신경인지장애의 정신행동증상에 대한 비약물학적 개입 (Non-Pharmacological Interventions for Behavioral and Psychological Symptoms of Neurocognitive Disorder)

  • 신경인지장애 환자들은 인지저하 뿐만 아니라 초조, 공격성, 우울, 배회 등의 정신행동증상들을 나타내며 이로 인해 환자와 가족이 상당한 고통을 받게 된다. 정신행동증상의 치료를 위해서는 환자 중심의, 비약물학적인 치료가 우선되어야한다. 본 논문은 신경인지장애 환자에서 나타나는 정신행동증상을 관리하고 치료하기 위한 비약물학적인 개입에 대하여 기술하였다. 초조, 우울, 무감동, 불면, 배회 등의 정신행동증상을 조절하기 위해서는 환경변화, 약물 등의 유발 인자를 파악하고 평가한 뒤, 그러한 문제를 해결해주는 것이 중요하다. 비약물학적인 개입 방법으로는 안심시키기, 활동 격려하기, 주의를 분산시키기, 환경을 변화시키기 등을 들 수 있다. 환자의 관점에서 행동을 이해하고, 그들의 요구와 능력에 맞게 접근해야한다. 비약물학적인 개입 방법으로 회상요법, 음악치료, 향기요법, 다중감각자극요법, 운동요법, 광선치료, 마사지 요법, 인지중재치료, 애완동물요법 등의 다양한 요법이 시행되고 있으며, 이러한 접근 방법이 환자의 우울, 무감동, 초조, 공격성, 불안, 배회, 불면 등의 증상을 호전시켜준다고 알려져 있다. 그러나, 효과에 대한 증거 수준이 높지 않아 향후에 좀 더 광범위하고 정확한 효과 검증 연구가 필요하다.

부인과 수술환자의 통증조절을 위한 비약물성 요법의 인식수준 및 통증변화 양상 (Perception of Non-pharmacological Therapy for Pain Control and Pattern of Postoperative Pain in Gynecological Surgery Patients)

  • Purpose: Women who undergo gynecological surgery have moderate and severe sensation and distress of pain despite the advent of patient controlled analgesia (PCA). The purposes of this study were to describe perception of non-pharmacological therapy for postoperative pain control and examine changes of pain sensation and distress in women who had gynecological surgery. Method: The sample consisted of 52 women who were having gynecological surgery. Subjects who agreed to participate in the study were asked for their opinion about non-pharmacological approaches for postoperative pain control using a structured study questionnaire. Pain sensation and distress were assessed by VAS in the morning and afternoon for 2 days following the surgery. Result: About 50% of the subjects thought that non-pharmacological methods such as relaxation, music, massage, or meditation would be helpful for their postoperative pain control. If both pharmacological and non-pharmacological therapy were given for pain control, 96% of subjects reported it would be effective. Nurses can apply techniques of relaxation, deep breathing, meditation, and music therapy to surgical patients along with PCA. Expected sensation and distress of pain was high, but pain levels gradually decreased over time. However, subjects experienced moderate levels of pain postoperatively although they used PCA. Conclusion: The effect of a combined method of pharmacological and non-pharmacological approach needs to be tested if postoperative pain is to be decreased more.

유방암 생존자들의 자가관리에 대한 현상학적 연구: 비약물적 접근방법을 중심으로 (The phenomenological study of self-management intervention among breast cancer survivors: Non-pharmacological approaches)

  • 본 연구는 현상학적 방법을 적용하여 유방암 생존자들의 비약물적 추후 관리 경험에 대한 본질적 구조와 의미에 대해 심도 있게 파악하는 것을 목적으로 한다. 이번 연구에서 총 참여자는 10명으로 S시 E병원 외래에 정기적으로 내원하는 유방암 생존자로 수술, 화학요법, 방사선요법을 완료한 자들이었다. 자료수집 기간은 2014년 8월부터 2015년 2월까지로 수집된 자료는 심층면담 후 Colaizzi의 방법 적용하여 분석하였다. 연구결과, 7개의 주제와 16개의 하위범주가 도출되었다. 7개의 주제는 '1. 증상을 완화하기 위해 용이한 방법을 적용함, 2. 변화된 신체 상태를 위해 움직이려 함, 3. 재발 방지를 위해 특정 식용 작물을 섭취함, 4. 건강을 유지하기 위한 식생활 양식이 있음, 5. 보완대체요법에 대해 항상 예의주시함, 6. 주치의 처방외 필요한 치료가 있음, 7. 자연친화적인 환경에서의 삶을 지향함' 이었다. 이번 연구는 유방암 생존자들이 치료 후에 일상생활에서 어떻게 추후관리를 하는지 심도있게 파악하는 데 도움을 줄 것이다. 나아가 본 결과는 향후 유방암 생존자들에게 실제 관리 양식에 근거한 실행 가능한 지침과 프로그램을 제공하는데 있어 기초자료로써 제공될 것이다.

주의력결핍 과잉행동장애에서 비정신자극제 약물치료 (Non-Stimulant Medications in the Treatment of Attention-Deficit Hyperactivity Disorder)

  • The aim of this paper is to evaluate the use of non-stimulants, including atomoxetine, bupropion and modafinil, as alternative approaches to the treatment of children with attention-deficit hyperactivity disorder. A comprehensive review of the empirically based literature regarding the efficacy and the safety of the non-stimulants was performed. There is a large and increasing body of data supporting the efficacy and the safety of non-stimulants. Although the treatment effect sizes for non-stimulants may be smaller than those for stimulants, non-stimulants alone have been shown to be effective in the treatment of attention-deficit hyperactivity disorder as well as several comorbidities. These results suggest that non-stimulants are effective in the treatment of attention-deficit hyperactivity disorder. Further studies are needed to improve our understanding of alternative pharmacological medications in the treatment of attention-deficit hyperactivity disorder.

  • Depression is an important medical problem in today's world. Despite its high prevalence, treatment of depression remains problematic, as its cause is still not fully understood. Of the ways recommended to tackle this problem is implementing the potentials of various medical schools. A medical school which has played an important role in the history of medicine in the world is Persian medicine. This study was intended to identify non-pharmacological interventions for depression in the most reliable references on Persian medicine, classify these interventions and compare them with the latest medical findings. The study was set to review the most reliable references on Persian medicine. In addition, relevant keywords were used to search the PubMed and Scopus databases. In Persian medicine sources, melancholia is categorized into three main types. One type is regarded as cerebral melancholia, which resembles modern psychiatry's depression closely. Therefore, some key points put forth by Persian medicine scholars regarding melancholia can be used to answer questions concerning depressive disorders and their etiology and treatment. Although recent studies have confirmed most approaches offered by Persian medicine physicians in ancient Persia, it is imperative to design and conduct clinical trial studies according to these approaches.

  • There have been many trials of clinical efficacy of multi component and single component treatments for irritable bowel syndrome(IBS). We reviewed effects of non phamacological treatments in the IBS. Though the efficacy of multi-component approaches was unclear, several results suggest that cognitive behavioral therapy was effective in improving gastrointestinal symptoms of IBS. As a single component, cognitive therapy and relaxation with or without biofeedback could improve the symptoms and psychological health of IBS patients. Yoga, meditation, self-help information and hypnotherapy could be applicable to IBS.

기분장애 환자의 비만에 대한 약물치료 (Pharmacotherapy for Obesity in Mood Disorders)

  • 기분장애에서 과체중과 비만의 문제가 일반인구보다 더 심각하고, 심혈관 치명률의 증가와 무관하지 않기 때문에 비만에 대한 치료가 필수적이다. 일반적인 상황에서 비만에 대한 치료와 동일하게 생활습관교정과 같은 비약물치료가 선행되어야 한다. 이 같은 시도가 실패하는 경우에는 약물치료가 필요하다. 기분장애의 비만치료로 공식 승인된 약은 없다. 따라서 일반인구의 비만에 승인된 약을 처방하거나 여러 연구를 통해서 효과가 입증된 약을 처방하게 된다. 여러 치료 지침과 연구 등은 orlistat, metformin, topiramate와 bupropion 등이 효과적임을 지지한다.

섬망에 대한 정신신체의학적 중재 (Psychosomatic Intervention of Delirium)

  • 섬망은 일반 의학 상태와는 독립적으로 입원 기간 및 사망율을 증가시켜 환자의 전반적 예후에 부정적인 영향을 미친다. 다양한 내, 외과적 치료 환경에서 보고된 섬망의 발생 빈도는 15~70%로 알려졌으며, 정신신체 의학적 중재가 요구되는 흔한 정신과 의뢰 사유 중 하나이다. 섬망의 치료는 1) 위험 인자와 촉발 요인의 확인, 2) 지남력 회복 및 기능 유지를 위한 적절한 치료 환경의 구성과 환자 가족 및 치료진에 대한 교육을 제공하는 비약물적 중재, 그리고 3) 심한 초조, 환각, 그리고 행동 상의 문제 등을 조절하기 위한 약물학적 중재로 구분될 수 있다. 본고를 통해, 섬망에 대한 정신신체의학적 중재의 전반에 대해 기술하고 실제 임상에서 섬망치료 시에 흔히 접하게 되는 문제에 대해서도 논의하고자 한다.

Update on Irritable Bowel Syndrome Program of Research

  • Purpose: This article provides an update and overview of a nursing research program focused on understanding the pathophysiology and management of irritable bowel syndrome (IBS). Methods: This review includes English language papers from the United States, Europe, and Asia (e.g., South Korea) from 1999 to 2013. We addressed IBS as a health problem, emerging etiologies, diagnostic and treatment approaches and the importance of a biopsychosocial model. Results: IBS is a chronic, functional gastrointestinal disorder characterized by recurrent episodes of abdominal pain and alterations in bowel habit (diarrhea, constipation, mixed). It is a condition for which adults, particularly women ages 20-45, seek health care services in both the United States and South Korea. Clinically, nurses play key roles in symptom prevention and management including designing and implementing approaches to enhance the patients' self-management strategies. Multiple mechanisms are believed to participate in the development and maintenance of IBS symptoms including autonomic nervous system dysregulation, intestinal inflammation, intestinal dysbiosis, dietary intolerances, alterations in emotion regulation, heightened visceral pain sensitivity, hypothalamic-pituitary-adrenal dysregulation, and dysmotility. Because IBS tends to occur in families, genetic factors may also contribute to the pathophysiology. Patients with IBS often report a number of co-morbid disorders and/or symptoms including poor sleep. Conclusion: The key to planning effective management strategies is to understand the heterogeneity of this disorder. Interventions for IBS include non-pharmacological strategies such as cognitive behavior therapy, relaxation strategies, and exclusion diets.

Ginsenoside Rg3, a promising agent for NSCLC patients in the pandemic: a large-scale data mining and systemic biological analysis

  • Introduction: Non-small cell lung cancer (NSCLC) patients are particularly vulnerable to the Coronavirus Disease-2019 (COVID-19). Currently, no anti-NSCLC/COVID-19 treatment options are available. As ginsenoside Rg3 is beneficial to NSCLC patients and has been identified as an entry inhibitor of the virus, this study aims to explore underlying pharmacological mechanisms of ginsenoside Rg3 for the treatment of NSCLC patients with COVID-19. Methods: Based on a large-scale data mining and systemic biological analysis, this study investigated target genes, biological processes, pharmacological mechanisms, and underlying immune implications of ginsenoside Rg3 for NSCLC patients with COVID-19. Results: An important gene set containing 26 target genes was built. Target genes with significant prognostic value were identified, including baculoviral IAP repeat containing 5 (BIRC5), carbonic anhydrase 9 (CA9), endothelin receptor type B (EDNRB), glucagon receptor (GCGR), interleukin 2 (IL2), peptidyl arginine deiminase 4 (PADI4), and solute carrier organic anion transporter family member 1B1 (SLCO1B1). The expression of target genes was significantly correlated with the infiltration level of macrophages, eosinophils, natural killer cells, and T lymphocytes. Ginsenoside Rg3 may benefit NSCLC patients with COVID-19 by regulating signaling pathways primarily involved in anti-inflammation, immunomodulation, cell cycle, cell fate, carcinogenesis, and hemodynamics. Conclusions: This study provided a comprehensive strategy for drug discovery in NSCLC and COVID-19 based on systemic biology approaches. Ginsenoside Rg3 may be a prospective drug for NSCLC patients with COVID-19. Future studies are needed to determine the value of ginsenoside Rg3 for NSCLC patients with COVID-19.
