Background: Small cell lung cancer (SCLC) tends to grow more rapidly and spread much faster than non-small cell lung cancer (NSCLC). A concurrent combination of chemotherapy and thoracic radiotherapy is suggested as the standard conventional treatment, but it is more challenging for elderly patients having pulmonary and cardiovascular comorbidities. Case presentation: Here we present a case of an 80-year-old male, current smoker diagnosed with SCLC in limited stage T3N0M0 (36mm right upper lobe, satellite nodule) in Dec, 2015. The standard concurrent chemoradiotherapy was not available for his comorbidities, which included chronic obstructive pulmonary disease (COPD) and angina pectoris. Furthermore, he and his family refused the recommended chemotherapy or radiotherapy exclusively. Alternatively, he received various non-conventional treatments including local radiofrequency hyperthermia, mistletoe, and Traditional Korean medicine including acupuncture, moxibustion and herbs since Jan. 2016. Despite the progression in primary tumor size, there have been no other distant relapse so far, and the patient has been in stable condition ever since. Conclusion: We suggest that a combination of various alternative treatments could be a candidate for elderly patients intolerable to conventional cytotoxic treatments.
Purpose: The purposes of this study were to develop an elderly care nursing education program and evaluate its effect on knowledge, attitudes and nursing performance of nurses toward older adults. Methods: A quasi-experimental study with a non-equivalent control group pretest-posttest design was used. Participants were recruited from two geriatric hospitals. The sample consisted of an intervention group (n=29) that participated in the educational program and a control group (n=30). The data were collected prior to and 6 weeks after the intervention using self-administered questionnaires. Data analyses utilized $x^2$-test, Fisher's exact probability test, and t-test. Results: After the intervention, knowledge and nursing performance about the elderly care increased significantly in the experimental group compared to the control group. However, there was no statistically significant difference between the groups in their attitudes toward the elderly. Conclusion: Findings indicate that to provide better nursing care to the elderly, nurses need to be able to practically apply the knowledge they have acquired from the elder-care nursing education programs in hands-on situations. To achieve this, nurses should work to change their personal attitudes about the elderly. Also, evidence-based guidelines for elderly nursing care, administrative support of institutions, and multidisciplinary approaches are required.
Purpose: As population of elderly people continues to grow, successful aging has risen to the top of the nursing science agenda. The successful aging includes maintenance of the cognitive and physical functions, as well as emotional well-beings. This study was carried out to evaluate effects of the environmental enrichments on cognition of institutionalized elderly. Method: The population was selected among the elderly aging over 65 residing at two of institutions. A quasi experimental design was used with non-equivalent control group. Study subjects were thirteen for each group. For the experimental group, physical, social and symbolic environmental enrichment program was provided for six weeks. The data were analyzed by repeated measure ANOVA and repeated measure ANCOVA using SPSS Win 11.0. Result: Compared to control group, the experimental group showed a significant difference on DSF(F=3.29, p=.046), and TMTA(F=4.76, p=.013) of cognitive function, and depression (F=5.56, p=.007) of emotional distress after 1 and 12 weeks of environmental enrichment program. Conclusion: Findings indicate that physical, social, and symbolic environmental enrichment was effective to partially prevent from cognitive decline, and to decrease emotional distress of elderly. As a nursing intervention, environmental enrichment program for elderly should be expanded for nursing practice to promote healthy aging and to offer support to the growing population of elderly. Further research should be conducted to evaluate the effect on the community elderly.
Objectives : The purpose of this study was to examine the effects of an oral health promotion program. And suggest that education on oral health should be given by a professional. Methods : Sixty-four people over the age of 65 years were selected from three elderly care facility located in Gyeongsangbuk-do, A city. From June 7 to August 19, 2016, an oral health promotion program was given to two experimental groups (N=21 and 20) for a total of eight times. One program was conducted by a professional and the other by a non-professional although both were considered experts. Additionally, a control group(N=23) was a observed. Results : While both experimental groups showed a positive change from the oral health promotion program, the experimental group that was given the oral health promotion program by a professional showed more positive changes in oral cavity function. Conclusions : By using professionals, it is possible to provide a health promotion program that actively monitors the oral cavity of the elderly as well as provide elderly care services for oral hygiene. Appropriate elderly health policies and oral health business model for elderly are needed.
This study was performed to estimate living cost for the elderly couple living in a city in Korea. Living cost means expenditure per month for elderly couple. It was assumed that the elderly couple will need different living cost according to their circumstances. The circumstances are health status, retirement status, and the level of living they want. The subjects were the elderly couple households over the age 65 of household head. Total number of subject was 1,649 households. Used data was Annual Report surveyed by National Statistical Office on the Family Income and Expenditure. Analysis of data was done through frequency, percentage, means, median using SAS Program. The results of this study were as follows: Their standard living cost was 844,980 won by pure relative standard line and 842,300 won by quasi relative standard lines. And minimum living cost was 713,400 won by the former, by the latter was 557,600 won (3/2 of median). And abundant Living cost was 1,068,020 won by the former, by the latter 1,263,450 won. The living cost of elderly households was about 81-83%, comparing with non-elderly households. Among the item of expenditure, the proportion of housing and medical care cost was larger than any other items.
This study aims to enhance the accuracy of severity classification by examining the usage patterns and characteristics of emergency department visits. It focuses on adult and elderly patients who visited a public hospital in Seoul. This descriptive study retrospectively reviewed the electronic medical records of patients who visited the emergency department of a public hospital between November and December 2023. The total number of participants was 1,033, with 46.4% (n=479) being elderly and 53.6% (n=554) being adults. The chief complaints of the participants were as follows: for the elderly, nervous system symptoms at 8.2% (n=85) and digestive symptoms at 7.5% (n=77) were the most common, while for adults, gastrointestinal symptoms at 11.0% (n=114) and trauma at 8.6% (n=89) were more prevalent. In the case of the elderly, patients classified as urgent accounted for the highest percentage at 23.9% (n=247), while for adults, non-emergency were more prevalent at 32.2% (n=333). The initial severity classification error rate for elderly patients in the urgent was 3.8%, indicating that the suitability of KTAS for elderly patients with high severity was low. To minimize severity classification errors and enhance KTAS accuracy, it's essential to address its current limitation of only classifying adults and children separately by developing a KTAS classification system that reflects the diverse characteristics of elderly patients.
Purpose: The aims of this study were to describe general characteristics and needs of home health care, and to find the differences between home health care needers and non-needers. Method: In this study, 642 subjects participated who lived in Muan, Jollanam-do. Data were collected in August 2001 using a self-reported questionnaire. The questionnaire was a revised and simplified form of the Organization of Community Health System Program at the Seoul National University. Collected data were analyzed through Kruskal-Wallis test. t-test. and Chi-squire for cross-sectional analysis. Result: The average age of the subjects was 52.6 years and 33.3% of them aged over 65 years. Twenty six percent of them had chronic degenerative diseases. The percentages of hypertension patients and D.M. patients were 6.4% and 2.5%, respectively. The number of family members was 2.95 on the average, 2.19 in cases of families with the elderly and 3.33 in cases of families without the elderly. The rate of disability of the elderly was 10.5%. Marital status (p=.000), the number of family members (p=.000), education (p=.000), job (p=.000) and health insurance (p=.027) were significantly different between home care needers and non-needers. Home care needers had less living expenses (p=.001), more frequent hospital admissions (p=.004), higher chronic disease rate (p=.000) and more frequent visits to public health center (p=.027) than non-needers. Home care needers who wanted free service were twice as many as non-needers. Conclusion: Home care need was very high in rural areas and the needers had worse characteristics (low educational level. low income, no job and no family). Therefore, it is necessary to develop cheaper and more accessible services for home care needers in rural areas.
Purpose: The study sought to identify the differences in chronic diseases and physical activity in elderly women by BMI. Method: The subjects of this study were 644 elderly women 60-80-years-of-age living at home. The research instruments were physical activity levels and chronic diseases. Subjects were given a self-report questionnaire. Data were analyzed using the SPSS win program. Result: Hypertension, prevalence of diabetes mellitus and hyperlipemia were significantly different in the subjects according to body mass index, being higher in obese subjects than in non-obese subjects. Physical activity in each of the body mass index groups did not differ significantly. Conclusion: Obesity increases the risk of chronic diseases. This knowledge could help elderly women control their weight, reduce chronic diseases, and ultimately, gain better health.
Using the 1996 National Survey of Family Income and Expenditure Survey this study investigated the effect of employment status on food expenditure patterns of elderly households. The effect of employment status of elderly households was not significant in food-at-home expenditures while it was significant in food-away-from-home expenditures. As the results of this study income age family size education number of earners home ownership and region were found to influence the food-at-home expenditures of elderly households whereas income family size number of earners and education were significantly associated with the food-away-from-home expenditures of elderly households.
To investigate the effects of wearable near-infrared ray-emitting knee pads on knee pain among elderly adults in Korea. Randomized controlled trial evaluating the effects of near-infrared rays (NIR) on knee pain in Korean elderly adults. Five community-based research facilities (two welfare centers, a senior citizen center, and two churches). Forty-seven participants aged 65 years and older who had experienced knee pain. The experimental group (n = 25) wore NIR-emitting knee pads for one month at nighttime while sleeping. The control group (n = 22) wore knee pads without NIR. Demographic characteristics, intensity and duration of knee pain, amount of analgesic medication used, range of motion, gait speed, and health-related quality of life were collected using questionnaires. The experimental group showed decreased intensity (t = -6.17, p < 0.001) and duration (t = -3.34, p = 0.002) of knee pain and reduced analgesic use (t = -2.30, p = 0.026) compared to the control group. NIR may be an effective non-pharmacological option for relieving knee pain in elderly adults.
