Application of Bovine Viral Diarrhoea Virus as an Internal Control in Nucleic Acid Amplification Tests for Hepatitis C Virus RNA in Plasma-Derived Products

  • Plasma-derived products are produced from plasma via fractionation and chromatography techniques, but can also be produced by other methods. In the performance of nucleic acid amplification tests (NAT) with plasma-derived products, it is necessary to include an internal control for the monitoring of all procedures. In order to avoid false negative results, we confirmed the usefulness of the bovine viral diarrhoea virus (BVDV) for use as an internal control in the detection of hepatitis C virus (HCV) RNA in plasma-derived products. These products, which were spiked with BVDV, were extracted and then NAT was performed. Specificity and sensitivity were determined via the adjustment of primer concentrations and annealing temperatures. BVDV detection allows for validation in the extraction, reverse transcription, and amplification techniques used for HCV detection in plasma-derived products.

Recent Advances in Tuberculosis and Nontuberculous Mycobacteria Lung Disease

  • Tuberculosis (TB) is one of the largest health problems in the world today. And the incidence of nontuberculous mycobacteria (NTM) lung disease appears to be increasing worldwide. Recently, an automated, nucleic acid amplification assay for the rapid detection of both Mycobacterium tuberculosis and rifampin resistance was developed (Xpert MTB/RIF). And fixed-dose combinations of anti-TB drugs and linezolid have been introduced in the treatment of TB. And new NTM species, named Mycobacterium massiliense, which is very closely related to Mycobacterium abscessus was reported. In this review, these recent advances in the diagnosis and treatment of TB and clinical characteristics of M. massiliense lung disease are discussed.

식물체에 감염성 질병을 유발하는 바이로이드 검출 및 진단 방법 (The Detection and Diagnosis Methods of Infectious Viroids caused Plant Diseases)

  • 바이로이드는 매우 작은 RNA 분자로 구성되어 있으며, 외피 단백질이 없고 오로지 식물에만 감염되어 질병을 유발한다. 바이로이드 감염 질병을 예방하거나 진단하는 것은 상당히 어려운 일이며, 이는 병징이 초기에는 발견되지 않고 수확기에 접어들어서 발견되기 때문이다. 한편, 혈청학적인 방법은 식물 병원체를 검출하기 위해 주로 사용되었으나 바이로이드는 핵산인 RNA로만 구성되어 있기 때문에 이 방법으로 검출할 수가 없다. 때문에 바이로이드를 검출하기 위해 주로 사용되는 방법은 분자 생물학적인 방법으로, 초기에는 바이로이드의 분자적인 크기와 구조적 특징을 이용한 겔 전기 영동 방법이 주로 사용되었다. 그 후에는 역전사 반응과 중합효소 연쇄반응을 접목시킨 역전사 중합효소 연쇄반응(RT-PCR) 방법이 활용되었고, 그에 대한 효율적인 결과 확인을 위해 형광 물질을 도입한 실시간 역전사 중합효소 연쇄반응(Real-time RT-PCR)이 도입되었다. 그러나 그들은 온도를 변화시키기 위한 값비싼 기기와 전문적인 인력이 필요함으로 현장에서는 활용되기가 어렵다. 최근 개발된 고리 기반의 등온 증폭법(Loop-mediated isothermal amplification)의 경우, 온도의 변화가 필요 없어 비싼 온도 조절 기기가 필요하지 않다. 또한 매우 높은 증폭 효율을 지니며 반응 시간이 짧은 등의 여러 장점을 지니고 있기에 최근 현장 진단용 기술에 도입되고 있다. 이러한 배경으로, 이 총설에서는 바이로이드 유발 질병에 대하여 요약하고 그에 대한 검출 및 진단 방법에 대한 연구 동향에 대하여 기술하였다.

등온증폭법을 이용한 고감도 JC polyomaviruses 진단법 개발 (Diagnostic Method for the Detection of JC Polyomavirus Using Loop-mediated Isothermal Amplification)

  • JC polyomavirus (JCPyV)는 viral group I, Polyomaviridae로 분류되는 사람 병원성 바이러스 이다. JCPyV는 최근 하수 등 수질오염 지표로 제안됨에 따라 수질환경에서 모니터링 필요성이 제기되고 있다. 임상 및 환경 시료 중 JCPyV 검출을 위해서 PCR 시스템이 사용되어 왔다. 그러나 신속하고 높은 검출 민감도를 가진 방법의 필요성에 따라 이번 연구에서는 등온증폭 프라이머 조합을 개발하였다. 이번에 개발한 방법은 기존 PCR 시스템에 비해 더욱 신속 및 약 10배 더 높은 검출 민감도를 보여주었다. 또한 본 연구에서 개발한 검출 방법의 재 검정을 위해서 HaeIII 제한효소를 이용한 방법을 함께 고안하였다. 따라서 이번 연구는 임상, 환경 등의 시료에서 JCPyV를 모니터링 하기 위한 방법으로 활용이 기대된다.

Real-time Nucleic Acid Sequence Based Amplification (Real-time NASBA) for Detection of Norovirus

  • Noroviruses (noroV) are the major cause of nonbacterial gastroenteritis in humans worldwide. Since noroV cannot yet be cultured in vitro and their diagnosis by electron microscopy requires at least $10^6$ viral particles/g of stool a variety of molecular detection techniques represent an important step towards the detection of noroV. In the present study, we have applied real-time nucleic acid sequence-based amplification (real-time NASBA) for simultaneous detection of NoroV genogroup I (GI) and genogroup II (GII) using standard viral RNA. For real-time NASBA assay which can detected noroV GI and GII, a selective region of the genes encoding the capsid protein was used to design primers and genotype-specific molecular beacon probes. The specificity of the real-time NASBA using newly designed primers and probes were confirmed using standard viral RNA of noroV GI and GII. To determine the sensitivity of this assay, serial 10-fold dilutions of standard viral RNA of noroV GI and GII were used for reverse transcription polymerase chain reaction (RT-PCR) and real-time NASBA. The results showed that while agarose gel electrophoresis could detect RT-PCR products with 10 pg of standard viral RNA, the real-time NASBA assay could detect 100 fg of standard viral RNA. These results suggested that the real-time NASBA assay has much higher sensitivity than conventional RT-PCR assay. This assay was expected that might detect the viral RNA in the specimens which could have been false negative by RT-PCR. There were needed to perform real-time NASBA with clinical specimens for evaluating accurate sensitivity and specificity of this assay.

Changes in Diagnostic Methods for Pulmonary Tuberculosis between 2005 and 2013

  • Background: Diagnostic methods for pulmonary tuberculosis (TB) have recently advanced. The aim of this study was to evaluate the changes in TB diagnostic tests that prompted the initiation of anti-TB treatment over time in South Korea, an industrialized country with an intermediate TB burden. Methods: Patients diagnosed with pulmonary TB in the first halves of 2005 and 2013 at a tertiary referral hospital were included. Diagnostic methods that prompted the initiation of anti-TB treatment were compared between the 2 groups of patients. Results: A greater proportion of patients were diagnosed with pulmonary TB using bronchoscopy in 2013 than in 2005 (26.7% vs. 6.6%, respectively; p<0.001), while the proportion of patients clinically diagnosed with pulmonary TB was lower in 2013 than in 2005 (24.7% vs. 49.0%, respectively; p<0.001). Additionally, more patients started anti-TB treatment based on positive polymerase chain reaction (PCR) results for Mycobacterium tuberculosis DNA in 2013 than in 2005 (47.3% vs. 7.9%, respectively; p<0.001). Conclusion: The initiation of treatment for pulmonary TB in South Korea has become more frequently based on PCR and the use of bronchoscopic specimens.
