A Study on High School Health Education Teachers' Activities and Other Influential Variables (고등학교 보건교육 관련 교사의 보건교육수행 및 관련요인에 대한 연구)

  • High school is regarded as the period when many important physical, mental and social developments occur, and when many health-related behaviors are formed. School health education is one of the major learning resources influencing health potential in the home and community as well as for the individual student. High school health education in Korea has a fundamental systemic flaw however, in that health -related subjects are divided and taught under various subjects at school. In order to achieve quality health education, it is essential to assess the learners' and teachers' educational needs. So far, most of the research projects that had been carried out for improving high school health education were limited to only the learners' educational need. They failed to in elude an educational assessment of the teachers. Therefore, in this study the high school health education teachers' needs relating to health education were investigated through a focus on the teachers' health education activity level, health education activity self-efficacy level, and perceived level of importance in health education content. In this study, research instruments these factors were constructed by Yoo(1997) on the basis of the PRECEDE model. The data for this study were collected from a sample consisting of twenty general and vocational high schools in Seoul and Chongju for a two month period beginning in July, 1996. In analyzing the data, an ANOVA test and stepwise multiple regression were accomplished using an SPSS - PC+ program. The results were as follows: The average level of health education activity and self-efficacy among high school health edu cation teachers were found to be low. But, teachers' perceived importance of health education contents was high. Teachers' activity and perceived importance concerning sex education were lower than in other health education areas. Health education activity of Military drill teachers was higher than that of physical education teachers as well as school nurses. But it was not significant. Health education activity self-efficacy of school nurses was higher than that of other teachers(p<.05). Perceived level of importance of health education contents was the most influential variable in teachers' health education activity. Health education activity self-efficacy level was not an influential variable in teachers' health education activity. The significance of this study is that it has diagnosed the needs of high school health education through the teachers' assessment of a variety of health factors related. These findings suggest that the management of an integrated health education, program requiring large changes in the curriculum of health education is necessary.

A Model Curriculum Development for Clinical Nurse Specialist Training Program in Organ Transplant (장기이식 전문간호사를 위한 교육과정 연구)

  • The twenty-first century may be said to be entering into a specialized qualification age to meet the needs of new technical innovations such as environmental changes, demographical changes, changes in the constitution of diseases, changes in the needs of the national health, reforms of information and knowledge, etc., which requires the provision of competitive services that can fulfill the high level needs of consumers. In consequence, it is needed to apply a practical nursing model that can serve as a guide for healthy society and to secure the sphere that can affect nursing policy-making by keeping pace with the changing environment. Furthermore, it is also urgent to expand more the activity sphere of nurse specialists with authority and autonomy, establish their legal foundation, establish a qualification accreditation system for nurse specialists, and develop educational programs. In Korea, the law relative to organ transplant past the national assembly on February 9, 2000, legally acknowledged brain death, which indicated to us the emergence of an age of organ transplant. Therefore, it necessitates to find out those of brain death from whom organ transplant is feasible in clinical practices, with their families' consent link to those terminal organ failure patients who are in need of an organ, and mediate both parties so that smooth transplant can be accomplished. A series of these complicated procedures require systematically trained specialists with high level techniques of organic management. With this in mind, this study was conducted on 69 clinical nurse specialists for organ transplant, accredited by the hospital, who are in active service in clinical practices. The resultant findings were revealed, as follows: 1. The qualifications of clinical nurse specialists for organ transplant should be accredited by Ministry of Health and Welfare or Korea Nurses Association. 2. The validity of qualifications should be for three years, and their renewal should be based on marks of a supplemental training or an education course for more than 12 hours a year. 3. The qualification of the clinical nurse specialist necessitates theoretical lectures and practices on those nurses who have had clinical experience in the pertinent field. 4. The course of training is required to be one year in the length of training and take more than 20 credits (320 hours) and 5 credits (240 hours).

A Study on Needs of the Families of Head Injury Patients (두부 손상환자 가족의 간호요구에 대한 연구)

  • The purpose of this study is to identify the needs which are perceived by one of the familiy members who have head injury parients by traffic accidents in the intensive care units. Subjects were 70 families members of head injury patients admitted to 2 general hospitals NICU located Pusan city, 1 general hospital NICU located Ulsan city from December 1. 1998 to February 28. 1999. For this study, 70 family members were interviewed with aid of a Likert scale which was developed by researcher for this study. For development of the survey instrument, literature review and open questionnaire technique with family members and the nurses working in NICU. The 50needs-items were classified into 7 groups according to the homogeneity of the items with the support of literature review. For the content validity, the instrument was reviewed by 1 nursing professor and the internal reliability of this instrument was Cronbach alpha=0.94 which is highly accepted. Data was analyzed by a SPSS computer program. Data analysis included frequency. percentage, mean, standard variance and t-test or ANOVA. The results were as followings : 1. The general characteristics of head injury patients shows that the male was 74.3%, the female was 25.7% and age distribution shows that the fifty-fifty nine years was 30%, the highest. Of religion the buddhism was the most, The diagnosis distribution shows that epidural hematoma was 32.9% and subdural hematoma was 24.3%. The mentalility distribution shows that semicoma was 31.4% and stupor was 31.4%. Hemiplegia was 42.9% 2. The general charaterisrics of the family needs of head injury patients shows that thirty-thirty nine years was 31.4%, the highest. sex distribution shows that the male was 20%, the female was 80%. Of religion the buddism was the most. 3. The family needs of head injury patients was $3.03{\pm}0.42$, needs for the information of a patient's condition was $3.65{\pm}0.48$, the highest. And needs for the information of care and treatment was $3.48{\pm}0.48$, needs to be supplied with comfortable facilities for family was $3.04{\pm}0.66$, needs to be participate in a patient's care was $2.90{\pm}0.55$, needs to be informed about the available resources was $2.83{\pm}0.59$, needs to be supported emotionally for family was $2.79{\pm}0.55$, needs for religious assistance was $2.51{\pm}0.85$. 4. Examining the family needs of head injury patients according to patient's characteristics, mentality, plegia degree and operation were shown be variables to make an effect on the needs for the head injury patients family. At all, according to severity of head. injury, the family needs of head injury patients was high. 5. Examining the family needs of head injury patients according to their general characteristics, we could know that religion, job. income were shown to be variables to make an effect on the family needs. Through the examination it can be seen that the characteristics of head injury patients and the family needs of head injury patients. In conclusion, the family needs of head injury patients was almost same the family needs of ICU patients. Therefore we must involve the family's care of head injury patients and we must provide exact and repeated explanation, education and support the family of head injury patients. As this study was resulted in selecting the families admitted to NICU of some general hospital, we couldn't stretch the result in our favor. Therefore, continuous studies are suggested.

The needs of nursing home services for frail elderly people receiving home care nursing services (가정간호서비스 이용 노인환자의 간호요양원(nursing home) 필요 예측에 관한 연구)

  • The purpose of this study was to estimate the population requiring nursing home services for frail elders who received home care nursing services. This study identified the need of nursing home services and the proportion of elders over 60 years of age requiring nursing home services according to the admission eligibility including items with intensity of home care nursing services and the criteria for screening of patients requiring home care nursing services. In this study, survey research design was conducted. A total of 49 home care nurses were collected. They were asked to assess the need of nursing home services based on items with intensity of home care nursing services and the criteria for screening of patients requiring home care nursing services. Using secondary data analysis from the survey on 'The Evaluation of Home Care Nursing Service Program in 1994 in Korea', the proportion of elders requiring nursing home services was estimated. The estimated numbers and proportion of frail elders requiring nursing home services were 169 (86.2%) aged persons among community dwelling elders based on admission eligibility. However, the proportion of aged persons was 68% using the only criteria for screening of patients requiring for screening home care nursing services.

Prerequisite for Korean Advance Directives: From the View of Healthcare Providers (한국형 사전의료의향서의 구성 요건: 의료인을 중심으로)

  • Purpose: The purpose of this study was to explore the main prerequisite for Korean Advance Directives [KAD] to ensure their better use. Methods: Data were collected from two focus group interviews and individual email responses. Study participants were 5 doctors and 6 nurses. All interview data were transcribed and analyzed using qualitative content analysis. Results: Three main themes emerged; establishing a philosophy of KAD, protocol to practice KAD, and the KAD document itself. A philosophy is needed to ensure individual needs, consensus to practice AD and identify principle agents. The core of protocol was found to be as follows; 1) process, 2) premise, 3) procedure, 4) contextual preparation, 5) timing, 6) feasibility. Component and feasibility need to be established for the document itself. Conclusion: For a positive acceptance of KAD in Korean society, a culture sensitive, reality based, and user friendly AD needs to be developed.

Developing a Community Capacity Builded Exercise Maintenance Program for Frail Elderly Women (지역사회 역량강화 전략을 이용한 재가 허약여성노인의 운동유지 프로그램 개발)

  • Purpose: This study was conducted to develop a community capacity builded exercise maintenance program for frail elderly women. Methods: As a guideline to develop the exercise maintenance program, the intervention mapping framework, including needs assessment, setting program goals, selecting theory-informed intervention methods, producing program components, planning program implementation and evaluation, was used. Focus group interviews with public health nurses and frail elderly women were conducted for needs assessment. Intervention strategies and components were formulated based on community capacity theory. Results: The developed exercise maintenance program consisted of strategies focusing on leadership development, partnership construction, organization development, community systematization of dimension of community capacity. A exercise maintenance program using health leader, health contract, exercise pocketbook, rhythmic activity suiting song and self-help group was included. Conclusion: The intervention mapping method was found to be useful to develop theory-based valid and community capacity builded exercise maintenance strategies for frail elderly women.

Wound Care Needs of Hospital-based Home Care Nursing at Elderly Care Facilities (노인요양시설의 간호인력과 비간호인력간 상처관리에 대한 병원중심 가정간호 요구도 비교)

  • Background: The purpose of this study is to investigate the needs for wound care services in the elderly care institutions by analyzing hospital-based home care nursing in select facilities in Korea. Methods: A total of 92 staffs at the elderly care institutions, located throughout the country, completed self-report questionnaires between February 1 and November 31, 2009. SPSS ver. 17.0 was used for data analysis, regarding frequency and chi square. The 5% level of significance was the critical level for acceptance of the study's hypotheses. Results: Results were as follows; group A and group B were rated as 'high' and 'very high', respectively, in care of simple pressure ulcer, care of complicated pressure ulcer, stitch out, care of simple surgical wound, ostomy care, complicated ostomy care, simple dressing and complicated dressing. Conclusion: Political decisions intended to promote visits by hospital- based home care nurses to the elderly care institutions would be a prudent course.

The Effects of Pre-operational Education on Nursing Care Satisfaction and Anxiety;A Comparison between Traditional Face-to-face Education and Mobile Text Messages (정보제공 방법에 따른 암수술환자 가족의 간호요구 만족도와 불안)

  • Purpose: This study was aimed to compare two different kinds of pre-operational education methods on nursing care satisfaction and anxiety among family members of cancer patients. Methods: This research used a quasi-experimental design. A total of 80 patients participated in the study. Forty subjects who were in the experimental group were assigned to be received mobile text-information via cellular phone, whereas control group was given the direct messages from nurses during peri-operative time. Peri-operative family needs and anxiety were measured and analyzed by X2-test and t-test. Result: There was no statistically significant difference in the levels of peri-operative family needs satisfaction and the levels of state anxiety between the two groups (p= .05). Conclusion: Based upon these findings, the nursing intervention with cellular phone short message could be as effective as direct peri-operative information in reducing the level of state anxiety and in increasing of the level of nursing care satisfaction among the family members of cancer patients.

Needs Assessment for Information and Support of Parents of Children with Epilepsy (간질환아 부모들의 정보와 지지에 대한 요구조사)

  • Purpose: This study investigated the need for information and social support in parents of children with epilepsy. Method: A total of 119 parents of children with epilepsy were recruited and asked to fill out questionnaires. Result: Of 119 parents, two-third reported that they received full and sufficient information about their child's disease and its management but one-third felt the information was insufficient and incomplete. Most parents (62.2%) felt at loss when their child had a seizure, either at home or at school. They wanted information on the causes of seizures, adequate steps deal with seizure and steps they should take to become adequate and supportive parents for their children. However, most parents were reluctant to disclose the disease or to receive support from outsiders. Conclusion: Regardless of the fact that most parents received sufficient information about the management of epilepsy, they felt at a loss when their child had a seizure attack. Therefore nurses should give specific instruction on seizure management and assess the needs of parents on a regular basis.

The Mediating Effect of Nurse's Communication on the Relationship between Nursing Needs and Nursing Satisfaction in hospital patients. (입원환자가 인지하는 간호요구도와 간호만족도의 관계에 영향을 미치는 간호사 의사소통 매개효과)

  • This study aimed to identify the mediating effect of nurse's communication on the relationship between nursing needs and nursing satisfaction in hospital patients. After IRB approval, this study was performed by administering questionnaires to 180 university hospital patients between April and June, 2017. Collected data were analyzed with SPSS 22.0. The average score of nurse's communication, nursing needs, and nursing satisfaction were $3.88{\pm}0.52$, $3.96{\pm}0.55$, and $3.74{\pm}0.59$ out of 5, respectively. There were significant differences in educational and therapeutic needs versus satisfaction. As for nurse's communication and nursing satisfaction, there were significant differences by patient's room. There were positive correlations among nurse's communication and nursing needs (r=.286, p<.001) as well as nurse's communication and nursing satisfaction (r=.524, p<.001), In addition, there were positive correlations between nursing needs and satisfaction (r=.488, p<.001). Nursing communication had a partial mediating effect between nursing needs and satisfaction, which showed significance in Sobel's test (Z=2.450, p=.014). Therefore, it is suggested that a continuous program for strengthening communication of hospital nurses is needed.
