Outcome of Surgery and Post-Operative Radiotherapy for Major Salivary Gland Carcinoma: Ten Year Experience from a Single Institute

  • Aims: To determine the clinical characteristics, pathological features, local and distant failure patterns in patients with carcinoma of major salivary glands treated with surgery and postoperative radiotherapy (PORT). Materials and Methods: We retrospectively reviewed 106 cases of major salivary gland tumor seen at our centre (1998-2008). Sixty five cases of major salivary gland carcinoma were selected for analysis (exclusions: benign, palliative, non-carcinomas). The patient population treated by surgery and PORT was divided into two groups: 1) Patients who underwent surgery and immediate PORT (Primary PORT); 2) Patients with recurrent carcinoma who underwent at least two surgeries and received PORT in the immediate post-operative period of the last performed surgery (Recurrent PORT). Recurrence free survival (RFS) was assessed using the Kaplan-Meier method. Results: Median age was 35 years with a male: female ratio of 1.3:1. The majority of cancers were located in the parotid gland (86.2%) and the most common histology was mucoepidermoid carcinoma (43%). Thirty nine cases (60%) were primary while 26 (40%) were recurrent. Optimal surgery was performed in 59/65 patients (90.8%). 43 patients (66.2%) underwent neck dissection, of which 14 (32.5%) had nodal metastasis. Overall, 61 (93.8%) patients complied with the prescribed radiotherapy. Median dose of PORT was 60 Gy. Median follow-up was 13.1 months (range 2-70). Relapse free survival was 50.4% at 60 months. Some 12 cases (18.5%) recurred with a median time to recurrence of 16.9 months. Conclusions: Surgery and PORT is an effective treatment for major salivary gland carcinoma with over 90% compliance and <20% recurrence. Early treatment with postoperative radiotherapy may increase the survival rate in major salivary gland carcinoma patients.

  • Various away methods are required in the electrical resistivity survey in order to find anomalous zone reliably. Array methods are classified as several groups. Among these group, a curved survey along the fixed elevation is designed to increase the mobility of men and survey equipments at the rough terrain. Another method is performed at the survey using inclined, curved, and horizontal boreholes. A survey can also be conducted in an arbitrary location by measurements of potentials for a multi sources. The complex data acquired using various away methods are represented by a correct images reconstructed from the 3D inversion. The element division is applied to the region in which the boreholes are curved and inclined because of a spatial discrepancies between the coordinate of each electrode and the nodal point in a model. The resistivity images are obtained from a good agreement for the anomalous zones in open slope and in survey using an inclined borehole.

  • An analysis of convection, in a fully developed laminar steady flow through the vertical square duct under the condition of variational symmetric heat flux, is considered. Finite element solution algorithm by Galerkin's method with triangular elements and linear interpolation polynominals for the temperature and velocity profiles are derived for the vertical square duct. The comparison of temperature distribution due to variational symmetric heat flux in the duct were made with available the other data when the condition of peripheral heat flux were uniform and zero. Numerical values for the dimensionless temperatures and Nusselt numbers at selected Rayleigh numbers and pressure gradient parameters were obtained at a few nodal points for the vertical square ducts and effects of corner in the duct were investigated.

Significance of nm23, Bcl2 and Bax Expression in Papillary Thyroid Carcinoma (유두상 갑상선암에서 nm23, Bcl2, Bax 발현도의 임상적 의의)

  • Objective: The nm23 gene has been identified as a potential metastasis suppressor gene in various human neoplasms. Both Bcl-2, which promotes cell survival, and Bax, which promotes cell death, have been considered as major factors in controlling the apoptotic pathway. This study was carried out to determine whether these markers are useful in distinguishing potential intrinsic differences in tumor virulence of papillary thyroid cancers. Material and Method: The expressions of nm23, Bcl-2 and Bax have been evaluated using immunohistochemical techniques in 100 pure papillary thyroid cancers and 20 metastatic lymphnodes. The intensity of immnunoreactivity was graded on arbitrary four point scale(grade 0 or 1 : negative reactivity, grade 2 or 3 positive reactivity). The immunoreactivities were analyzed in relation to TNM atage, AMES score, local recurrence and distant metastasis, and that of metastatic LNs was compared with the tumors. Results: The expression of Bcl-2 and bax did not show any statistical differences by TNM stage, AMES score, recurrence, distant metastasis and also between the tumor and metastatic LN. However, the nm23 showed higher expression of Ki67 in distant metastasis than in control group and in metastatic LNs than in the tumors(p<0.05). Conclusion: Although the expression of Bcl-2 and Bax protein showed no correlation with clinical parameters representing tumor virulence, the nm23 expression could be an useful prognostic factor, especially in predicting nodal or distant metastasis in papillary thyroid cancer.

  • Since the subpanels of polygonal-section shell have the corners of an obtuse angle larger than 90 degree unlike general plate or box-section structures, this could have an influence on forming nodal lines against local plate buckling or stress distributions. However, there is not sufficient material in the relevant study results or design recommendations. The very feasible models of the initial imperfections were acquired through the literature studies and then the parametric studies were conducted along with the initial imperfection models by using the finite element method. The parameters like the size of residual stresses, the portion of compressive residual stresses, and steel grades were considered. From the parametric studies, it was found that the maximum residual stress is more influential factor than the distribution pattern of residual stresses. In addition, The design strength equations for the simply supported plates can be applicable to the determination of the local buckling strength of the polygonal cross-section shell structures.

Postoperative radiotherapy in salivary ductal carcinoma: a single institution experience

  • Purpose: We reviewed treatment outcomes and prognostic factors for patients with salivary ductal carcinoma (SDC) treated with surgery and postoperative radiotherapy from 2005 to 2012. Materials and Methods: A total of 16 patients were identified and 15 eligible patients were included in analysis. Median age was 61 years (range, 40 to 71 years) and 12 patients (80%) were men. Twelve patients (80%) had a tumor in the parotid gland, 9 (60%) had T3 or T4 disease, and 9 (60%) had positive nodal disease. All patients underwent surgery and postoperative radiotherapy. Postoperative radiotherapy was delivered using 3-dimensional conformal radiotherapy or intensity-modulated radiotherapy. Locoregional failure-free survival (LRFFS), distant failure-free survival (DFFS), progression-free survival (PFS), and overall survival (OS) were calculated using the Kaplan-Meier method. Differences in survival based on risk factors were tested using a log-rank test. Results: Median total radiotherapy dose was 60 Gy (range, 52.5 to 63.6 Gy). Four patients received concurrent weekly chemotherapy with cisplatin. Among 10 patients who underwent surgery with neck dissection, 7 received modified radical neck dissection. With a median follow-up time of 38 months (range, 24 to 105 months), 4-year rates were 86% for LRFFS, 51% for DFFS, 46% for PFS, and 93% for OS. Local failure was observed in 2 patients (13%), and distant failure was observed in 7 (47%). The lung was the most common involved site of distant metastasis. Conclusion: Surgery and postoperative radiotherapy in SDC patients resulted in good local control, but high distant metastasis remained a major challenge.

  • This paper presents a geometrically non-linear and elastoplastic F.E. formulation using a total Lagrangian approach for the two dimensional isoparametric curved beam elements. The beam element is derived by using plane stress elements. The basic element geometry is constructed using the coordinates of the nodes on the element center line and the nodal point normals. The element displacement field is described using two translations of the node on the center line and a rotation about the axes normal to the plane containing the center line of the element. The layered approach is used for the elastoplastic analysis of the plane framed structure with the arbitrary cross section. The iterative load or displacement incremental method for non-linear finite element analysis of the frame structure is used. Numerical examples are presented to demonstrate the behavior and the accuracy of the proposed beam element for geometric and elastoplastic non-linear applications. Comparisons made with present theory and other published data show that tilt' beam element products accurate results with good convergence characteristics.

Metastatic Squamous Cell Carcinoma of the Lower Lip: Analysis of the 5-Year Survival Rate

  • Background: The author analyse the impact of extracapsular lymph node spread and bone engagement in the ipsilateral neck of patients suffering squamous cell carcinoma (SCC) of the lower lip. Methods: The data of 56 neck dissections performed in patients suffering SCC of the lower lip between January 2000 and December 2008 were retrospectively analysed. Statistical analysis was performed with the Kaplan-Meier life table method, and the survival rate was investigated with the log rank statistic and significance test. The values were considered statistically significant at p<0.05. Results: Nine patients took advantage from simultaneous treatment of tumor and prophylactic neck dissection (level I-III), reaching 100% survival rate. Patients suffering metastasized disease, who received radical neck dissection at the time of tumor treatment, presented 83.3% survival rate. Patients who underwent previous surgery and radiotherapy presented worse prognosis although radical neck dissection in case of extra-capsular spread only (24.7%) and osseous engagement (22.2%). Conclusion: Prophylactic neck dissection (level I-III) is recommended in T3-T4 N0 SCC. Simultaneous treatment of tumor and cervical lymph nodes provides a better prognosis as respect to delayed nodal management. Extra-capsular spread with or without bone engagement represents independent risk factor responsible for high mortality rate of SCC of the lower lip.

Distribution of Ki67 Proliferative Indices among WHO Subtypes of Non-Hodgkin's Lymphoma: Association with other Clinical Parameters

  • Background: Non-hodgkin lymphoma (NHL) is a diverse group of disease encompassing divergent tumor types with contrasting clinical behaviors. We aimed to evaluate the usefulness of Ki67 index in segregating indolent from aggressive NHL and its association with clinical parameters. Materials and Methods: During a study period of 4.5 years, a total of 215 cases of lymphomas were diagnosed among of which 172 cases were NHL. Ki67 immunohistochemical staining was performed by the DAKO envision method. Average proportion of tumor cells stained was calculated to determine the proliferative index. Results: The mean age at diagnosis was 46.2 years +19.8 (3-81) with a male to female ratio of 1.5:1. Mean Ki67 index for indolent NHL included 23% for small cell, 25% for mantle cell, 28.5% for marginal zone and 34.6% for follicular lymphoma. On the other hand, mean Ki67 index for aggressive lymphomas were 66.4%, 66.9%, 80.3%, 83.3% and 94.4% for diffuse large B cell, T cell (NOS), anaplastic large cell, lymphoblastic and burkitts lymphoma respectively. No significant correlation was found between Ki67 index and other clinical parameters like age and extra nodal involvement. Conclusions: Ki67 index is a valuable IHC marker to distinguish indolent from aggressive lymphomas especially in small needle biopsies where exact typing may not be possible.

  • A dynamic mathematical model of a thermal power plant with a single drum-type boiler is described with the base on modular concept. The present process model, which is including full scope of the components and high load changes, is based on physical approach through lumped parameters. The module, which means a component of the power plant and must essentially depict the characteristics of the component well, might be interconnected using pressure nodal method in a arrangement determined by users. With the results of the load changes of 75 MW to 95 MW and 95MW to 75 MW with the rate of 3 MW/min, the applicability of the process model is examined connecting to DCS(Dispersion Control System), which has a real running logic of 100 MW real power plant.
