Pancreas-preserving limited duodenal resection: Minimizing morbidity without compromising oncological adequacy

  • Backgrounds/Aims: Pancreaticoduodenectomy is the most common procedure for the management of duodenal pathologies. However, it is associated with substantial morbidity and a low risk of mortality. Pancreas-preserving limited duodenal resection (PPLDR) can be performed under specific scenarios. We share our experience with PPLDR and its outcome. Methods: We retrospectively analyzed a prospectively maintained database of patients undergoing limited duodenal resection in the form of wedge (sleeve) resection or segmental resection of one or more duodenal segments from March 2016 to March 2021 at a tertiary care center in North India. Results: During the study period, 10 patients (including 9 males) underwent PPLDR. Five of these 10 patients showed primary duodenal or proximal jejunal pathology, while the remaining five had duodenal pathology involving an adjacent organ tumor. Four patients underwent wedge (sleeve) resection, while the remaining six underwent segmental duodenal resection of one or more duodenal segments. Mean hospital stay was 6 days (range, 3-11 days) without 30-day mortality. Morbidity occurred in 4 patients (Grade I-II, n = 3; Grade III, n = 1). All patients were alive and disease-free at the time of last follow-up. The mean follow-up duration was 23 months (range, 2-48 months). Conclusions: PPLDR is a safe and effective alternative for pancreaticoduodenectomy when selected carefully for specific tumor types and location.

실제 도로거리를 이용한 농촌지역 생활권의 의료시설 접근성 평가 -평창군을 중심으로- (Assessment of Accessibility to Medical Facilities in Rural Areas using Real Road Distance focusing on Pyeongchang-gun)

  • Since most medical facilities and medical personnel are concentrated in urban areas in South Korea, an imbalance of access to medical facilities causes a gap in medical resource availability between urban and rural areas. Accessibility to medical facilities is a means of assessment that evaluates a measure of inequality in utilization of medical resources. The evaluation of accessibility uses Euclidean distance, in general; however, this method has its limits in that Euclidean distance cannot reflect actual distance. This study aims to estimate real road distance from village halls to medical facilities in rural areas using Open Application Programming Interface (Open API) of an internet portal site. Using real road distance, we evaluated medical accessibility and compared it with Euclidean distance. The accessibility to primary medical institutions was valued relatively well, but secondary and tertiary medical institutions were vulnerable in Pyeonchang-gun. Comparing Euclidean distance with real road distance from village halls to medical facilities, real road distance appeared to be approximately 1.4 times that of Euclidean distance. This calculation is similar to the circuity factor of Gangwon-do in Korea that estimated to fix the limits of Euclidean distance and assumed real road distance.

Seroprevalence and Spatial Distribution of Toxoplasmosis in Sheep and Goats in North-Eastern Region of Pakistan

  • Toxoplasmosis is a protozoan disease that is caused by Toxoplasma gondii in livestock and humans. Due to its medical and veterinary importance, it is essential to study the seroprevalence of T. gondii infection among humans and animals in various parts of the world. The major objective of this study was to determine the seroprevalence and spatial distribution of toxoplasmosis in small ruminants (sheep and goats) of north-eastern region, Pakistan. A total of 1,000 animals comprising of sheep (n=470) and goats (n=530) were examined for T. gondii infection by using ELISA. An epidemiological data was collected in the form of questionnaire. A surface has been generated by using method of interpolation in Arc GIS with the help of IDW (inverse distance weight). The results showed higher seroprevalence of T. gondii in goats (42.8%) as compared to sheep (26.2%). The seroprevalence was higher in females as compared to males in all examined ruminants. Similarly, there is a wide variation in the seroprevalence of T. gondii in different breeds of sheep and goats showing higher seroprevalence in Teddy (52.8%) and Damani breed (34.5%) of goat and sheep's, respectively. The geographical and spatial distribution of T. gondii shows that it is widely distributed in different parts of the north-eastern region of Pakistan. Our results suggest widespread environmental contamination with T. gondii oocysts and that small ruminants could be a potentially important source of T. gondii infection if their infected meat is consumed undercooked.

Presence of Two Apocarotenoids in Volatile Constituents of Onosma dichroanthum

  • Volatile constituents obtained by water distillation from the aerial parts and root of Onosma dichroanthum Boiss (Boraginaceae) native to the north of Iran were investigated by GC and GC/MS for the first time. Palmitic acid (39.61%) and decane (31.39%) were the major components in the root while decane (26.26%) and phytol (25.52%) were the predominant constituents in the aerial parts. Ketones, aldehydes, alkanes, fatty acids, oxygenated diterpenes and sesquiterpenes were characterized as the most phytochemicals in the aerial parts. Alkanes and fatty acids were identified as the main groups in the root volatile substances. There were two ketone derivatives, belong to apocarotenoids, in the aerial parts; β-ionone and hexahydrofarnesyl acetone.

A Case of Ocular Angiostrongyliasis with Molecular Identification of the Species in Vietnam

  • A 23-year-old female residing in a village of Cao Bang Province, North Vietnam, visited the Hospital of Hanoi Medical University in July 2013. She felt dim eyes and a bulge-sticking pain in her left eye for some days before visiting the hospital. In the hospital, a clinical examination, an eye endoscopy, and an operation were carried out. A nematode specimen was collected from the eye of this patient. The body of this worm was thin and long and measured $22.0{\times}0.3mm$. It was morphologically suggested as an immature female worm of Angiostrongylus cantonensis. By a molecular method using 18S rRNA gene, this nematode was confirmed as A. cantonensis. This is the first molecular study for identification of A. cantonensis in Vietnam.

The First Human Case of Thelazia callipaeda Infection in Vietnam

  • A 26-year-old man residing in a village of Thai Nguyen Province, North Vietnam, visited the Thai Nguyen Provincial Hospital in July 2008. He felt a bulge-sticking pain in his left eye and extracted 5 small nematode worms by himself half a day before visiting the hospital. Two more worms were extracted from his left eye by a medical doctor, and they were morphologically observed and genetically analyzed on the mitochondrial cytochrome c oxidase 1 gene. The worms were 1 male and 1 female, and genetically identical with those of Thelazia callipaeda. By the present study, the presence of human T. callipaeda infection is first reported in Vietnam.

  • Hospice movement in Taiwan emerged early in 1983. There was a nurse visiting terminal cancer patients by herself in Taipei city. It was ceased after one year. This stage of hospice movement might be called as "compassionate era". In early 1990, the first in-patient hospice ward was set up in north Taiwan. She demonstrated high touch in the high technology medical atmosphere. There was a great echo in Taiwan society to this action. In the following years, quite a few new hospice settings were founded. Medical professionals were aroused again to talk and think about life and death, dignity of dying and holistic care. This stage of hospice movement might be called as "ethical stage". Around 2000, obstructions were discovered in our development. We do need system and rules. Standard of setting and care, Curriculum of education and training, Accreditation system and specialist system and Nature Death Act are some of the systems we approached. This stage of hospice movement might be called as "Act stage". Among the "Act stage", the Nature Death Act is actually the mile stone in our history. 한국 섬유기업의 스마트섬유 개발 동향 - 대구경북지역 업체를 대상으로 - (Examination of Development State of Smart Fiber in Korean Textile Industry - Focused on Companies in Daegu/North Gyeongsang Province -)

  • The purpose of this study was to examine the development trend of smart fiber by textile companies in Daegu/North Gyeongsang Province. This examination includes the development fields of smart fiber, investment size, information sources and the use degree of those, research-related infrastructure, relationship between companies' characteristics and their interests in smart fiber. Research data were collected by a survey. The definition and classification(4 areas and 12 development products) of smart fiber followed ones of Smart Fiber Technology Roadmap by the Ministry of Commerce, Industry and Energy. Data were analyzed using SPSS 11.0 program for frequency, means, t-test, and $X^2$-test. Among respondents, the numbers of dying and finish companies and export traders were the highest. It revealed that 19.6% of companies were developing smart fibers. Within 12 development products, vapor permeable/waterproof fiber showed to be most being developed, while medical fiber for human has not been developed and wasn't made an investment plan for developing. It was discovered that the biggest problem of smart fiber development was the lack of experts and the best outcome of smart fiber development was preoccupation of future market. It showed that companies got information from textile fairs but didn't often use information sources. The companies appeared to have a weak intention about research of smart fiber. The more important a company considered functionality of textile products and the more innovative and stabler the company was, the higher interest in smart fiber companies had. It was concluded that textile companies in Daegu/North Gyeongsang Province were aware of the importance of smart fiber development but they were not strongly interested in it and not enthusiastic in taking action on it.

Current Status of Taeniasis and Cysticercosis in Vietnam

  • Several reports on taeniasis and cysticercosis in Vietnam show that they are distributed in over 50 of 63 provinces. In some endemic areas, the prevalence of taeniasis was 0.2-12.0% and that of cysticercosis was 1.0-7.2%. The major symptoms of taeniasis included fidgeted anus, proglottids moving out of the anus, and proglottids in the feces. Clinical manifestations of cysticercosis in humans included subcutaneous nodules, epileptic seizures, severe headach, impaired vision, and memory loss. The species identification of Taenia in Vietnam included Taenia asiatica, Taenia saginata, and Taenia solium based on combined morphology and molecular methods. Only T. solium caused cysticercosis in humans. Praziquantel was chosen for treatment of taeniasis and albendazole for treatment of cysticercosis. The infection rate of cysticercus cellulosae in pigs was 0.04% at Hanoi slaughterhouses, 0.03-0.31% at provincial slaughterhouses in the north, and 0.9% in provincial slaughterhouses in the southern region of Vietnam. The infection rate of cysticercus bovis in cattle was 0.03-2.17% at Hanoi slaughterhouses. Risk factors investigated with regard to transmission of Taenia suggested that consumption of raw meat (eating raw meat 4.5-74.3%), inadequate or absent meat inspection and control, poor sanitation in some endemic areas, and use of untreated human waste as a fertilizer for crops may play important roles in Vietnam, although this remains to be validated.

산부인과 간호단위의 간호과정과 SNOMED CT를 이용한 간호진단 온톨로지의 구축 (Construction of the Nursing Diagnosis Ontology in Obstetric and Gynecologic Nursing Unit using Nursing Process and SNOMED CT)

  • Purpose: This study was performed to propose an ontology methodology based on standardized nursing process as framework in obstetric and gynecologic nursing practice. Methods: The instrument used in this study was based on the nursing diagnosis classification established by North American Nursing Diagnosis Association (NANDA) (2009-2011), fifth edition of the Nursing Interventions Classification (NIC) (2008), forth edition of the Nursing Outcomes Classification (NOC) (2008) developed by Iowa State University and systematized nomenclature of medicine clinical terms (SNOMED CT). The nursing records data were collected from electronic medical records of one hospital from August to October 2010. Results: One hundred and forty-one nursing diagnosis statements used in obstetric and gynecologic nursing unit were linked standardized nursing classifications and constructed nursing diagnosis ontology including interoperability. Conclusion: Not only will this result be helpful to complete nurse's lack of knowledge and experience, it will also help to determine nursing diagnosis logically by using standardized nursing process. It will be utilized as the method to construct ontology including interoperability in other nursing units. It will be presented nursing interventions according to nursing diagnosis and thus will be easier to establish nursing planning. This can provide immediate feedback of the nursing process application.
