Down syndrome with asymptomatic neuroglial cyst: A case report and review of the literature

  • Down syndrome (DS, trisomy 21) is associated with neuroanatomical abnormalities, including choroid plexus cysts and various types of brain tumors. Trisomy 21 is associated with oncogenic factor, especially in brain tumor. The brain of DS patients had a smaller volume of gray and white matter and an unbalanced cerebellum volume, indicating a smaller volume overall than normal. We report a case of a DS male patient who had an incidentally discovered neuroglial cyst in left cerebellar vermis. He visited our hospital with gait disturbance and fatigue. But, the neurologic exam was normal. To the best of our knowledge, this is the first reported case of a neuroglial cyst in a trisomy 21 patient. As the developmental mechanisms of a cyst and the choroid plexus are related, more research is needed.

A Modified Stabilization of Medial Shoulder Luxation by Biceps Tendon Transposition in a Dog (개에서 상완이두건 변형 전위에 의한 내측 견관절 탈구의 안정화 증례)

  • A 10-year-old intact female Chihuahua weighing 2 kg was presented for complaint of forelimb lameness with non-weight bearing posture. Greater tubercle was palpated medial to its normal location on palpation and crepitation was intermittent with manipulation. On radiographic examination, left medial scapulohumeral joint luxation was found. Bilateral medial patellar luxations also were found and evaluated as grade III. Surgically, the medial scapulohumeral joint luxation was reduced with transposition of the tendon of the biceps brachii muscle. The patient's gait became normal and non-weight bearing posture was resolved at 6 weeks postoperatively.

The Effect of a Wedged Rocker Sole on Ankle Joints during Gait (보행에서 외측 경사진 굽은 밑창이 발목 운동에 미치는 영향 분석)

  • Wedged soles and rocker soles are widespread shoe designs used to prevent the disorders and reduce the pain of the lower extremity caused by arthritis or diabetic feet. In this study, the effect of a shoe with a laterally wedged sole and a rocker sole simultaneously was analyzed on the kinematics and kinetics of the ankle joint during normal walking. Eight male participants without a history of lower extremity disorders were recruited. Each participant performed twenty walking cycles for each of three walking conditions: bare foot, wearing normal shoes and wearing shoes with laterally wedged rocker soles. The differences between the three walking conditions were statistically investigated including spatio-temporal variables, angular displacements, joint moments and ground reaction forces. The results showed that the laterally wedged rocker sole decreased the sagittal variation of angular displacements as well as the frontal/sagittal average moment on the ankle joints compared to the flat sole. In addition, the rate of angular displacements and loading decreased during the heel contact phase.

  • Eccentric muscle contraction is more effective than concentric and iosmetric muscle contraction in increasing muscle strength. Also, eccentric or concentric-eccentric training has greater effective in neural activation and muscle hypertrophy than concentric training. In some study, eccentric exercises have been shown to reduce pain and improve function on Achilles tendinopathy. The purpose of this study was to evaluate the effect of eccentric isokinetic exercise in a patient with dislocation of the tarsometatarsal joint by traffic accident. After eccentric isokinetic training, peak torque, average work, and average power were increased. Also, the patient was fully weightbearing with a pain free normal gait thus making good recovery.

Walking Pattern Analysis Using an Acceleration Sensor Device

  • In this paper, a device to analyze gait pattern was developed by using a 2-axis acceleration sensor attached to the foot. The 1st low-pass filter was adapted to limit the frequency band up to 5 Hz. An algorithm to detect the peak value exceeding the threshold voltage of an X-axis acceleration sensor and a Z-axis acceleration sensor was developed and normal and abnormal walking patterns were thus differentiated. Also, MCU and Bluetooth were combined to transfer the data to other MCUs to display on an LCD; the size of the device could then be reduced. The new algorithm and the device allowed the individual walking patterns to be easily measured at a low cost and with less restriction on activities compared to conventional multiple pressure sensors or motion camera system.

  • An electrical stimulator was designed to induce locomotion for paraplegic patients caused by central nervous system injury. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. The electrical stimulator restored partially gait function in paraplegic patients.

  • In this study, an experiments and numerical simulation of a three chamber pneumatic cylinder for an intelligent AK-knee prosthesis is performed. The cylinder has a variable orifice which can be controlled automatically through a microprocessor controller as needed while amputee gaits. In the experiment, the cylinder was driven by a cam whose trajectory of simulates the normal gait and axial forces of cylinder with different of orifice opening was measured. The numerical simulations was based on thermodynamic and fluid mechanical consideration. The experimental results and the numerical results were in good agreement.

Study for Squat Scoring Scale (쭈그려 앉기의 평가척도에 대한 연구)

  • The purposes of this study are to develop 'Squat Scoring Scale' and identify relationship between squatting score and flexibility of the lower extremity muscles such as quadriceps femoris and soleus in terms of relationship between squatting and functions of lower extremity. Subjects have normal gait patterns only with or without musculoskeletal problems of lower extremities, lumbar and pelvis and gender. There are sixty subjects who participated in this study. The scoring are checked after squatting posture according to 'Squat Scoring Scale', and measured ROM of the ankle dorsiflexion and knee flexion for flexibility of the soleus and quadriceps femoris, respectively. Fourteen of sixty subjects have 10 score, seventeen subjects have 9 score, seven subjects have 8, 7 and 6 scores, three subjects have 5 score, and five subjects have 4 score. The gap between 10 to 9, 9 to 8 and 8 to 7 score in all values of angles has the statistical significance for t-test(p<.05). This information indicates that flexibility of quadriceps femoris and soleus acts as important factors in squatting.

Effect of the Exercise Load on the Locomotor-Respiratory Coupling (운동부하가 움직임-호흡 결합에 미치는 영향)

  • The nature of entrainment between the locomotor and the respiratory rhythm was investigated while normal human subjects were walked or running on a treadmill. The purpose of this study was to analyze the incidence and type of coordination between the locomotor and the respiratory rhythm during running at different work load. The experiments were carried out on 12 untrained volunteers exercising at 3 work loads (2 METs, 3 METs, 4 METs in randomized order). The gait cycle was measured by electromyography (EMG) signal of gastrocnemius firing and the respiratory cycle was measured by a thermometer. We found that the ratio between the locomotor and the respiratory rhythm existed and 2:1 ratio between the locomotor-respiratory coupling was dominant at 2 METs and 3 METs.

Application of Z-plasty for Skin Defects by Mastectomy in a Dog (개에서 유방절제술에 의한 피부결손의 Z-plasty응용)

  • A fourteen-year-old mongrel female dog that was 22 kg and had two large mammary tumor lesions was admitted to the Animal Teaching Hospital of Chonbuk National University. Two large mammary tumor lesions were observed in the right caudal thoracic and the left inguinal mammary glands, which were sized 6 cm and 5 cm in diameter, respectively. These tumor lesions were removed by block mastectomy, then large skin defect of 7 cm diameter in caudal abdominal wall were recoverd by classic Z-plasty in both medial thigh. Classic Z-plasty was designed with 60 in angle and, with 6 cm in length of central limb in right medial thigh and 4 cm in left, respectively. This patient had been recoverd to normal gait and behavior at 20 days after the plastic operation.
