장기요양시설 규모별 간호서비스 요구 및 제공현황 비교 (Residents' Nursing Care Needs and Provision of Nursing Care by the Size of Long Term Care Facilities)

  • Purpose: The aim of this study was to provide information on the extent and variations of elderly residents' nursing care needs, and provision of nursing care across long term care facilities. Methods: A nationwide survey was conducted on nurse managers from 1,041 long term care facilities, by e-mail or fax, from August 16 to September 30 in 2017. A self-reported questionnaire consisting of 5 domains was used to collect data. Results: Facilities with more than 30 residents were more likely to need skilled nursing services and to obtain the nursing staff such as a registered nurse and a nurse's aide. Awareness and satisfaction of hospital-based home nursing care was high in all facilities. In addition, there are some differences in nurse managers' perceptions of the level of healthcare resources and required action by facility size. Nurse managers of senior congregate housings were more likely to have considerable difficulty in dealing with healthcare needs of residents and recognizing the healthcare resource shortage. A majority of nurse managers agreed on the need to employ a registered nurse. Conclusion: This study confirmed that it is essential to increase nurse staffing level and to reform the long term care insurance for enhancing the accessibility of healthcare services, especially for the residents in small long term care facilities. There is also a need to provide diverse education and training opportunities for nursing staff working in long term care facilities.

장기요양시설 노인에 대한 간호서비스 제공시간 분석 (Care Time of Elderly in Long-Term Care Facilities)

  • Purpose: This study was to define the care time of elderly in long-term care facilities and to compare and analysis the care time by staff and facility types. Method: The data were collected from 530 elderly over sixty, residing in two long-term care hospitals for dementia, three long-term care hospitals for the elderly and two skilled nursing facilities. Care time for individual residents was measured the total time spent by nurses, aides. Result: The average care time measured by nursing staff was 158.6 minutes a day. The average care time for one resident by nurse was 40.4 minutes, and by aides, 118.2 minutes. The difference of the care time showed statistical significance between long-term care facility types(p<0.001) : average care time for dementia hospital(199.1min) was twice as long as that of skilled nursing facility(94.1min). Conclusion: The results of the study showed that the care time differentiates care time provided to elderly in long-term care facilities in Korea. The study suggests the need to emphasize the importance of standardization of level of staff and service programs by the long-term facilities.

노인요양시설에서 활용되는 간호중재의 중요도와 수행도 분석 (The Importance and Performance of Nursing Interventions Perceived by Nurses in Long-term Care Facilities for Elderly)

  • Purpose: This study was to identify nursing interventions which were perceived highly in the importance and performance of nursing interventions by nurses in long-term care facilities for elderly. Method: Data was collected from nurses working in long-term care facilities for elderly over 2 years who participated in continuous education by Korea Association of Senior Citizens Welfare Institution. Data was analyzed using mean and paired t-test to compare difference between the importance and performance of each nursing intervention. Results: Among 264 nursing intervention, 49 nursing interventions were considered highly important and performed very often by nurses. Especially, 11 nursing interventions had significant difference between the importance and the performance, which meant that nurses perceived them as the most important and they were not implemented often as much as that by nurses. Conclusion: The results of this study revealed that what kinds of the nursing interventions were perceived highly important and performed very often by nurses in long-term care facilities for elderly. These nursing interventions can be utilized in the development of standardized nursing intervention classification to be used for the long-term care facilities for elderly.

서울시 노인의료복지시설의 공급과 배치에 관한 연구 (A Study on the Supply Estimation and Allocation of Healthcare Facilities for the Elderly People in Seoul)

  • Recently, as the population of the elderly rapidly increases, the number of the special care facility for the elderly is a big social issue. Especially, special care facilities(nursing home, skilled nursing home, etc) for the elderly in Seoul are insufficient now. So this study quantitatively estimates nursing home beds needed in seoul in 2007, 2012 and 2017, and proposes an allocation of these facilities in Seoul. This study is to clarify problems in supply of long-term care facilities and present a solution for them. The main outcome of this study can be summarized into three parts; first, as architectural planning and supply estimation of long term care facility, an aged population of cities and countries in Seoul should be considered. Second, when the allocation of long term care facility in Seoul, the type of facilities, regional equilibrium etc. should be considered. Third, nursing home and residential care home are linked with the other area.

  • This purposes of this study is to develope policies on the facilities for the elderly care in Korea. Methodology of this study adopts comparative analysis on the facilities for the elderly care in 6 countries which have experienced various problems on the elderly. These 6 countries are Japan, Sweden, America, Australian, England and Germany. Major issues for comparative analysis are the standard of care facilities, residence condition, finance of facilities establishment and expense, operating system, management, and the law and administrative structure about the facilities for the elderly care. The elderly people need various kinds of welfare services such as medical care facilities, nursing home facilities, home care facilities etc.. Thus the public policies for the aging population nations are compose of income maintenance program, health and medical care services and social welfare services. The policies of facilities for elderly care are very important since these policies include the characteristics of income maintenance, me\ulcorner미 care program and welfare program. This study willsupply basic data for the development of facilities for the elderly care in Korea, especially conceming the system and institutional device of the facilities.

경인지역 요양시설의 식사지원 서비스 실태 (Meal Services at Long-term Care Facilities in Kyung-In Area)

  • Purpose: This study was aimed at understanding meal services provided at long-term care facilities. Method: Interview survey was conducted using questionnaires at 254 facilities located in the Kyung-In area. Result: Of the residents, 37.4% were eating meals unassisted. Eating places included living rooms and residents' rooms in most facilities. Major noise source was television in 63.8%. Apron was applied to all elderly residents at mealtimes in 49.6% of the facilities. Half of the facilities used feeding utensils except for ordinary spoon and chopsticks. Of the facilities having individual prosthetic devices, dentures were applied before eating in 98%, glasses in 20.2% and hearing aids in 9.2%. Most facilities included the residents' favorite foods in menu: wheres, only 9.4% offered the menu which residents could choose. Conclusion: Standard guidelines and staff education for meal services need to be provided for elderly residents.

노인장기요양시설 서비스의 질 평가 탐색 : 포커스 그룹 연구 (Evaluation of Service Quality in Aged Care Facilities : A Focus Group Study)

  • Purpose: This study was conducted to explore the meaning and contents of high-quality aged care facilities and provide basic data for evaluation of service quality in such facilities. Methods: The focus group interviews and participants consisted of two user groups, for a total of 16 family caregivers of the elderly living in facilities and four service provider groups, for a total of 26 chief managers and caregivers working in aged care facilities. All interviews were recorded and transcribed as they occurred. Content analysis was used and debriefing notes were referred to in order to analyze the data. Results: Four themes of a high-quality aged care facility emerged from the analysis; 1) a place to rest for comfortable later years; 2) systematic value-based management; 3) providing professional care; 4) comprehensive service provision in response to diverse needs. Conclusion: The findings of the study showed the importance of client centered care and ethical mindset of providers, which had not been included in the existing quality evaluation programs. Based on these results, medical treatments, end-of-life care and more comprehensive and extended services including family care need to be provided in facilities to ensure good quality aged care.

요양시설노인을 위한 구강간호 임상실무지침 수용개작 (Adapting Oral Health Care Guidelines for Nursing Home Residents in South Korea)

  • Purpose: This study attempted to adapt evidence-based oral health care guidelines for nursing home residents in South Korea. Methods: The manual for guideline adaptation, version 2.0, developed by the Korean National Evidence-based Healthcare Collaborating Agency was applied. On the basis of a comprehensive literature review of current research and multidisciplinary panel discussion on adapting such guidelines, we developed evidence- based guidelines for oral health at nursing homes. Results: The guidelines have three domains: oral assessment, oral hygiene care, and support and facilities. Oral assessment includes recommendations on the frequency of and tools for conducting oral health assessment. Oral hygiene care involves recommendations for care of natural teeth, denture care, xerostomia, and behavioral problems. Support and facilities refer to six components of the support system provided by facilities to encourage oral hygiene care among nursing home residents. Conclusion: These adapted guidelines could be an effective method to improve oral hygiene among nursing home residents.

  • Purpose: COVID-19 infections have been erupting in places of worship, long-term care facilities, and call centers in Korea since January 2020. This study aims to diagnose and present an infection control system solution for long-term care facilities where at-risk elderly individuals are actively engaged in communal life. Methods: We conducted comparative analyses of infection control systems between long-term care facilities and medical institutions respective of relevant laws and this study's evaluation system. Results: To prepare for future infectious diseases, it is necessary to establish a long-term care facility infection control system and strengthen the standards thereof, to strengthen long-term care facility evaluation standards and to newly establish medical charges for infection control. Conclusion: Systematic procedure fortification and financial support provisions are necessary for infection control at long-term care facilities.

요양보호사의 환자안전행위에 관한 영향요인 (Influencing Factors on Patient Safety Behaviors of Care Workers in Long Term Care Facilities)

  • Purpose: The purpose of this study was to identify factors influencing care workers' patient safety behaviors (PSB), we hoped to provide baseline data for its activation. Methods: The subjects of this study were chosen by convenience sampling, which included 114 care workers at long term care facilities. Data were collected from August 22 to October 10, 2018. The gathered data were analyzed using the SPSS program, which was used to Cronbach's alpha, descriptive statistics, t-test, ANOVA, Pearson correlation coefficient and stepwise regression. Results: The care workers' average PSB was at 7.81 points, patient safety culture (PSC) 3.86 points and safety of long-term care facilities 7.07. PSB positively correlated with safety of facilities(r=.802, p<.001) and PSC(r=.558, p<.001). Work attitude and satisfaction of job were explained 35.1% of the PSB. Conclusion: Administrators of long term care facilities should be interested in patient safety culture and improving job satisfaction of care workers in order to improve patient safety behaviors.
