흉부 CT 영상에서 비소세포폐암 환자의 재발 예측을 위한 종양 내외부 영상 패치 기반 앙상블 학습 (Ensemble Learning Based on Tumor Internal and External Imaging Patch to Predict the Recurrence of Non-small Cell Lung Cancer Patients in Chest CT Image)

  • In this paper, we propose a classification model based on convolutional neural network(CNN) for predicting 2-year recurrence in non-small cell lung cancer(NSCLC) patients using preoperative chest CT images. Based on the region of interest(ROI) defined as the tumor internal and external area, the input images consist of an intratumoral patch, a peritumoral patch and a peritumoral texture patch focusing on the texture information of the peritumoral patch. Each patch is trained through AlexNet pretrained on ImageNet to explore the usefulness and performance of various patches. Additionally, ensemble learning of network trained with each patch analyzes the performance of different patch combination. Compared with all results, the ensemble model with intratumoral and peritumoral patches achieved the best performance (ACC=98.28%, Sensitivity=100%, NPV=100%).

Update on Adjuvant Treatment in Resectable Non-Small Cell Lung Cancer and Potential Biomarkers Predicting Postoperative Relapse

  • A significant proportion of patients with non-small cell lung cancer (NSCLC) is diagnosed in the early and resectable stage. Despite the use of platinum-based adjuvant chemotherapy, there was only a marginal increase in overall survival and a 15% decrease in relapse. With the advents of immunotherapy and epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor (TKI), the landscape of adjuvant treatment in completely resectable NSCLC is changing. Postoperative radiotherapy can be beneficial to patients who underwent surgical resection in certain clinical settings. In addition, new biomarkers that predict efficacy of EGFR TKI and immunotherapy as adjuvant treatment are also necessary. In this review, recent updates in adjuvant treatment in resectable NSCLC were briefly explained.

  • After the successful development of targeted therapy and immunotherapy for the treatment of advanced-stage non-small cell lung cancer (NSCLC), these innovative treatment options are rapidly being applied in the adjuvant setting for early-stage NSCLC. Some adjuvants that have recently been approved include osimertinib for epidermal growth factor receptor-mutated tumors and atezolizumab and pembrolizumab for selected patients with resectable NSCLC. Numerous studies on various targeted therapies and immunotherapy with or without chemotherapy are currently ongoing in the adjuvant setting. However, several questions regarding optimal strategies for adjuvant treatment remain unanswered. The present review summarizes the available literature, focusing on recent advances and ongoing trials with targeted therapy and immunotherapy in the adjuvant treatment of early-stage NSCLC.

Increased Serum S-TRAIL Level in Newly Diagnosed Stage-IV Lung Adenocarcinoma but not Squamous Cell Carcinoma is Correlated with Age and Smoking

  • Background: Lung cancer is the leading cause of cancer mortality in the world. Many factors can protect against or facilitate its development. A TNF family member TRAIL, has a complex physiological role beyond that of merely activating the apoptotic pathway in cancer cells. Vitamin D is converted to its active form locally in the lung, and is also thought to play an important role in lung health. Our goal was to investigate the possible clinical significance of serum sTRAIL and 1,25-dihydroxyvitamin D(3) levels in patients with non-small cell lung cancer (NSCLC). Materials and Methods: Totals of 18 consecutive adenocarcinoma and 22 squamous cell carcinoma patients with stage-IV non-small cell lung cancer referred to our institute were included in this study. There were 12 men and 6 women, with ages ranging from 38 to 97 (mean 60.5) years with adenocarcinoma, and 20 men and 2 women, with ages ranging from 46 to 80 (mean 65) years with squamous cell carcinoma. Serum levels of sTRAIL and 1,25-dihydroxyvitamin D(3) were measured in all samples at the time of diagnosis. Results: sTRAIL levels in NSCLC patients were higher than in the control group. Although there was no correlation between patient survival and sTRAIL levels, the highest sTRAIL levels were correlated with age and cigarette smoking in the adenocarcinoma patients. sTRAIL level in healthy individuals were correlated with serum 1,25-dihydroxyvitamin D(3). Conclusions: Serum sTRAIL concentrations were increased in NSCLC patients, and correlated with age and smoking history, but not with overall survival.

Antiproliferative effect of Citrus junos extracts on A549 human non-small-cell lung cancer cells

  • Purpose: This study investigates the alterations in A549 human non-small-cell lung cancer (NSCLC) cells exposed to Citrus junos extract (CJE). We further examine the antiproliferative and apoptotic effects of CJE on NSCLC cells. Methods: Inhibition of proliferation was examined by applying the 3-(4,5-dimethylthiazol-2-yl)-2,5- diphenyltetrazolium bromide (MTT) colorimetric assay on CJE-treated A549 NSCLC cells. The lactate dehydrogenase (LDH) assay was performed to measure the degree of toxicity of CJE on NSCLC cells. The effect on migratory proliferation was confirmed using the scratch wound healing assay. The antiproliferative effect of the CJE on human lung cancer cells was verified through morphological observation, fluorescence microscopy, and caspase-3 colorimetry. Results: Exposure of NSCLC cells to CJE resulted in a dose- and time-dependent decrease in cell activity and increased toxicity to the cells. In addition, microscopic observation revealed a reduced ability of the cancer cells to migrate and proliferate after exposure to the CJE, with simultaneous morphological apoptotic changes. Fluorescence staining and microscopic examination revealed that this death was a process of self-programmed cell death of NSCLC cells. Compared to unexposed NSCLC cells, the expression of caspase-3 was significantly increased in cells exposed to CJE. Conclusion: Exposure of A549 human NSCLC cells to CJE inhibits the proliferation, increases the cytotoxicity, and decreases the ability of cells to migrate and grow. Moreover, the expression of caspase-3 increases after CJE treatment, suggesting that the apoptosis of NSCLC cells is induced by a chain reaction initiated by caspase-3. These results indicate that Citrus junos is a potential therapeutic agent for human non-small-cell lung cancer.

  • Small cell carcinoma of the lung is characterized by cells with finely stippled chromatin and scanty cytoplasm as well as a particularly aggressive clinical course and favorable response to the chemotherapy. Recently percutaneous fine needle aspiration (FNA) biopsy has become both widely established and highly respected for the diagnosis of lung cancer. However metastatic small cell carcinoma of lymph node should be cytologically differentiated from the small round cell tumor of particular sites, especially malignant lymphoma, because small ceil carcinoma of classic oat cell type nay simulate small cell non-Hodgkin's lymphoma. We report five cases of metastatic small cell carcinoma of in-termediate cell type diagnosed by FNA of the enlarged lymph nodes of the neck and axilla. The cytologic smears contained diffuse small neoplastic cells larger than lymphocytes with dense, pyknotic nuclei and extremely scanty cytoplasm. Apparently viable large tumor cells have vesicular nuclei with granular, sometimes very coarse chromatin. The characteristic cytologic features of small cell carcinoma as compared to malignant lymphoma were as follows.: 1) small cells with dense pyknotic nuclei are evenly distributed in the background of apparently viable larger tumor cells, admixed with mature lymphocytes and phagocytic macrophages. 2) small loose aggregates of cells with nuclear melding are indicative of small cell carcinoma rather than non-Hodgkin's lymphoma. 3) the cytoplasmic and nuclear fragments of tumor necrosis are more dominant in the smears of small cell carcinoma. 4) nuclear membrane and nucleoli are generally indistinct in small cell carcinoma due to condensation of chromatin.

Subtype-Based Microbial Analysis in Non-small Cell Lung Cancer

  • Background: The human lung serves as a niche for a unique and dynamic bacterial community related to the development and aggravation of multiple respiratory diseases. Therefore, identifying the microbiome status is crucial to maintaining the microecological balance and maximizing the therapeutic effect on lung diseases. Therefore, we investigated the histological type-based differences in the lung microbiomes of patients with lung cancer. Methods: We performed 16S rRNA sequencing to evaluate the respiratory tract microbiome present in bronchoalveolar lavage fluid. Patients with non-small cell lung cancer were stratified based on two main subtypes of lung cancer: adenocarcinoma and squamous cell carcinoma (SqCC). Results: Among the 84 patients analyzed, 64 (76.2%) had adenocarcinoma, and 20 (23.8%) had SqCC. The α- and β-diversities showed significant differences between the two groups (p=0.004 for Chao1, p=0.001 for Simpson index, and p=0.011 for PERMANOVA). Actinomyces graevenitzii was dominant in the SqCC group (linear discriminant analysis [LDA] score, 2.46); the populations of Haemophilus parainfluenza (LDA score, 4.08), Neisseria subflava (LDA score, 4.07), Porphyromonas endodontalis (LDA score, 3.88), and Fusobacterium nucleatum (LDA score, 3.72) were significantly higher in the adenocarcinoma group. Conclusion: Microbiome diversity is crucial for maintaining homeostasis in the lung environment, and dysbiosis may be related to the development and prognosis of lung cancer. The mortality rate was high, and the microbiome was not diverse in SqCC. Further large-scale studies are required to investigate the role of the microbiome in the development of different lung cancer types.

Cyclin D1의 발현이 비소세포폐암의 예후에 미치는 영향 (Prognostic Significance of Cyclin D1 Overexpression in Non-Small Cell Lung Cancer)

  • 연구목적: 폐암은 생불학적 및 병리학적인 특성에 따라 비소세포폐암과 소세포폐암으로 구분되는데 전체폐암의 약 80%가 비소세포폐암으로 국소적인 폐암인 경우 수술적 치료가 절대적 완치 방법인데 조기에 발견하여 광범위한 절제로 치료된 경우에도 환자의 약 50% 정도만이 5년간 생존한다. 확실히 비소세포폐암에서 비슷한 병기의 환자들도 생존 가망성은 상당한 차이를 보인다. 이러한 상황이 보다 정확히 생존 가망성을 예측하고 각각의 환자에게 보다 효율적인 치료를 제공할 수 있게 도울 수 있는 새로운 예측 인자를 밝혀야 하는 필요성이 제기되었다. 정상적인 세포주기 뿐만 아니라 암종의 세포주기에 중요한 역할을 하는 cyclin D계중 하나인 cyclin D1이 세포주기에서 G1기에서 S기로의 전환을 유도하므로써 세포주기를 진행시키는데 유방암, 식도암 및 방광암에서 이의 과발현이 암발생에 중요한 요인이 되다는 보고가 있다. 이에 본 연구에서는 광범위 절제된 비소세포폐암에서 cyclin D1의 과발현이 예후에 미치는 영향을 알아보기 위해 연구를 시행하였다. 방 법: 1983년 1월부터 1995년 7월까지 한양대학교 부속병원에서 비소세포폐암으로 진단된 술전 병기 IIIa 이하인 환자 총 81예를 대상으로 치료적 목적으로 광범위 폐절제술을 통해 얻어진 술후 조직에서 cyclin D1에 대한 단클론성 향체를 사용하여 면역조직화화적 염색을 시행하고 더불어 병리조직학적 특정과 임상적 특정, 특히 생존률과의 연관성을 알아보았다. 또한 술후 환자의 예후적 인자를 최소한으로 줄이기 위해 술전에 방사선 치료나 항암제 치료를 받은 환자는 제외하였고 혼합형의 조직학적 진단이 이루어진 경우와 술후 l달 이내에 사망한 경우는 본 연구의 대상에서 제외하였다. 조직병리학적 병기 판정은 TNM 병기 판정 기준에 맞추었고 조직학적 특징은 WHO 표준 기준에 맞추었다. 결 과: Cyclin D1의 발현은 총 81예에서 시행하여 26에에서 발현되어 30.9%의 발현율을 보였고 각각의 조직형이나 병기, 암의 크기에는 통계적 차이를 보이지 않았으나 TNM 병기 판정에 따라 NO와 나머지 N1-3의 두군으로 나누어 비교할 때 cyclin D1에 따른 통계적 의의를 보였고(p=0.035) cyclin D1 양성 발현군의 평균생존기간은 $22.76{\pm}3.507$개월, 음성 발현군의 평균생존기간이 $45.3{\pm}5.64$개월 (p=0.0515)로 생존율과외 상당한 통계적인 연관성의 가능성을 나타내었다. 결 론: 본 연구에서는 여러 가지 세포주기를 직, 간접으로 조절하는 인자중 cyclin D1의 과발현은 비소세포폐암에서 상당히 불량한 예후 인자로 작용할 수 있다고 생각되며 앞으로 보다 초기와 진행된 병기에 따른 cyclin D1의 발현이 예후에 미치는 영향 등 보다 세밀하고 대단위적인 연구가 진행되어 비소세포폐암에서 보다 정확한 치료와 예후를 예측활 수 있는 인자로서의 역할을 밝혀야 한다고 생각된다.

MiRNA Molecular Profiles in Human Medical Conditions: Connecting Lung Cancer and Lung Development Phenomena

  • MiRNAs are endogenous, single stranded ~22-nucleotide non-coding RNAs (ncRNAs) which are transcribed by RNA polymerase II and mediate negative post-transcriptional gene regulation through binding to 3'untranslated regions (UTR), possibly open reading frames (ORFs) or 5'UTRs of target mRNAs. MiRNAs are involved in the normal physiology of eukaryotic cells, so dysregulation may be associated with diseases like cancer, and neurodegenerative, heart and other disorders. Among all cancers, lung cancer, with high incidence and mortality worldwide, is classified into two main groups: non-small cell lung cancer and small cell lung cancer. Recent promising studies suggest that gene expression profiles and miRNA signatures could be a useful step in a noninvasive, low-cost and repeatable screening process of lung cancer. Similarly, every stage of lung development during fetal life is associated with specific miRNAs. Since lung development and lung cancer phenomena share the same physiological, biological and molecular processes like cell proliferation, development and shared mRNA or expression regulation pathways, and according to data adopted from various studies, they may have partially shared miRNA signature. Thus, focusing on lung cancer in relation to lung development in miRNA studies might provide clues for lung cancer diagnosis and prognosis.

Luteolin attenuates migration and invasion of lung cancer cells via suppressing focal adhesion kinase and non-receptor tyrosine kinase signaling pathway

  • BACKGROUND/OBJECTIVES: Non-small cell lung cancer is mostly recognized among other types of lung cancer with a poor prognosis by cause of chemotherapeutic resistance and increased metastasis. Luteolin has been found to decrease cell metastasis. However, its underlying mechanisms remain unresolved. The objective of this study was to examine the effect (and its mechanism) of luteolin on the migration and invasion of human non-small cell lung cancer A549 cells. MATERIALS/METHODS: Cell viability was investigated by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay. Wound healing and transwell assays were evaluated to assess migration and invasion, respectively. Western blot analysis and immunofluorescence were further performed to investigate the role of luteolin and its mechanisms of action. RESULTS: Administration with up to 40 μM luteolin showed no cytotoxic activity on lung cancer A549 cells or non-cancer MRC-5 cells. Additionally, luteolin at 20-40 μM significantly suppressed A549 cells' migration, invasion, and the formation of filopodia in a concentration-dependent manner at 24 h. This is similar with western blot analysis, which revealed diminished the phosphorylated focal adhesion kinase (pFAK), phosphorylated non-receptor tyrosine kinase (pSrc), Ras-related C3 botulinum toxin substrate 1 (Rac1), cell division control protein 42 (Cdc42), and Ras homolog gene family member A (RhoA) expression levels. CONCLUSIONS: Overall, our data indicate that luteolin plays a role in controlling lung cancer cells' migration and invasion via Src/FAK and its downstream Rac1, Cdc42, and RhoA pathways. Luteolin might be considered a promising candidate for suppressing invasion and metastasis of lung cancer cells.
