Effect of Ginseng Saponins on Nicotine-Induced Dopamine Release in the Rat Nucleus Accumbens and Striatum (인삼 사포닌이 흰쥐 측핵과 선조체에서 니코틴에 의한 도파민 유리에 미치는 효과)

  • Purpose and Methods: We investigated the effect of ginseng total saponin (GTS) on nicotine-induced dopamine (DA) release in the striatum and nucleus accumbens of freely moving rats using in vivo microdialysis technique. Results: Systemic pretreatment with GTS decreased striatal DA release induced by local infusion of nicotine into the striatum. However, GTS had no effect on the resting levels of extracellular DA in the striatum. GTS also blocked nicotine-induced DA release in the nucleus accumbens. Conclusion: The results of the present study suggest that GTS acts on the DA terminals to prevent DA release induced by nicotine. This may reflect the blocking effect of GTS on behavioral hyperactivity induced by psychostimulants.

Clinical Application of $^{99m}Tc-DISIDA$ Scintigraphy with Nonvisualization of Biliary Excretion (담도계로 배설되지 않는 $^{99m}Tc-DISIDA$ 신티그램의 임상적 응용)

  • Authors analysed biochemical studies and scintigraphic findings of obstructive jaundice and nonobstructive jaundice in 44 cases of $^{99m}Tc-DISIDA$ scintigraphy with nonvisualization of biliary excretion till 120 min or 240 min after injection of $^{99m}Tc-DISIDA$. Causative diseases of $^{99m}Tc-DISIDA$ scintigraphy with nonvisualization of biliary excretion were in order to choledocholithiasis (25%), hepatitis (25%), cholangiocarcinoma (14%), cholangitis (14%) and pancreas head tumor (11%). In obstructive jaundice, statistically significant findings were elevated alkaline phosphatase above 300 IU/L on biochemical study and single lobe enlargement of the liver, irregular radioisotope uptake of the liver and concave indentation of the gall bladder fossa of the liver on scintigraphy. In nonobstructive jaundice, statistically significant findings were persistent renal excretion of $^{99m}Tc-DISIDA$ and more increased uptake density of the heart than the liver on scintigraphy.

  • In this study, we developed three dimensional FDG brain PET and MRI coregistration technique. The boundaries of the head in PET and MRI were segmented using sinogram of emission PET scan and T1-weighted MRI. We registered both boundaries by minimizing the mean Euclidean distance of those. To display the registered PET and MRI simultaneously, we used weighted normalization method and interleaving method.

Usefulness of $^{99m}Tc$-HMPAO SPECT in Patients with Subarachnoid Hemorrhage due to Ruptured Intracranial Aneurysm (뇌동맥류파열에 의한 지주막하출혈 환자에서 $^{99m}Tc$-HMPAO SPECT 검사의 유용성)

  • We evaluated the usefulness of $^{99m}Tc$-HMPAO SPECT in 21 Patients with subarachnoid hemorrhage due to ruptured intracranial aneurysm and in 3 patients with unruptured intracranial aneurysm. SPECT study could detect the bilaterally hypoperfused cases in 10 patients (48%), but CT/MRI showed the bilateral abnormalities in only 3 patients (14%). The number of abnormal lesions were 56 in SPECT and 25 in CT/MRI. The lesions found in SPECT were well correlated with the neurological signs of the patients such as aphasia or hemiplegia. SPECT study during Matas test was helpful in evaluating the risk for carotid artery occlusion therapy. We thought that $^{99m}Tc$-HMPAO brain SPECT is helpful in evaluating the functional changes in patients with subarachnoid hemorrhage.

SPATIAL AND ENERGY RESOLUTIONS OF A HEXAGONAL ANIMAL PET SCANNER BASED ON LGSO CRYSTAL AND FLAT-PANEL PMT

  • The aim of this study was to explore the spatial and energy resolutions of a PET scanner that we have recently developed. The scanner, which consists of six detector modules with 1-layer LGSO crystals, has a hexagonal configuration with a faceto- face distance of 86.4 mm between two opposite PET modules; such properties facilitate the imaging of small animals. A $^{22}Na$ point source was employed to estimate horizontal and vertical spatial resolutions. To assess the energy resolution, a uniform $^{18}F$ cylindrical phantom was scanned. A software-based spectrum analysis of list-mode data was used to assign a local energy window centered on the photopeak position for every single crystal. For the image reconstruction, an ML-EM algorithm was used. The spatial resolutions at the center of the scanner were 0.99 mm in the horizontal direction and 1.13 mm in the vertical direction. The energy resolution averaged over each PMT ranged from 13.3%-14.3%, which gave an average value of 13.8%. These results show that this simple system is promising for small animal imaging with excellent spatial and energy resolutions.

A Systematic Review of MRI, Scintigraphy, FDG-PET and PET/CT for Diagnosis of Multiple Myeloma Related Bone Disease - Which is Best?

  • Aim: The purpose of the current study was to conduct a systematic review of the published literature to evaluate the diagnostic accuracy of FDG-PET, PTE/CT, MRI and scintigraphy for multiple myeloma related bone disease. Methods: Through a search of PubMed, EMBASE, and the Cochrane Library, two reviewers independently assessed the methodological quality of each study. We estimated pooled sensitivity, specificity, positive and negative likelihood ratios (PLR and NLR), and two sample Z-tests were conducted to evaluate for differences in sensitivity, specificity, area under the curve (AUC), and the $Q^*$ index between any two diagnostic modalities. Results: A total of 17 studies were reviewed. The MRI had a pooled sensitivity of 0.88, specificity of 0.68, AUC of 0.897, and $Q^*$ index of 0.828, whereas for MIBI, the corresponding values were 0.98, 0.90, 0.991, and 0.962, respectively, and for bone scan, they were 066, 0.83, 0.805, and 0.740, respectively. The corresponding values of MIBI were 0.98, 0.90, 0.991, and 0.962, respectively. For PET and PET/CT, the values were 0.91, 0.69, 0.927 and 0.861, respectively. Statistically significant differences were not found in the sensitivity, specificity, AUC, and $Q^*$ index between MRI, scintigraphy, FDG-PET and PET/CT. Conclusions: On the condition that X ray is taken as a reference in our study, we suggested that FDG-PET, PTE/CT, MRI and scintigraphy are all associated with high detection rate of bone disease in patients with MM. Thus, in clinical practice, it is recommended that we could choose these tests according to the condition of the patient.

Solution to Decrease Spatial Dose Rate in Laboratory of Nuclear Medicine through System Improvement (시스템 개선을 통한 핵의학 검사실의 공간 선량률 감소방안)

  • Objectives: This study aims at decreasing spatial dose rate through work improvement whilst spatial dose rate is the cause of increasing personal exposure dose which occurs in the process of handling radioisotope. Methods: From February 2013 until July 2013, divided into "before" and "after" the improvement, spatial dose rate in laboratory of nuclear medicine was measured in gamma image room, PET/CT-1 image room, and PET/CT-2 image room as its locations. The measurement time was 08:00, 12:00 and 17:00, and SPSS 21.0 USA was opted for its statistical analysis. Result: The spatial dose rate at distribution worktable, injection table, the entrance to the distribution room, and radioisotope storage box, which had showed high spatial dose rate, decreased by more than 43.7% a monthly average. The distribution worktable, that had showed the highest spatial dose rate in PET/CT-1 image room, dropped the rate to 42.3% as of July. The injection table and distribution worktable in the PET/CT-2 image room also showed the decline of spatial dose rate to 89% and 64.4%, respectively. Conclusion: By improving distribution process and introducing proper radiation shielding material, we were able to drop the spatial dose rate substantially at distribution worktable, injection table, and nuclide storage box. However, taking into account of steadily increasing amount of radioisotope used, strengthening radiation related regulations, and safe utilization of radioisotope, the process of system improvement needs to be maintained through continuous monitoring.

The Importance of Filter Integrity Test to Ensure Sterility of Radiophamaceuticals for Using PET Image

  • The radiopharmaceuticals are routinely injected to blood vessel for acquiring PET image. For this reason, It is imperative that they undergo strict quality control measures. Especially, Sterility test is more important than any other quality control procedures. According to the FDA guideline, It requires filter integrity test used in the processing of sterile solutions. Among several methods, we can decide to use bubble point test. We usually use vented GS-filters (Millipore co., USA) which are sterilizinggrade (0.22 um pore size) and are placed upper site on product vial. After the synthesis of $^{18}F$-FDG, solutions wet the membrane in filter and then go into the product vial. By all synthesis steps have finished, we can observe the presence of the bubbles in the product vial. Since we have started this study, we have never found any bubbles in the product vial. Because the maximum pressure intensity of the filter which has set by manufacturer is up to 5 bars, but helium gas pressure is up to 1 bar in our module system. So, we can make 5 bars pressure using helium gas bombe and increase pressure up to 5 bars step by step. However, it does not happen to anything in vial.

Investigation of a blind-deconvolution framework after noise reduction using a gamma camera in nuclear medicine imaging

  • A gamma camera system using radionuclide has a functional imaging technique and is frequently used in the field of nuclear medicine. In the gamma camera, it is extremely important to improve the image quality to ensure accurate detection of diseases. In this study, we designed a blind-deconvolution framework after a noise-reduction algorithm based on a non-local mean, which has been shown to outperform conventional methodologies with regard to the gamma camera system. For this purpose, we performed a simulation using the Monte Carlo method and conducted an experiment. The image performance was evaluated by visual assessment and according to the intensity profile, and a quantitative evaluation using a normalized noise-power spectrum was performed on the acquired image and the blind-deconvolution image after noise reduction. The result indicates an improvement in image performance for gamma camera images when our proposed algorithm is used.
