Nursing Outcomes Classification(NOC)를 이용한 일부 입원환자의 간호결과에 대한 분석 (Analysis of Patient Outcome using Nursing Outcomes Classification(NOC))

  • Purpose: The purpose of study was to determine differences in patient outcomes that exists in terms of Nursing Outcomes Classification (NOC) during hospital days of neurosurgical and respiratory patients. Method: Before starting clinical practicum, nursing students were received two hours' lecture on how to apply NOC to patient care plan and they were required to evaluate patient condition using NOC at the beginning and at the end of their clinical practicum. Data were extracted from 62 neurosurgical patients and 66 respiratory patients and analyzed by frequency and paired t-test. Results: The most frequently used NOC were Pain Level (37.1%), Mobility Level (25.8%), and Bowel Elimination (19.4%) in neurosurgical patients and Nutritional Status (37. 9%), Respiratory Status: Ventilation (37.9%) and Pain Level (25.8%) in respiratory patients. The numbers of outcomes used were 75 and 46 neurosurgical and respiratory patients respectively. During the hospital days, the level of patient outcomes increased significantly in all patient groups. Conclusion: The finding clearly suggests that nursing interventions make differences in patient outcomes and make contribution to the patient health achieved. To more effectively use NOC, however, nursing information system should be developed and included standardized nursing languages regarding nursing diagnoses and interventions.

간호사의 에니어그램 성격유형별 간호전문직 태도와 직무만족 (Enneagram Personality Types, Nursing Professional Attitudes and Job Satisfaction of the Nurses)

  • Purpose : The purpose of this study was to examine the enneagram personality types, nursing professional attitudes and job satisfaction of the nurses. Method: The subjects for this study were 93 nurses who did the enneagram workshop. Demographic characteristics, enneagram personality types, nursing professional attitudes and job satisfaction were measured. The data were analyzed through the SPSS 10.0 window program by were of frequency, percentage, t-test, ANOVA. Result : The highest of nurses' enneagram personality types was type 6 and type 5 and 8 were not existed. The triad of enneagram was consist of head type 40.9%, heart type 39.8%, gut type 19.3% orderly. Nursing professional attitudes and job satisfaction of nurses were positive. Nursing professional attitudes in type 2 were the highest but in type 9 were the lowest. also job satisfaction in type 1 were the highest but in type 2 were the lowest. But statistically nursing professional attitudes and job satisfaction no significant difference according to types of nurses' enneagram. Conclusion : According to the above findings, enneagram will utilize not only instrument for investigation of nurses' personality types but also human resource management program for understanding of nurses. And suggest that the further research is need to identify more variables related to the types of nurses' enneagram for enneagram' utility in nursing organization.

Nursing Management Minimum Data Sets (NMMDS) 연구의 최신 동향 (A Review of Practical Use and Research Trends on Nursing Management Minimum Data Sets (NMMDS))

  • Purpose: The purpose of this study was to review articles on Nursing Management Minimum Data Sets (NMMDS) and to suggest strategies to improve practical use of NMMDS in nursing management. Methods: A systematic search for articles published until 2013 was undertaken using the following biomedical databases: CINAHL, PubMed, and Google scholar. Seventeen articles were fully reviewed. Results: The results showed that studies were related to updating NMMDS reflecting current EHR use, mapping NMMDS to standardized national databases, and validating, translating and evaluating NMMDS for international uses. NMMDS has three dimensions and was developed reflecting the needs of nurse managers. Conclusion: The study findings provide a summary of recent trends in NMMDS. These results can serve as basic information to promote practical use of NMMDS in the healthcare organization to provide nursing management data for nurse managers.

간호사가 지각한 병원조직의 사회적 자본이 조직성과에 미치는 영향 - 지적자본에 의한 효과를 중심으로 - (Effects of Social Capital on Organizational Performance in Hospital Organization - Focusing on Effects of Intellectual Capital -)

  • Purpose: The purpose of this study was to identify the effects of intellectual capital in the relationship between social capital and organizational performance in hospital organizations. Methods: A cross-sectional design was used, with a convenience sample of 390 nurses from 3 general hospitals in Gwangju and Chonnam province. Data were collected through a questionnaire survey done from May 1 to 15, 2009. Data was analyzed with descriptive statistics, Pearson correlation coefficient and hierarchial multiple regression using the SPSS WIN 17.0 program. Results: There was a significantly positive relationship between social capital, intellectual capital and organizational performance. Intellectual capital showed partial mediating effects between social capital and organizational performance. However, intellectual capital did not show moderating effects. Conclusion: Accumulated social capital increased intellectual capital and the increased intellectual capital increased organizational performance. This finding suggests that as a contribution to the improvement of organizational performance in hospitals, creation and accumulation of social capital and intellectual capital are very critical.

간호관련인력별 직무표준 설정에 관한 연구;간호사와 간호조무사를 중심으로 (A Study on Standards Development for Nursing Practice)

  • The study was performed to develop standards for nursing practice. The quality and quantity of medical services, including nursing services, has been determined to date by the availability of providers rather than by the actual needs of the patients. In fact, there has been a serious gap between expected and actual servces. In order to provide high quality nursing services and to enhance professional status, the nursing profession should define their practice in a measurable way and identify the client outcomes to which they contribute. Therefore, this research was conducted through the three phases as follows : (1) Evaluation of current nursing practices and environment, and analysis of actual patient needs, by means of questionnaires. (2) Literature review to define the nature, purpose, and standards of professional nursing practice. (3) Expert group's modification of existing standards by integrating the results of first two phases. As a result, standards for each area of nursing practice were developed, such as shown in Table 3 and Table 4. However, these standards must receive ongoing attention by the profession to assure that they remain current and reflect advances in nursing knowledge and clinical technology.

노인요양시설의 질 관리 활동에 영향을 주는 구조적 요인 분석 (Structural Factors Influencing the Quality Management Activities in Nursing Homes)

  • Purpose: Nursing home quality indicators have been focused widely on result outcomes, not for the environment in that quality of service are delivered, This study aimed to examine structural factors influencing quality management activities in nursing homes. Method: Sample was 170 nursing homes responded to the survey questionnaire which was distributed to the 543 nursing homes nation-wide, Data were collected on structural characteristics, types of services, and quality management activities, Data were analyzed with the descriptive statistics, Pearson correlations, and multiple regression. Result: Most of the nursing homes were operated as free of charge by the social welfare ownership. Average number of residents was 52.1 with severe and mild dementia and bedridden status, In terms of quality management activities, 34% of the sample had CQI committee that focused their activities on services delivery process, performance appraisal, record keeping regularly. 30.6% of quality management activities were accounted for by the number of residents with dementia, the ratio of RN to residents, rehabilitation services, and social wefare services in nursing homes. Conclusion: We recommend that more comprehensive quality management activities should be developed as process quality indicators in conjunction with the outcome indicators.

간호사의 노동조합에 대한 태도와 간호전문직관, 헌신몰입에 관한 연구 (A study on the relation among attitudes toward unions, views on nursing profession and career commitment)

  • In studies on the hospital labor-management relations, research about the attitudes of workers and management toward unions was relatively untouched in Korea. This study was investigated to identify the relation among ideal, actual attitudes toward unions, ideal, actual views on nursing profession and career commitment. Data was obtained from a convenience sample of 285 nurses of varying positions, education, career, join union or not. The results of this study were as follows. 1. There was significant differences between ideal attitude toward unions and actual (p < .001). 2. There was significant differences between ideal view on nursing profession and actual (p< .001). 3. There were no correlations between ideal and actual attitude toward unions and nursing career commitment. 4. There were correlation between ideal and actual views on nursing and nursing career commitment(r=.32, r=.46). As the results show, views on nursing profession are more important factor to inhance nursing career commitment than attitude toward union. So the findings of this study suggest that antecedents and moderating variables need to be explored for further theoretical specification and empirical evaluation.

중환자실 간호사의 임상등급 (clinical ladder)별 간호역량 측정도구 개발 (Development of a Nursing Competency Scale according to a Clinical Ladder System for Intensive Care Nurses)

  • Purpose: This study was done to develop a nursing competency scale according to a clinical ladder system for intensive care nurses. Methods: Index of content validation was done by 20 clinical experts and 80 nurses in Intensive Care Units (ICU). Results: The process and results of study are as follows. First, 12 nursing competencies were used in the establishment of the clinical ladder system (Jang, 2000). Second, the first draft of the competency lists was developed. It was based on the clinical nurses' behavioral indicators of nursing competency by Jang (2000), and was modified and supplemented through various literature reviews including competency standards for specialist intensive care nurses in Australia and consultation with 2 clinical nurses with over 10 years experience in the ICU. Third, the draft was examined by 20 clinical experts for content validity. Finally, the final draft was analysed using clinical validity where 20 nurses in each ladder participated. The final number of items was fixed at 309. Conclusion: The tool represents expected nursing competency of nurses working in ICU. Intensive care nurses can recognize their strengths and weaknesses, and identify directions for their professional growth by analysing results of their competency evaluation using this tool.

우리나라 간호원가 연구의 동향 분석 (The Trends of Cost Analysis on Nursing Services)

  • Purpose: The objective of this literature review study was to identify research's trends and methodological issues of cost analysis on nursing services. Methods: Researches which analyzed nursing services cost, were selected from journal articles and master or doctoral dissertation studies. The total numbers of the collected studies were 23. Results: The number of studies on nursing services cost has been increased rapidly since middle-1990. The 5 methodological frameworks to classify the cost analysis researches was found. 4 researches were using the traditional costing method. 6 researches were using the clinical patient classification systems. 4 researches were using the Korean Diagnosis Related Group (KDRG). 5 researches were using the Resource Based Relative Value Scale (RBRVS). 4 researches were using the Activity Based Costing (ABC). Conclusion: These results will be used to provide the basic data for developing a more refined cost analysis method on nursing services. For further studies, we will suggest that the consent criteria of cost items need to measure nursing services be developed and the conducting cost analysis on nursing services be networked a hospital's cost management system.

병원 간호조직의 성과평가지표에 관한 조사 연구 (A Survey for Performance Measurement Indicators of Nursing Organizations in Hospitals)

  • Objective: The objective was to extract a preliminary performance measurement indicators of nursing organizations in hospitals using the BSC(Balanced Score Card) developed by Kaplan and Norton, and to analyze the content validity and evaluation methods of the performance measurement indicators with actual nurses in the nursing organization as participants in the study. Methods: The preliminary performance measurement indicators was created through a literature review and had the content validity by a professional. This survey was sent via post to 316 nurse managers and nurses with more than 5 years of experience in seven secondary and tertiary hospitals in the Seoul Gyonggi district. The completed questionnaires were returned by mail. Results: Fourteen indicators for finances, 16 for customer services, 27 for internal business processes, and 13 for learning and growth were selected. Conclusion: Amidst a rapidly changing medical environment, a first step was taken towards developing a performance measurement from various perspectives for nursing organizations in hospitals from various perspectives, rather than just one or a past-oriented perspective. However, as the most important thing is to actually use these indicators, continuous interest in publicity and education must be developed.
