Evaluation of an Integrated Community-based Home Care Service Pilot Project (지역사회 가정간호 통합서비스 시범사업 효과평가)

  • Purpose: This study evaluated the effectiveness of a community-based home care service pilot project conducted by C University Hospital for patients receiving home care services. Methods: This study employed a one-group pre-test-post-test design using secondary data. Between June 2021 and June 2022, 67 patients participated in a community-based home care service pilot project. The data were analyzed using SPSS/WIN 26.0, applying descriptive statistics, McNemar's Chi-square test, and paired t-tests. Results: The participants' average daily walking time increased from 35.40(±38.66) min to 46.49(±41.40) min (t=-2.23, p=.029) and the proportion reporting "good" perceived subjective health increased from 10.4% to 22.4% (χ2=10.47, p=.015). Family satisfaction averaged 4.27(±0.49), with reliability scoring the highest at 4.33(±0.49). The success rates of resource linkages were relatively high for medical treatment (medical consultations), examinations (83.7%), and residential environment improvements (62.7%). Conclusion: Home healthcare and registered nurses effectively collaborated to deliver comprehensive care, care even in patients excluded from long-term care. Home care nursing may contribute to the development of sustainable, community-based, integrated care models that require essential policy and institutional support.

The Evaluation of the Community based Opthalmologic Health Services for Elderly in Chuncheon City, Kangwon Province (지역사회 중심의 노인 안보건사업 평가 - 강원도 춘천시를 중심으로 -)

  • This study is designed to evaluation of the Community based Opthalmologic Health Services(CBOHS) for elderly carried out for last three years from 1994. The subjects were total 236 aged over 57 in Chuncheon city. Kangwon province. and the period of data collection was from Feb. 24th. 1996 to Feb. 27th. 1996. For evaluation of CBOHS. the questionnaire was consisted of participation rate. and perception about the service. the level of satisfaction and things to improve. Result were obtained as follows: 1. The number of participants was 95(40.3%) among total subjects. The places provided services were the health service center of Seoul National University in Chuncheon city and community health center. The participants were known from announcements of community staff(66.3%). 2. Non participants were 59.7% among total subjects. mam causes of missing wasn't known'. 3. The services that participants screening test(41.2%). provided provided operation(45.5%). 4. The question about things to improve were continuous follow up and more many glasses provision These fingings may indicate that the CBOHS need more announcement. continuous follow up and provide more many glasses. As proved the satisfaction of CBOHS for elderly. the next study can be performed about the relevant factor of the services.

Health Perception and Family Support as Influencing Factors on Depression in Hospitalized Elderly Patients (건강지각과 가족지지가 입원 노인환자의 우울에 미치는 영향)

  • Objectives : The purpose of this study was to test the influencing effects of health perception and family support on depression in elderly patients. Methods : Two-hundred forty-nine hospitalized geriatric patients were recruited in B city, South Korea. They were asked to complete a questionnaire, and 214 data sets were included in multiple regression analysis. Results : Subjects perceived a higher level of depression. Influencing factors of depression were hospitalization experience (t=3.476, p=.001), health perception (t=-3.310, p=.001), education (t=2.684, p=.008), economic satisfaction (t=2.579, p=.011), satisfaction with allowance (t=2.262, p=.025), and family support (t=-2.200, p=.029). The model was statistically significant explaining 36.3% of the variance (F=21.257, p<.001). Conclusions : In dealing with depression, nurses in elderly care facilities should be more concerned about patients with hospitalization experiences. Interventions to improve health perception should be provided to elderly patients, and educational programs on how to support patients are needed for the families of patients.

Effects of Nurse's Parenting Stress and Parenting Efficacy on Turnover Intension of Nurses at Medium-sized Hospitals (중소병원 간호사의 양육스트레스와 양육효능감이 이직의도에 미치는 영향)

  • Objectives : The aim of this research was to identify the relationships among parenting stress, parenting efficacy and turnover intension among nurses working in medium-sized hospitals. Methods : Two-hundred three nurses were recruited in 10 medium-sized hospitals in B city. They were asked to complete a questionnaire, and data were analyzed using SPSS program. Results : Subjects perceived a moderate level of turnover intension. Turnover intension showed significant differences by age (F=3.29, p=.039), working department (F=5.11, p=.007), working form (t=0.36, p=.037), reason for going to work (F=3.13, p=.027), and satisfaction with the job (F=17.94, p<.001), support from colleagues (F=12.82, p<.001). Factors affecting turnover intension were satisfaction of working, parenting support from colleagues, reason for working. The model was statistically significant explaining 24.1% of varience (F=10.141, p<.001). Conclusions : Nursing administrators should consider the experience of married nurses at medium-sized hospitals as a significant human resource, and be aware that supplementing this can reduce nurses' turnover intension.

Patient experience and recommendation intention at specialty hospitals (전문병원 입원환자의 환자경험 및 추천의향)

  • Purposes: Patient experience is a tool to evaluate the process and results of medical services provided by medical institutions from the patient's point of view. Patient satisfaction surveys are a meaningful and essential source of information for improving quality in healthcare organizations. This study aims to provide basic data for improving the quality of medical service that patients can feel by analyzing the recommendation intention and satisfaction of inpatients in specialty hospitals. Methodology: The subjects of this study were 879 inpatients in 28 specialty hospitals in 14 designated fields. We conducted a telephone survey with a structured questionnaire on the satisfaction and recommendation intention for specialty hospitals. Findings: In inpatients, hospital satisfaction was higher in nursing care services and hospital satisfaction was low in physicians care services. The overall patient satisfaction score was 91.4(SD=11.9) out of 100, and the intention of recommendation was 92.0(SD=14.1) out of 100. The factors affecting patient experience were designated fields, sex, age, residential area, monthly household income, and perceived health status. Practical Implications: This study confirmed the high level of patient satisfaction and recommendation intention among inpatients of specialty hospitals. Patient satisfaction can be of great value to healthcare providers in recognizing and improving the quality of care, as well as predicting patients' willingness to revisit medical institutions. This study can be used to improve the quality of hospital care services in specialty hospitals rather than general and tertiary general hospitals.

Evaluation of a Community-Based Rehabilitation Program in Wonju City (원주시 지역사회중심 재활사업 평가)

  • This study evaluated the community-based rehabilitation services provided by the Wonju Public Health Center from Jan. 2000 to Dec. 2002. Ninety-four persons with disabilities dwelling in the community participated and the surveys were completed in an interview during home visits. The respondents' demographic, socio-economic, and medical characteristics, rehabilitation service received, willingness to receive home-visit rehabilitation services, and satisfaction with the rehabilitation services were analyzed by frequency and percentage. A Likert scoring system consisting of five agreement-disagreement categories was applied to each item, consisting of Very Satisfied, Satisfied, So-So, Poorly Satisfied, and Very Poorly Satisfied. The major findings were as follows: 1) The rehabilitation services used included medical rehabilitation (26.9%), followed by social assistance (23.5%), diagnosis by a physician at home (17.3%), medical examination (12.3%), housekeeping services (6.2%), and vocational and educational rehabilitation (3.5%). 2) Of the medical services, the respondents desired physical therapy at home and free rental of rehabilitation equipment, such as wheelchairs, canes, walkers, the most, followed by home visit occupational therapy, nursing services, and oriental medicine service in descending order. 3) Some of the respondents expressed so-so satisfaction (50.0%) or dissatisfaction (16.9%) with the rehabilitation services provided by the Wonju Public Health Center. These findings should prove useful when planning or extending community-based rehabilitation programs for the homebound disabled in the community.

Research Trend on Internal Marketing of Medical Service Organization

  • In this research aimed to deduce internal marketing factors, purpose, and their practical application by analyzing preceding researches on internal marketing of Korean medical service organization and investigating the recent trend of its research. Subjects of research are ten preceding researches that have been published in KCI records for the last five years from 2011 to 2016. Summarize result of researches, first, internal factors that were most frequently used were internal communication, compensation system, and education and training, which were used by 8(.8). Second, occupations that had most interest in the internal marketing research appeared to be nursing(.9) and administration(.3). Third, the practical application of the internal marketing appeared to be job satisfaction(.8), followed by customer orientation(.6), and organizational commitment(.4). Suggestion do, necessary to develop subordinate factors regarding the realistic internal marketing, such as both-sided internal communication enhancement, education and training, compensation system differentiated by individuals and teams, fairness in performance rating, work environment improvement, delegation of authority, career development, shared organizational vision in order to maximize job satisfaction, job commitment, and organizational commitment of employees as internal customers, before establishing strategies to satisfy patients and guardians who are external customers.

Maternal Health Effects of Internet-Based Education Interventions during the Postpartum Period: A Systematic Review (인터넷 기반 교육 중재가 산욕기 어머니 건강에 미치는 영향에 대한 체계적 고찰)

  • Purpose: This study aimed to investigate the maternal health effects of internet-based education interventions on parturients during the postpartum period through a systematic review of randomized controlled trials. Methods: An electronic literature search of the Cochrane Library, CINAHL, EMBASE, Eric, PsycINFO, PubMed, RISS, and KISS databases was performed, using the combination of keywords such as 'parenthood education', '*natal education', '*birth intervention', 'internet-based intervention', 'randomized controlled trial'. The inclusion criteria were peer-reviewed papers in English regarding randomized controlled trials of internet-based postnatal education interventions. Educational interventions were delivered through any web, mobile, eHealth, mHealth, virtual reality, short message service, or social networking service platform. Quality appraisal was performed using the Risk of Bias 2 (RoB 2) for randomized controlled trials. Nine articles were yielded, and the intervention effects were analyzed. Results: Internet-based education interventions during the postpartum period affect maternal self-efficacy, postpartum depression, and successive breastfeeding; however, they do not affect maternal satisfaction and parenting confidence. Conclusion: This study demonstrated that internet-based education interventions affect maternal health status in terms of psychological, emotional, and physical wellness. Therefore, maternal health care professionals can utilize remote education using the internet or mobile-based interventions during the postpartum period.

An analysis on nursing activity in a hospital (병원 간호인력의 간호활동량에 관한 연구)

  • The management of nursing resource is very important. that is because nursing staff accounts for 30-40% of total staff in a hospital and nurses provide patoents with attentive service for hours daily. The continuous turnover of nursing staff. however, impedes the quality-oriented nursing care, which will consequently leads to the loss of human and material resource in competitive society. This study aimed to calculate nurse's activity and compare the activity amount based on career experience. Futhermore it aimed to find factors which would influence 'quality weighed direct nursing activity amount'. Questionnaires and check lists for this study were distributed to nurses in a suburban hospital outside of Seoul from October 20 to November 14. 1997. The nursing activities were calculated according to professionality. independency and working hour. And then it were accumulated by quality score. The collected data was analyzed by statistical methods as t-test, ANOVA, correlation, multiple regression. The results of this study were as follows ; Firstly, carrel' experience had no influence on the quality weighed nursing activity amount. But the quality level of service of skilled nurses was higher than that of new nurses. Secondly, career was a variable affecting the quality in nursing service. So career was positively related to the quality of nursing care. Patient's disease severity and number of patient were positively correlated with weighed nursing activity amount. But job satisfaction was negatively correlated v:ith the amount. Thirdly, the independent variables which had significant influence on the weighed nursing activity amount were disease severity and the number of patients, The severity score and number of patient were directly proportional to the weighed nursing activity amount. This results indicated· that weighed nursing activity amount was influenced by the number of patient and patient's disease severity. The quality score of nursing services for experienced members is higher than that of new staff. But both new and skilled staff showed no difference in the quality weighed nursing activity amount. Internal and. external environment influences nursing activities. The quality of nursing services is very important factor in nursing activity. Therefore nursing managers should make an effort to improve nursing care quality through continuous research. Also they should try to maintain experience nurses and assign nursing staff appropriately with patient's severity and other relevant factors being considered. The quality-improved nursing care in the hospital will strengthen hospital's competitiveness.

Development and Application of Telephone Counseling Services for Care of Patients with Colorectal Cancer

  • Background: The number of colorectal cancer (CRC) patients in Taiwan has increased in recent years; therefore, the effective dissemination of information related to symptom care has become especially important. Previous studies indicated that the physical and psychological status of cancer patients can be effectively improved by telephone counseling services (TCS). Thus, determining the most effective means of establishing a TCS to support the clinical practice of oncology has become a crucial goal for nursing. The purposes of this study were to analysis the content of the TCS for CRC and explore stratification of the TCS. Materials and Methods: The study design was retrospective. A total of 850 calls were made to CRC patients in the cancer center of Southern Taiwan during the period of January 2007- December 2011. A structure questionnaire was adopted to analysis satisfaction. Results: Responses provided by the TCS included information regarding nutrition, side effects resulting from chemotherapy and pain. Moreover, 28.7% of CRC patients needed advanced treatment. More than 90% satisfaction with all aspects of the calls was found. Conclusions: The TCS coulkd be shown to provide an effective means by which to expand the reach of nursing care to different times, places and patients, allowing for greater cost efficiency and more rapid service.
