The Effect of Preceptorship on OJT(On the Job Training) of New Nurses (신규간호사 배치후 교육시 프리셉터쉽 적용 사례연구)

  • The purpose of this study was to examine the effect of preceptorship on On the Job Training of new nurses. The research design was a retrospective survey and one group pre-post test design. The subjects were 49 entry nurses and 49 preceptors. The data were collected from Feburary 1, 1998 to December 1, 1998. One questionnair applied in this study consisted of general characteristics, the responses of new entry nurses and their preceptors and their qualities. The other questionnair consisted of job satisfaction of new entry nurses and preceptors and their competencies. The data were analyzed for frequencies, mean score, standard deviation, t-test, and ANOVA. The results of the study were as follows: 1) Preceptees's responses were that the nurses' affiliation of unit, cooperation of other nurses, and competencies of nursing skills improved after OJT applied preceptorship. 2) Preceptors's answers were that preceptorship had a positive effect on their continuing education. 3) While preceptees's job satisfaction scores were high in interaction, administration, and organization, they were low in autonomy and pay. 4) Pre-competency scores were the same as post-competency scores. 5) After preceptorship, preceptors responded that their nursing skill & communication competency increased, but their nursing administration competency decreased. 6) Finally, preceptorship's other effects were as follows: (1) Checklists for preceptee education at the nursing unit were developed separately. (2) It was proved that 'preceptor interview note' was effective on manage preceptorship. (3) We obtained duty overlap periods; medicalsurgical units-1 month, 5 ICU-2 months, operation room-3 months. (4) We prepared a preceptor program based on the study results.

Nurses' Work-family Boundary Management (간호사의 일-가정 경계선 관리)

  • Purpose: This study was done to explore how nurses manage the boundary for work-family balance. Methods: Data were collected through in-depth interviews from February 1 to May 28, 2012. Participants were 13 married nurses with children. The constant comparative method was adapted for data analysis. Results: In this study, nurses' work-family boundary management had characteristics of integration, maintenance and control, but it was adjusted and modified over time as the order of priorities was rearranged according to changes in internal cognition and family support. Nurses strategically persuaded their families to understand their work and share housework in an effort to integrate work and family, while controlling them by managing working hours and time for themselves. Conclusion: The results of the study indicate that nurses' work has an impact on family life due to heavy workload and insufficient time, however the work-family balance can be successfully maintained with the effective separation of the two areas.

Awareness and Experience of Nurses and Physicians on DNR (심폐소생술금지(DNR)에 대한 간호사와 의사의 인식과 경험)

  • Purpose: To examine the awareness and experiences of nurses and physicians on DNR. Method: The sample contained of 199 nurses and 98 physicians. The Instrument used in this study was The Nurses' Understanding and Attitude on DNR(AEDNR) developed by the Han et al and revised by the research the AEDNR included 29 items. Result: About 97.3% of them feel the necessity of DNR, Majority of subject(78.8%) have practiced DNR by the demand of 'the patient's family' and 91.8% of them made a medical record at the time they carried out DNR. There were no significant differences in their perceived necessity of DNR between nurses and physicians. Conclusion: This study can be a basis for making objective standards educational, ethical, and legal issues concerning DNR.

The Effects of Organizational Justice on Job Satisfaction, Organizational Commitment and Health Status among Nurses (조직공정성이 간호사의 직무만족, 조직몰입 및 건강에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effects of organizational justice on job satisfaction, organizational commitment and health among hospital nurses. Method: The sample of this study consisted of 267 nurses from 3 general hospitals located in Seoul, Korea. Data were collected with self-administrated questionnaires and analyzed hierarchical multiple regression. Result: The distributive and procedural justices had effects on nurses' job satisfaction and organizational commitment, but no effects on health. The procedural justice had more effects on nurses' job satisfaction and organizational commitment than distributive justice. There was no moderating effect of distributive and procedural justices on nurses' job satisfaction, organizational commitment and health. Conclusions: It is important for hospitals to be established fair reward systems which should focus on both outcomes and processes in order to evaluate nurses' rewards. It is suggested that new moderators like personality or organizational structure should be considered for future research.

A Study of the Head Nurse's Leadership and the Organizational Effectiveness as Perceived by Staff Nurses (간호사가 지각한 수간호사의 리더십과 조직유효성에 관한 연구)

  • Purpose: This descriptive study was designed to find out the relation between nurses' perceptions of leadership and organizational effectiveness. Method: The data was collected from nurses in 451, a tertiary hospital located in B city by means of structured questionnaires. The data was analysed by t-test. ANOVA, Pearson correlation coefficient. Results: The average age of the subjects was 29.1. Regarding the marital status, 68.1% were single. Regarding the nursing unit, 42.6% were working for the general surgical wards. The total period of clinical career was on average 73.7 months. Regarding the period in current ward, 51.0% were 1-less than 3 years. There was a positive correlation with transformational leadership, transactional leadership, job satisfaction, and organizational commitment, whereas the turnover intention had a negative correlation with transformational leadership and transactional leadership. Conclusion: The nurses' different perceptions of transformational leadership, transactional leadership, and organizational effectiveness have strong correlative relationships with each other. This finding suggests the importance of the head nurse's leadership in increasing organizational effectiveness.

A Review of Studies on Leadership in Nursing Organization (간호조직에서의 리더십에 관한 논문 고찰)

  • Purpose: The objective of this literature review study was to identify patterns and trends of studies on leadership among nurse managers in nursing organization and suggest the direction of future researches. Methods: Research Studies on the leadership were selected from thesis and related journals, which had been conducted from 1975 to 2006.5. Search Terminologies were used "leadership". The Total number of the collected studies were 69. Results: The number of studies on leadership has been increased rapidly since 1990's. In research design, Quantitative studies were 68 except for a Qualitative study. Correlation research were 45(65.2%) among quantitative studies. Conceptual frameworks, research questions and hypotheses were involved 31.9%, 98.6%, 31.9%. In subject, General nurse is 46.4%, Head nurse and General nurse was 23.2% and Head nurse is 2.9%. All the studies used questionnaire in data collection method. Measurement tools were used 50% for Bass' MLQ(MLQ-5X). For the statistical analysis, Correlation, ANOVA, t-test were used for 20.3%, 17.4%, 10.5% respectively. The number of leadership related variables were work-related, organization-related, personal recognition or attitude, empowerment, personality. The sub-variables were used for 31 Job satisfaction. Further studies were suggested the develop of leadership program, measurement for the effect of leadership, repeated studies on leadership, the development of measurement about leadership. Conclusion: These results will be used to provide the basic data for developing leadership theory on nursing organization.

Comparison of Professional Self-Concept, Clinical Nursing Competence, and Practicum Satisfaction between Nursing Students with and without Pediatric Nursing Clinical Preceptorship (아동간호 임상프리셉터십 경험 유무에 따른 간호학생의 전문직 자아개념과 임상수행능력 자신감, 실습 만족도 비교)

  • Purpose: The purpose of this study was to compare professional self-concept, clinical nursing competence, and practicum satisfaction between nursing students with and without pediatric nursing clinical preceptorship. Methods: This study used a comparative research design. A total of 85 junior nursing students participated in the study, and data from 76 students were analyzed using SPSS WIN 20.0. Among them, 44 students experienced pediatric nursing clinical preceptorship. Results: The students with clinical preceptorship showed a significantly higher score in nursing skill related items of professional self-concept and the confidence in knowledge of injection preparation and administration compared to the counterpart students. Practicum satisfaction was also significantly higher in the students with clinical preceptorship than in the other students. There were significant positive correlations among professional self-concept, performance confidence, and practicum satisfaction. Conclusion: The study findings suggest that clinical preceptorship is an effective educational method to teach clinical practicum of pediatric nursing. The use of clinical preceptorship needs to be considered to motivate nursing students actively involved into the clinical practicum. The roles of clinical preceptors should be clearly identified in clinical education.

Development of Clinical Evaluation Tool for Nursing Student (임상 간호실습교육 평가도구 개발)

  • This study is intended to develop a reliable and appropriate instrument of the clinical nursing education. This research consisted of 4 steps. First step is construction of the content validity by 1 Korean literature professor and 1 teaching professor in Ga Chon Gil College, the pilot study for the content validity by 14 professors and survey with four points Likert Scale, which includes from the point 'strongly valid' to the point of 'strongly non-valid', by 113 head nurses who guide and evaluate the students in clinical practice. The third step is the test of validity and reliability of the preliminary evaluation tool. The fourth step is the test of validity and reliability of the developmental evaluation tool. The data were collected from Sep. 10th, 2001 to Sep. 28th, 2001. This study was analyzed by SPSS PC+ for descriptive statistics, factor analysis and Cronbach's Co-efficient Alpha of the collected data. The results of these analysis are like as follows. 1. Evaluation tool of Clinical practice consists of 16 items including four categories : factor 1 was labeled 'desirable attitude'(5 items), factor 2 was labeled 'correctly judgement and nursing problem solving'(4 items), factor 3 was labeled 'adaptive ability of nursing knowledge and skill'(4 items), factor 4 was labeled 'desirable human relationship'(3 items) and these contributed 71.992% of the variance in the total score. 2. Cronbach's Co-efficient Alpha for internal consistency was .9128 for the total 16 items. For further research, it need to develop a variable and reliable instrument of the student self-evaluation and instrument that based on community.

Development of indicators for the evaluation of internet site for health information (인터넷 건강정보 사이트 평가지표 개발)

  • Objective : Use of Internet for health related information is rapidly increasing. There is, however, no consensus as to which information is correct and reliable. This study was conducted to develop an evaluation standards for health related information on the Internet for the purpose of delivering correct information to internet users. Methods : A Delphi technique, which involved a panel of 65 experts, was used as the research design for this study. In the first round, the initial evaluation criteria which consisted of 12 items was developed through a literature review by the researcher and the items represented 2 categories, structure and function. In the second round, a semi-structured questionnaire was sent to the experts with the initial evaluation tool and the respondents identified 55 items. In the third round, these 55 items were analyzed for content validity by the experts and a total of 35 items were included in the model; 11 items in the structural area, 24 in the functional area. Conclusions : Development of indicators for the evaluation of internet site for health information is crucial to provide correct and reliable information. For evaluation to be effective further evaluation programs need to be developed so that users can be sure that the information they received is accurate.

Concepts Analysis of Ethical Dilemma (윤리적 딜레마에 대한 개념 분석)

  • Purpose: Although ethical dilemma is important concept in nursing and many research is accomplished, some difference in defining ethical dilemma among researchers. Therefore the purpose of this analysis was to correct understanding and using about ethical dilemma by clarify the attributes of ethical dilemma. Method: The concept analysis model suggested by Walker and Avant(1988) is used to clarify what is meant by the term 'ethical dilemma'. Results: The defining attributes of ethical dilemma identified in this study were as follows. First, It is simultaneously occurrence that above two conflictive ethical alternatives. Second, It is ought to choose one of the above two conflictive ethical alternatives. Third, It is difficult to compare with two alternatives. Fourth, It is cannot delay the choices. Fifth, It is cannot do satisfactory resolution. Six, It is broken out fix, loss or feeling of powerlessness to an individual. Conclusion: In this study, a concept analysis of ethical dilemma was performed to more clearly understand ethical dilemma. It is hoped that this analysis will stimulate further study for developing strategies for prevention of nurses' ethical dilemma.
