A Study on Environment-related Factors that Promote Psychological·Behavioral Health and Quality of Life for People with Dementia - Focused on the Context of Long-term Care Facilities for People with Dementia (치매 환자의 건강과 삶의 질 향상을 위한 치유환경 요소에 관한 연구 - 치매 환자를 위한 노인요양시설을 중심으로 )

  • Purpose: The main purpose of this paper is to assess a body of research evidence that articulates the impact of environment-related factors on behavioral health and quality of life for people with dementia who reside in long-term care facilities. The study also aims at identifying design implications that can be incorporated into design process and design decisions to improve behavioral health and quality of life for people with dementia. Methods: Comprehensive literature review has been conducted to identify empirical studies and that link the design of dementia care facilities to health- and QOL-related outcomes and scrutinized peer-reviewed articles published in many different fields including architecture, psychology, to nursing. Results: The review identified a growing body of literature that articulates environment-related factors that improve health and quality of life for people with dementia living long-term care facilities Implications: The findings of the review can be translated to design decisions to promote psychological and behavioral health and quality of life of people with dementia in long-term care facilities.

The Effect of the Abdominal Draw-In Maneuver Using Breathing on Trunk Control and Hand Grip in Stroke Patients (호흡을 이용한 복부 드로우-인 기법이 뇌졸중 환자들의 몸통 조절과 악력에 미치는 영향)

  • PURPOSE: This study was carried out to examine the changes in the trunk control ability and hand grip when the abdominal draw-in maneuver using breathing was applied to stroke patients, and provide basic data regarding the trunk stabilization exercise. METHOD: After randomly placing patients in group I-applying the existing abdominal draw-in exercise and group II -applying the abdominal draw-in exercise using breathing, the intervention program was performed ten times per set (five sets per session), once a day, four times a week, for a total of four weeks. For the pre-tests before the intervention, trunk damage and hand grip were measured. After the four weeks of intervention, post-tests were conducted in the same way as the pre-tests for analyzing the study results. RESULTS: Both groups had significant differences in trunk control ability and hand grip (p < .05). The comparison between the two groups showed, significant differences only in the trunk control a ability(p < .05). CONCLUSION: The abdominal draw-in exercise using breathing led to the efficient contraction of abdominal muscles, which implies this is a more effective intervention to improve the trunk control ability.

Comparative length of emergency department stay of patients with different Korean Triage and Acute Scale severities: A descriptive analysis (응급실에 내원한 환자의 한국형 중증도 분류 결과)

  • Purpose: The severity classification in association with the time of visit to and the appropriateness of using a public ambulance for visiting the emergency department (ED) have not been thoroughly evaluated, and we aimed to evaluate these aspects. Methods: In this descriptive research, we retrospectively reviewed and analyzed the medical records of patients who visited the ED of the B General Hospital, Seoul from January to December 2019. Results: Of the 54,297 patients who were included in the analysis, 34,629 (63.8%) and 14,065 (25.9%) visited the ED directly and through public ambulances, respectively; 10,328 (73.4%) patients who used public ambulances were discharged home. In the daytime and nighttime, 24,891 (45.8%) and 29,406 (54.2%), respectively, visited the ED. The mean length of ED stay (LoS) of emergency and non-emergency patients was 326 and 159 minutes, respectively, and of patients classified as Korean Triage and Acuity Scale levels 1 and 2 was 427 and 430 minutes, respectively, which was longer than the total of 236 minutes. Conclusion: Patients who visited the ED using public ambulances constituted nearly 25% of all ED visits, and more than 70% of these patients were discharged home. Patients with high severity had a longer mean LoS, and daytime ED visits were characterized by higher numbers and severity of patients than nighttime ED visits.

Development and validation of a three-person cardiopulmonary resuscitation protocol (3인 CPR 프로토콜 개발 및 타당도 검증)

  • Purpose: This protocol aims to improve the simplicity of the existing two-person cardiopulmonary resuscitation (CPR) by focusing on chest compressions and artificial respiration, and classifying the roles within the scope that can be observed. Methods: In this study, the protocol was developed by forming an expert group consisting of a professor from the emergency and rescue department, a professor from the nursing department, and a first aid officer from the fire department. In addition, if the number of panels is 15 and the derived content validity ratio (CVR) value exceeds 0.49, a reference point that satisfies content validity at 95% reliability is presented. Results: All 11 performance items exceeded the CVR value of 0.49 to meet the reference point, and the CVR range of this protocol is 0.625 to 1.000. Conclusion: In order to improve the quality of three-person CPR, verification of simulation models and continuous protocol revision should be conducted by individuals specializing in various fields.

Education based on the health belief model to improve the level of physical activity

  • [Purpose] This study aimed to investigate the effect of education based on the health belief model on the physical activity of the staff of the University of Medical Sciences. [Methods] This semi-experimental study was conducted on 130 university staff aged 25-50 years from the Hamadan University of Medical Sciences. Inclusion criteria were having at least 1 year of work experience, lack of acute and chronic physical and mental illnesses, and not using drugs that affect physical activity. The samples were randomly divided into two groups. The experimental group received three training sessions based on the health belief model. Before and 2 months after training, the control and experimental groups were evaluated via the following questionnaires: (1) demographic information questionnaire, (2) Health Belief Model Questionnaire, and (3) International Physical Activity Questionnaire. Finally, data were analyzed statistically. [Results] The training process resulted in a significant increase in the mean scores of the health belief model constructs in the experimental group, but changes in the control group were not significant. Self-efficacy was the strongest predictor of physical activity. [Conclusion] The health belief model is a useful model for improving individuals' understanding of the benefits of physical activity.

Comparison of Abductor Hallucis Contractility between Regular and Flat Feet during SFE with SLSKB Test

  • Purpose: This study compared and analyzed the contractility of the abductor hallucis (AbdH), an intrinsic foot muscle, between flat feet and normal feet during a movement control test (single-leg small knee band test) using ultrasonography. Methods: A total of 23 subjects with (n = 11) and without (n = 12) flatfoot were included in the study. Each subject performed the short foot exercises (SFE) with a single-leg small knee bend (SLSKB) test, which is a functional movement. An ultrasound device was used to collect data regarding the changes in the contractility of the AbdH. Results: Intergroup comparison showed that dorsoplantar thickness was significantly reduced at baseline and during the SFE with SLSKB in the flatfoot group (p < 0.05). Intragroup comparison showed that the cross-sectional area significantly improved when the SFE was performed with SLSKB in the control group (p < 0.05). Conclusion: In this study, it was observed that the AbdH had inadequate contractility during the SLSKB test in subjects with flatfoot; therefore, it is important to train the contraction of the AbdH via functional movements during clinical interventions for subjects with flatfoot.

Analysis of factors affecting Korean Medicine utilization of multicultural family members (다문화가정의 한의의료이용에 미치는 요인 분석 연구)

  • Objectives: This study aimed to analyze the Korean medical utilization and Korean medical expenses by characteristics of multicultural family members to investigate whether it affects Korean medical utilization. Methods: This study utilized The Korea Health Panel data in 2018, with 238 final participants. We performed a t-test and ANOVA on the difference between Korean medical utilization and Korean medical expenses according to Anderson's Model of Health Service Utilization variables. Logistic regression analysis and generalized linear model analysis were conducted to analyze Korean medical utilization factors. Results: The Korean Medical utilization was 12.61% among the multicultural family members. As a result of regression analysis, the female had high Korean medical utilization(p=.008), and rural area residents had low utilization(p=.017). Korean medical expenses were high when they were female or married. Including the utilization of western medical services by outpatients, Korean medical expenses were high when they were female or outpatients who received western medical services. Conclusions: As a result of this study, the factors influencing Korean medical utilization were gender and residence area. There were differences in Korean medical expenses depending on western medical services use or gender. Therefore, it is necessary to use these factors to expand the Korean Medical utilization by multicultural family members, and research of the Korean medical utilization by disease is needed.

Effect of Job Embeddedness and Organization Attachment on Intention to Stay in Hospital Nurses (병원간호사의 직무배태성과 조직애착이 잔류의도에 미치는 영향)

  • This study aimed to identify the effect of job embeddedness and organization attachment on the intention to stay of hospital nurses in the convergence society. The participants were 211 general hospital nurses; data were analyzed using t-test, ANOVA, Pearson correlation, Multiple regression. This study found that: hospital nurses showed higher score at job embeddedness, higher organization attachment, higher scores for intention to stay. The main factors influencing intention to stay were job embeddedness and organization attachment. They explained about 45% of the intention to stay. Thus, it is necessary to allocate tasks and evaluate appropriate to the nurse's ability and support individual and organizational activities that can enhance organizational attachment.

The Effectiveness of Mindfulness-Based Cognitive Therapy-Korean (MBCT-K) for Anxiety and Depression in Patients with Anxiety Disorder (한국형 마음챙김 명상에 기초한 인지 치료가 불안 장애 환자의 불안과 우울에 미치는 효과 비교)

  • The objective of this study was to examine the effectiveness of newly developed program for the recovery and relapse prevention in patients with anxiety disorder. Twenty-four patients with anxiety disorder received Mindfulness-Based Cognitive Therapy (MBCT) session weekly for a period of overall 8 weeks-program. Changes of depression, anxiety, negative and positive automatic thought were compared before and after the program. Depression and anxiety were improved significantly after the MBCT program(Z=-1.9, p=.06, Z=-2.9, p<.001). Conclusions: MBCT may be effective at reducing negative automatic thought and relieving anxiety and depressive symptoms in patients with anxiety disorder. However, large-sample, randomized controlled trials will be needed for generalization.

  • In December 2019, the world faced a crisis in the international economy due to the coronavirus pandemic. Korea has made efforts to stimulate the economy in various ways, including basic income support for all citizens and income support for small business owners. The purpose of this study is to examine how prolonged COVID-19 affects the family, the basic unit of society, and each family member. The family conflicts and crisis solutions due to COVID-19 are as follows. It is to have intensive support for the family, program support for strengthening bonds between household members, and recognition of equal values.
