Recurrent Chondroid Syringoma of the Alar Rim

  • Chondroid synringoma (CS), pleomorphic adenoma of skin, is a benign tumor found in the head and neck region. CS was first reported in 1859 by Billorth for the salivary gland tumor. The usual presentation is an slowly growing, asymptomatic mass. A 53-year-old female with a history of chondroid synringoma had presented with multiple firm, nodular masses found in the left nostril area. The lesion had been excised 8 years prior and was diagnosed histopathologically, but had gradually recurred. Excision of the mass located in subcutaneous layer revealed four whitish, firm tumors surrounded with capsular tissue. Neither recurrence nor complications occurred during the 18 months follow-up period. In the head and neck region, chondroid syringoma should always be considered in differential diagnosis of soft tissue masses despite its rare incidence. For that reason, excisional biopsy with clear margin is the optimal diagnostic as well as therapeutic choice. We report a case of recurred chondroid syringoma on the nose in female patient.

Endoscopic Balloon Dilatation of Nasopharyngeal Stenosis in a Dog

  • A four-year old, intact male, mixed-breed dog had a history of chronic snoring sound and dyspnea. Based on the results of computed tomography, the patient was diagnosed as nasopharyngeal stenosis. Balloon dilatation in the area of stenosis was performed using a balloon dilator inserted retrograde fashion through the working channel of an endoscope. Clinical signs were relieved but reappeared after 3 weeks. Thirty-four days following the first balloon dilatation treatment, a second procedure was performed; the balloon catheter was inserted in antegrade fashion through the left nostril and was filled with contrast agent under fluoroscopic guidance. Because of the relapse of clinical signs after 6 months, the patient received additional balloon dilatation procedure by the antegrade approach. At the one-year follow-up, the owner said that the clinical signs of nasopharyngeal stenosis had been completely resolved. Balloon dilatation could be a minimally invasive and effective treatment for nasopharyngeal stenosis, although the repetition of the procedure may be required.

Nasal septum angiofibroma: a rare condition with an unusual onset

  • The characteristics of extra-nasopharyngeal angiofibromas tend to be different from angiofibromas of the nasopharynx according to patient gender, patient age, prevalence, affected site, pathogenesis, and clinical and epidemiological features. We report a case of an extra-nasopharyngeal angiofibroma in a 28-year-old man referred to the ENT Clinic for right-sided epistaxis, airflow impairment and nasal swelling. The right nostril was completely occluded works by a reddish-yellow mass that bled easily. The computed tomography scan revealed an "inhomogeneous solid lesion in the nasal fossa". With the patient under general anesthesia, the formation in the anterior portion of the right side of the nasal septum was removed up to its vascular base. Although electrical cauterization efficiently controlled the bleeding, we abraded the sub-perichondral area to prevent further bleeding as well as recurrence. The histological exam report confirmed the diagnosis of angiofibroma. As in our case, epistaxis is commonly the presenting sign of angiofibroma. Yet its onset was peculiar, given that the bleeding started with a low impact trauma. The nasal swelling was also a relevant feature as well as the breathing impairment. Although uncommon, nasal septal angiofibromas should considered in patients with epistaxis.

A Morphorlogical Study of Ear, Eye, Nose and Mouth according to the Sasang Constitution (사상인(四象人) 이목비구(耳目鼻口)의 형태학적(形態學的) 특징(特徵) 연구(硏究))

  • Objective This research is a study about constitutional diagnosis through the external appearances as a basic principle, and it is for finding shape differences of the ear, eye, nose, mouth according to the Sasang constitution Method We have collected 209 cases of patients of the Sasang Constitutional Department, including employees of the Kyung-Hee Medical Center and took pictures of the frontal view, lateral view, oblique view of face and measured heights, deapth, breadths of ear, eye, nose, mouth with 'The Measurement of R. Martin'. We analyzed shape differences of the face according to the Sasang constitution with certain results Results We got the morphologic characteristics of ear, eye, nose and mouth according to the Sasang constitution as Table 3. -Table 10. Conclusion : 1. The morphologic characteristics of Ear according to the Sasang constitution (1) Morphologic ear length, Physiognomic ear length, Ear lobule length is longer in Taeumin than Soeumin. (2) Physiognomic ear breadth is wider in Taeumin than Soeumin. (3) Physiognomic ear length, lobule length ratio is higher in Taeumin than Soyangin. 2. The morphologic characteristics of Eye according to the Sasang constitution. (1) Inner Palpebral fissure width, 5th Palpebral fissure length, Bizygomatic breadth-Outercanthal distance is the longest in Taeumin (2) Palpebral fissure inclination is widest in Soeumin. (3) Palpebral fissure length is longer in Taeumin than Soeumin. (4) Pupillary diameter ratio is the lowest in Taeumin (5) Palpebral fissure length, width ratio is higher in Soeumin than Taeumin. (6)zygomatic breadth, Bizygomatic breadth-Outercanthal distance ratio is the higher in Taeumin than Soeumin. 3. The morphologic characteristics of Nose according to the Sasang constitution. (1) Nasion depth is deepest in Soyangin. (2) Nasion to pupillary depth is deeper Soyangin than Taeumin. (3) Nasal tip depth, Nostril to Nasalalar depth is deeper Soyangin than Taeumin. (4) Subnasale to Nasalalar depth is the shallowest in Taeumin (5) Nasalalar height is lowest in Soeumin. (6) Nasalalar to Nostril distance is deeper Taeumin than Soeumin. (7) Nasal tip depth, Nasal depth ratio is the highest in Taeumin (8) Nasal depth Nasalalar heightratio is lowest in Soeumin. (9) Midfaceheight, Nasal tip depth ratio is higher Soyangin than Taeumin. 4. The morphologic characteristics of mouth according to the Sasang constitution. (1) Lower mid lip height, Lower philtrum height, Lower quarter lip height, Total middle lip height, Total philtrum height, Total quarter lip height is the shottest in Soyangin. (2) Upper mid lip height, Upper philtrum height is longer in Taeumin than Soyangin (3) Lip inclination is higher in Soeumin than Soyangin. (4) Intercheilion breadth, total height ratio is lowest in Soyangin. (5) Total lip height, Upper philtrum height ratio is higher in Soyangin than Soeumin. (6) Lower lip height Lower quarter lip height ratio is higher in Soyangin than Taeumin. (8) Total lip area is wider in Taeumin than Soyangin.

Acoustic Rhinometric Comparison of Cleft Side with Non-cleft Side after Repair of Unilateral Cleft Lip Nose Deformity (일측 구순열비변형에서 음향비계측법(acoustic rhinometry)의 이용: 개열측과 비개열측의 비교)

  • The upper and lower lateral cartilages provide the key to the lower cartilaginous portion of the nose. Lifting the cartilages is essential procedure for correction of unilateral cleft lip nose deformity. After correction of cleft lip nose deformity, authors used acoustic rhinometry (AR) to compare the lower nasal cavity of cleft side with non-cleft side. AR is a well known new, non-invasive diagnostic technique in which nasal geometry is assessed by means of acoustic reflection. From June 1996 to January 2004, we performed acoustic rhinometric analysis after correction of unilateral cleft lip nose deformity. This study involved 40 children of age ranged from 3 months to 8 years. Subjects were divided into the group of incomplete unilateral cleft lip nose deformity(20 subjects), and the group of complete unilateral cleft lip nose deformity(20 subjects). Results show that lower nasal cavity volume between non-cleft side and cleft side has no difference, and better results were obtained when nasal molding prong was applied at cleft side nostril. The results between incomplete type and complete type have no significant difference. In conclusion, AR is an effective method to calibrate cross sectional area and nasal cavity volume of unilateral cleft lip nose deformity, and furthermore effective in comparing the volume of cleft side with non-cleft side after unilateral cleft lip nose deformity correction with lifting the lower lateral cartilages to the upper lateral cartilages.

Histochemical Detection of Ionic Zinc in the Rat Olfactory Mucosa: Zinc Selenium Autometallography ($ZnSe^{AMG}$) (랫드 후각점막내 Zinc 이온의 조직화학적 동정)

  • The present study was designed to demonstrate ionic zinc in the rat nasal mucosa by means of zinc selenium autometallography ($ZnSe^{AMG}$). Rats were given sodium selenide either intraperitoneally (i.p) or intranasally (i.n). Prior to the i.n. administration the rats were anesthetized with pentobarbital sodium (30 mg/kg, i.p.). A thin plastic tube coupled to a Hamilton syringe was then inserted into the right nostril and $10{\mu}l$ of the solution was instilled. For the i.p. administration non-anesthetized rats were given $100{\mu}l$ of the sodium selenide solution (10 mg/kg). Control rats were instilled with saline. After 2 hrs survival, the rats were anaesthetized and transcardially perfused with 3% glutaraldehyde. The olfactory area was removed and put into same fixative. The nose was then sectioned ($30{\mu}m$) horizontally, autometallography (AMG) was performed according to Danscher et al. (1997). After silver enhancement, fine AMG grains were scattered in the whole length of the olfactory epithelium containing olfactory receptor neurons, sustentacular and basal cells. However, much higher concentration of the AMG grains occupied near the surface and in the basal region of the olfactory epithelium. Both groups of i.p. and i.n. administration showed almost same level in the concentration of the AMG grains. In i.n. group, few AMG grains were also found in olfactory nerves of the lamina propria, suggesting zinc transport into the olfactory bulb via olfactory axons. At the electron microscopic level, the AMG grains were most entirely found in the supporting cells of the olfactory epithelium, and they were mostly localized in lysosome-like organelles. The i.n. group showed various signs of tissue damage of the olfactory mucosa, where dense concentration of AMG grains were localized at crystalloid structures. The present study demonstrated dense population of ionic zinc in the rat olfactory epithelium. zinc may play a role in the olfactory functioin and in the pathogenesis of the neurodegerative disorders affecting nose.
