treatment and management of illness, and the preservation of health through services offered by the medical, dental, complementary and alternative medicine, pharmaceutical, clinical laboratory sciences (in vitro diagnostics), nursing, and allied health professions. Health care embraces all the goods and services designed to promote health, including "preventive, curative and palliative interventions, whether directed to individuals or to populations. The overall impact of managed care remains widely debated. Proponents argue that it has increased efficiency, improved overall standards, and led to a better understanding of the relationship between costs and quality. Practices can solicit feedback from patients in a variety of ways: phone surveys, written surveys, focus groups or personal interviews. What do I do with the results? While you don't have to act on every suggestion that your patients give you, you should take action on the key items that are causing dissatisfaction.

  • The main purpose of this study was to investigate the effect of the factors which cause the patients to choose hospital from the view of Christian missionary hospital. The subjects were 194 inpatients and 202 outpatients of the K Medical Center in Busan. The research had been conducted from 20 June to 31 July in 2013. The major results demonstrated that the medical and social services image factors strongly affected to the satisfaction and selectivity of patients. Social role image of religious medical institution should be considered important and it seems beneficent in CRM(Customer Relationship Management) based on this results.

A study on elderly people's satisfaction level with oral exercise program (입 체조 프로그램에 참가한 노인의 만족도에 관한 조사연구)

  • Objectives : This study was to examine the satisfaction level of participants in a oral exercise program geared toward the improvement of oral function, their reuse intention of the program and their willingness of recommendation in an effort to facilitate the advancement of the program. Methods : The subjects in this study were 85 senior citizens who used senior welfare centers and nursing homes in Seoul. A oral exercise program was implemented 24 times from April to June 2009, twice a week, and the selected elderly people participated in the program 20 times or more. An interview survey was conducted by two trained dental hygienists, and the collected data were analyzed. Results : 1. As for satisfaction level with each part of the program, the elderly people investigated expressed the best satisfaction with the service supplier(a mean of 3.81), followed by the usefulness of the procedure of the program(3.77) and oral exercise(3.64). 2. The satisfaction level with oral exercise had a significant impact on their overall satisfaction with the program, reuse intention and willingness of recommendation, and the entire satisfaction level with the program was correlated to reuse intention and willingness of recommendation(p<0.001). 3. Out of the demographic characteristics, marital status and whether to live with their families or not made a significant difference to their satisfaction level with oral exercise(p<0.05). Conclusions : It took 20 minutes to get oral exercise, which consisted of warming-up, exercise for each part of oral and cooking-down, and they found it appropriate to spend that time and had no difficulties in repeating after the instructor.

Influential Factors on Turnover Intention of Nurses in Long-term Care Hospitals (노인요양병원 간호사의 이직의도에 영향을 미치는 요인)

  • Objectives : The purpose of this study was to identify levels of turnover intention of nurses in long-term care hospitals, and to explore influential factors on turnover intention. Methods : Data were collected with a structured questionnaires from 165 nurses. The data were analyzed with SPSS/WIN 21.0. Results : First, the average score for the practice environment cognition, job satisfaction, reward importance, and turnover intention were 3.14±0.21, 3.18±0.32, 4.02±0.53, and 3.29±0.67, respectively. Second, there were significant differences in the turnover intention according to the average monthly wage, total clinical career, present clinical career, work form, average monthly night shift and turnover experience. Third, the significant predictors of turnover intention were monthly salary, practice environment cognition, reward importance, monthly night shift and type of work explaining 67.0%. of the variance. Conclusions : It is necessary to conduct continuous and systematic research and to find ways that can prevent the resignation of nurses and improve cognition in the practice environment in long-term hospitals nurses.

Structural Analysis on Quality of Life for Terminal Cancer Patients provided Hospice and Palliative Care Services in Community Health Centers (보건소 호스피스완화케어 서비스를 제공받는 재가 말기암 환자의 삶의 질 구조분석)

  • Objectives: The purpose of this study was to identify the predictors of quality of life (QOL) for terminal cancer patients admitted into the community health center, and to establish a hypothetical model to explain and verify causative relationships among the variables. Methods: Data were collected from January 2015 to June 2016. Participants were 237 registered patients in Busan Metropolitan City hospice & palliative care center. The descriptive and correlation statistics were analyzed using the SPSS/WIN 24.0, and the structural equation modeling procedure was performed using the AMOS 24.0 program. Results: The results of this study showed that the physical symptoms of terminal cancer patients were the most direct factors affecting the QOL, and satisfaction with health care services has a direct effect on the QOL. Conclusions: The study contributes to drawing up measures to improve QOL for terminally ill cancer patients who are living in the end-of-life section of the community by revealing the causal relationship to the QOL for terminal cancer patients.

  • The purpose of this case study is to apply for the elderly development of health promotion through exercise program in urban health center. The subject of the case study was 20 elder women of volunteer in district aged house. Performance of exercise case program took for 14 minute in once, weekly, enforced 7 times from Oct. 1 to Nov. 26. The result was appeared improvement physical health status and self-satisfaction after the exercise program. In conclusion, this case study indicates that exercise program for the elderly could be effective in health promotion and this program could lead to health promotion service in urban health center.

  • The purpose of this study was to understand the structure of the lived experience by poor elderly people. The research question was 'What is the structure of the lived experience of life of a poor elderly people.' The sample consisted of 21 single poor elderly persons in Pusan. The unstructured interviews were audio-recorded and analyzed using the Van Kaam method. This study was 368 responses which yielded of descriptive expressions and priority classifications. The result generated 74 common elements, 18 syntheses of hypothetical definitions and 5 identifications of the structural definition. The structural definitions and hypothetical definitions were as follows; 1. physical discomfort ; complaints of severe pain ; dysfunction of physical organs 2. emotional cognition ; despair ; resignation ; attitude toward death 3. support system ; interaction with family ; thinking about God ; economical difficulties ; expectancy of social services ; opinions about health service ; leisure ; interaction with neighborhood ; dissatisfaction due to inadequate environment 4. past reminiscence ; negative reminiscence of one's past ; past regret ; positive reminiscence of one's past 5. desire ; desire of unrealization life ; self satisfaction The significance of this study for nursing are; Comprehension of the lived experience of client and identification' of nursing approach method concerning the lifestyle of client.

Trajectories of subjective health status among married postmenopausal women based on the ecological system theory: a longitudinal analysis using a latent growth model (생태체계 이론을 적용한 기혼 폐경 여성의 주관적 건강상태에 대한 궤적: 잠재성장모형을 이용한 종단연구)

  • Purpose: This study investigated the trajectory of subjective health status in married postmenopausal women and aimed to identify predictive factors affecting subjective health status. Methods: Data were obtained from women who participated in wave 4 (2012) of the Korean Longitudinal Survey of Women & Families Longitudinal Study and continued to the latest phase (wave 7, 2018). A latent growth model (LGM) was used to analyze data from 1,719 married postmenopausal women in the framework of the ecological system theory. Results: The mean age of the participants at wave 4 was 56.39±4.71 years, and the average subjective health status was around the midpoint (3.19±0.84). LGM analysis confirmed that subjective health status decreased over time (initial B=3.21, slope B=-0.03). The factors affecting initial subjective health were age, body mass index, frequency of vigorous physical activity (microsystem level), marital satisfaction (mesosystem level), and medical service utilization (macrosystem level). Medical service utilization and the frequency of vigorous physical activity were identified as predictive factors affecting the slope in subjective health status. The model fit was satisfactory (TLI=.92, CFI=.95, and RMSEA=.04). Conclusion: This analysis of the trajectory of subjective health status of married postmenopausal women over time confirmed that subjective health is influenced by overall ecological system factors, including the microsystem, mesosystem, exosystem, macrosystem, and chronosystem. Therefore, it is necessary to assess physical activity and support policies promoting access to medical services in order to improve the subjective health status of married postmenopausal women.

The Factors Affecting Job Stress and Job Satisfaction of Nurses at Welfare Centers in Seoul (서울시 복지관에 근무하는 간호사의 직무스트레스와 직무만족도에 영향을 미치는 요인)

  • Purpose: The purpose of this research was to find out factors affecting job stress and job satisfaction of nurses working at the welfare centers. Methods: This study was performed with 140 nurses who were working at or had retired within 1 year from general welfare centers in Seoul during the period from March 29 to February 29 2004. We used a questionnaire asking about general characteristics. work characteristics. stress factors and job satisfaction. We analysed 103 subjects who replied to the questionnaires. Results: The mean job stress level was 2.84 out of 5 and. as for the sub-factors of job stress. income was 4.19, career development 3.69, service environment 3.26 and support system 1.90. The mean job satisfaction level was 3.32 out of 5 and. as for the sub-factors of job satisfaction, job control was 3.58, efficiency of organization 3.27, job burden and responsibility 2.92 and role ambiguity 2.25. When the nurses' job stress was compared according to their general and work characteristics. stress level was significantly high in nurses who had a short experience. were working at the department of senior welfare center. or were the only staff at their workplace (p <0.05). In addition. satisfaction level was significantly high in nurses who were working at the department of senior welfare center and medical welfare team. were the only staff at their workplace or had a heavy work load (p<0.05). According to the results of multiple regression analysis. job stress level was influenced by age (R2=0.212. p<0.01). and job satisfaction level by the number of staffs and work' load (R2=0.272. p<0.1). Conclusions: The results of the study suggest that good atmosphere should be made up to communicate between the departments at the workplace in order to lessen job stress and enhance job satisfaction of nurses working at welfare centers. In addition. role division and job duties among special staffs should be reviewed in order to achieve high efficiency in performing job duties.
