Work Experiences of Delivery Room Nurses: A Phenomenological Study (분만실 간호사의 근무 경험에 관한 현상학적 연구)

  • Purpose: This study aimed to understand meaning and essentials in work experiences of delivery room nurses. Methods: A phenomenological perspective was used for this qualitative research. Ten delivery room nurse with more than 3 years of work experience participated in this study. Data were collected through individual in-depth interviews with the nurses, between December 2012 and April 2013. Data were analyzed using Colaizzi's method. Results: Nine theme clusters and four categories emerged from the data as follows: 1) factors influencing career decision-making, 2) gap between reality and expectations, 3) difficulties with working in the delivery room, and 4) motivation to work in the delivery room. Conclusion: Based on the results of this study, strategies to improve working environment of the delivery room nurses are necessary. Also, better policy are required to firmly establish the role of the delivery room nurses' role as recognized professionals.

Awareness and Experience of Nurses and Physicians on DNR (심폐소생술금지(DNR)에 대한 간호사와 의사의 인식과 경험)

  • Purpose: To examine the awareness and experiences of nurses and physicians on DNR. Method: The sample contained of 199 nurses and 98 physicians. The Instrument used in this study was The Nurses' Understanding and Attitude on DNR(AEDNR) developed by the Han et al and revised by the research the AEDNR included 29 items. Result: About 97.3% of them feel the necessity of DNR, Majority of subject(78.8%) have practiced DNR by the demand of 'the patient's family' and 91.8% of them made a medical record at the time they carried out DNR. There were no significant differences in their perceived necessity of DNR between nurses and physicians. Conclusion: This study can be a basis for making objective standards educational, ethical, and legal issues concerning DNR.

Ethical Dilemma of Nurses (간호사의 윤리적 딜레마)

  • Purpose: The purpose of this study is to explore the lived experience of ethical dilemma of nurses and to describe the structure of the meaning of their experience for nurses. Method: The participants in this study were seven nurses with 3years and more careers who worked at general hospital and university hospital in T, P, K city. The data were collected from May, 2004 to September, 2004 through in-depth interviews untill saturated. The data were analyzed by phenomenological method suggested Giorgi. Results: Five main meanings were identified: Role limitation, Difficulty of decision, Self defensive position, Difficulty of interpersonal relation, A custom of unable expose. Conclusions: Based on the results of this study, there is need to study for identify the ethical dilemma of nurses from the viewpoint of korea, and development of nursing ethics guidebook to guide ethical dilemma situation in nursing field.

The Process of Overcoming the Professional Burnout of Clinical Nurses (임상간호사의 직무소진 극복과정)

  • Purpose: The purpose of this study was to identify the process of overcoming professional burnout among clinical nurses. Methods: Data were collected from 12 clinical nurses through in-depth interviews about their actual experiences. The main question was "How do you describe your experience of the process of overcoming professional burnout?" Qualitative data from the field notes and transcribed notes were analyzed using the grounded theory methodology developed by Strauss & Corbin. Results: The core category of experience of the process of professional burnout among clinical nurses was identified as "Keeping a balance by awakening-relaxing-coexisting". The participants used six interactional strategies such as 'Confronting problems', 'Exposing problems', 'Laying the mind's burdens down', 'Taking a breather', 'Seeing from a different perspective', 'Accepting realities'. The results of overcoming process of professional burnout were 'Coexisting' and 'Revitalizing'. Conclusion: The results of this study provide useful information for designing supportive programs and policies to solve professional burnout in clinical nurses.

Incidence of Cutaneous Injury in Clinical Nurses (병원 간호사의 경피 상해 발생 실태)

  • Purpose: To identify the incidence of cutaneous injury in clinical nurses. Method: From Feb.1 to 28, 2005, 276 clinical nurses were surveyed by questionnaire. Results: 1. Of the nurses, 53.6% had at least one incidence of cutaneous injury, and the mean number of injuries was 1.34. A higher incidence rate for cutaneous injury was found in nurses who were under the age of 25, unmarried and who had less than 3 years career experience. 2. The major causes of injury were syringe needles at 65.0%, and medical instrument were next followed by sharp objects or blades. The injuries occurred when the nurses were rearranging equipment after care (25.2%), taking blood samples (22.8%), separating syringes and needles (17.1%), during surgical operations (14.2%), and distribution of medications, treatments and recapping of needles (5.7% each). The hands were the most common body parts injured, and the most prevalent pathogens contaminating the instruments causing the injury were HBV syphilis, HCV and HIV in that order. 3. Of the injured nurses, 77.9% did not report the accident and 25.8% did not receive any treatment because there were no pathogens, it was a bother or there was difficulty reporting the incident. Conclusion: To reduce cutaneous injuries, intensive training and supervision may be needed for those of nurses under the age of 25, unmarried and with less than 3 years career experience.

Content Analysis and Job Satisfaction of the Nurse in the Free Elderly Home and Nursing Home (무료 양로.요양시설 간호사의 업무내용과 직무만족정도)

  • Purpose: The study was performed to compare the contents of jobs and job satisfaction of resident nurses at free elderly homes with nursing homes in Korea. Method: The subjects consisted of 138 resident nurses at free elderly homes and nursing homes. Data were collected from the 1st of January to the 22nd of February in 2005 using a structured questionnaire containing questions on general characteristics. contents of the nurses' daily tasks, health program. job satisfaction and pay satisfaction. Collected data were analyzed using SPSS (version 11.5) through descriptive statistics, $X^2-test$, t-test and ANOVA. Result: For nurses at free elderly homes and nursing homes. their daily tasks consisted of health assessment. medication care and eating care. The general job satisfaction of the nurses was similar between those at free elderly homes and those at nursing homes, but working conditions and pay were significantly different according to the sub content of job satisfaction. Job satisfaction was significantly different according to marital status in resident nurses at the free elderly home, and according to the experience of living with the elderly and the experience of receiving special training or lectures in elderly nursing in resident nurses at free nursing homes. Conclusion: Resident nurses at free elderly homes and nursing homes had various types of tasks, and it is necessary to differentiate tasks and payment systems for them according to their job.

Influence of Internal Marketing Perception, Empowerment, and Job satisfaction on Customer Orientation of Clinical Nurses (병원간호사의 내부마케팅 인식, 임파워먼트, 직무만족이 고객지향성에 미치는 영향)

  • Purpose: This descriptive study attempted to identify the influence of internal marketing perception, empowerment, and job satisfaction on customer orientation of clinical nurses. Methods: The subjects were 411 nurses with over one year working experience in a university hospital, located in I-city. The data were collected from April 20 to 30, 2012. using a self-report questionnaire. The data were analyzed using hierarchical multiple regression with the SPSS/WIN 17.0 program. Results: There were significant differences in customer orientation depending on general characteristics(age, marital status, education, position, clinical experience, and department). Internal marketing perception, empowerment, and job satisfaction correlated with customer orientation. As the results of hierarchical multiple regression analysis for impact factors on nurses' customer orientation show, factors are the outpatient department in working places(${\beta}$=.10), education and training in internal marketing perception(${\beta}$=.12) and empowerment(${\beta}$=.44), which accounted for 31.1% of customer orientation. Conclusion: Therefore, nurse managers should identify the needs to perceive nurses as internal customers, and the internal marketing strategy should be performed to empower nurses. Also, it is needed to place nurses with high customer orientation at the outpatient department.

A Study on Experience of Transition from New Clinical Nurse to Competent Step (임상 신규간호사의 상급 초보자에서 적임자로 되어가는 경험)

  • Purpose: This study aimed to describe nurses' experience in transition from new graduate nurses to professional clinical nursees in a hospital. Methods: Ten new clinical nurses working in a hospital participated in this study. Data collected through in-depth interviews were analyzed using phenomenology. Results: The analysis yielded five core category themes from new nurses for becoming a professional nurse: (a) beginning of hardship journey (b) feeling of inability in workplace (c) struggling hard to develop skills (d) enduring hardships by soothing oneself and peer support (e) beginning of an exciting journey. Conclusion: Over time, new nurses face many difficulties in transition to a professional status. To enhance the role development of new clinical nurses in hospital settings, nursing schools and clinical fields must teach and prepare them. Nursing schools should teach a strong theoretical base as well as nursing skills. In addition, in the clinical nursing department, education, teaching the integrated systems, staff development, support, evaluation, and feedback are needed for new nurses's development.

Job Stress and Assertiveness Behaviors among ICU Nurses (중환자실 간호사의 직무 스트레스와 자기주장 행동)

  • Purpose: This study was designed to identify levels of job stress and assertiveness behaviors and the relationship between them among ICU nurses. Methods: A correlational descriptive survey was used to these relationships among 454 ICU nurses from 25 hospitals which were members of Korean Association of Critical Care Nurses. Data were collected using a structured questionnaire which included 58 items of job stress and assertiveness scale from November 4 to 14, 2008. Descriptive statistics, Pearson correlation coefficients were used to analyze the data. Results: ICU nurses experience mild to moderate degree of job stress, A score of assertiveness behaviors was 2.86. There was a negative relationship between job stress and assertiveness behaviors(r=-.106, p=.024) as influenced by role conflict, lack of professional knowledge and skills, and inappropriate treatment. Conclusion: ICU nurses, especially nurses with 3-6 year clinical experience, are in need of support to manage their job stress. These nurses should be supported through provision of adequate stress management and assertiveness program.

Comparison of Effects of Nursing Process Reeducation according to Academic Credit Bank System Student Nurses' Clinical Experience - Expertise, Critical Thinking Disposition, Clinical Decision Making - (학점은행제 교육과정 간호사들의 임상경력에 따른 간호과정 재교육의 효과 비교 - 숙련성, 비판적사고 성향, 임상의사결정 능력을 중심으로 -)

  • Purpose: This study investigated the effects of reeducation of nursing process on nurses' expertise, critical thinking disposition, and clinical decision making. Methods: Data were collected from March to June, 2007. The subjects were 47 Academic Credit Bank System Student Nurses. They were taught 'nursing process' during 1 semester(15weeks). 47 nurses were divided into three groups according to clinical experience(under 3 years, 3 to under 5 years, 5 years or more). After 15 weeks, the effects of education were compared using a paired t-test between pre-test and post-test. Results: There was a significant difference in the 3 to under 5 years experienced nurses's expertise(t=-3.659, p=.004) between the pre-test and post-test. There was a significant difference in the 5 years or more experienced nurses's expertise(t=-5.781, p<.001) and critical thinking disposition(t=-3.345, p=.003) between the pre-test and post-test. There were no significant differences in clinical decision making(accuracy, proficiency, and confidence) of 3 groups. Conclusion: Reeducation of nursing process is a valuable teaching and an evaluation strategy for 5 years or more experienced nurses to improve expertise and critical thinking disposition. We need to develop continuing education program for improving nurses' clinical decision making.
