Kissing Aneurysms of Distal Anterior Cerebral Arteries

  • The incidence of distal anterior cerebral artery[ACA] aneurysm is relatively rare, and only a few cases of bilateral symmetrical distal ACA aneurysms which were adhered together have been reported. They are also called kissing aneurysms. We treated bilateral asymmetrical distal ACA kissing aneurysms in a 44-year-old woman. We successfully clipped the double aneurysmal sacs individually by interhemispheric approach in spite of intraoperative aneurysmal rupture. The patient was discharged without any neurological deficits two weeks after the operation.

  • Although the ventriculo-peritoneal shunt is the treatment of choice when hydrocephalus should be corrected, the ventriculo-atrial shunt still holds strong alternative when the peritonium is precluded due to the several reasons. During the ventriculo-atrial shunt operation, it is not always easy to dissect and find the corresponding venous structures. In this technical note, the author describes a simple method of percutaneous insertion for placement of the atrial end.

Cervical Radiculomyelopathy due to Calcification of the Ligamentum Flavum

  • The authors experience a rare case of the cervical radiculomyelopathy due to calcification of ligamentum flavum at the level of C4-6 and report it with review of the literature. A 60-year-old woman was admitted, complaining progressive quariparesis, gait disturbance and neck pain. She had been treated with diabetes mellitus and hypertension for several years. On radiologic study, calcification of ligamentum flavum[CLF] at the C4-6 level was demonstrated on cervical spine CT and MR scan. Decompressive laminectomy of the C 4-6 and removal of CLF were performed. During operation, thecal sac was severely compressed by hypertrophic ligamentum flavum and there were tight adhesion between calcified ligamentum flavum and dura. After the operation, her motor function and neck pain improved and she could walk with an assistant.

  • Aspergillosis is an opportunistic infection associated with diabetes, alcoholism, use of corticosteroid and transplantation procedure. Aspergillosis of central nervous system is a rare disease and known to show an aggressive course with high mortality. We managed two cases of cerebral aspergillosis, following cranial operation, with combination therapy of surgery and antifungal agents. One patient recorvered but the other patient died. The clinical course of these two patients is presented with review of pertinent literature.

Intraventricular Malignant Meningioma with CSF-Disseminated Spinal Metastasis : Case Report and Literature Review

  • The authors report a case of 42-year-old woman with an intraventricular tumor in the trigone of the left lateral ventricle. The first operation achieved a microscopically complete resection. The tumor was histologically atypical meningioma. After 26 months, there were recurrences of intraventricular meningioma. Complete resection of the tumor and adjuvant radiation therapy were performed, and the histological diagnosis was malignant meningioma. Sixteen months after the second operation, spinal metastasis in cervicolumbar lesion was diagnosed and a subtotal removal of cervical intradural extramedullary mass was performed. We describe an unusual case of intraventricular malignant meningioma with cerebrospinal fluid-disseminated spinal metastases with review of the clinical courses of previous reports.

Vertebral Metastasis from Hepatocellular Carcinoma of Unknown Origin

  • This 51-year-old man suffered from paraparesis of 1-week history. On preoperative images, spinal cord compression by infiltrative vertebral mass was shown at T3 and T4 level. Several months earlier, he underwent surgical resection of left 2nd to 4th ribs, due to painful growing chest wall mass, which was proved to be hepatocellular carcinoma. All available diagnostic procedure failed to uncover origin of malignancy. Operation was followed by adjuvant irradiation and chemotherapy to the vertebral mass, however he only to die in 3 months after operation. This is an extremely rare case of ectopic hepatocellular carcinoma at thoracic vertebrae which showed very aggressive clinical course. Possible pathogenic process is presented and discussed.

Intraventricular Atypical Meningiomas

  • A rare case of intraventricular meningioma that arose in the atrium of the left lateral ventricle was identified in a 51-year-old woman. Gross total removal was performed by transcortical approach. Histopathological findings showed meningothelial meningioma with a focal atypical area which had 8% of Ki-67 labeling index (LI). A large recurrence extending into the ipsilateral quadrigeminal cistern and opposite medial occipital lobe developed approximately 41 months after the first operation. The specimens obtained from the second resection showed atypical meningioma with 20% of Ki-67 LI but there were no anaplastic area. The patient underwent fractionated stereotactic radiotherapy. However, multiple local distant metastases were found in the occipital and cerebellar cortex suggesting cerebrospinal fluid dissemination apparently 24 months after the second operation. This report presents chronological progression of a rare intraventricular atypical meningioma with more aggressive transformation.

로봇을 이용한 두개골 드릴링 시스템의 프로토타입 개발 (Prototype Development of a Robotic System for Skull Drilling)

  • This paper presents an overview of automated robotic system for skull drilling, which is performed to access for some neurosurgical interventions, such as brain tumor resection. Currently surgeons use automatic-releasing cranial perforators. The drilling procedure must be performed very carefully to avoid penetration of brain nerve structures; however failure cases are reported. The presented prototype system utilizes both preoperative and intraoperative information. Preoperative CT image is used for robot path planning. A NeuroMate robot with a six-DOF force sensor at the end effector is used for intraoperative operation. Intraoperative cutting force from the force sensor is the key information to revise an initial registration and preoperative path plans. Some possibilities are verified by path simulation but cadaver experiments are required for validation of this prototype.

A Case of Broncho-Paraspinal Fistula Induced by Metallic Devices : Delayed Complication of Thoracic Spinal Surgery

  • We present a case report of a 45-year-old woman with spontaneous pneumocephalus accompanied by pneumorrhachis of the thoracic spine, which is a very rare condition generally associated with trauma and thoracic or spinal surgery. The patient had undergone an operation about 10 years earlier to treat a giant cell tumor of the thoracic spine. During the operation, a metallic device was installed, which destroyed the bronchus and caused the formation of a broncho-paraspinal fistula. This is the suspected cause of her pneumocephalus and pneumorrhachis. To our knowledge, this is a very rare case of pneumocephalus accompanied by pneumorrhachis induced by metallic device, and when considering the length of time after surgery these complications presented are also exceptional.

  • Thirty nine-year-old man who required urgent shunt operation due to rapidly deteriorating visual acuity suffered from ventriculitis after aneurysmal operation. Daily dose of 20mg of vancomycin and amikyn were given intraventricularly via external ventricular catheter after failure of various kinds of systemic antibiotics. The exit of the catheter was made on the upper chest wall to prevent superinfection. External ventricular drainage could finally be switched to ventriculo-peritoneal shunt and he was discharged with clinical improvement.
