Aims: To study the effectiveness of human recombinant endostatin injection (Endostar(R)) combined with cisplatin doublets in treating advanced non-small cell lung cancer (NSCLC), and to evaluate outcome by CT perfusion imaging. Methods: From April 2011 to September 2014, 76 patients with advanced NSCLC who were treated with platinum-based doublets were divided into group A (36 patients) and group B (40 patients). Endostar(R) 15mg/day was administered 4 days before chemotherapy and combined with chemotherapy from day 5 in group A, and combined with chemotherapy from the first day in Group B. Endostar(R) in the two groups was injected intravenously for 14 days. Results: Treatment effectiveness in the two groups differed with statistical significance (p<0.05). Effectiveness evaluated by CT perfusion imaging, BF, BV, MTT and PS also demonstrated significant differences (all p<0.05). Adverse reactions in the two groups did not significantly vary (p> 0.05). Conclusions: The response rate with Endostar(R) administered 4 days before chemotherapy and combined with chemotherapy from day 5 in group A was better than Endostar(R) combined with chemotherapy from the first day, and CT perfusion imaging could be a reasonable method for evaluation of patient outcomes.
Background: We strived to evaluate the status of nivolumab use and associated factors on the clinical efficacy of the drug. Methods: The study was retrospectively conducted in patients who had been administered nivolumab at least once at the cancer center of Seoul National University Hospital from June 2015 to April 2017. Data were collected from electronic medical records. A medication-use evaluation was performed based on the American Society of Health-System Pharmacists mediation-use guidelines. Results: Sixty-six of the 74 patients (89.2%) showed indications approved for nivolumab use by the Korean Ministry of Food and Drug Safety (MFDS; n=55) or the US Food and Drug Administration (FDA; n=11). Approximately 73.0% of the patients were administered the approved dose of 3 mg/kg but 25.7% were administered an unapproved fixed dose of 100 mg. The overall response rate was 21.7%, and the response rate of non-small cell lung cancer patients, who accounted for the largest number of indications, was 18.8%. Adverse reactions were found in 90.1% of the patients and were mostly mild (86%). The expression of programmed death-ligand 1 (PD-L1) was analyzed as a factor affecting treatment response (p=0.028, odds ratio [OR]=11.331). Conclusion: PD-L1 expression was found to affect treatment response. However, caution is required while using an unapproved dosage and in the absence of monitoring for effectiveness and safety. Therefore, an effective protocol or instruction manual for the proper use of nivolumab should be considered.
The nucleoside analogue gemcitabine (2', 2-difluorideoxycytide) is potential against a wide variety of solid tumors and considered to be one of the most active drugs in the treatment of non-small cell lung cancer (NSCLC). In this study, we investigated the signals of gemcitabine-induced apoptosis, especially in point of caspase pathway in A549. We exposed A549 cells to gemcitabine for dose/time dependent manner and the results showed that gemcitabine induced apoptotic cell death in a time/dose-dependent manner. We also treated to gemcitabine and Z-VAD-fmk as a pan-caspase inhibitor for 24 hours. Gemcitabine alone induced 35.3% cell death, and co-treatment with gemcitabine and Z-VAD-fmk induced 15.1% apoptotic cell death. Our results demonstrated that Z-VAD-fmk as a pan-caspase did not completely block the gemcitabine-induced apoptosis. Western blotting analysis showed that gemcitabine increased caspase-3, active caspase-8, p21 and p53 protein expressions in A549. Co-treatment with Z-VAD-fmk completely blocked caspase-3 and active caspase-8 protein expressions, but did not change the level of p21 and p53 protein expressions. Our data indicate that gemcitabine induced apoptosis through caspase-dependent and -independent pathways in A549.
연구배경: 악성 흉수가 있는 환자는 예후가 좋지 않아 이를 감별하는 것은 임상적으로 중요한 일이다. 하지만 흉수 내 세포진 검사가 음성일 경우 진단이 쉽지 않다. 따라서 본 연구는 흉수 탈락 세포에서 추출한 DNA에서 종양억제 유전자로 알려진 retinoic acid receptor b2 (RARB2)와 p16 유전자의 과메틸화 측정이 악성 흉수 진단에 도움이 될 수 있을지를 확인하기 위하여 실시하였다. 방법: 43명의 환자에서 흉수를 천자하여 흉수 내 탈락 세포에서 메틸화 특이 PCR 방법으로 RARB2와 p16 유전자의 과메틸화를 측정하고, 이를 흉막 생검 및 흉수 세포진 검사법과 비교하였다. 결과: 43명의 환자 중 17명은 폐렴, 결핵에 의한 양성 흉수 환자였고, 26명은 흉막 침범 폐암 환자였다. 17명의 양성 흉수에서는 흡연 유무와 상관없이 RARB2와 p16 유전자의 과메틸화가 관찰되지 않았다. 26명의 악성 흉수에서 과메틸화는 각각 14명, 5명에서 관찰되었으며, 두 유전자 중 어느 한 쪽이라도 과메틸화가 생긴 경우는 15명이었다. 흉막 생검, 흉수 내 세포진 검사, RARB2 단독, p16 단독, 두 유전자 동시 측정 과메틸화 검사의 민감도는 각각 73.1%, 53.8%, 53.8%, 19.2%, 57.7%이었고, 음성 예측도는 각각 70.8%, 58.6%, 58.6%, 44.7%, 60.7%로서 두 유전자를 동시에 검사할 때의 민감도와 음성 예측도는 흉막 생검보다는 낮았지만, 흉수 내 세포진 검사보다는 높았다. 또, 소세포암에서 p16를 이용한 민감도가 14.3%로 떨어져서 조직 유형에 따른 차이를 보였다. 결론: 본 연구 결과에서 p16과 RARB2 유전자의 과메틸화의 발견은 악성 흉수를 양성과 감별하는데 높은 특이도를 보였고, 민감도 역시 흉수 내 세포진 검사보다 높은 방법이었다. 폐암의 세포 유형에 따른 분자 생물학적 병리를 이해하고 적절한 유전자를 선정한다면 이런 결과는 더욱 향상될 수 있을 것으로 생각된다.
히스톤 디아세틸라이제 저해제(HDACI)는 최근에 새로운 미래형 항암제로 주목을 받고 있으며 다른 항암요법 및 치료제와의 병용치료에도 효과적으로 적용될 수 있을 것으로 기대하고 있다. HDACI은 다양한 조직기원의 암세포에서 증식억제 및 세포사멸 유도능이 시험되어 왔으나 비소세포폐암 세포에서 그 작용 및 기전이 명확히 조사된 바가 없다. 본 연구는 HDACI 중의 하나인 sodium butyrate (SB)를 비소세포폐암 세포주인 H460에 처리하여 세포생존율, 세포주기 분석, 세포사멸도를 평가하고, 이와 관련하여 세포사멸 관련 단백질, p53, ERK의 변화를 조사하고자 하였다. 3가지 다른 농도(2.5, 7.5, 20 mM)의 SB에 H460 세포가 48시간 노출되었을 때, 세포 생존율은 농도의존적으로 감소되었으나 7.5 mM 이상의 농도에서 대조군에 비해 유의한 생존을 감소를 보였고, 20 mM에서 생존을 50% 전후를 나타냈다. SB노출은 H460 세포의 사멸을 유발하였는데, 세포사의 유형은 아포토시스와 괴사가 동시에 발생함이 Annexin-V 분석으로 확인되었다. H460 세포에서 SB에 의해 유발되는 뚜렷한 세포주기의 변화양상은 G2/M기정지였으며, 이러한 세포주기의 지연현상으로 세포사멸이 초래되는 것으로 생각된다. SB처리는 아포토시스 발생관련 효소인 caspase-3과 caspase-7의 활성화를 유발하였으며, 이에 의한 PARP 단백 질의 분절화도 관찰되었다. 동시에, 항세포사별 단백질인 XIAP의 단백질 함량은 감소함을 보였다. SB 노출에 의한 세포주기의 G2/M기의 정지현상과 관련하여는 p53 단백질의 증가가 주목할 만한 하였다. SB의 H460세포에의 처리는 일반 ERK단백질의 함량 변화를 유도하지 않았으나, 인산화형의 ERK는 SB처리농도에 의존적으로 그 단백질 함량이 감소하였다. 이는 ERK가 비소세포폐암 세포인 H460에서 세포생존 및 유지와 관련된 단백질의 인산화에 계속적으로 관여하고 있다는 사실을 암시한다. 즉, SB의 처리는 ERK의 인산화를 유의하게 억제하는 기전과 관련이 있을 가능성이 높다고 추측된다. 향후 SB의 노출에 의한 PERK 감소 기전에 대한 연구가 추가적으로 진행되면 SB의 더욱 효율적인 암세포 사멸 유도 전략수립에 도움을 줄 수 있을 것이라 예상된다.
연구배경: TRAIL 은 death 수용체(DR4, DR5)와 decoy 수용체(DcR1, DcR2)라는 독특한 수용체 시스템 덕분에 정상세포는 보호하고 종양선택적 살상능을 가진 유망한 항암제로 주목받고 있다. DR5 TRAIL 수용체는 wild-type p53 세포에서 DNA 손상항암제인 doxorubicine에 의해 발현 유도되는 p53-조절 death 수용체 유전자로 처음에 발견되었다. 폐암 조직에서 이러한 DR5와 p53 발현에 관연 연구 보고는 많지 않다. 본 연구에서 비소세포폐암 조직에서 면역조직화학적 방법을 이용하여 DR5 발현과 p53과의 상관관계를 중심으로 분석하였다. 방법: Avidin-biotinylated horseradish peroxidase complex를 이용하여 89예의 수술적 절제된 비소세포폐암 포르말린 고정조직에서 면역조직화학적 염색을 시행하였다. 1차 항체로는 anti-DR5 polyclonal antibody (Pro Sci Inc., Poway, CA)와 anti-p53 monoclonal antibody (DO-7, Novocastra, Inc, Manhasset, NY)를 이용하였다. DR5 발현에 대한 면역조직화학적 판정은 면역염색의 범위와 강도를 합산하여 다음과 같이 판정하였다; 0, non-staining; 1+, weak and diffuse(>50%) or focal (<50%) or moderate and focal; 2+, strong and focal or moderate and diffuse ; 3+, strong and diffuse. 2+ 이상을 고발현(high expression)으로 분류하였다. p53 발현 정도에 따른 p53 wild type과 mutant type의 구분은 3+의 강양성인 경우를 mutant type으로 판정하였고, 나머지 0, 1+, 2+ 발현은 wild type p53으로 판정하였다. 결과: 주변 정상폐조직에서의 DR5발현은 기관지점막에서 89%의 고발현율을 보였으나, type I, II 폐상피세포, 림프구, 평활근세포 등의 정상 세포에서는 거의 발현되지 않았다. 폐암 조직에서 DR5 고발현율은 편평상피암에서 28% (15/53), 선암에서 47% (15/32), 대세포암에서 50% (2/4)의 빈도를 보여 전반적으로 36%의 고발현율을 보였다. DR5 고발현율은 임상병기, 생존기간 등 임상 지표와 유의한 상관관계를 보여주지 못하였지만 p53 발현과는 유의한 역상관관계를 보여주었다. 결론: 비소세포폐암 조직에서 면역조직화학적 분석을 통하여 DR5 발현이 기관지점막을 제외하고는 종양 선택적임을 확인하였고 p53 발현과 유의한 역상관관계가 있음을 확인하였다. 이는 DR5가 p53 조절 유전자임을 in vivo에서 확인한 유력한 증거일 수 있다고 생각된다.
연구 배경 : 비소세포 폐암의 암화 과정에서 에스트로겐과 프로제스테론 단백의 역할에 대한 면역조직화학 염색을 이용한 연구들이 진행 중이다. 그러나 이 연구들은 아직 일치된 결과를 보이고 있지 않으며 이는 상용하는 면역조직화학 염색법이 한 문제로 제시되고 있다. 저자 들은 최근 새로 개발된 조직미세배열법을 이용하여 비소세포 폐암 환자의 조직에서 이들 호르몬 수용체 발현을 연구하였다. 대상 및 방법 : 대상은 70예의 비소세포 폐암 환자로 남성이 74%, 여성이 26%이었다. 이들의 포르말린 고정, 파라핀 포매조직을 이용하여 조직미세배열을 구축하였다. 가열을 통한 항체 재생 후에 폐암 조직에서 일차 단일클론 항체 (ER1D5와 PR1A6)를 이용한 면역조직화학 염색을 시행하였다. 결 과 : 흡연력은 현재 흡연자가 49%이었고, 비흡연자와 금연자는 각각 27%와 24%이었다. 폐암의 조직학적 분류는 편평상피세포암이 34예이었고, 선암, 편평상피선암, 기타 세포형은 각각 24예, 9예와 3예이었다. 단일클론 항체를 이용한 염색에서 양성 결과를 보이는 비소세포 폐암 세포는 관찰되지 않았다. 결 론 : 미세조직배열법을 이용한 에스트로겐과 프로제스테론 수용체 연구는 모든 비소세포 폐암 조직에서 음성 결과를 보였다. 현재 면역조직화학 염색에 사용되는 호르몬 수용체가 비소세포 폐암 조직에서 발현이 되지 않을 가능성을 시사해주는 소견으로 향후 적절한 항체들을 이용한 추가적인 연구가 필요하겠다.
Background: The objective of this research is the computed axial tomography (CT) imaging grading of radiation induced pneumonitis (RP) and its correlation with clinical and radiotherapeutic parameters. Materials and Methods: The chest CT films of 20 patients with non-small cell lung cancer who have undergone threedimensional conformal radiation therapy were reviewed. The proposed CT grading of RP is supported on solely radiological diagnosis criteria and distinguishes five grades. The manifestation of RP was also correlated with any positive pre-existing chronic obstructive pulmonary disease (COPD) history, smoking history, the FEV1 value, and the dosimetric variable V20. Results: The CT grading of RP was as follows: 3 patients (15%) presented with ground glass opacity (grade 1), 9 patients (45%) were classified as grade 2, 7 patients (35%) presented with focal consolidation, with or without elements of fibrosis (grade 3), and only one patient (5%) presented with opacity with accompanying atelectasis and loss of pulmonary volume (grade 4). Both univariate and multivariate analysis revealed as prognostic factors for the radiological grading of RP the reduction of FEV1 and the V20 (P=0.026 and P=0.003, respectively). There was also a significant (P<0.001) correlation of radiological grading of RP with FEV1 and V20 (spearman rho 0.92 and 0.93, respectively). Conclusions: The high correlation of the proposed radiological grading with the FEV1 and the V20 is giving a satisfactory clinical validity. Although the proposed grading scale seems relevant to clinical practice, further studies are needed for the confirmation of its validity and reliability.

Objective : This study was performed to assess the efficacy of GKS in patients with ten or more brain metastases. Methods : From Aug 2002 to Dec 2007, twenty-six patients (13 men and 13 women) with ten or more cerebral metastatic lesions underwent GKS. The mean age was 55 years (32-80). All patients had Karnofsky performance status (KPS) score of 70 or better. According to recursive partitioning analysis (RPA) classification, 3 patients belonged to class I and 23 to class II. The location of primary tumor was lung (21), breast (3) and unknown (2). The mean number of the lesions per patient was 16.6 (10-37). The mean cumulated volume was 10.9 cc (1.0-42.2). The median marginal dose was 15 Gy (9-23). Overall survival and the prognostic factors for the survival were retrospectively analyzed by using Kaplan Meier method and univariate analysis. Results : Overall median survival from GKS was 34 weeks (8-199). Local control was possible for 79.5% of the lesions and control of all the lesions was possible in at least 14 patients (53.8%) until 6 months after GKS. New lesions appeared in 7 (26.9%) patients during the same period. At the last follow-up, 18 patients died; 6 (33.3%) from systemic causes, 10 (55.6%) from neurological causes, and 2 (11.1 %) from unknown causes. Synchronous onset in non-small cell lung cancer (p=0.007), high KPS score (${\geq}80$, p=0.029), and controlled primary disease (p=0.020) were favorable prognostic factors in univariate analysis. Conclusion : In carefully selected patients, GKS may be a treatment option for ten or more brain metastases.
Background: With development and application of new and effective anti-cancer drugs, the median survival post-progression (SPP) is often prolonged, and the role of the median SPP on surrogacy performance should be considered. To evaluate the impact of the median SPP on the correlation between progression-free survival (PFS) and overall survival (OS), we performed simulations for treatment of four types of cancer, advanced gastric cancer (AGC), metastatic colorectal cancer (MCC), glioblastoma (GBM), and advanced non-small-cell lung cancer (ANSCLC). Materials and Methods: The effects of the median SPP on the statistical properties of OS and the correlation between PFS and OS were assessed. Further, comparisons were made between the surrogacy performance based on real data from meta-analyses and simulation results with similar scenarios. Results: The probability of a significant gain in OS and HR for OS was decreased by an increase of the SPP/OS ratio or by a decrease of observed treatment benefit for PFS. Similarly, for each of the four types of cancer, the correlation between PFS and OS was reduced as the median SPP increased from 2 to 12 months. Except for ANSCLC, for which the median SPP was equal to the true value, the simulated correlation between PFS and OS was consistent with the values derived from meta-analyses for the other three kinds of cancer. Further, for these three types of cancer, when the median SPP was controlled at a designated level (i.e., < 4 months for AGC, < 12 months for MCC, and <6 months for GBM), the correlation between PFS and OS was strong; and the power of OS reached 34.9% at the minimum. Conclusions: PFS is an acceptable surrogate endpoint for OS under the condition of controlling SPPs for AGC, MCC, and GBM at their limit levels; a similar conclusion cannot be made for ANSCLC.

④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)

제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
