교모세포종 세포주 U-87에서 세포내 PKC 농도와 종양침습성과의 상관 관계 (The Relationship between Intracellular Protein Kinase C Concentration and Invasiveness in U-87 Malignant Glioma Cells)

  • 교모세포종은 비교적 흔한 원발성 뇌종양이며 생물학적 특성상 빠른 성장률을 보이는 것 외에 침습성이 강하여 종양과 인접한 부분을 파괴 시킬 뿐 아니라 직접접촉하지 않는 부분의 파괴도 일어나게 되어 그 결과 치료 예후가 매우 불량한 것으로 되어 있다. 이러한 불량한 예후를 개선 시키기 위해서는 이들 종양의 침습에 대한 기전의 정확한 이해가 필요하며 이를 이용한 새로운 치료방법이 요구된다할 것이다. Protein kinase C(PKC)는 세포내 신호전달체제 과정에서 매우 중요한 역할을 하는 효소로 세포막 수용체 신호를 핵으로 전달하는 역할을 하며 세포내 여러 생물학적 작용이 알려져 있다. 본 실험은 종양침습과 연관하여 세포내 PKC가 어떠한 작용을 하는지에 대해서 악성교종 세포를 대상으로 하여 알아보고자 하였다. 따라서 PKC가 종양침습에 중요한 역할을 할 것이라는 가설을 세웠고 이 가설을 증명하기 위해 세포내 PKC농도를 길항제 및 촉진제를 이용하며 높고 낮게 조절함으로써 그에 따른 침습성의 변화를 살펴보았다. 방법으로는 교모세포종 세포주인 U-87 세포를 약제로 처리한 후 인위적으로 조절된 세포내의 PKC에 대해 효소의 활성도를 측정하였고 침습성은 matrigel artificial basement membrane assay 및 tumor spheroid fetal rat brain aggregate(FRBA) confrontation assay를 이용하여 측정하였다. 결과로 PKC의 길항제인 tamoxifen과 hypericin으로 처치한 세포는 PKC의 활성과 침습도가 모두 감소하였으며 이는 약제농도에 비례하여 나타났다. 반면 PKC 자극제인 TPA로 처치된 세포는 증가된 PKC 활성도나 침습도을 보이지 않았다. 이러한 결과를 종합해 보았을 때 PKC는 종양세포의 침습성에 중요한 역할을 함을 알 수 있었으며 PKC의 길항제는 종양 치료에 유용한 화학 요법 제가 될 수 있을 것으로 사료된다.

동결견(凍結肩) 환자의 동서협진 치료의 임상효과 비교연구 - 견관절 가동운동범위(ROM) 변화를 중심으로 - (Comparison Research of Clinical Effect of Eastern and Western Medical Treatment on Frozen Shoulder Patients)

  • Objectives : To establish an effective collaborate medicine treatment of acupuncture and western medicine for treating frozen shoulder patients. Methods : 59 voluntary patients were randomly assigned to Eastern treatment group(E group, n=22), Western treatment group(W group, n=17) and East-West treatment group(EW group, n=20). The E group received acupuncture treatment on LI15, TE14, GB21 and Master Dong's acupuncture points, Shin-gwan and Gyun-joong, twice a week for 4 weeks. The W group received suprascapular nerve block, subacromial injection and trigger point injection, twice a week for 4 weeks. The EW group received acupuncture and injection treatment including nerve block All groups were instructed to practice self exercise during their daily lives. Evaluations were made before treatment and after treatment based on the change in shoulder Range of Motion(ROM) and the patient's satisfaction concerning the treatment was measured by Visual Analogue Scale(VAS). The obtained data were analyzed and compared. Results : The patient's satisfaction scores were E group 5.67, W group 7.73 and EW group 7.67. The E group and the EW group showed significant improvement in abbduction, adduction and flexion(p<0.05). The W group showed significant improvement in adduction(p<0.05). Abduction significantly improved(p<0.05) in the EW group compared to E group and W group. Flexion also showed improvement in the EW group, but the difference among the 3 groups was statistically insignificant. The three group's difference of change in extension and adduction was insignificant(p>0.05). Conclusion : Acupuncture and nerve block alone significantly improved ROM in frozen shoulder patients. Also collaborate treatment of acupuncture and nerve block significantly improved ROM in frozen shoulder patients. But the difference of the three treatments were significant only for improving abduction(p<0.05).

Pectorobrachioepicondylaris musculoaponeurotic band: case description with evidence of median nerve compression

  • Upper limb muscle variations can be encountered on imaging or at surgery. We report an unusual muscle and band found during routine dissection of the arm in a cadaver. This case is described and salient literature reviewed. A band was found that traveled from the insertion of the pectoralis major tendon distally and obliquely toward the medial intermuscular septum and medical epicondyle. Fibers of the brachialis were found to interdigitate into the band. A tunnel was formed that carried the median nerve and brachial vessels. Evidence of median nerve compression was observed. We considered this an example of a pectorobrachioepicondylaris muscle. However, some can lead to clinical presentations. Although the significance of the case reported herein is not certain, signs of median nerve compression were identified. We believe that the term pectorobrachioepicondylaris bests describes the muscle reported herein and that our case represents a previously unreported variant of this muscle.

동의보감 풍문 내 중풍증과 비병증, 역절풍증, 파상풍증 처방의 본초 조합 네트워크 비교 (A Comparative Study on the Herb Network of Prescriptions in the Dongui-Bogam Wind Chapter)

  • Objectives: This study was carried out to investigate whether a prescription's composition varies according to the disease being caused by wind, which is one of the migratory pathogenic factors. Methods: An initial database and binary matrix of Pungmun in Dongui-Bogam, including its herbs and prescription, was constructed. With this data, a network map about wind stroke, arthralgia, acute arthritis, and tetanus in Dongui-Bogam was constructed. Results: Analysis of the network map about Pungmun in Dongui-Bogam revealed that the complete prescription network has more isolated nodes than does each disease's network map. Conclusions: The composition of prescriptions in Dongui-Bogam Pungmun differ according to the disease being caused by wind.

18F-THK5351 PET Imaging in Nonfluent-Agrammatic Variant Primary Progressive Aphasia

  • Background and Purpose: To analyze $^{18}F-THK5351$ positron emission tomography (PET) scans of patients with clinically diagnosed nonfluent/agrammatic variant primary progressive aphasia (navPPA). Methods: Thirty-one participants, including those with Alzheimer's disease (AD, n=13), navPPA (n=3), and those with normal control (NC, n=15) who completed 3 Tesla magnetic resonance imaging, $^{18}F-THK5351$ PET scans, and detailed neuropsychological tests, were included. Voxel-based and region of interest (ROI)-based analyses were performed to evaluate retention of $^{18}F-THK5351$ in navPPA patients. Results: In ROI-based analysis, patients with navPPA had higher levels of THK retention in the Broca's area, bilateral inferior frontal lobes, bilateral precentral gyri, and bilateral basal ganglia. Patients with navPPA showed higher levels of THK retention in bilateral frontal lobes (mainly left side) compared than NC in voxel-wise analysis. Conclusions: In our study, THK retention in navPPA patients was mainly distributed at the frontal region which was well correlated with functional-radiological distribution of navPPA. Our results suggest that tau PET imaging could be a supportive tool for diagnosis of navPPA in combination with a clinical history.

청폐배독탕 및 청폐배독탕거마황을 사용해 COVID-19 증상의 개선을 보인 증례 보고 : 후향적 사례군연구 (Treating COVID-19 with Qingfei Paidu Decoction with and without Ephedra Herba: A Retrospective Case Series)

  • The purpose of this report is to describe the effects of Qingfei Paidu decoction (QPD) and QPD without Ephedra Herba on COVID-19 patients. One COVID-19 patient was treated with QPD and two others who had been sensitive to ephedra were treated with QPD without it. The fever, cough, and sore throat of all three were monitored three times a day through non-face-to-face assessment for a total of five to seven days, and cough and throat pain changes were evaluated with a numeric rating scale (NRS). After treatment, the fever and NRS scores were improved for all three patients. These results indicate that both QPD and QPD without Ephedra Herba is an effective treatment for COVID-19 symptoms.

사물안신탕가미방과 침 치료로 임상 증상에 호전을 보인 완전방실차단 환자 치험 1례 (A Case Report of Complete Atrioventricular Block Treatment with Samulanshin-tang-gamibang and Acupuncture)

  • Introduction: This study reports the effect of herbal medicine (Samulanshin-tang-gamibang) and acupuncture on complete atrioventricular (AV) block. Case presentation: A 63-year-old female with complete AV block was experiencing dyspnea, palpitation, dizziness, headache, bradycardia, and insomnia, and she was treated with Samulanshin-tang-gamibang and acupuncture for 12 days. To evaluate the treatment, a numeric rating scale (NRS) and the New York Heart Association (NYHA) functional classification was used. The patient's NRS scores decreased from 6 to 2 for dyspnea and palpitation and from 5 to 1 for dizziness and headache. Her NYHA Class improved from Class II to Class I. No side effects were observed during treatment. Conclusion: This study suggests that herbal medicine and acupuncture may be effective in relieving symptoms caused by complete AV block. However, the long-term effects of the treatment were not observed, and so further studies are still needed.

The Attenuation of Pain Behavior and Serum COX-2 Concentration by Curcumin in a Rat Model of Neuropathic Pain

  • Background: Neuropathic pain is generally defined as a chronic pain state resulting from peripheral and/or central nerve injury. There is a lack of effective treatment for neuropathic pain, which may possibly be related to poor understanding of pathological mechanisms at the molecular level. Curcumin, a therapeutic herbal extract, has shown to be effectively capable of reducing chronic pain induced by peripheral administration of inflammatory agents such as formalin. In this study, we aimed to show the effect of curcumin on pain behavior and serum COX-2 level in a Chronic Constriction Injury (CCI) model of neuropathic pain. Methods: Wistar male rats (150-200 g, n = 8) were divided into three groups: CCI vehicle-treated, sham-operated, and CCI drug-treated group. Curcumin (12.5, 25, 50 mg/kg, IP) was injected 24 h before surgery and continued daily for 7 days post-surgery. Behavioral tests were performed once before and following the days 1, 3, 5, 7 after surgery. The serum COX-2 level was measured on day 7 after the surgery. Results: Curcumin (50 mg/kg) decreased mechanical and cold allodynia (P < 0.001) and produced a decline in serum COX-2 level (P < 0.001). Conclusions: A considerable decline in pain behavior and serum COX-2 levels was seen in rat following administration of curcumin in CCI model of neuropathic pain. High concentration of Curcumin was able to reduce the chronic neuropathic pain induced by CCI model and the serum level of COX-2.

In-House Developed Surface-Guided Repositioning and Monitoring System to Complement In-Room Patient Positioning System for Spine Radiosurgery

  • Purpose: This study aimed to develop a surface-guided radiosurgery system customized for a neurosurgery clinic that could be used as an auxiliary system for improving the accuracy, monitoring the movements of patients while performing hypofractionated radiosurgery, and minimizing the geometric misses. Methods: RGB-D cameras were installed in the treatment room and a monitoring system was constructed to perform a three-dimensional (3D) scan of the body surface of the patient and to express it as a point cloud. This could be used to confirm the exact position of the body of the patient and monitor their movements during radiosurgery. The image from the system was matched with the computed tomography (CT) image, and the positional accuracy was compared and analyzed in relation to the existing system to evaluate the accuracy of the setup. Results: The user interface was configured to register the patient and display the setup image to position the setup location by matching the 3D points on the body of the patient with the CT image. The error rate for the position difference was within 1-mm distance (min, -0.21 mm; max, 0.63 mm). Compared with the existing system, the differences were found to be as follows: x=0.08 mm, y=0.13 mm, and z=0.26 mm. Conclusions: We developed a surface-guided repositioning and monitoring system that can be customized and applied in a radiation surgery environment with an existing linear accelerator. It was confirmed that this system could be easily applied for accurate patient repositioning and inter-treatment motion monitoring.
