Factors Influencing Clinical Nurses' Intention to Report Medication Administration Errors (임상간호사의 투약오류보고 의도에 영향을 미치는 요인)

  • Purpose : This study aimed to identify factors influencing clinical nurses' intention to report medication administration errors. Methods : This cross-sectional study collected data from 121 nurses in charge of administering medication at a university hospital in Korea using structured questionnaires. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson's correlation coefficient, and multiple linear regression. Results : Participants' mean age was 26.90±3.99 years, and 89.3% were women. Their mean clinical career duration was 3.88±4.26 years. The average levels of patient safety culture, attitude toward reporting medication administration errors, and intention to report medication administration errors were 7.51 out of 10, 3.36 out of 5, and 4.85 out of 6, respectively. The multiple regression analysis results indicated that the statistically significant influencing factors were patient safety culture (𝛽=.21, p =.018) and attitude toward reporting medication administration errors (𝛽=.22, p =.015). Conclusion : To improve the intention to report medication administration errors among clinical nurses, a patient safety culture must be established, along with an education provision for improving their attitudes toward reporting such administration errors.

A Comparison of Shared Governance and Nursing Unit Culture in Three Hospitals (공유관리와 간호단위문화에 대한 조사;세개 종합병원 대상)

  • The primary purpose this descriptive study was to identify, describe, and compare the patterns of shared governance and nursing unit culture in the hospital settings. The secondary purpose of the research was to identify, describe whether the participation style or responsibility style of nursing management activities shared through nurses in a consistent way. Methodology included survey and in-depth interviews with a total 145 members of 15 nursing units in 3 hospitals. One was a national hospital, another was a corporation hospital and the other was a teaching hospitals. Conclusions from this research included the following: 1. The degree of shared governance in nursing management activities was the highest in the corporation hospital. 2. In the participation style of nursing management activities, 'all participation' was the highest in the corporation hospital. 'Nursing administration only' was the highest in the national hospital. 3. Distribution of responsibility style differed from that of participation style. Three hospitals showed high in nursing management activities such as 'nursing administration only' and 'head nurse only' style. 4. Five experts surveyed showed that the ideal level of nurses' participation in nursing management activities was a traditional nursing governance pattern. 5. There was a distinct difference in the nursing unit culture throughout the institutions in the professional growth. 6. There was no significant difference in the same nursing units of three hospitals in nursing unit culture. According to these results, the following implications can be made; 1. In nursing administration, there should be an emphasis on preparing staff nurses' potential decision-making ability through continuing education so that staff nurses' autonomy and responsibility will be developed and increased. 2. It is necessary to develop a strategic nursing unit for improving nursing quality in hospital setting. 3. The relationship of shared governance, nursing unit culture and nursing outcome should be researched further.

An Empirical Analysis of Costs related to Nursing Practice (간호 관련 비용의 실증적 사례분석 연구)

  • Purpose: The purpose of this study was to identify nursing service costs associated with all health care costs incurred by the institution. Methods: This study was an empirical case study research in which the nursing cost was separated from total medical cost. The nursing cost index was calculated through a cost allocation method after summarizing costs for personnel, raw materials and administration of each department in one public hospital. The 2014 budget plan, published in 'Public Hospitals Alert', was used as data and the data were analyzed using the Microsoft Office EXCEL 2013 program. Results: When comparing total medical costs and nursing costs, the nursing cost were 27.14% of the total medical cost. The nursing cost per nurse per hour was calculated as \29,128 The nursing cost per inpatient per day was calculated as \157,970, and the administration cost per patient was calculated as \133,710. Conclusion: The results of the research present the process of cost allocation of specific cost elements in the hospital and evidence for administrative costs which in the past have been only vaguely formulated. These are the significant implications of this study.

The Application of Organization Theories to Nursing Research: A Critical Review and Implications (간호 연구와 조직 이론의 통합적 접근: 동향과 함의)

  • This article explores the potentials of the application of organization theories to nursing research on health care organizations in Korea. The critical review of the literature reveals that the important topics on organizations in health services research have been also dealt with by nursing researchers. Notwithstanding this theoretical closeness between the two fields, some issues remain: the paucity of attention to 'level issue', which requires to increase the attempts at 'meso-' or 'macro-' analysis on nurses' behavior in organizations; the need for theoretical frameworks specifying organizational factors relating to nursing processes; the necessity of interdisciplinary approaches to bridge the recent developments of organizational study and nursing research. This article finishes with a few of research agendas for future research to fill these gaps.

Development of Agenda Priority for Nursing Service Research and Development (간호서비스 연구개발 분야에서의 우선순위과제 도출)

  • Purpose: The purpose of this study was to suggest agenda priorities for nursing service R&D (Research and Development) related policies development. Methods: Two steps in developing the agendas and priorities were performed in this descriptive study. First, nursing service R&D agendas were extracted through needs assessment of nursing researchers and practitioners. Then, the priority of agendas was set by Analytic Hierarchy Process by ten experts who were representatives of nursing and other healthcare professionals. Results: Six core areas and forty-six nursing service R&D agendas were developed. The priority of agendas was different according to the evaluation criteria depending on weight value of nursing services. Conclusion: In order to select and promote nursing service R&D projects within national healthcare policy, nursing service R&D policy should be proposed with consideration to the importance of the criteria in reflecting characteristics of nursing care. By strengthening R&D capabilities for quality improvement and sensitive awareness of national directions for healthcare R&D policies, nursing service R&D can be appropriately promoted.

Nursing Service R&D Strategy based on Policy Direction of Korean Government Supported Research and Development (국가보건의료 R&D 정책 방향에 따른 간호서비스 R&D 전략 연구)

  • Purpose: To develop strategies for research and development (R&D) in nursing service based on the policy direction of government supported R&D in Korea. Methods: This was a descriptive study to develop strategies for promoting R&D in nursing by analyzing investment trends and status quo, policy directions, and implementation of the details of government supported R&D through government reports, websites of relevant agencies and literature reviews. Results: Few nursing experts participated in clinical research on overcoming major diseases and in R&D for well-being and care. Development of nursing topics that meet the direction of government supported R&D were lacking. Insufficient implementation of nursing service R&D in a timely manner equipped with a performance-based system. Few research studies in R&D projects that included research using big data or contributing to developing medical instruments. Finally, an insufficient number of nursing specialists participated on government R&D advisory committees. Conclusion: For nursing service R&D development efforts should be toward quantitative expansion and qualitative improvements by sensitively recognizing policy direction of government supported R&D. The promotional capacity of nursing service R&D must be reinforced through a multidisciplinary approach and collaborative association with other professionals and the inclusion of nurse specialists on government R&D advisory committees.

The Trends of Nursing Management Research (간호관리 연구의 동향)

  • The study was conducted to provide basic data for the development of nursing management research and suggesting the direction of the future research and the theory of nursing management. In this study, 456 pieces of articles that had been published for the last 10 years from 1987 to 1996 were analyzed based on the nursing research methodology of Lee et al. (1991). The results were as follows: 1. The whole articles were divided into two groups. that is. degree dissertations of 69.7%(307 pieces of master's and 11 pieces of doctor's) and non-degree papers of 30.3%. 2. In the light of graduate school types. the papers of the graduate school of administration formed 41.8% of the entire articles and those of the general graduate schools formed 32.4% of them. 3. In the methodology of research design. survey research held 86.8% of the whole articles. 4. In the research subject. nurses held 68.4%. others 14.6%. patients 9.4%. 5. In the data collecting method. questionnaire held 85.1%. 6. In the statistics method. 88.1% of the entire articles used inference method. 7. In the area of nursing management. direct stage formed 26.8%. control stage 21.9%. and staffing stage 20.6% of the whole articles. 8. In the light of frequency of the key concept. job satisfaction held 10.9%. nursing task 8.5%. role 7.0%. stress 4.7%, and perception 4.0%. On the basis of the results. the following suggestions were made: 1. Qualitative studies should be done to recognize the phenomena. and experimental studies to prove the intervention for the improvement of future nursing management researches. 2. For the development of nursing management. all-inclusive studies. the group or the whole organization-centered studies rather than nurse-centered, should be required. 3. In order to be valid. data should be collected by various methods. 4. For the balanced studies of nursing management. getting out of the concentrated study of just one concept, repetitive study of various concept should become the subject of them.

Analysis of Studies on Nursing Image Performed in Korea from 2003 to 2014 (국내 학술지에 발표된 간호 이미지 논문 분석: 2003년부터 2014년도까지)

  • Purpose: This study was done to review and summarize trends in research related to image of nursing in Korea and to explore directions for future research. Methods: Thirty-five studies published from 2003 to 2014 were reviewed, and the analysis was performed according to criteria adopted from Kim & Lee (2003). Results: There was a three-fold increase in the number of studies compared to a prior analysis study. Of these studies 50% were carried out with nursing students and 71.4% were survey type research. Elementary school students, patients, and guardians had positive images of nursing compare to middle school students and health related personnel. Experience of hospital admission or patient visit did not influence the nursing image of participants. Satisfaction with nursing as a career and clinical practicum were related to nursing image among nursing students and their satisfaction, and their nursing professional value and self-esteem had a positive correlation with nursing image. Conclusion: The findings of this study suggest that recent trends of nursing research on nursing image indicate a need for further research to eliminate negative stereotypes and build a more professional image for nursing are recommended.

Nurses' Experience of Practice in Nursing Homes (노인요양시설 간호사의 실무 경험)

  • Purpose: This article aimed to identify nurses' experience of practice in nursing homes. Method: Using phenomenological approach for qualitative research, the participants were 8 nurses who were working at nursing homse in C city. The data was analyzed by using phenomenological methodology by Colaizzi. Results: Five theme clusters were derived from the data. They were: burden (independent decision making in clinical situations, tension of as a multifunctional member); lowered self-esteem (exhaustion, relative self-evaluation is low, comparing with others); conflict (negative image of elderly, communication break with elderly family, monitoring of nursing home staff); worth (slight improvement of elderly health state, trust and recognition, intimacy); and self-reflection (need for knowledge extension, effort towards enhancing virtue, perception of importance in their health management). Conclusion: The results showed that nursing home nurses experienced negative aspects of various nursing practice, suggesting the ways to solve their negative experience.
