Ultrasonographic evaluation of common compression neuropathies in the upper limb

  • Neuromuscular ultrasonography has emerged over the last decade as a useful tool for diagnosing peripheral nerve disorders. It has been studied extensively with a particular focus on the assessment of compression neuropathies. Neuromuscular ultrasonography complements electrodiagnostic studies well by visualizing both the nerve anatomy and surrounding structures, providing useful data that cannot be obtained using the latter methodology only. This review article summarizes and synthesizes the literature focusing on the diagnostic role of neuromuscular ultrasonography in common compression neuropathies of the upper limb.

  • Clinical and genetic heterogeneity in association with overlapping spectrum is characteristic in pediatric neuromuscular disorders, which makes confirmative diagnosis difficult and time consuming. Considering evolution of molecular genetic diagnosis and resultant upcoming genetically modifiable therapeutic options, rapid and cost-effective genetic testing should be applied in conjunction with existing diagnostic methods of clinical examinations, laboratory tests, electrophysiologic studies and pathologic studies. Earlier correct diagnosis would enable better clinical management for these patients in addition to new genetic drug options and genetic counseling.

The effects of neuromuscular electrical stimulation on skeletal muscle architecture and qualitative properties in vivo

  • The purpose of this study was to evaluate the changes in skeletal muscle architecture and qualitative properties by muscle contraction force when neuromuscular electrical stimulation (NMES) of 50% MVIC was applied. Sixteen subjects (8 male, 8 female) without neuromuscular disease volunteered to participate in the study. All subjects were divided into two subgroups: control (no electrical stimulation) group and 50% maximal voluntary isometric contraction (MVIC) group. NMES training program was performed in the calf muscle three times a week for 10 weeks. Before and after the experiments, the MVIC of ankle plantar flexor was measured by the use of dynamometer, and the ultrasonography in the gastrocnemius medialis muscle was measured. The following results were obtained; MVIC was significantly increased in the electrical stimulation groups. Pennation angle, muscle density, and white area index also considerably changed in the electrical stimulation groups. In conclusion, the NMES training of 50% MVIC, comparative low level, improved the skeletal muscle architecture and the qualitative properties as well as the muscle contraction force.

  • A system has been developed for monitoring the effect of neuromuscular blocking frugs and the neuromuscular function during anesthesia and surgery. This system is composed of software and hardware, the latter are nerve stimulator, force transducer, interface board(preamplifier, filter, peripheral input/output) and personal computer (apple ll) , the former are programmed in ASSEMBLY and BASIC language. The nerve stimulator which is controlled by personal computer is capable of delivering single shocks at o.)Hz, train of four at 2Hz and tetanic stimulation at 30, 100, 200Hz. The response, adduction of the thumb, is sensed by the force transducer. The output of the force transducer Is amplified, filtered, converted digital signal and then processed by the per- sonal computer. The personal computer quantia4es twitch and traln of four tesponse and calculates the 74 ratio (Ta/Tl )between the first and fourth response of train of four. This ratio is used to estimate the level of the neuromuscular block. This system has reaserch potential for determining the effect of newer neuromuscular blocking drugs for comparlsion with presently used drugs of alternatively, for delerminig the effects of blocking drugs in altered physilogical states.

  • Neuromuscular junction consist of presynaptic membrane, synaptic cleft and postsynaptic membrane. In the neuromuscular junction, presynaptic membrane is the motor nerve terminal, have many synaptic vesicle. Postsynaptic membrane is the motor end plate of muscle fiber and the most striking structural features are the deep infolding of the sarcolemma. Between the nerve and muscle cells, there is a synaptic cleft of some 50-100nm. This review shows the ultrastructure and function of neuromuscular junction, summarizes the current knowledge.

Airway anesthesia with lidocaine for general anesthesia without using neuromuscular blocking agents in a patient with a history of anaphylaxis to rocuronium: a case report

  • We experienced a case of induction of general anesthesia without using neuromuscular blocking agents (NMBAs) in a 40-year-old woman with a history of anaphylaxis immediately after the administration of anesthetics lidocaine, propofol, and rocuronium to perform endoscopic sinus surgery 2 years before. The skin test showed a positive reaction to rocuronium and cis-atracurium. We induced general anesthesia without using NMBAs after inducing airway anesthesia with lidocaine (transtracheal injection and superior laryngeal nerve block). Deep general anesthesia was maintained with end-tidal 4 vol% sevoflurane. Hypotension was treated with phenylephrine infusion. The operation condition was excellent, and patient recovered without complications after surgery. Airway anesthesia with local anesthetics may be helpful when we cannot use NMBAs for any reason, including hypersensitivity to NMBA and surgery that needs neuromuscular monitoring.

Effects of the He-Ne IR Laser Irradiation and Neuromuscular Electrical Stimulation on the Change of the Serum Biochemical Components in the Experimentally Induced Muscle Injured Rats

  • Low power laser and neuromuscular electrical stimulation have been utilized in many clinical applications for the treatment of musculoskeletal lesion. This study was to investigate the effects of low power He-Ne IR laser and neuromuscular electrical stimulation on the change of the serum biochemical components in experimentally induced muscle injured rats. The twenty Sprague-Dawley male rats were randomly assigned to the 4 groups: the normal group (n=5), the control group (n=5), the low power laser irradiation group (LLI) (n=5) and the neuromuscular electrical stimulation group (NMES) (n=5). The experimentally induced muscle injury was induced by the subcutaneous injection of a 0.1 ml Freund's Complete Adjuvant into the right hind paw and calf muscles in control, LLI and NMES groups. The LLI group was irradiated with He-Ne IR laser for 5 minutes every day during 10 days. Also, the NMES group was electrically stimulated 15 minutes every day during 10 days. The normal and control groups were not received with any therapeutic intervention. The authors performed tests which were the concentration of the serum biochemical components to detect the effects of therapeutic interventions. The results were as follows: 1. The level of the aspartate aminotransferase (AST) was significantly decreased in the LLI and NMES groups compare to the control group. 2. The level of the serum lactate dehydrogenase (LDH) was significantly decreased in the LLI and NMES groups compare to the control group. 3. The level of the serum creatine kinase (CK) was significantly decreased in the LLI and NMES groups compare to the control group. From the results, we could come to the conclusion that low power laser and neuromuscular electrical stimulation could be accelerating healing processes of skeletal muscle injury and further clinical work will be required.

저출력레이저와 전기자극에 의한 골격근 손상 흰쥐의 IL-1β, IGF-1, IGF-2 활성 (Activation of IL-1β, IGF-1 and IGF-2 in Injured Rat Skeletal Muscle by Low Power He-Ne IR Laser and Electrical Stimulation)

  • 본 연구는 골격근 손상에 대한 물리치료 중재의 치료 효과를 근전도, 초음파 영상 그리고 IL-$1{\beta}$, IGF-1, IGF-2의 변화를 통해 알아보았다. 본 연구를 위해 실험동물을 정상군, 대조군, 레이저치료군, 신경근전기 자극군으로 무작위 배치하였다. 연구의 결과 근전도상 모든 실험동물에서 비정상적인 자발전위는 관찰되 지 않았으며 근육의 최대 횡단 직경은 대조군에 비해 레이저치료군과 신경근전기자극군이 유의하게 증가 하였다. IL-$1{\beta}$의 수준은 대조군에 비해 레이저치료군과 신경근전기자극군에서 보다 많이 감소하였으며 IGF-1과 IGF-2는 대조군, 레이저치료군 그리고 신경근전기자극군 모두 정상군에 비해 유의하게 높았으 나 대조군과 레이저치료군 그리고 신경근전기자극군 사이에 유의한 차이는 없었다.

Effects of adenosine receptor agonist on the rocuronium-induced neuromuscular block and sugammadex-induced recovery

  • Background: Several types of receptors are found at neuromuscular presynaptic membranes. Presynaptic inhibitory $A_1$ and facilitatory $A_{2A}$ receptors mediate different modulatory functions on acetylcholine release. This study investigated whether adenosine $A_1$ receptor agonist contributes to the first twitch tension (T1) of train-of-four (TOF) stimulation depression and TOF fade during rocuronium-induced neuromuscular blockade, and sugammadex-induced recovery. Methods: Phrenic nerve-diaphragm tissues were obtained from 30 adult Sprague-Dawley rats. Each tissue specimen was randomly allocated to either control group or 2-chloroadenosine (CADO, $10{\mu}M$) group. One hour of reaction time was allowed before initiating main experimental data collection. Loading and boost doses of rocuronium were sequentially administered until > 95% depression of the T1 was achieved. After confirming that there was no T1 twitch tension response, 15 min of resting time was allowed, after which sugammadex was administered. Recovery profiles (T1, TOF ratio [TOFR], and recovery index) were collected for 1 h and compared between groups. Results: There were statistically significant differences on amount of rocuronium (actually used during experiment), TOFR changes during concentration-response of rocuronium (P = 0.04), and recovery profiles (P < 0.01) of CADO group comparing with the control group. However, at the initial phase of this experiment, dose-response of rocuronium in each group demonstrated no statistically significant differences (P = 0.12). Conclusions: The adenosine $A_1$ receptor agonist (CADO) influenced the TOFR and the recovery profile. After activating adenosine receptor, sugammadex-induced recovery from rocuronium-induced neuromuscular block was delayed.
