족부 보조기를 통해 비구조적 척추 측만증을 치료한 3례 -2례는 족부 보조기만으로, 1례는 족부 보조기와 한의학적 치료법을 병행함 (Nonstructural Scoliosis Treated with Foot Orthosis: Three Clinical Cases Report -2 Cases Treated with Only Foot Orthosis, 1 Case Treated with Foot Orthosis and Korean Medicine Therapy)

  • Objectives : The objective of this study is to report the effect of foot orthosis on by observing 3 clinical case studies. Methods and Results : We investigated for the changes in Cobb's angle and pelvic height by using the standing full X-ray. After application of foot orthosis, Cobb's angle and pelvic height was reduced in cases. We treated 2 cases with only Foot Orthosis, 1 case with Foot Orthosis and Korean Medicine therapy. Conclusions : This study showed that foot orthosis can be effective way on nonstructural scoliosis and statistics study should be followed.

  • Objectives : This case report describes the management of five patients suffering from nonstructural scoliosis with lumbar herniated intervertebral disc all treated with complex Korean medicine treatment. Methods : Five patients were hospitalized and treated with herbal medicine, acupuncture, pharmacopuncture, cupping, and Chuna manual therapy. Patients were assessed for Numerical Rating Scale (NRS), Oswestry Disability Index (ODI), EuroQol-FiveDimensions (EQ-5D), and Cobb's angle. Results : In all patients, NRS, ODI, and Cobb's angle decreased, while the EQ-5D score increased. Conclusions : As seen in these five cases, complex Korean medicine treatment with Chuna manual therapy has effects on the management of nonstructural scoliosis.

Correlation of Radiographic and Patient Assessment of Spine Following Correction of Nonstructural Component in Juvenile Idiopathic Scoliosis

  • Objective To evaluate the association between progression of curvature of scoliosis, and correction for functional component in patients with juvenile idiopathic scoliosis (JIS). Methods We retrospectively reviewed medical data of patients prescribed custom molded foot orthosis (FO) to correct inequality of RCSPA (resting calcaneal stance position angle), and chose 52 patients (26 females, 26 males) with Cobb angle ${\geq}10^{\circ}$ in radiology and uneven pelvic level at iliac crest by different RCSPA (${\geq}3^{\circ}$) as a factor of functional scoliosis. They had different hump angle ${\geq}5^{\circ}$ in forward bending test, for idiopathic scoliosis component. Their mean age and mean period of wearing FO were $79.5{\pm}10.6months$ and $18.6{\pm}0.70months$. Results Cobb angle was reduced from $22.03^{\circ}{\pm}4.39^{\circ}$ initially to $18.86^{\circ}{\pm}7.53^{\circ}$ after wearing FO. Pelvis height difference and RCSPA difference, were reduced from $1.07{\pm}0.25cm$ initially to $0.60{\pm}0.36$, and from $4.25^{\circ}{\pm}0.71^{\circ}$ initially to $1.71^{\circ}{\pm}0.75^{\circ}$ (p<0.01). Cobb angle improved most in 9 months. However, there was no significant improvement for those with more than $25^{\circ}$ of Cobb angle initially. Mean Cobb angle improved in all age groups, but patients less than 6 years had clinically significant improvement of more than $5^{\circ}$. Conclusion JIS can have functional components, which should be identified and managed. Foot orthosis is useful in correcting functional factors, in the case of pelvic inequality caused by different RCSPA, for patients with juvenile idiopathic scoliosis.

척추 측만증을 동반한 흉추에 발생한 동맥류성 골낭종 - 증례 보고 - (Aneurysmal Bone Cyst of the Thoracic Spine with Scoliosis - Case Report -)

  • Aneurysmal bone cysts are uncommon bony lesions of the spine. Approximately 3-20% of the aneurysmal bone cysts occur in the spine, predominantly in the lumbar region, but they may occur at the any level of the spine. These lesions commonly arise from the neural arch and occasionally invade the pedicle and the vertebral body. The clinical diagnosis of a spinal lesion can be very difficult in the early stages of the disease because specific symptoms and signs are usually absent or only amount to back pain. However, depending on the level of involvement and the extent of neurological compression, a wide variety of neurological symptoms and signs may appear, ranging from mild radicular symptoms to complete paraplegia or tetraplegia. Available treatment options include complete excision or curettage of the lesion with bone graft, but where excision cannot be achieved, low dose radiation or arterial embolization may be used. We report a case of aneurysmal bone cyst in the pedicle of the T10 spine with nonstructural scoliosis of $40^{\circ}$ Cobb's angle which was treated successfully with only curettage of the lesion.
