  • Objectives : In order to distinguish morphological characteristics of trunk bark and root bark of Ulmus davidiana var. japonica (Rehder) Nakai and the trunk bark and root bark of Hemiptelea davidii Planchon were sampled and compared in terms of their external and internal features with flour states according to their medical use, through microscopic examination. Methods : The slice of the tested material made by paraffin section technique was colored with Safranine Malachite Green contrast methods, and the flour of it was mounted by the liquid made by the same ratio of each of glycerin, acetic acid, and water, and then observed and photographed by olymphus-BHT. Results : 1. Internal Features 1) A large parenchymatous cell was observed in the phloem of the slice of both trunk bark and root bark of Ulmi Cortex, However, both of the trunk bark and root bark of Hemipteleae Cortex did not have parenchymatous cell in the phloem; instead, stone cells including much square crystal of calcium oxalate were distributed around fiber bundle, and the parenchymatous cell included much druse crystal of calcium oxalate. 2) In both the Ulmi Cortex and Hemipteleae Cortex, rhytidome was observed in trunk bark, but not in root bark, but in the parenchymatous cell of the root bark of the Ulmi Cortex contained starch grain. 2. Flour States 1) In the flour of root bark of the Ulmi Cortex, a large parenchymatous cell was observed. However, in the flour of trunk bark and root bark of Hemipteleae Cortex, no parenchymatous eel was found; instead, stone cell including square crystal of calcium oxalate and druse crystal of calcium oxalate were observed. 2) There was no remarkable difference between the trunk bark and root bark of Hemipteleae Cortex. However, starch grain was contained in the parenchymatous cell of the root bark of Ulmi Cortex but not in the trunk bark of it. Conclusions : There were some morphological differences in external, internal, and flour parts of Ulmi Cortex and Hemipteleae Cortex. In particular, there was a morphological difference in flour states between the trunk bark and root bark of Ulmi Cortex, it is possible to use microscope to distinguish their flour states.

  • Objectives The purpose of this study is to investigate recent clinical studies on the effect of herbal medicine for pediatric epistaxis in China, and to seek better methods to treat and study for pediatric epistaxis in Republic of Korea. Methods We searched the clinical studies from the China Academic Journal (CAJ) in China National Knowledge Infrastructure (CNKI) by key words 鼻出血', '鼻衄', '衄血', '中藥', '中医藥', '本草', '湯', '丸', '散', '方','顆粒', '膠囊', '自擬', '外用', '膏 from January 2000 to November 2019. We analyzed the literature in regards to the treatment methods and results. Results Among the 1141 searched studies, 12 randomized controlled trials were selected and analyzed. In most of the studies, the effectiveness of herbal medicine on pediatric epistaxis was significantly high. The most commonly used herbs for oral administration were Rehmanniae Radix (生地黃), Scutellariae Radix (黃芩), Imperatae Rhizoma (白茅根). The most commonly used herbs for external applications were Notoginseng Radix (三七), Bletilla striata (白芨), Crinis Carbonisatus (血餘炭). Conclusions Herbal medicine treatment can help improve pediatric epistaxis. Additional studies need to be performed to solidify these findings. In addition to that, unified criteria for treatment of pediatric epistaxis should be established in Republic of Korea.

  • Objectives: This was a double blinded, randomized, placebo-controlled clinical study for evaluation of safety and effective dose finding of Cardiotonic Pills$^{(R)}$ in patients with chest pain and discomfort. Cardiotonic Pills$^{(R)}$ are composed of Salviae Miltiorrhizae Radix (丹蔘), Notoginseng Radix (三七根) and Borneolum (龍腦). Major effects of Salviae Miltiorrhizae Radix and Notoginseng Radix are vasodilatation, sedation and analgesic action. Borneolum has an antibacterial effect, and can stimulate the central nervous system. All of these substances are oriental herbs that have been used for a long time in east Asia. Cardiotonic Pills fi received Investigational New Drug (IND) approval from the Food and Drug Administration (FDA) in the USA and 40 million people in the world take this pill. We performed a phase IV clinical study to confirm its efficacy and safety in patients who have probable cardiogenic or psychogenic chest pain or chest stifling. Methods: This study was planned for a multi-center clinical trial including four university hospitals of oriental medicine in Korea. This was the first time to evaluate the 'planning treatment according to diagnosis (辨證施治)' of chest pain or chest discomfort according to oriental medical guidelines. The patients who were included in this trial were adult volunteers from 20 to 70 years old who had chest pain or chest discomfort more than twice during a recent month, and we received written consent to participate in this study from all of them. After administration of Cardiotonic Pills$^{(R)}$ for 8 weeks, number of occurrences, duration, appearance and degree of chest pain or chest discomfort was observed and degree of symptoms (severity of illness, global improvement) were measured using a patient's global assessment composite scale. Results: In the patient's global assessment scale, the severity of illness of the Cardiotonic Pills$^{(R)}$ group (n=25) was 14/25=0.56 but of the placebo group (n=25) was 7/25=0.28 (p-value=0.0449). This result indicates Cardiotonic Pills$^{(R)}$have a positive effect on the symptoms of chest pain and discomfort. However, the global improvement of the Cardiotonic Pills$^{(R)}$group was 23/25=0.92, and of the placebo group was 22/25=0.88 (p-value=0.6374). The total symptom score of the Cardiotonic Pills$^{(R)}$ group was $1.68\pm20.06$, and of the placebo group was $16.76\pm72.l4$(p-value=0.2285). The number of symptom events of the Cardiotonic Pills$^{(R)}$ group was $72\pm29.78$, and of the placebo group (n=25) was $10.80\pm38.42$ (pvalue=0.3660). We could not find any effects on the other factors examined besides the severity of illness, beyond the difference of standard deviations. Conclusions: Cardiotonic Pills$^{(R)}$ significantly reduced chest pain and chest discomfort in patients. Therefore, we expect that Cardiotonic Pills$^{(R)}$ will be helpful for patients with chest pain and chest discomfort not only caused by heart disease but also by other diseases.

  • This study was performed to investigate the antithrombotic activity and protective effect of hexane fraction of Kamihyulbuchukeotang (KHCTH) on brain injury by KCN and MCA occlusion a prescription of HCT added with Lumbricus and Notoginseng Radix. Experiemental parameters are brain ischemia by MCA occlusion assay, KCN-induced brain injury, pulmonary thrombosis and platelet aggregation assay. The results were summarized as follows; 1. KHCTH extracts significantly inhibited the duration of KCN-induced coma (67%) and mortality (80%). 2. KHCTH extracts significantly suppressed brain ischemic area and edema following MCA occlusion and protected neuron cells as compared with control data. 3. KHCTH extracts inhibited pulmonary thrombosis induced by collagen and epinephrine. 4. KHCTH extracts inhibited platelet aggregation induced by collagen, ADP as agonist up to 76.9% and 32.3% respectivey at 1 mg/ml more effective than water extract of KHCT These data suggested that KHCTH could be applied as the protector of brain ischemia and injury and antithrombotic agent.
