A Study on the Ethics of Reproduction in Alain Resnais's Film -Focusing on , , and (알랭 레네 영화로 본 재현의 윤리 연구 -<밤과 안개>, <히로시마 내 사랑>, <뮤리엘>을 중심으로)

  • This paper focuses on Alain Resnais's representative works (1955), (1959), and (1963), and analyzes how he implements a representation of memory though cinematic apparatus. These three films deal with horrific memories that seem impossible to reproduce aesthetically such as the Holocaust, the Hiroshima Atomic Bomb, World War II, and the war in Algeria. The reappearance of events that stripped humans of even their minimum dignity can naturally be associated with ethical issues. These events can never be reproduced because they cannot be explained in the human language. It is also impossible to reproduce in a way that doesn't invade other peoples' sufferings, nor displays the pain of others as spectacles. Alain Resnais was a director who realized that if factual representation was not possible from the beginning, truthfulness would have to be approached through cinematic form. Therefore, he tries to overcome these problems through cinematic forms. First, he shifts to action films to avoid the obscenity of documentary. shows the records of camps captured by German forces in the past, while shows the pain of others in a fictional form of representation. Next, he describes how the trauma affects the identity of the main character through a flashback in , but also shows a main character who is experiencing trauma without a flashback in Flashbacks have the effect of showing the effects of trauma on the main character, but at the same time they involve the obscenity of enjoying the suffering of others. Nonetheless, the absence of flashbacks highlights the impossibility of representation. This is because it is not silent in the impossibility of representation but is constantly approaching. The attitude that repeatedly circles around impossibility is an ethical form that maximizes the impossibility of representation. In conclusion, this is the ethics of representation that Alain Resnais showed in his films.

The effect of tulobuterol patches on the respiratory system after endotracheal intubation

  • Background: Endotracheal intubation during anesthesia induction may increase airway resistance ($R_{aw}$) and decrease dynamic lung compliance ($C_{dyn}$). We hypothesized that prophylactic treatment with a transdermal ${\beta}2$-agonist tulobuterol patch (TP) would help to reduce the risk of bronchospasm after placement of the endotracheal tube. Methods: Eighty-two American Society of Anesthesiologists (ASA) category I or II adult patients showing obstructive patterns were divided randomly into a control and a TP group (n = 41 each). The night before surgery, a 2-mg TP was applied to patients in the TP group. Standard monitors were recorded, and target controlled infusion (TCI) with propofol and remifentanil was used for anesthesia induction and maintenance. Simultaneously, end-tidal carbon dioxide, $R_{aw}$, and $C_{dyn}$ were determined at 5, 10, and 15 min intervals after endotracheal intubation. Results: There was no significant difference in demographic data between the two groups. The TP group was associated with a lower $R_{aw}$ and a higher $C_{dyn}$, as compared to the control group. $R_{aw}$ was significantly lower at 10 min (P < 0.05) and 15 min (P < 0.01), and $C_{dyn}$ was significantly higher at 5 min (P < 0.05) and 15 min (P < 0.01) in the TP group. A trend towards a lower $R_{aw}$ was observed showing a statistically significant difference 5 min after endotracheal intubation (P < 0.01) in each group. Conclusions: Prophylactic treatment with TP showed a bronchodilatory effect through suppressing an increase in $R_{aw}$ and a decrease in $C_{dyn}$ after anesthesia induction without severe adverse effects.

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Korean Medical Treatment on a 79 years old Korean female with shoulder pain after stroke. ■ Methods She was treated with moxibustion, acupuncture, electroacupuncture and herbal medicine. We evaluated the improvement by Numeric Rating Scale(NRS) of shoulder pain and frequency of awakening due to shoulder pain ■ Results After 24 Days, total score of NRS was decreased from 7 to 2.1. Average sleep time was increased and waking up due to shoulder pain was decreased during the night. ■ Conclusions This study suggests that Korean medical treatment, especially Moxibustion could be effective to reduce shoulder pain and alleviate sleep disturbance. Further studies are needed.

Relationship between musculoskeletal subjective symptoms and work environment in dental hygienists (치과위생사의 근골격계 자각증상과 근무환경의 관련성)

  • Objectives : The purpose of this study was to identify the factors that affect working environment related musculoskeletal subjective symptoms among dental hygienists. Methods : The subjects in this study were 232 dental hygienists working at dental clinics and general hospitals in Daegu. The data has collected through the self-questionnaire survey from July 9th 2012 to July 31th. Results : 1. Musculoskeletal subjective symptoms were 85.3% in shoulder(right), 81.9% in neck, 74.6% in shoulder(left), 65.5% in wrist(right), 56.5% in lower leg(right). 2. Pain frequency of musculoskeletal subjective symptoms were the highest 24.7% in neck. Seeing the severity pain was the highest 9.0% in foot(left). The investigation of work interference related to substantially pain showed the highest 18.5% in wrist(right). 3. Musculoskeletal subjective symptoms correlated with general characteristics such as age, marital status, regular exercise and medical check-up(p<.05). 4. Musculoskeletal subjective symptoms correlated with working environments such as working career, the night treatment, the average daily number of patients handled, the average daily standing work hours, the regular rest, the major job in work place and physical burden(p<.05). Conclusions : The education or program on wrong working habits and bad postures of dental hygienists is needed to prevent musculoskeletal disorder.

A descriptive statistical analysis of inpatients with lumbar disc herniation at a Korean medicine hospital in 2014

  • Objectives : This is a retrospective statistical analysis of the demographic and therapeutic characteristics influencing the continued improvement of symptoms in patients treated in 2014 for herniated lumbar discs at a Korean medicine hospital; thereby, providing clinical data to further improve medical services of Korean medicine. Methods : We investigated the demographic and therapeutic variables of all patients who were diagnosed with a herniated lumbar intervertebral disc and were hospitalized for more than 1 night at Dunsan Korean medicine hospital from January 1, 2014, to December 31, 2014. IBM SPSS 21.0 was used to conduct a logistic multiple regression analysis and a covariance analysis (ANCOVA) of the demographic and therapeutic variables collected from the electronic medical records and telephone surveys. Results : 1. A longer duration of hospitalization was significantly better for the maintenance of pain relief or a decrease in the pain after discharge. 2. Younger patients were significantly less likely to be treated with a Western medical treatment after discharge. 3. Most of the demographic and therapeutic variables were not statistically significant in regards to treatment for lower back pain since discharge. Conclusion : Some of the demographic and therapeutic variables had a positive effect on the prognosis at one year or greater in patients who received integrative Korean medical treatment for lumbar disc herniation. Continued and systematic research will be needed.

Case Report of Polyarteritis Nodosa Treated with Oriental Treatments Including Acupotomy (침도침 시술을 가미한 복합한방치료를 시행한 결절성 다발 동맥염의 치험례)

  • Objectives : Polyarteritis nodosa is a progressive disease of connective tissue that is characterized by nodules along arteries; nodules may block the artery and result in inadequate circulation to the particular area. This report is intended to estimate the efficacy using oriental complex treatment on a patient with Polyarteritis nodosa. Materials and Methods : From 19th March, 2012 to 12th May, 2012, One male inpatient diagnosed with polyarteritis nodosa was treated with general oriental medicine therapy : needle-embedding therapy ; acupuncture ; pharmacopuncture ; acupotomy therapy and herbal medication. VAS(visual analogue scale) was used for evaluation of both leg pain. Other subjective symptoms including night sweat, tinnitus, upper heat were evaluated by percentage comparing the symtoms before and after treatment. Results : The patient showed a certain degree of improvement in both leg pain and other subjective symtoms. Conclusions : Oriental treatments such as needle-Embedding therapy, acupuncture and moxibustion therapy, pharmacopuncture therapy, acupotomy therapy and herbal medication can be effective for controlling pain and other accompanied symtoms due to polyarteritis nodosa.

  • Hinman syndrome is a condition representing urinary voiding dysfunction in the neurologically intact child. The syndrome is probably caused by acquired behavioral and psychosocial disorders manifested by bladder and/or bowel dysfunction mimicking neurologic disease. Clinically, the symptom complex may include day and night time enuresis, encopresis, constipation, and recurrent urinary tract infections. Cystoscopy frequently demonstrates normal vesicourethral anatomy. Voiding films usually demonstarate a carrot-shaped proximal urethra with a persistent narrowing at the external sphincter. The bladder is large and often appears trabeculated with a thickened wall and significant postvoid residual. A 13-year-old male child was admitted due to fever, urinary tract infection, enuresis and flank pain. His neurologic examination was normal. Renal sonograms showed moderate hydronephrosis. Voiding cystourethrograms showed a huge, trabeculated bladder without vesicourethral reflux and urethral valves. No abnormal findings was found in spinal MRI.

Solitary Lymphoblastic Lymphoma of the Thoracic Spine

  • Non-Hodgkin's lymphoma rarely originates from bone, and even more infrequently from a vertebral body. Lymphoblastic lymphoma is a rare type of non-Hodgkin's lymphoma, and results from an abnormality in adaptive immune cells. A 27-year-old man presented with a two-month history of night sweats, weight loss, and severe back pain. Radiological studies demonstrated an osteolytic lesion compressing the subarachnoid space at the T11 level. Posterolateral fusion with decompression was performed and a pathologic examination confirmed lymphoblastic lymphoma of the B-cell precursor type. To our knowledge, this is the first report of solitary lymphoblastic lymphoma from B-cell precursors in of the thoracic spine. Herein, we discuss the presenting symptoms and the management of this rare case of lymphoblastic lymphoma.

Round Acupuncture for the Treatment of Recurrent Carpal Tunnel Syndrome

  • Objectives: Round Acupuncture having blunt end has developed from acupotomy. This case report is to find out that Round Acupuncture is effective in treating patients with recurrent carpal tunnel syndrome (CTS), which has not improved by steroid injection or acupotomy. Methods: Round Acupuncture was inserted into the distal fibers of transverse carpal ligament and released toward the proximal fibers. Treatment was performed three times in total. T ingling, numbn ess, night pain and swelling sensation were assessed, and provocative maneuvers were also used. Results: After treat ment, all symptoms completely disappeared and the patient had no recurrence until 3 months after treatment. Conclusion: Round Acupuncture co uld be an effective treatment for recurrent CTS.

Sleep patterns and it's influencing factors of hospitalized elderly in long-term care hospital (요양병원 입원 노인의 수면 양상 및 영향요인)

  • This study was conducted to identify the sleep patterns and influencing factors of hospitalized elderly in a long-term care hospital. The sleep patterns of 142 subjects were recorded using Sleep Charts. The average sleep time of subjects was 10.7 hours a day (3.9 hours in daytime and 6.8 hours in nighttime). Sleep regularity among participants were 71.7% in all day (58.1% in day time and 80.5% in night time). The presence of dementia patients in the room (PDPR) has been identified to be a statistically significant predictor of all day sleep, and pain, PDPR, and physical function have been found to be a significant predictors of sleep regularity in all day among subjects. It suggested that elderly patients in a long-term care hospital do not slept well during night, which leads to increase in daytime sleep and decrease the quality of their sleep. Therefore, an intervention program should be developed to promote the quality of sleep among hospitalized elderly.
