Treatment of Metastatic Cervical Cancerous Nodes from an Unknown Primary Site (근원불명 전이성 경부암의 치료)

  • Guidelines for the optimal management of patients with cervical cancerous nodes from an unknown primary site, has not been yet settled. However. radical treatment has been advocated employing either a surgery, or radiotherapy, or a combination of the two in relation to the location, stage and histologic features of the nodes in the neck. Of 43 patients who presented with 'Cervical metastases from an unknown primary' since 1971, $31.2\%$ survived 3 years after adequate management. Combined modality with surgery and radiation showed more favorable treatment results: in local control rate, 3 year survival rates and the subsequent appearances of the contralateral side of neck node through the retrospective analysis. In 11 cases, the primary tumors became apparent later, carcinoma of the hypopharynx, being the most frequent site, rather than the nasopharynx.

Surgical Treatment of Esophageal Cancer (식도종양의 외과적 치료)

  • Fourty nine patients out of 127 esophageal cancer were managed surgically from January 1986 to December 1991, at the Department of Thoracic and Cardiovascular Surgery, Jeonbuk National University Hospital. Most frequent preoperative symptom was dysphagia and its mean duration was 3.1 months. In histopathologic examination, squamous cell carcinomas were 44 cases [89.8% , and adenocarcinomas 5 [10.2% . The tumor location were the upper esophagus in 6.1%, middle esophagus in 57.2%, lower and cardiac portion of stomach in 36.7%. Involved and metastatic organs, which were detected perioperatively, were celiac lymph nodes in 6 cases, aorta 2, stomach 2, pericardium 2, cervical lymph node 1. The esophagus was resected radically, and the procedures for esophageal replacement were performed with esophagogastrostomy in 45 cases, esophagocologastrostomy 3, and esophagojejunostomy 1. Postoperative complications occurred in 16 cases [hospital morbidity = 32.6% ,anastomotic leak 3, anastomotic stricture 2, respiratory insufficiency 2, hemoperitoneum 1, chylothorax 1, intussusception 1, empyema 1, non-A,non-B hepatitis 1, and mediastinitis 1. Hospital deaths were experienced 3 cases [ hospital mortality = 6.1% . The 6 month, one, two, and five year actuarial survival rates were 85.7%, 71.4%, 57.1%, and 27.9%, respectively. One year survival rates of stages were 100% in stage I, 90.9% in stage IIa, 63.6% in stage IIb, 25.0% in stage III, and 7.2% in stage IV.

  • Castleman's disease (CD) is an uncommon benign lymphoproliferative disorder of unknown etiology. Histopathologically, it is divided into three types: hyaline-vascular, plasma cellular, and multicentric CD. The mass usually presents asymptomatically; however, it can cause non-specific symptoms such as fever and fatigue. Although CD can be found wherever lymph nodes are present, 75% of cases are reported in the mediastinum, and occurrence in the head and neck is rare. Herein, we report a rare case of CD presenting as a superficial mass in the temporal region. To the best of our knowledge, this is the first report of temporal CD in Korea involving a young patient.

  • 본 연구는 한국형 Reach File을 개발하기 위한 선행 연구로써 한국형 Reach File의 필수 구성요소 중 하나인 속성을 설계하는 것을 목표로 하였다. 이를 위해 우선 미국 Reach File 구축 사례를 검토하였고, 특히 최종 버전인 RF3(Reach File version 3)의 속성 내역을 정리하여 설계에 참조하였다. 국내 TMDL 업무 분석을 수행하여 업무 활용 측면에서 요구되는 속성도 모델링을 통해 속성 설계에 반영하였으며, 아울러 아크-노드 모델(Arc-node model) 기반의 네트워크 형식의 공간자료를 구축하는 것을 가정하여 네트워크 분석에도 활용이 가능하도록 선형 및 점형 도형자료 간 위상관계도 속성 설계에 반영하였다. 연구 결과에서 한국형 Reach File의 속성은 '위상(topology)', '위치(location)', '주제(theme)'의 세 가지 대분류에 따라 모델링되었으며, 각 분류에 따라 세부 속성이 정의되었다. 고유 식별자를 포함하여 선형 도형자료의 경우 총 53개, 점형 도형자료의 경우 총 13개의 세부 속성이 정의되었고, 각 속성에 대한 속성명, 필드명, 데이터 타입 및 길이 등에 관한 상세 설계서가 작성되었다. 본 연구를 통해 개발된 속성 설계서는 향후 한국형 Reach File 구축에 직접 활용이 가능하며, 따라서 한국형 Reach File의 구체적인 개발 방향의 설정에도 기여할 것으로 예상된다. 향후 연구에서는 속성 설계서 결과를 바탕으로 한국형 Reach File 구축 및 활용에 관한 연구가 뒤따라야 한다.

Improved Hierarchical Prefix Delegation Protocol for route optimization in nested NEMO (중첩된 NEMO에서의 경로 최적화를 위한 개선된 계층적 프리픽스 할당 프로토콜)

  • Hierarchical Prefix Delegation (HPD) protocol refers to a type of solution to problems inherent in non-optimal routing which occurs with Network Mobility (NEMO) basic solution. However, because HPD cannot improve the micro-mobility problems, problem surfaces each time Mobile Network Node (MNN) changes the attachment point; as happens also in a Mobile IPv6 (MIPv6) protocol in sending Binding Update (BU) messages to Home Agent (HA) / Correspondent Nodes(CNs). By applying Hierarchical Mobile IPv6 protocol concept to HPD, this study proposes an algorithm for effectively handling micro-mobility problems which occur with HPD in a nested NEMO environment. By sending BU only to nearby Mobility Anchor Point(MAP) during MNN location change within a MAP's domain, the proposed protocol will alleviate service disruption delays and signaling loads during the handover process, overcoming the limitations of HPD.

  • MANET has a highly dynamic topology because it consists of only mobile nodes. Various attacks using these characteristics exist. Among them, damage of the attacks based flooding such as DoS or DDos is large and traceback of the attack node is not easy. It is because route information by moving of intermediate nodes which pass the data changes frequently. In this paper, we propose table-based traceback technique to perform efficient traceback although route information by moving of nodes changes frequently. Cluster head manages route management table in order to form cluster status table and network topology snapshot for storing the location information of mobile nodes when cluster member nodes change. Also, bloom filter is used to reduce the amount of storing route information. The performance of the proposed technique is confirmed through experiment.

Adaptive Wireless Localization Filter Containing NLOS Error Mitigation Function

  • Range-based wireless localization system must measure accurate range between a mobile node (MN) and reference nodes. However, non-line-of-sight (NLOS) error caused by the spatial structures disturbs the localization system obtaining the accurate range measurements. Localization methods using the range measurements including NLOS error yield large localization error. But filter-based localization methods can provide comparatively accurate location solution. Motivated by the accuracy of the filter-based localization method, a filter residual-based NLOS error estimation method is presented in this paper. Range measurement-based residual contains NLOS error. By considering this factor with NLOS error properties, NLOS error is mitigated. Also a process noise covariance matrix tuning method is presented to reduce the time-delay estimation error caused by the single dynamic model-based filter when the speed or moving direction of a MN changes, that is the used dynamic model is not fit the current dynamic of a MN. The presented methods are evaluated by simulation allowing direct comparison between different localization methods. The simulation results show that the presented filter is more accurate than the iterative least squares- and extended Kalman filter-based localization methods.

Schwannoma of the tongue: a case report with review of literature

  • Background: Schwannomas (or neurilemmomas) of the tongue are benign, usually solitary, encapsulated masses derived from Schwann cells. Clinical evidence indicates that schwannoma is painless and slow growing. In general, schwannoma is treated by surgical excision. Here, we describe a case of schwannoma of the tongue, include a review of the literature from 1955 to 2016, and provide data on age, gender, location, presenting symptoms, size, and treatment methods. Case presentation: A 71-year-old female patient presented with a swelling at the base of the tongue of unknown duration. Magnetic resonance images (MRI) showed a large well-circumscribed solid mass and no significant lymph node enlargement. The mass was excised without removing overlying mucosa. Conclusions: The authors report a case of lingual schwannoma that was completely removed intraorally without preoperative biopsy. No sign or symptoms of recurrence were observed at 12 months postoperatively.

  • In this paper, we propose a safety management system which prevents the occurrence of crime and accident from rising in school neighborhood. This system offers a monitoring method for safety of children which make intelligent space from environment and location sensing data through wireless communication between sensors attached to children and fixed sensor network in school neighborhood. Also, We propose a strategic safety management plan using opportunistic network when node is out of school zone network. Many students can easily receive the service of low cost from WSN-based School zone network than existing service from GPS individually by using our scheme.

  • To provide good-quality services of multimedia in wireless mobile networks, QoS for multimedia traffic should be guaranteed. However, since the existing QoS guaranteed schemes are designed for fixed networks, they are inadequate in the wireless network environments. Recently, as the size of cells like micro cell or pico cell becomes smaller. Mobile hosts on micro cellular networks, change their location frequently. It causes many drawbacks such as delayed delivery, packet losses and signaling overheads. Because the existing schemes supporting interoperability between RSVP and Mobile If are designed for macro cell environments. Their uses in micro cellular networks can be restricted. In this paper, we propose the CRSVP(Cellular RSVP Protocol) which supports QoS in micro cellular network environments.
