Development and Evaluation of an Education Program for Professional Palliative Care Nursing

  • Purpose: This study aimed to develop a "Palliative Care Professional" education program and evaluate its effects on the recognition of good death, palliative care, and the meaning of life for nurses. Methods: It was developed based on the hospice care program for volunteers being used in the Hospice Palliative Care Research Center of S University in Seoul. It was also based on the studies which investigated the educational needs of nurses in palliative care. This program consisted of 5 sessions and 16 content items for 2 weeks. A non-equivalent control group non-synchronized design was utilized and participants were assigned to the experimental group (n=42) or the control group (n=44). Results: The recognition of a good death (F=11.44, p=.001), palliative care (F=4.15, p=.045), and the meaning of life (F=5.12, p=.026) increased more significantly for participants in the experimental group than in the control group. Participants felt that they gained further knowledge in palliative patient management and refined their clinical practice. Conclusion: The results of the study indicate that this program could serve as a practical program for palliative care nursing in the nursing field and suggests that more attention should be directed to the diverse educational needs of nurses.

Development and Evaluation of Suicide Prevention Nursing Competency Programs for Visiting Nurses (방문간호사의 자살예방 역량강화를 위한 프로그램 개발 및 평가)

  • Purpose: The purpose of this study was to develop a suicide prevention nursing competency program for visiting nurses, and to examine the effect of this program on suicide prevention-related knowledge, attitudes, and behaviors. Methods: A total of 66 visiting nurses were recruited from 10 public health centers and divided equally into the experimental and control group. For the experimental group, the suicide prevention nursing competency program was provided twice a week for 120 minutes across 3 weeks. Participants were asked questions related to suicide prevention knowledge, attitudes, and behaviors at pre, post, and 1 month after the intervention. Data were analyzed using descriptive statistics, a t-test, repeated measure ANOVA, and Friedman test. Results: There were significant differences in knowledge and behaviors at the measured time periods, and significant differences in attitudes and behaviors between the two groups. There were also significant interactions between groups and times in attitudes and behaviors. These results suggest that the effects of the program were persistent until the 1-month follow-up. Conclusion: The developed suicide prevention nursing competency program is effective in evidence-based education for visiting nurses to increase suicide prevention-related knowledge, attitudes, and behaviors.

Effects of a Rational Emotive Behavior Therapy Counseling Program on Self-efficacy and Adjustment to College Life for Nursing Freshmen (인지정서행동집단상담 프로그램이 간호대학 신입생의 자기효능감과 대학생활적응에 미치는 영향)

  • Purpose: This purpose of this study was to examine the effects of a rational emotive behavior therapy (REBT) counseling program on self-efficacy and adjustment to college life for nursing freshmen. Methods: This study used a nonequivalent control pretest-posttest quasi-experimental design. Thirty-eight new nursing students were randomly assigned to either an experimental group (n=18) or a control group (n=20) recruited from a four-year nursing college. An REBT counseling program was composed of 8 group sessions of orientation, understanding the theory of REBT, finding irrational beliefs, internalizing rational beliefs, and closing over a duration of 5 weeks. The duration of each session was 90 minutes. Self-efficacy and adjustment to school life were measured by the Sherer's Self-Efficacy Scale and Baker & Siryk's Student Adjustment to College before and after the REBT counseling program. Data were analyzed using t-tests and paired t-tests. Results: The experimental group showed a significant improvement in self-efficacy compared to the control group. There was no significant change in college life adjustment between the experimental and control groups. Conclusion: An REBT counseling program could be applied as an effective program in enhancing self-efficacy resulting in positive and goal-oriented behavior for new nursing students in college.

Development of a Decision Support Computer Program for Pain Management in Institutionalized Patients with Dementia (노인요양시설에서의 치매환자 통증관리를 위한 컴퓨터 의사결정지원시스템 개발)

  • Purpose: The purpose of this study was to develop a computer decision making support system that enables nurses to utilize the computer in selecting the best decision for pain management for patients with dementia institutionalized in nursing homes. Methods: To formulate the protocol for the management of patients' pain, the researcher analyzed content of interviews with 30 nurses in three nursing homes and an expert group. A decision support computer program was formalized based on existing protocols. To evaluate the effectiveness and applicability of the system, analysis of data on patient pain management and nurse satisfaction with the system were done after the formalized decision support computer program was complete. Results: The decision support computer program for pain management for institutionalized patients with dementia was finalized after adjustments following the evaluation. Nurse satisfaction with the program was moderate. It also provided opportunity to reassess thinking about pain and pain management. Conclusions: The results indicate that this program provides nurses with useful knowledge for pain management in institutionalized patients with dementia and aids in decision making in nursing practice in nursing home.

The effect of Holistic Health Program on spiritual well-being and mental health in nursing students (전인건강중재 프로그램이 간호대학생의 영적안녕과 정신건강에 미치는 효과)

  • The purpose of this study is to examine the effects of Holistic Health Program on spiritual well-being and mental health of nursing students. A one-group pretest-posttest research design was used. The subjects were 42 nursing students attending C College located in Y city. All subjects participated in 13 week holistic health program. The data were analyzed using frequency, paired t-test, Pearson correlation coefficient. The results of this study were as followings; First, the score of spiritual well-being was significantly increased after Holistic Health Program in the subjects. Second, the score of mental heath was significantly increased after Holistic Health Program in the subjects. Based on these results, it was concluded that Holistic Health Program is effective for improving spiritual well-being and mental health of nursing students. Therefore, we recommend this program to be utilized as an effective method for improving spiritual well-being and mental health of nursing students. And this results will provide basic data to develop nursing practice curriculum in the nursing college.

An Evaluation of the Home Care Nursing Services Conducted jointly by Catholic Churches and Hospital (일부 가톨릭교회와 연계된 병원중심 가정간호사업의 평가)

  • Purpose: This study was performed to evaluate the outcomes of the home care nursing program conducted jointly by thirty two catholic churches and C hospital in Seoul. Method: The subjects included 173 patients who registered for the program during a 4 month-period from November 1, 2004 to February 28, 2005 and received home care services for more than 4 times and 32 professional nurses participating in the program. Using the concept of medical outcome study (MOS), the structure, process, outcome elements were analyzed. Result: 1) A Catholic homecare nursing center and nurses of the C hospital played a central role in providing nursing care, and each church operated its own vehicle from its own office. Home care nurse's job satisfaction was 2.8 out of total score of 4. The major illness was cerebrovascular disease including stroke followed by skeletomuscular disease including degenerative arthritis cancer, and diabetes. Among reasons for accessing the home care nursing program, hypertension management was most prevalent. More than half of the registration was done through catholic churches. Most people who referred the patient to the program was through the church. Most patients received home care nursing 1-2 times a week for 30 to 60 minutes in average and the most frequent type of service provided was basic nursing. 3) The most frequent reason for terminating home care services was death. The change in PPS(Palliative Performance Scale) level from the time of registration and after 4 visits was the same in 45%, decreased in 30%, and improved in 25%. Patient satisfaction was very high, showing 3.4 out of total score of 4. Conclusion: These results proved that the home care nursing program was highly appreciated by subjects and nurses were providing professional care. Thus the two parties involved in the program were actively supporting the program to fulfill their mission. However, several areas needed to be improved such as relating with local community, relating with family doctor, and issue of improving the working conditions for home care nurses.

Development and Implementation of a Self-directed Critical Care Nursing e-Learning Program (자기주도형 중환자간호 이러닝 프로그램 개발 및 적용)

  • Purpose: The purpose of this study was to develop a self-directed e-learning program for ICU nurses and to evaluate how the proposed e-learning program affects the level of knowledge, nursing performance and job satisfaction of ICU nurses. Methods: The e-learning program was developed with the Computer Assisted Instruction Design Model using sources of self-efficacy which included the inactive attainment, vicarious experience, and verbal persuasion of ICU nurses. The program was evaluated by experts. Following the revision of the program, it was applied to a total of 59 ICU nurses (experimental group n=29, control group n=30) from three hospitals. Four weeks later, we measured the level of knowledge, nursing performance and job satisfaction. Results: The level of knowledge significantly improved in the experimental group (t=5.691, p<.001). Moreover, the level of nursing performance significantly increased in the areas of circulatory diagnostic test (t=2.143, p=.039), EKG (t=2.911, p=.006), aortic balloon pump (t=2.491, p=.017), and nebulizer therapy (t=2.085, p=.044). Overall, job satisfaction did not significantly increase (t=1.664, p=.105); however, job satisfaction in terms of relationships with coworkers (t=2.371, p=.023) and the needs of nursing performance (t=2.940, p=.006) were significantly increased. Conclusion: The results showed that the proposed e-learning program using sources of self-efficacy from ICU nurses was an effective learning method to increase the level of knowledge and nursing performance. The propose de-learning program would be a useful teaching tool for ICU nurses improving the quality of ICU patient care.

The Effects of a Multi Agent Obesity Control Program in Obese School Children (학령기 아동을 위한 멀티에이전트 비만관리 프로그램의 효과)

  • Purpose: The purpose of this study was to identify the effects of a multi agent obesity control program in obese school children. This program was composed of strategies to modify diet and exercise habits and to change cognitive behavior variables(stress, coping, and self-efficacy). Method: The subjects were 40 obese school children who participated in our project voluntarily via homepage, TV, newspaper, public paper and school official documents. The program was implemented daily for 4 sessions per day for ten days from August 16 to 26, 2004. The daily program consisted of exercise therapy, dance therapy, cognitive behavior therapy and aroma therapy. The data was analyzed by paired t-test using the SPSSWIN program. Result: There was a significant decrease in children's waist-hip ratio(p=.04) and in children's stress(p=.00) after the program. There was a significant increase in children's self-confidence after the program(p=.02) and a significant decrease in children's diet habit after the program(p=.02). Conclusion: This study provides evidence that a multiagent obese control program is effective in changing waist-hip ratio, stress, self-confidence, and diet habits in obese school children.

Effects of a Ready Planned Self Growth Program through MBTI on Interpersonal Relationships and the Career Identity of Nursing College Students (MBTI를 활용한 성장프로그램이 간호대학생의 대인관계 및 진로정체감에 미치는 효과)

  • Objectives: This study was conducted to investigate the effects of a self growth program through MBTI on interpersonal relationships, and the career identity of nursing college students. Methods: A randomized controlled pre-post test experimental design was used for this study. The 24 nursing students were randomly assigned into two groups: 12 for the experimental group, and 12 for the control group. The subjects were junior nursing students, who were randomly selected from an undergraduate nursing program at K university located in Daegu, Korea. The data was collected from March 18 to April 1, 2002. The MBTI self growth program used in this study was based on the one developed by Shim and Kim (1997). The 12 experimental group students received the group self growth program through MBTI for 15 hours, while the 12 control group students received no treatment, except a series of tests. The Korean version MBTI test used in this study was developed and verified for reliability and validity by Kim and Sim (1990). Change in inter-personal relationships was measured using the Relationship Change Scale developed by Schlein and Guerney (1971), and modified by Lee and Mun (1982). Career identity was measured using the My Vocational Situation developed by Holland. Daiger and Power (1980), and revised by Kim (1997). The data were analyzed with the SPSS Win 10.0 program, through which the 2 test, t-test, and Repeated measures ANOVA were examined. Results: 1) The first hypothesis, 'the experimental group that received the self growth program through MBTI will obtain a higher interpersonal relationship score than the control group', was supported (F=5.295. p=.031. Interaction: p=.000). 2) The second hypothesis, 'the experimental group that received the self growth program through MBTI will obtain a higher career identity score than the control group', was supported (F=28.575, p=.000, Interaction: p=.000). Conclusion: The results showed that the ready planned self growth program through MBTI had positive effects on improving the interpersonal relationships, and the career identity of nursing college students.

Effect of The Spiritual Care Module Education Program for Nurses (간호사의 '영적간호모듈' 간호교육프로그램의 적용 효과)

  • Purpose: This study was to verify the effects of spiritual care module education programs by applying it to nurses. Method: The study employed a non-equivalent control group pretest-posttest design in a quasi-experimental basis. Subjects were 93 nurses (46 in an experimental group and 47 in a control group) with more than two years clinic experience, attending a bachelor program of K University, in I city, Korea. The program consists of courses with 2.5 hours per week for seven weeks. Result: Scores of spiritual needs and spiritual nursing competence increased significantly in the experimental group. The score of spirituality and spiritual well-being also increased in the experimental group, but not significantly. Conclusion: The spiritual care module education program was considered to be an effective nursing intervention education course. Nurses educated with this program seemed to perform better nursing interventions for subjects facing difficulties or confusion by helping them restore and cope with those problems by themselves. Therefore, it is recommended that spiritual care module education should be settled as a regular course of nursing college with consideration to the corrections and supplements mentioned in this study.
