고유수용성신경근촉진법이 편마비 환자의 보행능력에 미치는 영향 (The Effects of Proprioceptive Neuromuscular Facilitation on Gait Ability in Hemiplegic Patients)

  • Purpose : The aim of this study was to determine the effect of Proprioceptive Neuromuscular Facilitation (PNF) on gait ability in hemiplegic patients. Method : The subjects of this study were 11 hemiplegic patients. Each subject was taken PNF exercise with 3 times per week for 4 weeks. Pre- and post-intervention change in gait ability were measured using an 6m walking test, stride length, and step length. The data were analyzed using the paired t-test. Results : The results of this study showed significantly improvement in 6m walking test, stride length, and step length after intervention. Conclusion : These results suggest that the PNF coordination exercise is an effective way of improving gait ability for hemiplegic patients.

  • Purpose: The purpose of this study was to compare the strength and walking ability of chronic stroke patients following either proprioceptive neuromuscular facilitation (PNF) pattern training with pressure biofeedback units (feedback group) or PNF pattern training without pressure biofeedback units (control group). Methods: Eighteen participants with chronic stroke were recruited from a rehabilitation hospital. They were divided into two groups: a feedback group (n = 8) and a control group (n = 10). They all received 30 minutes of neurodevelopmental therapy and PNF training for 15 minutes five times a week for three weeks. Muscle strength and spatiotemporal gait parameters were measured. Muscle strength was measured by hand-held dynamometer; gait parameters were measured by the Biodex Gait trainer treadmill system. Results: After the training periods, the feedback group showed a significant improvement in hip abductor muscle strength, hip extensor muscle strength, step length of the unaffected limb, and step time of the affected limb (p<0.05). Conclusion: The results of this study showed that proprioceptive neuromuscular facilitation pattern training with pressure biofeedback units was more effective in improving hip muscle strength and walking ability than the proprioceptive neuromuscular facilitation pattern training without pressure biofeedback units. Therefore, to strengthen hip muscles and improve the walking ability of stroke patients, using pressure biofeedback units to improve trunk stability should be considered.

  • Purpose: The current study aims to identify the effects of proprioceptive neuromuscular facilitation (PNF) on pain and grip strength in patients with cervical herniated intervertebral disc (HIVD) and provide effective interventions for such patients. Methods: Included in this study were 20 subjects (10 men, 10 women) in their 30s-50s who had been diagnosed with cervical HIVD. Intervention methods included PNF neck patterns, dynamic reversal of antagonists, and a combination of isotonics. The visual analogue scale (VAS) was used to measure pain and dynamometers were used to measure grip strength. Results: After four weeks of muscle strengthening exercises, neck pain was significantly reduced, and hand grip strength significantly increased. Conclusion: PNF can be utilized as an effective intervention to reduce the pain of patients with cervical HIVD and to increase grip strength.

Effect of Neuromuscular Stabilization Exercise Program Using Whole Body Vibration on Patients with Low Back Pain

  • Objective: The purpose of this study is to investigate the effect of stabilization exercise on whole-body vibration on pain, dysfunction, psychosocial factors, balance ability, and abdominal contraction with patients with low back pain. Design: A randomized controlled trial Methods: A total of 34 patients with low back pain were assigned randomly to experimental group (n=17) and control group (n=17). Both groups underwent a neuromuscular stabilization exercise program. In addition, the experimental group implemented the neuromuscular stabilization exercise program using whole-body vibration. All interventions were applied 60 min per session, 3 times per week for total 4 weeks. Numeric Rating Scale (NRS), Korean version of Oswestry Disability Index (K-ODI), Fear-Avoidance Beliefs Questionnaire (FABQ), balance ability, muscle thickness and contraction ratio were compared to evaluate the effect on intervention. Results: Both groups showed significant differences in NRS, balance ability, and muscle thickness in contraction, contraction ratio before and after intervention (p<0.05). In addition, the experimental group showed significant difference in the amount of change in NRS, balance ability and muscle thickness in contraction, contraction ratio values than the control group (p<0.05). Conclusions: Neuromuscular stabilization exercise program combined with whole-body vibration stimulation has been proven to be an effective and clinically useful method to decrease pain, dysfunction, increase balance ablilty, and transverse abdominis muscle thickness in contraction and contraction ratio for patients with low back pain.

신경근전기자극 적용양식에 따른 대퇴사두근의 중앙주파수 변화 (Changes in Median Frequency of Quadriceps Muscle According to Application Modes of Neuromuscular Electrical Stimulation)

  • This study assigns each 8 of 24 normal persons to control group(Group I), strength increase group(Group II) and endurance increase group(Group III) to analyze differences in changes of strength and endurance with surface electromyography and kinetics according to application modes of neuromuscular electrical stimulation(NMES). Group I had not any treatment, group II performed 15 repeated contraction with 60% intensity of maximal voluntary isometric contraction(MVIC) by setting 10-sec on time and 50-sec off time and group III conducted 30 repeated contraction with 30% intensity of MVIC by setting 10-sec on time and 20-sec off time. For neuromuscular electrical stimulation, 2,500 Hz of Russian current, 35 pps of pulse rate and 200 of pulse width. Neuromuscular electrical stimulation was conducted by five times for total 4 weeks. Before and after experimentmotor unit action potential of vastus medialis, rectus femoris and vastus lateralis were measured with sEMG, median frequency(MDF) was analyzed, and thus the following results were obtained. There was significant difference in the period of measuring vastus medialis and rectus femoris in change of MDF and interaction among groups with analysis of surface electromyography before and after neuromuscular electrical stimulation(p<.001) and in particular, there was a remarkable change among groups according to the period of measurement. In conclusion, NMES influenced changes of strength and endurance according to its application modes and in particular, it was found that strength increment application had a significant influence on strength increment in applying short-time NMES.

  • PURPOSE: This study examined the effects of both lower extremities proprioceptive neuromuscular facilitation training with functional electrical stimulation on the balance and gait of stroke patients. METHODS: Ten patients with stroke were divided randomly into two groups of five patients each who met the selection criteria. The training was conducted five times a week, for 60 minutes. The experimental group received both lower extremities proprioceptive neuromuscular facilitation training with functional electrical stimulation while the control group received general physical therapy with functional electrical stimulation. RESULTS: The timed Up-and-Go (TUG) test result was statistically significant after the intervention in the experimental group (p<.041). Berg Balance Scale (BBS) assessment was statistically significant after the intervention in the experimental group (p<.047) and between the experimental and control groups (p<.012). The cadence assessment was statistically significant after intervention in the experimental group (p<.031) and between the experimental and control groups (p<.015). The stride length assessment was not statistically significant after intervention in the experimental group and between the experimental and control groups. Gait velocity assessment was statistically significant after the intervention in the experimental group (p<.031) and between the experimental and control groups (p<.015). CONCLUSION: Both lower extremities proprioceptive neuromuscular facilitation training with functional electrical stimulation had positive effects on the balance and gait of stroke patients.

경부근막동통증후군 환자의 가동범위에 대한 연구 (The Effects of Joint Mobolization on Neck Myofacial Pain Syndrome)

  • Purpose : To identify the effects of Hold-Relax of proprioceptive neuromuscular facilitation (PNF) on the functional improvement of patients with neck myofascial pain syndrome. Methode : The present research investigated 60 patients with neck myofascial syndrome, dividing them into a group doing Hold-Relax of proprioceptive neuromuscular facilitation and a group doing Stretching, This study examined degree of recovery from neck pain by comparing their neck myofascial pain syndrome before and after the treatment, and compared two groups to find difference in the degree of recovery from myofascial pain syndrome. Results : 1. For Hold-Relax of proprioceptive neuromuscular facilitation group, the pain rating score (PRS) were significantly decreased for six weeks treatment. 2. For Stretching group, the pain rating score (PRS) did not indicate significant decrease for six weeks treatment. 3. For Hold-Relax of proprioceptive neuromuscular facilitation group, range of motion of flexion, left rotation, right rotation indicated significant difference after pre test and after two week but no significant difference after four week. 4. For Stretching group, range of motion of flexion, left rotation, and right rotation except extension indicated significant difference after pre test and after two week but no significant difference after four week. Conclusion : The Hold-Relax of proprioceptive neuromuscular facilitation is considered to be effective in increasing joint range of motion of patient and decreasing pain. Accordingly physical therapists has to learn a wide range of patterns and to develop various treatment techniques.

  • Nerve conduction study (NCS) is an electrophysiological tool to assess the overall function of cranial and peripheral nervous system, therefore NCS has been diagnostically helpful in the identification and characterization of disorders involving nerve roots, peripheral nerves, muscle and neuromuscular junction, and are frequently accompanied by a needle Electromyography. Furthermore, NCS could provide valuable quantitative and qualitative results into neuromuscular function. Usually, motor, sensory, or mixed nerve studies can be performed with using NCS, stimulating the nerves with the recording electrodes placed over a distal muscle, a cutaneous sensory nerve, or the entire mixed nerve, respectively. And these findings of motor, sensory, and mixed nerve studies often show different and distinct patterns of specific abnormalities indicating the neuromuscular disorders. The purpose of this special article is to review the neurophysiologic usefulness of NCS, to outline the technical factors associated with the performance of NCS, and to demonstrate characteristic NCS changes in the setting of various neuromuscular conditions.

뇌졸중으로 인한 삼킴장애 환자의 경부근육전기자극치료에 따른 음성 변화 (Voice Change Associated with Swallowing Disorder Caused by a Stroke After Neuromuscular Electrical Stimulation)

  • 이 연구는 뇌졸중으로 인한 삼킴장애 환자를 대상으로 반복된 경부근육전기자극치료(NMES) 과정에서 나타나는 음성의 변화를 음향음성학적 분석 방법으로 분석하고, NMES 중재가 음성에 미치는 영향을 파악하고자 하였다. 2010년 2월 부터 2011년 3월까지 인천광역시의 2개 병원에서 삼킴장애로 진단을 받은 발성이 가능한 15명의 남성을 대상으로 2개월 동안 반복된 경부근육전기자극치료를 실시하였다. 음성은 강도, 음도($F_0$), 음질(Jitter, Shimmer, NNE; Normalized Noise Energy)을 측정하였고, 중재가 종료된 1주 후에 변화된 음성이 유지되었는지를 분석하였다. 연구결과, NMES 중재 후 대상자의 삼킴 기능 회복과 함께 음성 강도가 향상되었고, 성대 진동의 주기성(Jitter, Shimmer)이 안정되었다(P<0.05). 그러나 음도는 유의한 차이가 없었고, NNE는 중재 후 향상되었지만 여전히 비정상적인 수치를 보였다. 이 결과는 반복된 경부근육전기자극치료가 삼킴 능력의 회복과 함께 음성 강도의 향상 및 성대 진동 주기성의 안정화에 영향을 미칠 수 있는 가능성을 시사한다.

고유수용성신경근 촉진법 훈련과 동작관찰 훈련이 뇌졸중 환자의 상지 운동기능에 미치는 효과: 단일사례 연구 (Effect of Proprioceptive Neuromuscular Facilitation and Action Observation Training on Upper Extremity Motor Function in Stroke Patient: A Single-Subject Study)

  • This study evaluated and compared the effectiveness on upper motor extremity function between proprioceptive neuromuscular facilitation which has been frequently used in clinical practice, and action observation training in terms of improving upper motor extremity function. A study with a single-subject design (A-B-C-A') was conducted with a patient who was diagnosed with left hemiplegia. A repeated-measure analysis was conducted to assess results of the Wolf Motor Function Test (WMFT), Box and Block Test (BBT), and grip and pinch strength test performed daily in the study for 4 weeks. The results of the analysis indicated that the WMFT score, BBT score, grip strength, and pinch strength were improved from 29.60 to 39 (24.10%), from 1.67 to 4.93 each (EA) (66.22%), from 2.06 to 2.66 libras (lbs) (22.61%), and from 1.57 to 1.93 lbs (18.94%), respectively, from the baseline period to treatment period B. The values were improved from 29.60 to 42.20 (29.86%), from 1.67 to 7 EA (76.21%), from 2.06 to 3.47 lbs (40.57%), and from 1.57 to 1.67 lbs (6.12%), respectively, from the baseline period to treatment period C. From treatment period B to treatment period C, the WMFT score, BBT score, and grip strength were improved from 39 to 42.20 (7.58%), from 4.93 to 7 EA (29.56%), and from 2.66 to 3.47 lbs (23.20%), respectively, but pinch strength was decreased from 1.93 to 1.67 lbs (15.83%). In conclusion, proprioceptive neuromuscular facilitation and action observation training both have positive effects on upper extremity motor function. However, we suggest that the posttreatment effect of action observation training was better than that of proprioceptive neuromuscular facilitation.
