허혈성심질환 발생에 대한 대사증후군과 비만의 개별효과와 결합효과 (Combined Influence of Obesity and Metabolic Syndrome on Ischemic Heart Disease in Korean middle aged and older adults)

  • Purpose: The purpose of this study was to examine a combined influence of obesity and metabolic syndrome on ischemic heart disease in Korean middle aged and older adults. Methods: This study used secondary data from the 2013 Korea National Health and Nutrition Examination Survey. A total of 3,726 adults over age 40 were included. Logistic regression was used for analysis of complex samples. Gender, age, educational level, family income, family history of ischemic heart disease, physical activity, smoking, and heavy alcohol consumption were analyzed as covariates. Results: Ischemic heart disease was more prevalent among adults with metabolic syndrome regardless of obesity (non-obesity: Adjusted Odds Ratio [AOR]: 3.044, 95% Confidence Interval [CI]: 1.163-7.967, overweight: AOR: 2.805, 95% CI: 1.246-6.316, obese: AOR: 3.137, 95% CI: 1.548-6.358) compared to the reference group, defined as adults with non-obesity and non-metabolic syndrome. Odds of ischemic heart disease were not significant in the group with obesity and non-metabolic syndrome compared to the reference group. Conclusion: The results of this study show that the population with metabolic syndrome is an at-risk group for ischemic heart disease. Thus, management of metabolic syndrome is required for prevention of ischemic heart disease.

일 대학 보건의료전공학생의 건강행위특성과 골밀도와의 관련성 (Relationship between Health Behavior Factors and Bone Mineral Density among College Students in a Health-Related Department)

  • Purpose: The study investigated the relationship of health behavior with bone mineral density (BMD) for college students. Methods: A descriptive study was done using a convenient sampling of college students (n=116) enrolled in a health-related department. Height, weight, body mass index (BMI), health behaviors recognition of subjective health, smoking, drinking, exercise, food habits, intake of calcium, and family history of fracture were measured. BMD was measured at the right forearm using Hologic lunar duel-energy X-ray absorptiometry. Data were analyzed using descriptive analysis, t-test, ANOVA and multiple linear regression. Results: The prevalence of osteopenia and osteoporosis was 41.4% and 22.4%, respectively. Significant relationships were observed between BMD of right forearm and gender, BMI, subjective health recognition, drinking, and exercise (p<.05). Results of linear regression after adjusted BMD were increased concerning subjective health recognition and regular exercise (p<.05). Conclusions: Subjective health recognition and exercise carries positive effects on BMD. We recommend for college students that healthy behaviors like proper weight, smoking cessation, regular exercise, regular food habits, and health awareness are helpful to BMD.

사회경제적 요인에 따른 지역사회 저소득층 중년여성의 심혈관질환 위험요인 차이 (Cardiovascular Disease Risk according to Socioeconomic Factors among Low-income Midlife Women)

  • Purpose: To determine the degree of cardiovascular disease risk according to socioeconomic factors among midlife women in the community and thereby provide baseline data for the development of health promotion programs. Method: A total of 200 women participated in health screenings and a health survey. The survey was performed in November and December, 2006. The survey instruments included socioeconomic factors, health behavior (smoking and exercise), and family history. Biophysical measurement included BMI and blood pressure. Blood samples were drawn for glucose and total cholesterol tests. Results: The mean age was 52.5 years, 34.0% had received education less than 6 years, 70.0% earned a monthly income of less than \1,500,000, and 61.5% were homemakers. Cardiovascular disease risk was significantly different by age (BMI, systolic BP, and exercise), education (systolic BP), monthly income (T. cholesterol), marital status (smoking), and occupation (exercise). Most women had 2 or 3 cardiovascular disease risks. Older age, lower education, and lower income were significantly associated with increased cardiovascular disease risk. Conclusion: Efforts should be made to decrease the number and severity of cardiovascular disease risk factors for midlife women in the community by developing health promotion programs targeting to modify their cardiovascular disease risk factors.

류마티스질환 환자의 인지손상과 관련된 요인 규명: 체계적 문헌고찰 및 메타분석 (Correlates of Cognitive Impairment of Rheumatic Disease: Systematic Review and Meta-analysis)

  • Purpose: This study was conducted to synthesis the results of research on relationships of cognitive impairment with multi-dimensional correlates of rheumatic disease through a systematic literature review and meta-analysis. Methods: For the study purpose, 23 studies were selected through a systematic process of searching the literature. Results: The study results showed that among general characteristics, age and education were the variables having a significant relationship with cognitive impairment. Among health risk factors, obesity appeared to have a significant positive relationship with cognitive impairment. For past history, diabetes and hypertension were shown to have a significant positive relationship with cognitive impairment. It was noted also that aPL, one of the physiological factor, had significant association with cognitive impairment. None of the medication related factors had a significant relationship with cognitive impairment. Results showed that among disease related factors, disease activity had the highest relationship with cognitive impairment. Depression, among psychological factors, was the only variable having a significant relationship with cognitive impairment. Conclusion: The findings indicate that the variables strongly impacting on cognitive impairment in rheumatic disease are depression and disease activity.

문제음주 여성의 임신초기 경험 (Experience of Pregnant Women with Problem Drinking during First Trimester of Pregnancy)

  • Purpose: The purpose of this study was to explore the meaning of pregnant women's experiences with drinking alcohol during first trimester of pregnancy Methods: The data were collected through in-depth interviews of 7 pregnant women who drank alcohol in the first trimester. Giorgi's phenomenological method was used for data analysis. Results: Findings included 6 main themes and 14 themes. The main themes concerning pregnancy and drinking were: 'Open attitude in drinking, History of drinking in family or spouse, Seeking information in how drinking affects pregnancy, Regret not doing planned pregnancy and not quitting drinking before pregnancy, Willing to stop drinking until the child birth, Awareness about importance of preconception care. Conclusion: The results of this study provide a deeper understanding of pregnant women's experiences of drinking alcohol during the first trimester of pregnancy. These results can be used in the development of strategies to prevent drinking alcohol during first trimester and to support preconception care and prenatal care.

40세 이상 성인 남녀의 대사증후군 관련 심리사회적 요인 (Psychosocial Factors Associated with Metabolic Syndrome among Korean Men and Women Aged over 40 Years)

  • Objectives: This study identified biopsychosocial factors associated with metabolic syndrome among Korean men and women aged over 40 years. Methods: Secondary data from the 2010-2016 Korean National Health and Nutrition Examination Survey were used for this study. Based on the biopsychosocial model, psychosocial factors (stress, depression, smoking, binge alcohol consumption, physical activity) were assessed with control of biomedical (age, body mass index, family history of hypertension, dyslipidemia, type 2 diabetes mellitus, and cardiovascular disease) and biosocial factors (educational level and economic status). Data from 8,624 men and 7,321 women were analyzed by logistic regression analysis using a complex sample procedure. Results: Among men, current or past smoking habits (Adjusted Odds Ratio [AOR]: 1.349; 95% Confidence Interval [CI]: 1.155-1.575, p<.001) and binge alcohol consumption (AOR: 1.570, CI: 1.389-1.774, p<.001) were associated with metabolic syndrome. In addition, moderate (AOR: 1.205, CI: 1.038-1.400, p=.014) and low levels of physical activity (AOR: 1.296, CI: 1.109-1.514, p=.001) were associated with metabolic syndrome. Among women, low level of physical activity (AOR: 1.276, CI: 1.017-1.602, p=.036) was associated with metabolic syndrome. Conclusion: Gender specific interventions such as encouraging physical activity and prevention of smoking and excessive alcohol drinking are important to prevention of metabolic syndrome.

지방공무원의 직무스트레스와 음주, 흡연 행태에 관한 연구 (A Correlational Study of Job Stress, Drinking and Smoking of Local Government Staffs)

  • Purpose: The aim of this study was to identify the correlation among job stress, drinking, and smoking of local government staff. Methods: The participants of this study were 205 government staffs working in a local government office. The data were collected by self-report using questionnaires from March 2 to April 30, 2009. The data were analyzed using t-test, Chisquare test, ANOVA, and Man-Whitney test. Results: Physical stress was the highest in job stress components. Majority of the participants (96%) had drinking experiences more than once per month. 25.4% of respondents reported they drank more than 10 glasses each time. Smoking was habitual in 36.6% of the subjects and 28% of subjects had participation in a smoking cessation program. Respondents who reported high job stress had a longer history of smoking. Conclusions: Job stress was highly associated with drinking and smoking. Stress control programs and smoking and alcohol cessation/moderation programs could be beneficial for the health of government staffs.

일 지역 대학생의 음주거부 자기효능감 영향 요인 (Factors Influencing on Drinking Refusal Self-Efficacy among University Students)

  • Purpose: The purpose of this study was to identify factors influencing on drinking refusal self-efficacy among university students. Method: The subjects consisted of 228 university students. The data were collected using self-reported questionnaires, which were constructed to include satisfaction of campus life and the DRSEQ-R(Drinking Refusal Self-Efficacy Questionnaire-Revised). Data were analyzed using the SPSS/PC WIN. 12.0 program. Results: The mean score indicating satisfaction of campus life was 3.20, and drinking refusal self-efficacy was 4.36. Satisfaction of campus life was positively correlated with drinking refusal self-efficacy. The gender, social circle, family history of drinking, drinking frequency, drinking quantity, and satisfaction of campus life explained 33.7% of the variance for drinking refusal self-efficacy. Conclusion: The findings suggest that satisfaction of campus life is an important factor for improving drinking refusal self-efficacy in university students. Therefore, strategies should be developed in order to promote drinking refusal self-efficacy and increase satisfaction of campus life in order to prevent drinking problems.

출산력과 연령으로 그룹화한 유방암 환자에서 비만이 생존율 및 무병생존율에 미치는 영향 (Effects of Obesity on Survival Rate and Disease-free Survival Rate of Breast Cancer Patients sub-classified according to Reproductive History and Age)

  • 본 연구는 유방암 환자를 출산력과 연령으로 그룹화한 후 비만에 따른 생존율과 무병생존율을 비교분석하여, 비만 여부가 유방암 환자의 예후와 어떤 관련성이 있는지 알아보고자 하였다. 본 연구의 조사대상은 2006년부터 2008년까지 3년 동안 서울 일개 종합병원에 유방암으로 내원한 4181명의 환자였으며 2015년 5월 30일까지 생존여부와 재발여부를 추적 조사한 후향적 코호트 연구이다. 연구결과 전체 환자에 대해 출산경험이 없는 환자군이 출산경험이 있는 환자군보다 생존율이 낮았고(p=.000), 나이가 40세 미만인 환자군이 40세 이상인 환자군보다 생존율(p=.003)과 무병생존율(p=.000)이 낮았으나, 비만 여부에 따른 생존율과 무병생존율의 차이는 없었다. 그러나 출산력이나 연령을 기준으로 대상자를 그룹화한 후 분석한 결과 비만이 생존율과 무병생존율에 영향을 미치는 하위집단이 확인되었다. 출산력을 기준으로 분류한 환자군에 대해, 출산력이 있는 환자군 내에서 비만 환자의 생존율(p=.001)이나 무병생존율(p=.005)이 비비만 환자보다 낮았다. 연령을 기준으로 분류한 환자군에 대해서도, 나이가 40세 이상인 환자군 내에서 비만 환자의 생존율(p=.005)이나 무병생존율(p=.014)이 비비만 환자보다 낮았다. 연구대상자를 출산력이 있으면서 동시에 연령이 40세 이상인 환자들로 한정한 환자군 내에서도 비만 환자의 생존율(p=.000)이나 무병생존율(p=.003)이 비비만 환자보다 낮은 것으로 나타났다. 결론적으로 유방암 환자 전체에 대해서는 비만이 생존율과 무병생존율에 영향을 미치지 않았으나 출산력과 연령을 기준으로 그룹화한 후 하위집단 내에서 비교했을 때는 비만과 예후와의 관련성을 확인할 수 있었다. 이는 기존 연구들에서 구체적으로 확인하기 힘들었던 내용이므로 비만이 예후에 영향을 미치는 것으로 확인된 유방암 환자 하위집단에 대해 적절한 간호중재 방안이 마련되어야겠다.

범이론 모형을 기초로 한 농촌지역 성인의 금연행위에 영향을 미치는 요인 (Predictive Factors of Aspects of the Transtheoretical Model on Smoking Cessation in a Rural Community)

  • Purpose: This study was done to evaluate the predictive value of aspects of the Transtheoretical model (TTM) of behavior change as applied to smoking cessation in a rural population. Method: A convenience sample was recruited from a public health center in a community. A total of 484 participants were recruited, including 319 smokers, 116 ex-smokers and 49 non-smokers. A cross-sectional and descriptive design was used in this study. Data was analyzed using descriptive statistics, frequency statistics, ANOVA and Logistic regression. Result: The major findings were 1) The participants were assessed at baseline for their current Stage of Change resulting in a distribution with $42.1\%$ in Precontemplation, $24.1\%$ in Contemplation, $9.7\%$ in Preparation, $6.2\%$ in Active, and $17.9\%$ in the Maintenance stage. 2) There were statistically significant differences of processes of change, decisional balance and situational temptation across the stages of change. 3) The main factors that affect smoking cessation were age, number of years smoking, age when began smoking, self-liberation and negative/affective situations, which combined explained $33.2\%$ of the smoking cessation. Conclusion: TTM variables measured prior to a smoking cessation program added little predictive value for cessation outcome beyond that explained by demographic and smoking history variables.
