시상, 중뇌, 소뇌경색으로 인한 안검하수, 안구운동장애를 호소하는 환자의 한방 치험 1례 (A Case Report of a Thalamus, Midbrain, and Cerebellum Infarction Patient Suffering from Blepharoptosis and Ocular Motility Disorders Treated by Korean Medicine)

  • Objectives: This is a case report about the effect of Korean medicine on blepharoptosis and ocular motility disorders that occur with thalamus, midbrain, and cerebellum infarctions. Methods: The patient was treated using Korean medicine such as acupuncture, moxibustion, and herbal medicine (Boyanghwano-tang-gamibang). The change of blepharoptosis was evaluated by measurement of palpebral fissure width (PFW), marginal reflex distance-1 (MRD1), and marginal reflex distance-2 (MRD2). The change inocular motility disorders was evaluated by comparison using photographs of the extraocular movements of the patient. We used the Numeric Rating Scale-11 (NRS-11) of diplopia and dizziness to evaluate the effectiveness of treatment. Results: After treatment, blepharoptosis and ocular motility disorders were improved. The NRS-11 score of dizziness decreased from 10 to 7, and diplopia disappeared after Korean medicine treatment. Conclusions: According to this study, Korean medicine can be effective for treating blepharoptosis and ocular motility disorders in thalamus, midbrain, and cerebellum infarctions.

구강 내 균형장치 및 한방치료를 적용한 턱관절장애 환자에 대한 증례보고 (Intraoral Balancing Appliance and Korean Medical Treatment for Patient with Temporomandibular Disorder: A Case Report)

  • Objectives: The aim of our study was to report clinical improvement of a patient who suffered from Temporomandibular Disorder (TMD) being treated with Korean Medical treatments and Intraoral Balancing Appliance (IBA). Methods: During the admission period, the patient was treated with acupuncture, cupping, Korean Medicine, Chuna therapy and FCST (Functional Cerebrospinal Therapy) every day. And we observed patient's condition by Numeric Rating Scale (NRS), Pain Disability Index (PDI), EQ-5D-5L (Five-level EuroQol-5 dimensions), and assessment about range of motion(ROM) for temporomandibular joint and cervix. Results: After treated for 4 weeks, the patient's NRS, PDI, EQ-5D-5L and ROM were improved. Conclusions: Korean Medical treatments including FCST are estimated to be effective for patients with TMD. But the case was only one, so more cases and further research is needed to prove the effectiveness of the treatment.

요추 추간판 탈출증 환자에 대한 도침을 포함한 한방 복합치료 효과: 연속증례보고 (The Effect of Acupotomy on Lumbar Herniated Intervertebral Disc: Report of a Case Series)

  • Objectives : This is a case series reporting of the improvement in patients with a lumbar herniated intervertebral disc by means of acupotomy combined with oriental medicine. Methods : We treated seven patients with a lumbar herniated intervertebral disc with acupotomy combined with oriental medical treatment. We checked the numeric rating scale(NRS), oswestry low back pain disability index(ODI) and range of motion(ROM). Results : NRS and ODI decreased, and ROM improved in all cases. Conclusions : This study shows acupotomy has a meaningful effect when applied on a lumbar herniated intervertebral disc.

  • Objectives : The purpose of this study is to report 4 cases of shoulder pain treated by Ligamentum Pharmacopuncture. Methods : This study on shoulder pain was carried out on 4 patients who were treated at Department of Acupuncture & Moxibusition, Dongshin University, Korean Medicine Hospital from May 2016 to July 2016. All patients were treated 2~3 times based on the patient's condition. To evaluate change of pain and function, we used Shoulder Pain and Disability Index(SPADI), Numeric Rating Scale(NRS) and Range Of Motion(ROM). Evaluations were made at baseline and one week and two weeks after treatment. Results : There was remarkable improvement in SPADI, NRS and ROM. Conclusions : This study suggested the possibility that the ligamentum pharmacopuncture therapy may be useful for the treatment of shoulder pain. Follow-up study is needed.

Piriformis Syndrome in Knee Osteoarthritis Patients after Wearing Rocker Bottom Shoes

  • Background: Rocker bottom shoes (RBS) are popular among patients with different foot, leg, or back problems in Korea. Patients with knee osteoarthritis concurrent weakness in the quadriceps femoris muscle, who wear these shoes, are often assumed to develop piriformis syndrome (PS). This study was performed to improve the understanding about the effect of wearing such shoes on duration of the syndrome in knee osteoarthritis. Methods: We randomly assigned 150 patients with PS, who had used RBS daily for at least 6 months, to 2 groups, the S (stopped wearing) and K (kept wearing) groups. Both the groups were subdivided into the O and N groups, comprising patients with and without knee osteoarthritis, respectively. The effects of the treatment, including piriformis muscle injections and a home exercise program, were compared between the 2 groups by using a flexion-adduction-internal rotation (FAIR) test, a numeric rating scale (NRS), and the revised Oswestry disability index (ODI) during the 12-week follow-up. Results: The positive FAIR test ratios, mean NRS scores, and revised ODIs were higher in the KO group than the SN group from 4-12 weeks after treatment. Conclusions: RBS may extend duration of the PS in osteoarthritis patients.

재발성 족외과 점액낭염에 대한 도침요법: 증례 보고 (Effects of Acupotomy Treatment on Relapsed Lateral Malleolar Bursitis: A Case Report)

  • Objectives : This study details on a case of acupotomy for relapsed lateral malleolar bursitis. Methods : A 71-year-old woman with lateral malleolar bursitis was treated with acupotomy twice in an outpatient setting. A sterilized, disposable, blade width 0.5 mm × needle length 50 mm sized acupotomy was administered on GB40 and BL62 on each visit. Pain intensity assessment using Numeric Rating Scale (NRS) and the response evaluation criteria was conducted right after each treatment. Results : Pain on NRS declined from 5 to less than 1 after twice of acupotomy treatment. This result was regarded as 'Complete Response' using the response evaluation criteria. There were no side effects during whole treatment procedure at all. Conclusions : It is turned out that acupotomy can have a positive clinical effect on lateral malleolar bursitis in this case without any adverse effect. Further controlled studies on acupotomy for lateral malleolar bursitis are required for clinical application.

An in vivo electromyographic evaluation of pain relief using different therapies in masticatory myalgia patients

  • Objectives: This study is aimed to evaluate and compare the effect of moist heat fomentation therapy with ultrasound therapy in patients with the masticatory myalgia. Materials and Methods: The study was conducted on 42 patients with masticatory myalgia, dividing them into two groups; Group A (21 patients), received moist heat therapy and Group B (21 patients), received ultrasound therapy for seven effective days. Prior and after the treatment the numeric rating scale (NRS) and the electromyography (EMG) scores were recorded and compared. The observations were analyzed clinically and statistical support was taken to assess the NRS and EMG data. Results: Irrespective of the groups, patients testified a significant reduction in pain after the treatment. From the EMG readings; even though the standard deviation for each group was varied considerably, EMG recorded an improved muscle activity. Statistical analysis was used to assess and identify the best treatment methodology between the two modalities. Conclusion: From the statistical analysis, it is concluded that, though both the therapies had significantly reduced the symptomatic response, it is moist heat fomentation that improved muscle activity both statistically and clinically in comparison to ultrasound.

Physical Therapy for Esophageal Cancer Patient With Long Thoracic Neuropathy After Esophagectomy: A Case Report

  • Esophageal cancer is a representative cancer that occur physical deterioration but, physical problems after surgery were not well reported. The purpose of this study is to report on the long thoracic neuropathy after surgery, and to identify the symptoms and effects of physical therapy after esophageal cancer surgery. This is a case of a 61-year-old man who showed winging of the scapula with long thoracic nerve injury on the results of electromyography after an esophageal cancer surgery. Physical therapy programs were implemented 8 sessions during hospitalization. The quality of life, fatigue, shoulder range of motion (ROM), numeric rating scale (NRS), 6-minute walk test, and 30-second chair stand test were assessed. The quality of life, fatigue, shoulder ROM, NRS (pain), 6-minute walk test, and 30-second chair stand test were improved. However, the esophageal-specific symptom was not different after physical therapy program. As esophageal cancer suffers from physical difficulties after surgery, physical therapy programs are thought to be helpful.

성형 수술 후 부종에서 호박탕(琥珀湯)의 효과 : 2 증례보고 (Effect of Succium Decoction on Edema after Plastic Surgery: Two Case Report)

  • Objectives : In traditional Korean medicine literature, Succium(琥珀) is known to be effective in eliminating edema. However, pumpkins are misused in folk remedies to manage edema, because succium and pumpkin is homophony to 'Hobak' in Korean. The purpose of this study is to report the effects of Succium decoction on two patients with edema after plastic surgery. Methods : We treated two patients with edema after plastic surgery using Succium decoction. Numeric rating scale (NRS) of edema, photograph of changes in edema before and after treatment were analyzed to evaluate the effects of treatment. Results : After 2 weeks of administration, NRS for facial edema decreased from 5 to 3 and 2 to 1, respectively. Photograph of edema decreased after treatment, and there were no adverse events. Conclusions : This case report suggests that Succium decoction can be effective in treating edema after plastic surgery.

요관결석 쇄석술 후 심한 혈뇨 소견을 보인 환자 임상증례 (A Clinical Case Report of Severe Hematuria Patient after Ureterolith Lithotripsy)

  • Objectives : The purpose of this study is to report the improvement after the Korean medical treatment about a severe hematuria patient after ureterolith lithotripsy. Methods : This study was carried out on 60 year-old female patient who suffered from severe hematuria. We diagnosed a severe hematuria patient after ureterolith lithotripsy as kidney deficiency and blood deficiency pattern in the symptom-differentiation system of Korean medicine and applied herbal medicine, acupuncture and moxibustion to the patient. Results : After Korean medical treatment, we observed improvement of the symptom process by Numeric Rating Scale (NRS), hematologic findings and urinalysis. Blood count of blood and urine had been in the normal range, and NRS of three symptoms had dropped below 0.5. Conclusions : We concluded that Korean medical treatment was an effective treatment for a hematuria patient after ureterolith lithotripsy.
