Foreign student life experience in Korea after COVID-19

  • This study was a phenomenological qualitative research that analyzed the experiences of Korean students studying in Korea after the COVID-19 incident. Participants in this study consisted of 22 international students aged 20 to 40 attending the International Exchange Center at C University. The interview period was from September 10, 2020 to October 10, 2020. Giogi qualitative research method was used to analyze vivid experiences of international students. As a result of the analysis, 26 semantic units, 7 subcomponents were derived. The description of the general structure sentence of phenomenology was a description of the meaning of experience from the perspective of participants, and the context and structure descriptions were integrated. The results of this study showed that: The students who came to Korea to study were concerned about Korea in various ways, but they had to adjust to unexpected changes in education methods, anxious about the unexpected COVID-19 disaster. Participants chose to study in Korea based on existing information, so they felt anxiety, regret, fear, and frustration over sudden changes, but taking online classes helped them learn repeatedly and voluntarily became an experience that suited their learning speed. As commuting time has decreased, they were more opportunities to make money in Korea also. Based on the results of this study, the following is suggested: First, the government should establish systematic online infection prevention measures for international students who have poor Korean language skills in preparation for unexpected disasters. Second, non-face-to-face teaching methods should be prepared with the same weight in the face-to-face teaching methods that have been carried out so far in preparation for unexpected disasters.

서비스 디자인 관점에서 본 스마트 헬스케어의 선행 조건 : 고령자 경험 사례를 중심으로 (Prerequisites on Smart Healthcare in the Perspective of Service Design : Focusing on the Elderly Experience Case)

  • Due to the increasing interest in wellness aroused by the aging population and the pursuing feature of active old age, Korean elderly set importance on long life with their healthy condition. Following the change in the paradigm of the medical delivery system from hospital-oriented, treatment-oriented to personal-centered and self-care, Service design application of Smart Healthcare for the elderly became valuable. Smart Healthcare is a healthcare service provided through the fusion of ICT technologies including mobile/wearable devices, IoT, big data, and information technology, and it is utilized to prevent diseases managing abundant health information and living habits. As a methodology for delivering such Smart Healthcare to the elderly, Service design can be adopted. Therefore, this study would like to present the perquisites of Smart Healthcare design for the elderly through analyzing the results from in-depth interview methods between the elderly and medical staff. As a result of this study, guidelines for Service design application of health vulnerability management for the elderly utilizing smart phones were presented. Therefore, this study presented four prerequisites composed of 'high level of supplementation and ethical decision making', 'improvement of inequality in accessibility and experience', 'resolving problems in policy implementation' and 'user-friendliness' for the Smart Healthcare service design for the elderly. Overall, Service design is expected to play an innovative role in improving the quality of life for the elderly through the process of collecting and delivering information on Smart Healthcare centered on the experience of the elderly.

  • 본 연구의 목적은 아동학대 예방을 위한 방안 마련의 일환으로 빅데이터 분석과 머신러닝 알고리즘을 활용한 정책개발의 기초자료를 제공하는데 있다. 아동학대 예방을 위한 머신러닝 알고리즘 개발을 위한 빅데이터 분석을 위해 학술데이터베이스와 사회관계망서비스 자료를 빅데이터로 정의하고 빈도, 연관어, 감성분석을 시행하였다. 연구결과 예방적 아동학대 알고리즘은 학술빅데이터 분석에 나타난 아동학대 관련 세 주체 피해아동, 가해양육자, 정부당국의 관점에서 아동학대 예방을 위한 데이터 수집 및 공유 네트워크 시스템 마련을 통해 개발이 가능할 것이다. 또한 아동학대 피해아동의 특성에서 자아개념 저하 등으로 우울 및 불안이 나타남을 단서로 영유아 자아존중감 및 우울, 불안 검사를 제도화함으로써 가능할 것이다. 아동학대 예방을 위한 빅데이터 수집 및 분석, 알고리즘 개발 연구의 지속적 진행을 제안하며 아동학대 예방을 위한 실효적 정책 마련이 실현되어 아동학대범죄가 근절되기를 기대한다.

의료서비스 마케팅을 위한 품질지각과 만족에 관한 연구 (A Study on Qulity Perceptions and Satisfaction for Medical Service Marketing)

  • INSTRODUCTION Service quality is, unlike goods quality, an abstract and elusive constuct. Service quality and its requirements are not easily understood by consumers, and also present some critical research problems. However, quality is very important to marketers and consumers in that it has many strategic benefits in contributing to profitability of marketing activities and consumers' problem-solving activities. Moreover, despite the phenomenal growth of medical service sector, few researchers have attempted to define and model medical service quality. Especially, little research has focused on the evaluation of medical service quality and patient satisfaction from the perspectives of both the provider and the patient. As competition intensifies and patients are demanding higher quality of medical service, medical service quality and patient satisfaction has emerged as a critical research topic. The major purpose of this article is to explore the concept of medical service quality and its evaluation from both nurse and patient perspectives. This article attempts to achieve its purpose by (1)classfying critical service attibutes into threecategories(satisfiers, hygiene factors, and performance factors). (2)measuring the relative importance of need criteria, (3)evaluating SERVPERF model and SERVQUAL model in medical service sector, and (4)identifying the relationship between perceived quality and overall patient satisfaction. METHOD Data were gathered from a sample of 217 patients and 179 nurses in Seoul-area general hospitals. From the review of previous literature, 50 survey items representing various facets of the medical service quality were developed to form a questionnaire. A five-point scale ranging from "Strongly Agree"(5) to "Strongly Disagree"(1) accompanied each statement(expectation statements, perception statements, and importance statements). To measure overall satisfaction, a seven-point scale was used, ranging from "Very Satisfied"(7) to "Very Dissatisfied"(1) with no verbal labels for scale points 2 through 6 RESULTS In explaining the relationship between perceived performance and overall satisfaction, only 31 variables out of original 50 survey items were proven to be statistically significant. Hence, a penalty-reward analysis was performed on theses 31 critical attributes to find out 17 satisfiers, 8 hygiene factors, and 4 performance factors in patient perspective. The role(category) of each service quality attribute in relation to patient satisfaction was com pared across two groups, that is, patients and nurses. They were little overlapped, suggesting that two groups had different sets of 'perceived quality' attributes. Principal components factor analyses of the patients' and nurses' responses were performed to identify the underlying dimensions for the set of performance(experience) statements. 28 variables were analyzed by using a varimax rotation after deleting three obscure variables. The number of factors to be extracted was determined by evaluating the eigenvalue scores. Six factors wereextracted, accounting for 57.1% of the total variance. Reliability analysis was performed to refine the factors further. Using coefficient alpha, scores of .84 to .65 were obtained. Individual-item analysis indicated that all statements in each of the factors should remain. On 26 attributes of 31 critical service quality attributes, there were gaps between actual patient's importance of need criteria and nurse perceptions of them. Those critical attributes could be classified into four categories based on the relative importance of need criteria and perceived performance from the perspective of patient. This analysis is useful in developing strategic plans for performance improvement. (1) top priorities(high importance and low performance) (in this study)- more health-related information -accuracy in billing - quality of food - appointments at my convenience - information about tests and treatments - prompt service of business office -adequacy of accommodations(elevators, etc) (2) current strengths(high importance and high performance) (3)unnecessary strengths(low importance and high performance) (4) low priorities(low importance and low performance) While 26 service quality attributes of SERPERF model were significantly related to patient satisfation, only 13 attributes of SERVQUAL model were significantly related. This result suggested that only experience-based norms(SERVPERF model) were more appropriate than expectations to serve as a benchmark against which service experiences were compared(SERVQUAL model). However, it must be noted that the degree of association to overall satisfaction was not consistent. There were some gaps between nurse percetions and patient perception of medical service performance. From the patient's viewpoint, "personal likability", "technical skill/trust", and "cares about me" were most significant positioning factors that contributed patient satisfaction. DISCUSSION This study shows that there are inconsistencies between nurse perceptions and patient perceptions of medical service attributes. Also, for service quality improvement, it is most important for nurses to understand what satisfiers, hygiene factors, and performance factors are through two-way communications. Patient satisfaction should be measured, and problems identified should be resolved for survival in intense competitive market conditions. Hence, patient satisfaction monitoring is now becoming a standard marketing tool for healthcare providers and its role is expected to increase.

호스피스 환자의 돌봄 요구와 가족이 인지하는 환자의 돌봄 요구 비교: Q 방법론 (Care Needs of Hospice Patients in Comparison with Those from the Family Caregivers' Perspective: Q methodology)

  • 목적: 본 연구의 목적은 호스피스 병동에 입원한 호스피스 환자들의 돌봄 요구 유형과 가족간호자가 인지하는 환자의 돌봄 요구 유형을 파악하고, 두 군 간에 차이가 있는지 비교하여 호스피스 환자의 돌봄 요구를 만족시키기에 도움이 되는 방법을 모색하는데 필요한 기초 자료를 제공하고자 한다. 연구방법으로는 개인에 대한 내적 의미에 중심을 두고 인간의 주관성이나 태도 유형을 심층적으로 측정할 수 있는 연구 방법론인 Q방법론을 적용하였다. 방법: 본 연구를 위한 Q표본은 호스피스 환자들의 돌봄 요구에 대한 경험으로 구성된 23개의 진술문이다. 자료는 2002년 12월부터 2003년 2월까지 23개의 Q-표본을 이용하여 C 대학 K 병원 호스피스병동에 입원하고 연구에 동의한 환자 20명군과 이들을 간호하는 주 가족간호자 20명군으로 가족 20쌍의 P-표본으로부터 각각 수집하였다. 탈분포도는 정상 분포방식에 따라 9점 척도 상에 대상자의 의견과 일치하는 정도에 따라 분류하도록 하였으며, 면담시 대상자의 구술도 기록하였다. 자료 분석은 Quanl PC 프로그램을 통한 Q요인분석인 주인자 분석방법을 이용하여, 요구 유형을 파악하기 위하여 평균분석, 요인 가중치, 영역별 요인 분석, 요인 배열을 하였고, 요인수의 결정을 위해 아이겐값은 1.0 이상을 기준으로 하였다. 결과: 연구결과 호스피스 환자 군과 가족간호자 군이 인지한 호스피스 환자의 돌봄 요구 유형은 네 가지로 확인되었다. 신체적, 정서적, 영적, 그리고 사회적 돌봄 요구 유형. 돌봄 요구 유형에 대한 두 군 간의 전체 일치률은 48%이었다. 제 1유형은 '신체적 돌봄 요구형' 으로 통증조절이 안된 상태로 통증으로 인하여 심한 고통을 당하고 있어 주호소인 통증 및 신체적 안위에 대한 돌봄을 우선적으로 요구하였다. 두 군 간의 일치율은 62.5%이었다. 제 2유형은 '정서적 돌봄 요구형' 으로 가족과 친밀감과 사랑을 나누기를 원하고 죽음에 대한 두려움을 따뜻한 대화로써 정서적으로 지지 받기를 원하였다. 두 군 간의 일치율은 20%이었다. 제 3유형은 '영적 돌봄 요구형' 으로 대상자 모두 종교를 가지고 신심이 깊었으며 절대자에게 용서 받고 싶은 욕구가 많았고 기도와 사목자의 방문을 원하였다. 두 군 간의 일치율은 60%이었다. 제 4유형은 '사회적 돌봄 요구형'으로 자신이 하던 일에 대한 마무리와 봉사를 원하는 것으로 나타났다. 두 군 간의 일치율은 50%이었다. 이상으로 볼 때, 호스피스 환자와 가족간호자가 인지한 환자의 돌봄 요구는 큰 차이가 있다는 것을 알 수 있다. 따라서, 호스피스 간호사는 호스피스 환자의 돌봄 요구를 만족시키기 위해서 환자와 가족간호자간에 환자의 돌봄 요구에 대한 인지의 차이를 좁히기 위한 적절한 방법을 모색하고, 가족간호자에게 환자의 돌봄 요구를 사정하기 위한 교육을 시켜야 하겠다.

Viral Hepatitis and Liver Cancer in Korea: an Epidemiological Perspective

  • In the past, hepatitis B virus (HBV) infection was endemic in the general Korean population. The association of HBV infection with the occurrence of liver cancer has been well demonstrated in several epidemiologic studies. While the mortality rates of liver cancer in Korea have decreased steadily over the last decade, the presence of hepatitis B surface antigen (HBsAg) in mothers remains high at 3-4%, and 25.5% of these HBsAg positive mothers are positive for hepatitis B e antigen (HBeAg). HBV infection caused almost a quarter of hepatocellular carcinoma (HCC) cases and one-third of deaths from HCC. These aspects of HBV infection prompted the Korean government to create a vaccination program against HBV in the early 1980s. In 1995, the Communicable Disease Prevention Act (CDPA) was reformed, and the government increased the number of HBV vaccines in the National Immunization Program (NIP), driving the vaccination rate up to 95%. In 2000, the National Health Insurance Act (NHIA) was enacted, which provided increased resources for the prevention of perinatal HBV infection. Then in 2002, the Korean government, in conjunction with the Korean Medical Association (KMA), launched an HBV perinatal transmission prevention program. The prevalence of HBsAg in children had been high (4-5%) in the early 1980s, but had dropped to below 1% in 1995, and finally reached 0.2% in 2006 after the NIP had been implemented. After the success of the NIP, Korea finally obtained its first certification of achievement from the Western Pacific Regional Office of the World Health Organization (WPRO-WHO) for reaching its goal for HBV control. An age-period-cohort analysis showed a significant reduction in the liver cancer mortality rate in children and adolescents after the NIP had been implemented. In addition to its vaccination efforts, Korea launched the National Cancer Screening Program (NCSP) for 5 leading sites of cancer, including the liver, in 1999. As a consequence of this program, the 5-year liver cancer survival rate increased from 13.2% (1996-2000) to 23.3% (2003-2008). The development of both the primary and secondary prevention for liver cancer including HBV immunization and cancer screening has been of critical importance.

여대생의 자아존중감에 미치는 영향 (Effects on Self-esteem among Female Undergraduate Students)

  • 본 연구는 외모스트레스, 알코올 섭취량, 이상섭식태도가 여대생의 자아존중감에 미치는 영향을 파악하는데 목적이 있다. 본 연구의 총 참여자는 여대생 150명으로 2017년 3월부터 2017년 7월까지 설문조사를 시행하였다. 수집된 자료는 IBM SPSS Statistics 22.0을 이용하여 위계적 회귀 방법으로 분석하였다. 대상자의 외모스트레스 평균은 2.65점, 알코올 섭취 문제 평균은 9.6점, 이상섭식태도 평균은 2.24점, 자아존중감 평균은 29.77점으로 나타났다. 대상자의 자아존중감의 영향요인은 대학생활만족(${\beta}=-.226$, p=.026)과 외모스트레스(${\beta}=-.318$, p<.001)로 나타났으며 변수의 설명력은 11%였다. 본 연구결과를 통해 대학생활과 외모스트레스는 성격적 차원의 핵심요소인 자아존중감에 중요하게 작용하는 것을 확인할 수 있었다. 따라서, 여대생들이 직면하는 다양한 대학생활 환경을 객관적인 관점에서 인지하고 직면한 환경과 개인 상황과의 차이를 조절하여 자기 자신을 긍정적으로 바라 볼 수 있도록 실질적인 외적 자원이 학 내외적으로 제공되어야 할 필요가 있다. 또한, 외모스트레스 상황에서 유연하게 대처할 수 있는 긍정적인 자기내면화 모색, 건강한 자아상 형성, 감정 조절을 위한 성격자원을 활성화시키기 위한 완충자원을 제공하여 여대생들의 자아존종감을 향상시켜야 할 것으로 사료된다.

요양병원 간호사의 통증관리에 대한 지식, 태도, 수행정도 (Pain Management Knowledge, Attitudes, and Performance of Nurses in Long-Term Care Hospitals)

  • 목적: 본 연구는 요양병원 간호사의 통증관리 지식, 태도, 수행정도를 파악하여 추후 요양병원 통증관리 교육을 설계하는데 기초자료로 활용하고자 실시되었다. 방법: 본 연구는 4개 요양병원에서 근무하는 간호사 120명을 대상으로 한 횡단적 조사연구이다. 통증관리 지식과 통증관리 태도는 원래 연구도구는 Watt-Watson이 개발한 도구를 사용하였고, 통증관리 수행정도는 요양병원에서의 통증관리 가이드라인들을 근거로 연구자가 개발하여 사용하였다. 결과: 본 연구의 결과 통증관리 지식은 40점 만점 평균 26.2점이었다. 가장 많이 틀린 문항은 통증의 주관성에 대한 항목으로 '통증에 따르는 환자의 생리적, 행동적 반응을 보고 통증의 유무와 강도를 파악할 수 있다(89.2%)'였고, 56.7%는 내성의 증가는 마약성 진통제 중독을 의미한다고 생각하였다. 통증관리 태도와 실무에 관해서는, 80.2%가 통증을 자주 호소하는 환자에게 위약을 사용한다고 하였고, 위약사용은 간호사가 두 번째로 자주 사용하는 통증관리 방법이었다. 결론: 본 연구는 요양병원 간호사의 통증관리 지식이 낮고 태도가 부적절한 것으로 나타났고, 이는 통증관리 실무에 부정적 영향을 미칠 수 있다. 요양병원 간호사들에게 통증관리 교육, 특히 통증의 속성과 위약남용에 대한 교육이 시급하다.

간호사의 병원 내 성희롱 피해 경험: 현상학적 연구 (Experience Sexual Harassment of Nurses: Phenomenological Study)

  • 본 연구는 간호사가 경험한 성희롱의 본질적 구조와 의미를 파악하여, 성희롱을 예방하고, 체계적인 중재방안을 마련하고자 수행되었다. Giorgi의 현상학적 연구방법과 심층면담에 의한 연구로서 자료수집은 2022년 8월 6일부터 2023년 3월 25일까지 종합병원 간호사 7명의 연구참여자로부터 수집하였다. 면담에 사용된 주질문은 "귀하에게 성희롱이란 무엇을 의미합니까?"이었다. 필사된 자료는 Giorgi의 질적 분석방법에 따라 전체의 인식, 의미단위의 구분, 의미단위의 심리학적 표현으로의 변형, 일반적 구조로의 통합의 단계를 거쳐 분석하였다. 그 결과 총 159개의 의미단위, 37개의 본질적 심리의미, 13개의 하위 구성요소, 5개의 구성요소가 도출되었다. 5개의 구성요소는 '성희롱의 최전방에 노출됨', '혼자 감당하며 고군분투함', '조직으로부터 내팽개쳐진 심정', '함께 헤쳐나갈 방법을 찾음', '변화하고 있음을 느낌'이 포함되어 있다. 이상의 결과를 토대로 간호사의 성희롱을 예방하고 이들을 위한 체계적인 중재방안 마련과 제도 개선에 유용한 기초자료를 제공할 것으로 기대된다.

사춘기 여성들의 월경경험 (Menstrual Experience of Adolescent Girls)

  • Studies on menstruation have focused only on menstruation itself and menstrual disorders. The menstruating girls or women have been neglected. So, the purpose of this study was to understand menstrual experience of adolescent girls in their perspective and build a theory on it, The specific purpose of this study were to find initial reaction of the girls, their strategies to adapt to menstruation. consequences of their efforts, influencing factor, and patterns of experience. The subjects of this study were eleven adolescent girls who experienced menarche three months to twenty-six months before the interview time. They were selected purposively. Their ages were in range of twelve and sixteen. One of them was a elementary school girl, three high school girls, and seven middle school girls. Two girls were handicapped because of cerebral palsy. All of them had some knowledge about menstrual physiology and hygiene during menstruation. Data were collected from September, 1994 to July, 1995. Data collection & analysis were done according to the grounded theory methodology by Strauss & Corbin(1990). Data collecting method was the long interviews and observation. Each interview took from 1 hour to 2 hours. Interview were tape-recorded and transcribed later by author. Data were analyzed immediately after interviews. Based on the results of previous interview, next interview were planned until gathered data reached the saturation point. Results were as follows. One hundred and six concepts were found. Those concepts were grouped into twenty eight categories and then fourteen higher categories. Twenty eight categories were as follows. “want to hide”, “bewildered”, “sense of burden”, “sense of heterogeneity”. “gladness”. “sense of superiority”, “negative empathy”, “positive empathy”, “limited hygenic control”, “sense of timing”, “lack of knowledge”, “lack of support”, “advance knowledge”, “informational support”, “emotional support”, “endurance”, “prayer”, “disclosing”, “avoidance”, “diversion”, “sense of powerlessness”, “discovery of sex identity”, “sense of maturation”, “sense of stability”, “acceptance of menstruation ”. fourteen higher categories were as follows. “negative feeling”, “posive feeling”, “exchange of feeling”, “limited hygenic control”, “sense of timing”, “accumulated experience”, “dysmenorrhea”, “level of knowledge”, “need for support”, “perceived support”, “sharing of feeling”, “self-control”, “passive acceptance”, “active acceptance”. The core category was “emotional shaking”, which consisted of “positive feeling” and “negative feeling”. “Emotional shaking”comes up to every adolescent girls experiencing menarche, independently of any contextual conditions, and its dimension has two directions : positive one and negative one. Its influencing factors were time of menarche, advance knowledge, support from the significant persons, expression and self-regulation. Even if they showed different process of adaptation to menstruation, general process of adaptation were as follows : 1. stage of emotional shaking 2. stage of acceptance 3. stage of internalization of the menstrual experience. Seven patterns existed on the process of adaptation to menstruation after menarche. Those are as follows. 1. If girls thought their menarche came too early and they had not much knowledge on menstruation, they had a kind of negative feeling. If they did not get enough support and dysmenorrhea superimposed, they came to accept menstruation passively. 2. If girls had menarche too early. they had negative feeling, even though they had enough advance knowledge. But support helped them accept menstruation easily. 3. If girls had menarche too early, they had negative feeling, even though they had enough advance knowledge on menstruation. But by experiencing subsequent menstruations and disclosing feeling, they began to accept menstruation. 4. If girls had menarche too lately and they had enough advance knowledge on menstruation. they had positive feeling. If dysmenorrhea superimposed later, their feeling turned in to negative one. But they came to accept menstruation positively by disclosing feeling and getting support. 5. If girls had menarche too early, they had negative feeling, even though they had enough advance knowledge on menstruation. In addition to this. if dysmenorrhes superimposed while they did not get enough support, they felt powerless and came to accept menstruation passively. 6. If girls had menarche too early and did not get enough advance knowledge, they had negative feeling. But disclosing feeling and support made them get sense of homogeneity and began to accept menstruation. 7. If girls had handicap, they had negative feeling, even though they had enough advance knowledge and menarche was late. But Menarche made them get feel sexual identity. Their limited hygenic control and negative empathy from their mothers made them accept menstruation passively.
