상기도기침증후군으로 의심되는 만성기침 환자에 대한 인후부 근위취혈 침치료 및 한약 온열크림 도포 경과 : 증례보고 (Treatment of Chronic Cough in an Upper Airway Cough Syndrome (UACS)-Suspected Patient with Local Acupuncture Points Stimulation and Application of Topical Herbal Mixed Heating Cream : A Case Report)

  • Objectives: This study investigated the effect of local acupuncture point stimulation and the application of a topical herbal mixed heating cream in a chronic cough patient who had not responded to various medications for more than a few months. Methods: An 81 year-old female patient who was suspected to be suffered from Upper Airway Cough Syndrome (UACS) was examined. The patient was treated with local acupuncture point stimulation and the application of a topical herbal mixed heating cream. We used the Leicester Cough Questionnaire, Cough-Specific Quality-of-Life Questionnaire, and Verbal Numerical Rating Scale to assess the patient's respiratory symptoms. Results: Local acupuncture point stimulation and the application of a topical herbal mixed heating cream resulted in the improvement of cough symptoms. The quality of life due to the alleviation of symptoms also significantly improved. Adverse effects were not observed. Conclusions: This study suggests that local acupuncture point stimulation and the application of a topical herbal mixed heating cream may be an effective therapy for the treatment of chronic cough in patients with UACS.

응급실 방문 당시 통증 정도와 computed tomography 기반 충수염 진행 정도와의 상관관계 (Correlation between degree of pain at the emergency room and progression of appendicitis based on computed tomography)

  • Objective: Acute appendicitis is one of the most urgent surgical problems. Several factors have been considered as predictors of perforation, but this study focused on the change in pain pattern. The degree of pain and progression of appendicitis were analyzed assuming that the pain intensity would increase until the perforation and the degree of pain would decrease immediately after the perforation occurred. Methods: In this study, 385 out of 467 patients, who were diagnosed with appendicitis in a single institution and aged between 15 and 65 years, were reviewed retrospectively. The patients' pain scores and the diameters of appendices were analyzed along with the accompanying complications. Correlation analysis and a Student's t-test were performed. Results: In patients with complicated appendicitis, the mean numerical rating scale (NRS) was slightly higher than that of simple appendicitis, but there was no significant difference in the absolute value. Only the size of the appendix showed meaningful differences according to the combined computed tomography findings. The NRS distribution or appendiceal size did not correlate with the time duration from symptom onset. Conclusion: The appendiceal size tended to increase with progressing appendicitis. No significant correlation was observed between the patient's pain level and complications. The time duration from symptom onset did not show a relationship with the progression of appendicitis.

Comparison of Effects of Different Myofascial Meridians Methods on Pain and Postural Control of College Students with Forward Head Posture

  • PURPOSE: This study was conducted to investigate the effects of the myofascial meridians release technique on pain and postural control of college students with forward head posture and to compare the intervention effects of the Grastone massage and the Rollfing massage. METHODS: Thirty subjects with forward head posture were randomly and equally allocated to experimental group I (myofascial meridians release technique using Grastone massage, n=15) or experimental group II (myofascial meridians release using Rollfing massage, n=15). All subjects underwent 30 minutes of different myofascial meridians release techniques in addition to general physical therapy (hot pack: 15 min, interferential current therapy: 15 min, ultra sound: 5 min) three times a week for a total of 6 weeks. Outcome was measured before and after 4 weeks of intervention. RESULTS: In both groups, the numerical rating scale of pain was significantly decreased after intervention, with experimental group I showing a greater decrease than experimental group II. The neck disability index decreased significantly after intervention in both groups, with experimental group I showing a significantly greater decrease than the controls. The distance of the head forward displacement did not differ significantly between the two groups (p>.05). CONCLUSION: This study suggests that the Grastone method combined with the myofascial meridians release technique is a more effective intervention for improvement of pain and postural control in adults with moderate forward head posture.

계지가부자탕(桂枝加附子湯)을 이용하여 호전된 요실금 치험 1례 (A case report of urinary incontinence improved by Gyejigabuja-tang.)

  • Objective : The purpose of this case report is to offer an unprecedented target for Gyejigabuja-tang by analyzing a case in which the symptoms were improved by the herbal decoction therapy, and to accumulate a fundamental knowledge in herbal medicine therapy for urinary incontinence. Method : We studied a patient's chart with the patient's consent and displayed patient's timeline and adherence in a figure according to the CARE guideline. The progress of urination symptoms were assessed with the frequency of urination and how long the patient can stand urine. The accompanied lower limb pain was estimated with Numerical Rating Scale(NRS) Results : The patient's adherence was good during the medication period and symptoms related with urination and accompanied lower limb pain were improved with no specific side effects. NRS of the lower limb pain decreased from 7 to 2, the frequency of urination also improved from over 10 times to 6~8 times per day, and the patient can stand urine from less than 1 second to 2~3 minutes. Conclusions : Gyejigabuja-tang was effective for an urinary incontinence patient which corresponds to the provision No.20 in Shanghanlun.

  • This study was performed to evaluate the effectiveness of pharmacoacupuncture treatment of coccygodynia caused by perforating cutaneous nerve entrapment syndrome. Two patients were diagnosed as coccygodynia caused by perforating cutaneous nerve entrapment syndrome which pain was within the anatomical field of the nerve, worsened by pressure-inducing posture, no objective sensory loss and in presence of pin-point tenderness. They were treated by pharmacoacupuncture at perforating cutaneous nerve region penertrating the sacrotuberous ligament and local tenderness point of coccyx. The evaluation of clinical outcome was done by pain intensity numerical rating scale (PI-NRS), pressure pain threshold (PPT) and EuroQol five dimensions questionnaire (EQ-5D) index. After treatment, their PI-NRS was decreased, PPT and EQ-5D index were increased. The pharmacoacupuncture therapy at entrapment point of perforating cutaneous nerve could be an effective way to treat coccygodynia caused by perforating cutaneous nerve entrapment syndrome.

  • Objectives: The purpose of this case report was to propose Yangshimtang-Gamibang on three patients diagnosed with acute stress disorder. Methods: Yangshimtang-Gamibang was treated on three patients diagnosed with acute stress disorder after experiencing a traffic accident. The effects of Yangshimtang-Gamibang were evaluated through BAI (Beck Anxiety Inventory), NRS (Numerical rating scale), NDI (Neck Disability Index), ODI (Oswestry Disability Index), respectively, and quality of sleep was assessed with total sleep time and number of times waking up during sleep. Results: After the treatment, there was significant decrease in the value of BAI score, a measurement of anxiety, as well as the value of NRS, NDI, ODI, a measure of pain. Also, the sleep quality of three patients improved. Conclusions: The results indicate that Yangshimtang-Gamibang may have an effect on people with acute stress disorder who recently experienced a traffic accident.

A Patient with Ankylosing Spondylitis Treated with Acupotomy and Traditional Korean Medicine

  • This case report relates to a 40-year old male patient diagnosed with ankylosing spondylitis who was treated with acupotomy and traditional Korean medicine. He showed a significant improvement in joint range of motion (ROM) in cervical and lumbar vertebrae, and in pain and functional disorder symptoms. The patient received acupotomy, acupuncture, pharmacopuncture, herbal medicine and physical therapy (November 26, 2018 to December 17, 2018). Joint ROM for cervical and lumbar vertebrae was measured, and the pain level was assessed using a numerical rating scale. The functional disorder and quality of life scales were also assessed using the Bath Ankylosing Spondylitis Functional Index, Bath Ankylosing Spondylitis Disease Activity Index, Korean Health Assessment Questionnaire, and Modified Health Assessment Questionnaire. After applying acupotomy and traditional Korean medicine, the patient exhibited increased joint ROM and reduced pain, also in conjunction with improved responses in functional disorder and quality of life.

A Case of Dural Ectasia with Low Back Pain and Sciatica Treated with Integrative Korean Medical Treatment

  • Dural ectasia is defined as ballooning or expansion of the dural sac surrounding the spinal cord. This report describes a rare case of low back pain and sciatica, suspected as being dural ectasia. The patient was hospitalized for 45 days, and underwent integrative Korean medical treatment, including pharmacopuncture, acupuncture, herbal medicine, Chuna therapy, cupping therapy, and physiotherapy. The effect of the treatment was evaluated using the numerical rating scale, Oswestry disability index, European quality of life 5 dimensions, and subjective symptoms. After inpatient treatment, the pain the patient experienced was significantly reduced and the evaluation indices improved. This case report suggested that integrative Korean medical treatment could be an effective therapeutic choice for low back pain and sciatica, with dural ectasia. Further clinical studies are needed to support this observation.

The effect of needle tip position on the analgesic efficacy of pulsed radiofrequency treatment in patients with chronic lumbar radicular pain: a retrospective observational study

  • Background: Pulsed radiofrequency (PRF) is a treatment modality that alleviates radicular pain by intermittently applying high-frequency currents adjacent to the dorsal root ganglion. There has been no comparative study on analgesic effect according to the position of the needle tip in PRF treatment. The objective of this study is to evaluate the clinical outcomes of PRF according to the needle tip position. Methods: Patients were classified into 2 groups (group IP [group inside of pedicle] and group OP [group outside of pedicle]) based on needle tip position in the anteroposterior view of fluoroscopy. In the anteroposterior view, the needle tip was advanced medially further than the lateral aspect of the corresponding pedicle in group IP; however, in group OP, the needle tip was not advanced. The treatment outcomes and pain scores were evaluated at 4, 8, and 12 weeks after applying PRF. Results: At 4, 8, and 12 weeks, there were no significant differences between the successful response rate and numerical rating scale score ratio. Conclusions: The analgesic efficacy of PRF treatment did not differ with the needle tip position.

The Effect of Kinesiology Taping on Suprahyoid Muscles Activity in Community-Dwelling Elderly People

  • Background: The therapeutic exercise method using kinesiology taping (KT) has been reported to be effective in activating the suprahyoid muscle in healthy adults. However, its applicability and effectiveness are not known to the physically vulnerable elderly. Objectives: To investigate the effects of kinesiology taping on the activity of suprahyoid muscles in community-dwelling elderly people. Design: Repeated measure design. Methods: We enrolled 23 healthy elderly adults (age range 60-75 years) with no history of neurologic disease. Participants performed five consecutive spontaneous swallowing actions at 5-second intervals under three conditions (KT with 80% stretch, placebo-KT, and non-KT. Activation of the suprahyoid muscles during swallowing in the three conditions was measured using surface electromyography (sEMG). In addition, a 0-10 numerical rating self-report scale was used to evaluate the required effort and the resistance felt during swallowing. Results: KT with 80% stretch were significantly higher in sEMG mean value, peak value, required effort, and resistance felt during swallowing compared to other two conditions (P<.05, all). KT with 80% stretch required the most effort during swallowing and, consequently, has a positive effect on increasing suprahyoid muscle activation. Conclusion: Our results could be taken into consideration in therapeutic exercise method for suprahyoid muscle in dysphagia rehabilitation.
